Median cleft nose: Failure of cartilage assembly-a retrospective descriptive case series of three patients.

BACKGROUND: Median cleft nasal deformity is traditionally described as a disorder of midline development, encompassing varying degrees of tissue deficiency, malposition, and excess. However, intraoperative findings in the present series suggest that, in selected patients, the cartilaginous framework is present but structurally disorganized rather than absent, with implications for surgical strategy. METHODS: Three patients with median cleft nasal deformity within the frontonasal dysplasia spectrum underwent reconstruction between 9 and 12 months of age using a technique based on mobilization,

Influence of maxillary clockwise rotation with posterior impaction and mandibular setback by SSRO or IVRO for mandibular prognathism on volume change in upper airway.

PURPOSE: In orthognathic surgery for skeletal mandibular prognathism, maxillary clockwise rotation with posterior impaction and mandibular setback is often performed. Change in airway volume after orthognathic surgery is one of the concerns. The present study was performed to evaluate the effects of maxillary clockwise rotation with posterior impaction and the influence of different mandibular techniques, including sagittal splitting ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO), on upper airway volume changes before and after bimaxillary surgery for mandibular prognathi

Single Versus Double Pin Fixation of the Calcaneocuboid Joint During Calcaneal Lengthening Osteotomy for Pediatric Idiopathic Flatfoot.

BACKGROUND: Calcaneocuboid (CC) joint subluxation is a recognized complication after calcaneal lengthening osteotomy for pediatric idiopathic flexible flatfoot, and temporary pin fixation is often performed to prevent it. This study compared postoperative radiologic outcomes at 2 years of single- and double-pin fixation of the CC joint. METHODS: We retrospectively reviewed 50 feet from 30 patients aged 9 to 15 years who underwent calcaneal lengthening osteotomy for idiopathic flexible flatfoot with a follow-up of at least 2 years. Fourteen feet (28%) underwent single-pin fixation, and 36 (72%

Role of the posterior tibial tendon in plantar pressure distribution across progressive stages of ligamentous insufficiency in a cadaveric flatfoot model.

BACKGROUND: Progressive collapsing foot deformity involves failure of both static and dynamic medial arch stabilizers. The relative timing and interaction between posterior tibial tendon dysfunction and ligamentous attenuation remain unclear. This study investigates whether the posterior tibial tendon can compensate for progressive ligament failure in distributing pressure under the foot in a cadaveric progressive collapsing foot deformity model at balanced stance. METHODS: Eight cadaveric lower limbs underwent sequential ligament transections replicating progressive collapsing foot deformity

Interplay between genomic architecture alterations and GDF6 regulation: A candidate mechanism in Nablus mask-like facial syndrome.

Nablus mask-like facial syndrome (NMLFS) is a very rare disorder associated with 8q22.1q22 microdeletions and characterized by a distinctive facial phenotype and variable neurological, cardiological, and genital anomalies. However, not all affected individuals with overlapping deletions present the mask-like phenotype, thus suggesting the involvement of an additional regulatory mechanism. Here, we perform a comparative analysis of previously reported affected individuals (n = 30) with deletions in the 8q21q22 region, stratified by the presence or absence of the NMLFS phenotype, and including a

Targeting Interleukin-8-Mediated Cellular Crosstalk Reverses Hypertrophic Cardiomyopathy and Cardiac Fibrosis in Noonan Syndrome.

BACKGROUND: Genetic variants in components or regulators of the RAS-mitogen-activated protein kinase (RAS-MAPK) signaling pathway are causative for severe and early-onset hypertrophic cardiomyopathy (HCM) in patients with Noonan syndrome (NS). Despite paracrine communication being considered to play a pivotal role in the etiology of cardiomyopathies, there is a paucity of knowledge about the underlying pathomechanism that leads to the development of HCM and cardiac fibrosis in NS. METHODS: To dissect the impact of noncardiomyocytes in the development of NS, we employed two-dimensional and thr

Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Bentall Procedure in a Patient with Loeys-Dietz Syndrome: A Case Report.

Loeys-Dietz syndrome (LDS) is a rare autosomal dominant connective tissue disorder characterized by aggressive aortopathy, craniofacial anomalies and systemic involvement. Rapid aortic root dilatation and valvular dysfunction mandate an early surgical intervention. We report a case of 15-year-old boy with LDS, rapidly progressive aortic root dilatation and severe aortic regurgitation (AR) who underwent successful Bentall procedure. The case highlights the need of early detection, multidisciplinary involvement and meticulous anesthetic care.

FAP adipogenesis drives early spinal muscle loss in dysferlinopathy and reveals a therapeutic target.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology

Foot Progression Angle Modulates Knee Loading During Walking in Individuals with Flexible Flatfoot.

PURPOSE: This study evaluated tibiofemoral loading and medial meniscal stress distribution in individuals with flexible flatfoot (FFF) during walking under different foot progression angle (FPA) conditions. METHODS: This study analyzed the gait of 28 FFF patients (16 males, 12 females) under three FPA conditions (neutral, toe-in, toe-out). Kinematic (Vicon) and kinetic (Kistler) data were used to estimate tibiofemoral forces in OpenSim. Subsequently, joint angles and muscle forces at peak tibiofemoral forces were used to drive a finite element (FE) model of the knee, enabling the comparison o

Evaluation of the clicky hip as a screening tool in the diagnosis of developmental dysplasia of the hip.

AIMS: The early diagnosis of developmental dysplasia of the hip (DDH) is important because treatment in early infancy is usually nonoperative. A consensus statement from the British Society for Children's Orthopaedic Surgery identified ongoing uncertainty regarding the predictive value of a documented clicky hip. The aim of this study was to evaluate the association between a clicky hip at the newborn or six- to eight-week examination and pathological DDH requiring treatment in a selective screening programme in the UK. METHODS: This was a retrospective analysis of prospectively collected reg

Phenotypic spectrum of 1q21.1 copy number variants in 42 children: genotype-phenotype correlations and clinical implications.

UNLABELLED: Chromosome 1q21.1 copy number variants (CNVs) are associated with a broad and variable clinical spectrum encompassing neurodevelopmental, anthropometric, and systemic anomalies. The incomplete penetrance and variable expressivity of these rearrangements pose significant challenges for clinical interpretation and genotype-phenotype correlation. We retrospectively analyzed 42 pediatric patients with 1q21.1 CNVs, identified by chromosomal microarray analysis (CMA), evaluated at a single tertiary center between 2019 and 2025. Patients were classified according to CNV type and localizat

Complex presentation of neonatal arthrogryposis multiplex congenita: insights into a multifactorial disorder.

This case report details a neonate diagnosed with arthrogryposis multiplex congenita (AMC), specifically the amyoplasia subtype, presenting with bilateral talipes equinovarus, elbow contractures, ulnar deviation of the wrists and clinodactyly. Additional clinical findings included mild developmental dysplasia of the hips and a small atrial septal defect. Genetic analysis revealed heterozygous variants of uncertain significance in the SYNE1 and CHD4 genes, both of which were inherited from asymptomatic parents. While SYNE1 is associated with autosomal recessive AMC type 3 and CHD4 with Sifrim-H

Targeted modulation of thalamocortical and descending control networks in chronic shoulder pain: a longitudinal connectomics study.

BACKGROUND: Central sensitization involves maladaptive plasticity in ascending nociceptive and descending modulatory pathways. This pilot study was designed with three integrated aims: (1) to characterize baseline thalamocortical and descending control network abnormalities in chronic shoulder pain (CSP) relative to healthy controls; (2) to examine whether these network features change following a standardized course of peripheral myofascial stimulation (PMS); and (3) to explore potential mechanistic pathways linking peripheral intervention to central network reconfiguration. While PMS has bee

Photon-counting CT in musculoskeletal imaging: New horizon for understanding bone health in osteoarthritis.

Osteoarthritis is a heterogeneous disease affecting many joint tissues. The significant role of bone, and in particular abnormalities in the subchondral bone, in OA is increasingly understood. Bone marrow lesions are believed to play an important role in OA symptom perception and progression of OA. Consequently, there is an increasing interest in the characterization of subchondral bone and bone marrow lesions by means of imaging. Computed tomography (CT) has shown to be a suitable imaging method to visualize various bony abnormalities in OA, however, the spatial resolution of conventional CT

A Sedation Protocol for Endoscopic Baton-Plate Adjustment in Neonates With Pierre Robin Sequence.

BACKGROUND: Robin sequence (RS) is characterized by upper airway obstruction leading to life-threatening desaturations in neonates. Procedural sedation in this population presents unique challenges due to anatomical airway anomalies, young patient age, and associated comorbidities. The pre-epiglottic baton-plate (PEBP) procedure is a non-invasive, endoscopically guided intervention that alleviates obstruction, facilitates oral feeding, and promotes mandibular growth. Sedation for PEBP adjustment requires the maintenance of spontaneous respiration while ensuring patient comfort and procedural t

Histologically Confirmed Adult Tracheomalacia at Autopsy After Single-Lung Transplantation.

A man in his 50s developed persistent ventilator dependence, recurrent pneumonia, and lung abscess after right single-lung transplantation, and later died following complications from native lung pneumonectomy. Autopsy confirmed the diagnosis of tracheomalacia, allowing detailed histological examination that revealed extensive cartilage degeneration, disruption, and fibrosis of the trachea. Because histopathological studies of adult tracheomalacia are extremely rare, this case provides valuable insight into its pathology.
