Selective percutaneous myofasciotomy in persistent toe-walking: Anatomical rationale and surgical technique.

UNLABELLED: Persistent toe-walking is a gait abnormality characterized by forefoot initial contact and reduced or absent heel strike beyond early childhood. Although often regarded as a transient developmental variant, persistence of this pattern may lead to progressive limitation of ankle dorsiflexion, fixed equinus contracture, and long-term functional impairment. Toe-walking may occur as an isolated idiopathic condition or in association with neurological, neuromuscular, or genetic disorders. Increasing evidence suggests that a subset of patients previously classified as idiopathic harbor u

Surgical and orthodontic management of class III malocclusion in monozygotic twins using bilateral sagittal split osteotomy.

This case report discusses the genetic influence and treatment of class III malocclusion in identical twin sisters, in their early 20s, who presented with mandibular prognathism. Both exhibited concave profiles with hypodivergent growth patterns, but Sister A had a more pronounced concavity. Diagnosis revealed skeletal class III malocclusion with mandibular prognathism and negative overjet (9 mm in Sister A, 6 mm in Sister B). Treatment involved pre-surgical orthodontics, bilateral sagittal split osteotomy (BSSO) with mandibular setbacks (7 mm for Sister A, 5 mm for Sister B), followed by post

Lung disease in crisponi/cold-induced sweating syndrome type 1: expanding the clinical phenotype spectrum.

UNLABELLED: Crisponi/Cold-Induced Sweating Syndrome Type 1 (CS/CISS1) is an ultra-rare genetic disorder caused by biallelic variants in the cytokine receptor-like factor-1 (CRLF1) gene and is associated with several comorbidities. Comprehensive data on the respiratory phenotype of this condition are lacking despite being associated with high mortality in infancy, often due to severe respiratory complications like aspiration pneumonia and sudden death related to hyperthermic crises. Our study investigates respiratory complications and lung ultrasound characteristics, enhancing the comprehension

Negative symptom dimensions link cognitive impairment to global and social functioning in 22q11.2 deletion syndrome.

BACKGROUND: 22q11.2 deletion syndrome (22q11DS) confers a 20-30% lifetime risk of schizophrenia, with negative symptoms associated with poorer outcomes and higher psychosis risk. High rates of neurocognitive deficits, autism spectrum traits (ASD), and attention-deficit/hyperactivity disorder (ADHD) may further complicate symptom profiles and functional outcomes. We examined relationships among positive and negative symptom severity, ASD traits, ADHD symptoms, and global functioning in 22q11DS. METHODS: Individuals with 22q11DS (n = 38) and healthy comparison subjects (n = 34) completed clinic

The R203W substitution drives PACS-1 syndrome by disrupting intramolecular regulation.

The c607C>T mutation in the PACS1 gene results in an Arg203Trp substitution in the multifunctional protein PACS-1, and drives a syndrome characterized by intellectual disability, seizures, craniofacial dysmorphisms, and various characteristics of the autism spectrum. On the molecular level, this syndrome, in part, results from enhanced association of PACS-1 with the protein deacetylase HDAC6. PACS-1 uses its Furin binding region (FBR: amino acids 101-273) to directly interact with the catalytic domains of HDAC6. We present the solution structure of a chimeric PACS-1 FBR and use NMR to demonstr

Consensus-based detection of aetiologic copy number variants for syndromic orofacial clefts utilising whole exome sequencing of case parent trios.

BACKGROUND: Orofacial clefts (OFCs) are the most common craniofacial congenital anomalies, with complex aetiology involving both genetic and environmental factors. Most genetic studies on the condition have focused on the contribution of single nucleotide variants (SNVs) and small insertions and deletions (indels). However, the contribution of copy number variants (CNVs), especially in African populations, remains underexplored despite their known contribution to congenital anomalies. This study aimed to identify high-confidence CNVs potentially contributing to the aetiology of syndromic OFCs

Diffusion Tensor Imaging of Knee Cartilage for Detection of Early Osteoarthritis and Prediction of Radiographic Progression.

BACKGROUND: The progression of early osteoarthritis (OA) is difficult to predict radiographically. T2 mapping provides information about cartilage degeneration, but its sensitivity to early matrix injury is limited. Diffusion tensor imaging (DTI) may detect cartilage microstructural abnormalities beyond T2 mapping. PURPOSE: To test whether baseline cartilage DTI metrics are associated with early knee OA and 3-year radiographic progression, and to compare their performance with T2 mapping and baseline Whole-Organ Magnetic Resonance Imaging Score (WORMS). STUDY TYPE: Prospective longitudinal c

Mosaicism for Cohen-Gibson Syndrome in a Female Individual With Overgrowth and Neurodevelopmental Delay.

BACKGROUND: The Embryonic Ectoderm Development (EED) gene encodes a core component of the Polycomb Repressive Complex 2 (PRC2), which regulates chromatin structure and plays a key role in embryonic development and cell differentiation. Pathogenic variants in EED are associated with overgrowth-intellectual disability syndrome (Cohen-Gibson syndrome, OMIM #617561), though reported cases are rare and typically involve heterozygous germline variants. This study presents an individual with confirmed postzygotic mosaicism in EED, contributing to diagnostic complexity. METHODS: A 12-year-old girl wi

Management of Upper Limb Deformities in Arthrogryposis Multiplex Congenita: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) presents complex therapeutic challenges in upper limb reconstruction, in which establishing an optimal positioning-reaching-grasping axis is critical for patient independence. However, clinical evidence is often limited by low-level data and significant phenotypic heterogeneity, leaving management strategies fragmented. This review synthesizes current evidence to provide a strategic framework for managing complex upper extremity deformities in AMC. A selective literature search was conducted via the MEDLINE database in April 2026, focusing on advancemen

Two Atypical Cases of Adams-Oliver Syndrome With DOCK6 Genetic Variants.

Adams-Oliver syndrome (AOS) is characterized by scalp aplasia cutis congenita (ACC) and terminal transverse limb defects (TTLD). Autosomal recessive AOS type 2 caused by DOCK6 variants is usually accompanied by central-nervous-system and ocular anomalies. We report two unrelated children who each carried compound heterozygous DOCK6 variants. Both presented with variable degrees of global developmental delay and visual impairment, but neither showed ACC or TTLD. Our observations highlight the phenotypic variability in DOCK6-related AOS and a possible ascertainment bias in screening patients onl

Veterinarians' attitudes towards the treatment and surgical management of brachycephalic dogs - a survey among small animal veterinarians in Austria.

BACKGROUND: The rising popularity of flat-faced dog breeds has intensified discussions around brachycephaly and its associated health risks. The structural shortening of the skull coupled with a lack of a concurrent reduction in the volume of the soft tissues in these breeds often results in chronic health issues, notably Brachycephalic Obstructive Airway Syndrome (BOAS), which remains incurable but may be alleviated through corrective surgical interventions. In light of these challenges, veterinarians are increasingly called upon to provide informed guidance and support to pet owners navigati

A Novel Single-Stage Pleated Nailbed Flap for Toe Macrodactyly Debulking.

The authors describe a novel single-stage, nailbed-preserving debulking procedure for macrodactyly used in their unit. This simple technique preserves perfusion and venous drainage of the nailbed complex through a subperiosteal dissection and use of a dermal bridge by de-epithelialization of the proximal area, followed by folding the nailbed flap back onto itself in a pleated fashion. Four toes of 3 patients were treated with this technique, and all achieved a shorter, less bulky appearance with nail preservation at 2-year follow-up. The single-stage pleated nailbed flap is a simple and robust

FBXL21 regulates diurnal proteostasis in skeletal muscle by targeting DNAJB6 and client proteins.

Circadian regulation of proteostasis, a key determinant of muscle health, remains poorly understood. Here, we identify DNAJB6, an Hsp40 (DnaJ) co-chaperone, as a substrate of the circadian E3 ligase FBXL21. FBXL21 mediates the ubiquitination-dependent proteasomal degradation of both DNAJB6 and its client proteins, including Desmin. In contrast, myopathy-causing mutations of DNAJB6 confer resistance to FBXL21-directed degradation. Fbxl21 KO C2C12 cells display aberrant Desmin accumulation, and show aggravated cytoplasmic accumulation of TDP-43, another DNAJB6 client protein, in response to heat

Evaluation of Salter and Pemberton osteotomies in developmental dysplasia of the hip: the role of the congruity index as a prognostic factor.

BACKGROUND: The Salter osteotomy (SO) and the Pemberton osteotomy (PO) are common surgical methods for treating paediatric developmental dysplasia of the hip (DDH). Although they each have their own advantages, complications resulting from open surgeries are still relatively common. This study aimed to evaluate the clinical outcomes of SO and PO in the treatment of DDH and to identify prognostic factors associated with postoperative outcomes. METHODS: Between April 2011 and May 2018, we retrospectively reviewed 72 DDH patients who underwent open reduction combined with pelvic osteotomy (SO/PO
