Review of Ultrasound Diagnosis of Ichthyosis in Fetuses.

Congenital ichthyoses are inherited disorders in which the outer layer of the skin forms abnormally. Prenatal ultrasound can show a recognizable pattern in some affected fetuses, but most published evidence comes from case reports and small case series. This narrative review summarizes sonographic findings that have been reported, when they became visible, and how they should be interpreted in clinical practice. The strongest imaging literature concerns harlequin ichthyosis. Reported findings include ectropion, eclabium or persistent mouth opening, a flattened nasal contour, fixed flexion of t

Towards Personalised Therapeutics for Epidermal Differentiation Disorders: Integrating Host Genetics, Immuno-Endotypes and the Skin Microbiome.

Epidermal differentiation disorders (EDDs), are a rare, heterogeneous grouping of monogenic disorders. EDDs, comprising the previously termed congenital ichthyosis, are characterised by epidermal barrier dysfunction, cutaneous inflammation, and associated morbidity. Immunomodulatory therapies, particularly repurposed biologics, are an emerging approach for management. However, clinical implementation has been impeded by a variable and unpredictable response rate, reflecting the genetic and immunological heterogeneity of EDD as well as raising questions over other contributory pathogenic mechan

Making Antibiotic Guidance Work in the Emergency Department.

PURPOSE: Atopic dermatitis (AD) is a chronic, relapsing eczematous inflammatory skin disease. Mutations in the filaggrin gene (FLG) are major predisposing factors for AD. Ethnic differences exist between Asian and European populations in the frequency and spectrum of FLG mutations. Moreover, a distinct set of FLG mutations has been reported in Asian populations. The aim of this study was to examine the spectrum of FLG mutations in Koreans with AD. We also investigated the association of FLG mutations and clinical features of AD and compared the Korean FLG landscape with that of other East Asia

Harlequin Ichthyosis in Resource Limited Environments: Practical Strategies to Improve Neonatal Survival.

Harlequin ichthyosis is a rare and life-threatening congenital disorder with high neonatal mortality, particularly in settings lacking specialized neonatal intensive care. We report a preterm newborn managed in a rural hospital without modern incubators, continuous monitoring, systemic retinoids at birth, or specialized personnel. Survival was achieved through pragmatic, low-cost interventions including an improvised humidified incubator, intensive topical emollient therapy, basic respiratory support, umbilical venous access for fluid and electrolyte management, and later initiation of oral is

Exploring Patterns of Palmar Hyperlinearity in Pediatric Population With Atopic Dermatitis or Ichthyosis Vulgaris Attending a Tertiary Care Hospital in Jaipur, India.

INTRODUCTION: Ichthyosis vulgaris (IV) and atopic dermatitis (AD) are skin diseases occurring in children and having important quality-of-life implications. Palmar hyperlinearity has been characterized as a visible sign of skin barrier abnormality in these disorders. METHODS: In this hospital-based cross-sectional study, children aged six months to 18 years with a confirmed diagnosis of AD, IV, or both were assessed for five pre-defined palmar patterns. Patterns were graded as hyperlinear or non-hyperlinear, and their correlation with disease groups was examined. Inter-rater reliability was e

Atopic Dermatitis with Ichthyosis, Recurrent Fungal and Viral Infections: A Case of Concurrent FLG and Multiple Immune-Related Gene Mutations.

Atopic dermatitis (AD) is frequently associated with epidermal barrier dysfunction and may be complicated by recurrent infections. We report a 31-year-old Han Chinese woman with lifelong ichthyosis vulgaris and severe AD, complicated by recurrent vaginal Nakaseomyces glabratus (N. glabrata) infection and herpes simplex virus type 1 (HSV-1) reactivation. Antifungal susceptibility testing demonstrated reduced susceptibility to multiple azole agents. Whole-exome sequencing identified a heterozygous FLG frameshift mutation (c.3321delA) together with heterozygous variants in TNFRSF13B, MPO, and DOC

A mixed-methods online survey approach using retrospective self-reporting to characterise congenital ichthyoses across age groups.

BACKGROUND: The ichthyoses are a group of rare, dermatological conditions characterised by dry, thickened scales across the body and impaired skin barrier function. Although ichthyosis may be acquired, the majority of cases are congenital and caused by specific genetic variants. The ichthyoses can cause multiple skin-related issues (e.g. increased risk of infection) and can also have substantial downstream effects on mental health and quality of life; moreover, they are commonly comorbid with a range of medical conditions. Although the symptoms associated with the various ichthyosis subtypes h

Secukinumab for lamellar ichthyosis in an adolescent: a case report.

BACKGROUND: Lamellar ichthyosis (LI) is a rare hereditary disorder of keratinization, most commonly inherited in an autosomal recessive manner and frequently associated with pathogenic variants in the TGM1 gene. Patients usually present with generalized scaling, xerosis, and pruritus from birth. Current therapeutic options remain limited, and systemic retinoids are often associated with considerable adverse effects. CASE DESCRIPTION: A 14-year-old female individual presented with generalized pruritic, lamellar scale-like skin changes present since birth. The diagnosis of LI was established ba

Identification of Pathogenic Variants in CYP4F22, FLG, ALOX12B, and NIPAL4 in a Case Series of Inherited Ichthyosis.

Inherited ichthyoses are clinically and genetically heterogeneous disorders of cornification caused by disruption of epidermal barrier genes involved in keratinization and lipid homeostasis. Pathogenic variants in more than 50 genes have been implicated in nonsyndromic ichthyosis vulgaris (IV) and autosomal recessive congenital ichthyosis (ARCI). Here, we investigated the genetic basis of ichthyosis in four consanguineous Pakistani families presenting with IV or ARCI phenotypes. Exome sequencing followed by segregation analysis identified pathogenic variants in four established ichthyosis-asso

Novel Compound Heterozygous Variants in TGM1 and CYP4F22 in Two Newborns with Non-Syndromic Epidermal Differentiation Disorders (TGM1-nEDD and CYP4F22-nEDD).

Background: Congenital ichthyoses are a clinically and genetically heterogeneous group of Mendelian disorders of cornification. According to the 2025 classification, non-syndromic epidermal differentiation disorders (nEDDs) are categorized based on the biological function of the causative gene products. Variants in TGM1 cause a transglutaminase-related nEDD (TGM1-nEDD), typically corresponding to the traditional phenotype of lamellar ichthyosis (LI), whereas biallelic variants in CYP4F22 cause a lipid-metabolism-related nEDD (CYP4F22-nEDD), formerly classified as ARCI type 5. The latter form i

Tape strips capture immune and epidermal hyperplasia markers in the major orphan ichthyoses.

Prior studies in ichthyosis have demonstrated cutaneous and/or systemic immune abnormalities with barrier defects; however, the transcriptomes of the major orphan forms of ichthyosis have yet to be characterized through tape stripping, a minimally invasive sampling method validated in other inflammatory skin diseases. Skin tape strips from 27 patients with ichthyosis (9 with Netherton syndrome, 6 with congenital ichthyosiform erythroderma, 7 with lamellar ichthyosis, and 5 with epidermolytic ichthyosis) and 18 demographically matched healthy controls were analyzed with RNA sequencing. Congenital ichthyosis is associated with increased risk of allergic disorders.

BACKGROUND: The congenital ichthyoses are rare hereditary epidermal differentiation disorders characterized by severely impaired skin barrier function. Patients with atopic dermatitis and one form of congenital ichthyosis, Netherton syndrome, are known to be at increased risk of atopy, presumably linked to barrier impairment. However, an increased risk of atopy has not been assessed in individuals with other congenital ichthyoses despite their severe barrier impairment. OBJECTIVE: Our aim was to explore rates of atopy in patients with congenital ichthyoses versus in a control population. MET

CYP4F22-Related Autosomal Recessive Congenital Ichthyosis Associated With Hirschsprung Disease and Bartter-Like Renal Manifestations.

Autosomal recessive congenital ichthyosis (ARCI) is a heterogeneous group of inherited cornification disorders caused by defects in epidermal barrier formation. Mutations in CYP4F22 are an uncommon cause of ARCI and are associated with variable phenotypes including lamellar ichthyosis (LI) and congenital ichthyosiform erythroderma (CIE). This is a report of a genetically confirmed case of CYP4F22-related ARCI in a child born to consanguineous parents who presented with collodion membrane at birth followed by persistent ichthyosiform scaling and palmoplantar keratoderma. The First Molecular Profiling of JAK2 Exon 12 and Flanking Intronic Sequences in Indonesia.

OBJECTIVE: Data on JAK2 Exon 12 mutations in Indonesia are not available. This study aimed to explore the presence of known JAK2 exon 12 coding mutations in V617F-negative myeloproliferative neoplasm (MPN) patients in Indonesia and to describe any novel intronic changes downstream of exon 12. METHODS: A total of 198 V617F-negative MPN samples were screened using allele-specific polymerase chain reaction (AS-PCR) for four commonly reported canonical exon 12 mutations. Of these, 57 cases were selected at random or for atypical PCR band patterns and underwent bidirectional Sanger sequencing of a

Bi-allelic RNU6ATAC variants cause a minor spliceopathy characterized by transcriptome-wide minor intron retention and multisystem manifestations.

We report three individuals with bi-allelic variants in RNU6ATAC, which encodes the U6atac minor spliceosomal small nuclear RNA (snRNA), causing a multisystem minor spliceopathy. Through RNA sequencing analysis, we identified a distinctive excess of minor intron retention (MIR) in two unrelated individuals, which guided the identification of bi-allelic RNU6ATAC variants. The discovery cohort presented with variable multisystem manifestations. One individual presented with refractory epilepsy, microcephaly, developmental delay, ataxia, bilateral toe syndactyly, hypereosinophilia, and short stat
