Microcirculatory Alterations and Endothelial Glycocalyx in Fontan Circulation: Association With Central Venous Pressure.

OBJECTIVE: Fontan circulation is characterized by chronically elevated central venous pressure (CVP) and reduced cardiac output, which may adversely affect the systemic microcirculation. We investigated whether Fontan circulation is associated with alterations of the endothelial glycocalyx. METHODS: Forty-four patients with congenital heart disease undergoing postoperative endothelial glycocalyx evaluation were included. The perfused boundary region (PBR), an inverse indicator of glycocalyx dimensions, was measured noninvasively using GlycoCheck. PBR was compared between patients with Fontan

The impact of nationwide folic acid fortification on genetic variants associated with conotruncal heart defects.

Folate deficiency is associated with increased risk of conotruncal heart defects (CTHD), but its interaction with genetic factors remains unclear. Our objective was to investigate genome-wide associations between common genetic variants and birth before versus after universal folic acid fortification among children with CTHD and other heart defects. Sequencing data were available through the Pediatric Cardiac Genomics Consortium, aligned to the human reference genome (GRCh38/hg38), and jointly processed to ensure uniform variant detection and minimize batch effects. Analyses were restricted to

Discovery of novel alleles in Kartagener's syndrome.

Kartagener's syndrome (KS) is a rare, typically autosomal recessive ciliopathy and a subtype of primary ciliary dyskinesia (PCD), characterized by situs inversus, bronchiectasis, and chronic sinusitis. KS accounts for approximately 50% of PCD cases and results from impaired motile cilia function. Defective embryonic nodal ciliary beating leads to stochastic left-right axis determination and situs abnormalities. Although numerous genes have been implicated in PCD, up to 30% of cases remain genetically unresolved, and studies specifically focused on KS cohorts remain limited. Here, we performed

Co-occurrence of aortic dissection and polycystic kidney disease: a case report.

BACKGROUND: Aortic dissection is a rare and often fatal condition which arises when there is aortic wall weakening. Multiple risk factors could lead to this condition, including hypertension, atherosclerosis, connective tissue diseases such as Marfan's, Ehlers-Danlos and Loeys Dietz syndromes, as well as smoking, cocaine usage, bicuspid aortic valve, pregnancy and trauma. Polycystic kidney disease (PKD) is a rare condition characterized by the development of multiple renal cysts, which is caused by genetic abnormalities leading to autosomal dominant or autosomal recessive inheritance. However,

Cutaneous Neoplasms with Sebaceous Differentiation: A Comprehensive Update on Clinical, Histopathological and Molecular Classification.

Cutaneous sebaceous neoplasms are rare adnexal tumours differentiated towards sebaceous glands, spanning a biological spectrum from benign (sebaceous adenoma, sebaceoma, nevus sebaceus) to outright malignant (sebaceous carcinoma). Most tumours are sporadic and solitary, with a predilection for the head and neck of elderly adults, but a clinically important subset is observed in the setting of Lynch syndrome/Muir-Torre syndrome. Recent large-scale genomic and transcriptomic studies have provided insights into the pathogenesis of these unique neoplasms, revealing a genetically heterogeneous land

The short-term and long-term effect of burn on organ dysfunction: mechanisms and strategies.

Burns constitute a form of severe trauma and may affect multi-organ systems. Secondary multi-organ dysfunction is the primary driver of disability, death, and poor prognosis in patients with severe burns, and has become a pressing global public health challenge. In the acute phase of burns, hypovolemic shock and uncontrolled systemic inflammatory response are the most important lethal mechanisms. In addition to localized skin damage, burns can rapidly induce acute dysfunction in the cardiovascular, immune, endocrine, coagulation, musculoskeletal, digestive, and central nervous systems. The pat

Inducible chronic NAD deficiency in mice reveals multi-systemic phenotypical, metabolic, and transcriptional changes.

Nicotinamide adenine dinucleotide (NAD) is a metabolic cofactor regulating energy metabolism, epigenetics, DNA repair, and signaling. Although NAD declines with vitamin B3 deficiency, disease, and aging, systemic consequences of chronic depletion remain unclear. Here, we characterize an in vivo model of inducible NAD deficiency using acquired niacin dependency (ANDY) mice, in which NAD synthesis becomes dependent on dietary niacin. On a niacin-free diet, ANDY mice developed widespread tissue NAD depletion and sex-independent multisystem abnormalities, including kyphosis, reduced bone, lean, an

[Low-frequency trisomy 21 mosaicism diagnosed following conflicting prenatal testing].

Down syndrome is caused by trisomy 21. Far less common is trisomy 21 mosaicism, in which only a proportion of cells carry the chromosomal abnormality. In this case report, first-trimester combined screening and non-invasive prenatal testing (NIPT) showed an increased risk of trisomy 21. However, chromosomal microarray performed after amniocentesis was normal and confined placental mosaicism was deemed most likely. After birth, the child showed slight stigmata for Down syndrome, and a blood sample confirmed the presence of low-frequency mosaicism at 6 months of age.

Metformin improves mitophagy-related pathways and mitochondrial-lysosomal homeostasis in chromosome 21 trisomic fibroblasts.

Mitochondrial dysfunction is a core cellular abnormality in Down syndrome (DS), yet the mechanisms underlying impaired mitochondrial quality control remain poorly defined. Here, we provide an integrated characterization of the autophagy-mitophagy-lysosome axis in human trisomic fetal fibroblasts (DS-HFFs). We show that trisomic cells display a paradoxical combination of elevated basal autophagy/mitophagy-related markers and defective mitochondrial clearance, indicative of a saturated and inefficient quality control system. DS-HFFs exhibit increased steady-state levels of PINK1, PARKIN, OPTN, N

Muscle stem cells produce a protective Fibrillin-1 matrix to prevent precocious activation.

Diverse biological mechanisms regulate muscle stem cell quiescence, including inhibition of differentiation, adhesion, and translational control. Here, we identify the matrix glycoprotein Fibrillin-1 (FBN1) as a Notch-regulated, cell-autonomous effector, essential for maintaining quiescence. Known for its causal role in Marfan syndrome (MFS), our work positions FBN1 as a critical niche component that protects muscle stem cells from aberrant growth factor signaling. Targeted deletion of Fbn1 in muscle stem cells leads to dose-dependent quiescence defects, characterized by loss of cellular proje

MEK inhibitors in NF1-associated high-grade glioma: partial responses and outcomes from a single-institution cohort and literature review.

PURPOSE: NF1-associated high-grade gliomas (HGGs) are rare, biologically heterogeneous tumors with poor outcomes. Although MEK inhibitors (MEKis) are established options for selected NF1-associated low-grade gliomas, their role in NF1-associated HGG remains undefined. We describe clinical and radiographic outcomes with MEKis, including mirdametinib. METHODS: We retrospectively reviewed adult patients with NF1-associated HGG treated at The University of Texas MD Anderson Cancer Center who initiated selumetinib, mirdametinib, or dabrafenib plus trametinib from December 1, 2019, to January 31, 2

The pathogenic role of high-titer anti-Ro52 antibodies in neonatal lupus erythematosus: insights from a 9 case series.

BACKGROUND: Neonatal Lupus Erythematosus (NLE) is a rare acquired autoimmune disease caused by the transplacental transfer of maternal anti-Ro/SSA and anti-La/SSB antibodies. Despite the high prevalence of these antibodies in mothers with Systemic Lupus Erythematosus (SLE) and Sjögren's Syndrome (SS), the clinical manifestations in neonates are highly heterogeneous, ranging from transient cutaneous lesions to life-threatening congenital heart block (CHB). METHODS: This study analyzed a case series of nine mother-infant pairs diagnosed with NLE. We integrated clinical outcomes, maternal serolo

Patient-derived neomorphic SEPTIN6 mutations induce hematopoietic stem cell senescence underlying congenital neutropenia and myelodysplasia.

Septins are RAS-like GTP-binding proteins involved in cytoskeletal organization and cellular division. We previously described 2 pediatric patients harboring SEPT6 germline mutations (GM1: SEPT6 c.1282T>C and GM2: SEPT6 c.1282T>A) presenting with congenital neutropenia associated with the development of myelodysplastic syndrome. In one patient, an additional somatic mutation (SM: SEPT6 c.43C>T) was detected at a low allelic fraction in the BM. Here, we describe the consequences of these mutations for hematopoiesis. Transgenic expression or gene editing of GM1 or GM2 in human CD34+ cells led to
