Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Epidermolysis bullosa dystrophica in a goat kid.

In a dairy herd of Thuringian Forest goats, a male goat kid of around three weeks of age was detected with skin lesions, particularly on the distal limbs, chest and ears. These skin lesions were characterized by alopecia, pus-filled blisters, incrustations and erosions. The goat kid was referred to the clinic for treatment but had to be euthanized despite intensive treatment due to increasing deterioration in its general condition and was subsequently referred to necropsy. Histologic investigation of the altered areas of skin and mucosa revealed a separation of epithelium from the underlying d

ProNA3D: Distance-Based Analysis of Nucleic Acid-Containing Interfaces.

Biomolecular interactions are central to many essential cellular processes, but RNA-containing complexes remain challenging to resolve structurally, even as experimental methods and AI-based prediction have expanded structural coverage. Tools for the integrated analysis of interfaces within such complexes remain limited. We present ProNA3D, a tool that provides a unified platform for analyzing protein-nucleic acid and nucleic acid-only complexes, bridging the gap between structure prediction and functional interpretation. ProNA3D supports both experimental and computationally predicted structu

Topical gene therapy for severe recessive dystrophic epidermolysis bullosa: clinical outcomes and multidisciplinary management in three patients treated with beremagene geperpavec (B-VEC) in Italy - a case series.

Recessive dystrophic epidermolysis bullosa is a severe inherited blistering disorder characterized by chronic wounds, fibrosis and systemic complications. Beremagene geperpavec (B-VEC), a topical HSV1-based gene therapy delivering full-length COL7A1, has shown efficacy in clinical trials; however, real-world data remain limited. This retrospective, descriptive case series reports on three patients with genetically confirmed recessive dystrophic epidermolysis bullosa - two children and one young adult - treated between 2024 and 2026 at two Italian tertiary centres. All patients had extensive ch

Zinc as a Biomarker of Nutritional Status and Clinical Burden in Recessive Dystrophic Epidermolysis Bullosa: Implications for Preventive Monitoring.

Background/Objectives: Recessive dystrophic epidermolysis bullosa (RDEB) is a severe congenital genodermatosis characterized by skin and mucosa fragility, chronic inflammation, recurrent infections and high nutritional demands due to increased metabolism and epithelial barrier-related losses, placing patients at risk of zinc deficiency. We aimed to investigate the clinical relevance and biochemical determinants of zinc deficiency as a potentially modifiable contributor to disease burden in RDEB. Methods: In this cross-sectional study (n = 84), serum zinc levels were analyzed in association wit

Amelioration of Dominant Dystrophic Epidermolysis Bullosa Ulceration by Combination Gentian Violet and Trichloroacetic Acid Therapy.

INTRODUCTION: Dominant dystrophic epidermolysis bullosa (DDEB) is a hereditary genetic disorder with a mutation of the type VII collagen gene (COL7A1), leading to a destabilized dermal-epidermal junction. Current treatments for DDEB are supportive, and new gene therapies are being developed to target DDEB. However, gene therapy can be expensive. CASE REPORT: A 59-year-old woman presented with eroded blisters on her right lower extremity. Genetic testing identified a pathogenic COL7A1 mutation, confirming the diagnosis of DDEB. She was treated with 6 weeks of gentian violet and trichloroacetic

An evaluation of prademagene zamikeracel for the treatment of recessive dystrophic epidermolysis bullosa.

INTRODUCTION: Prademagene zamikeracel is an autologous, genetically corrected epidermal graft therapy designed to address the underlying molecular defect in recessive dystrophic epidermolysis bullosa (RDEB). The product is manufactured from a patient's own keratinocytes, which are modified ex vivo to overexpress a normal copy of COL7A1, the gene coding for type VII collagen (C7), which is essential for anchoring fibril formation and stable dermal-epidermal cohesion. In individuals with biallelic COL7A1 variants, loss of C7 results in severe skin fragility, painful and widespread wounds, debili

Observations of dystrophic epidermolysis bullosa patients with collagen VII NC2 truncation provide new insights into anchoring fibril assembly.

Type VII collagen (C7) assembles into anchoring fibrils (AF) which are basement membrane zone (BMZ) structures that reinforce dermal-epidermal cohesion. Mutations in COL7A1, which encodes C7, cause dystrophic epidermolysis bullosa (DEB), a blistering disorder characterized by skin fragility. C7 consists of a central collagenous domain flanked by non-collagenous NC1 and NC2 regions. During AF assembly, C-proteinase-mediated cleavage of the NC2 domain enables anti-parallel dimer formation, followed by C7 lateral association into mature, banded AFs that loop through the dermis to anchor the epide

MesenSistem-EB: systemic haploidentical mesenchymal stem cell therapy in recessive dystrophic epidermolysis bullosa associated with clinical benefits and correlated with MCP1 and sCD40L dynamics.

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is a devastating genodermatosis caused by biallelic COL7A1 mutations, where chronic inflammation drives disabling complications affecting quality of life and survival. Effective inflammatory control remains a major unmet need. While systemic administration of mesenchymal stromal cells (MSC) from various sources has proven safe with transient benefits yet only one recent placebo-controlled study in RDEB. Consistent type VII collagen (C7) deposition is lacking, and therapeutic mechanisms and predictive biomarkers remain undefined. The

Challenges of Ontology-Based Concept Normalization for Deep Phenotyping in Rare Skin Diseases.

Extracting and standardizing phenotypic information from free-text medical reports remains a main challenge in biomedical natural language processing (NLP). Conceptual normalization, which maps textual mentions to standardized vocabularies such as the UMLS or HPO, is a key step for deep phenotyping. Dystrophic epidermolysis bullosa (DEB) is a rare severe and heterogeneous skin disorder causing skin and mucosal fragility that leads to early morbidity and mortality. To better understand the disease's natural history and severity spectrum, it is important to comprehensively identify DEB-related p

Identification of a long-term surviving human mesenchymal stromal cell subpopulation and implications for recessive dystrophic epidermolysis bullosa treatment.

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is a severe skin disease caused by loss-of-function pathogenic variants in COL7A1 encoding type VII collagen (C7). Patients with RDEB suffer since birth from skin and mucosal blistering and develop severe local and systemic complications resulting in poor prognosis. Mesenchymal stromal cells (MSCs) have demonstrated their potential to enhance wound healing and reduce skin inflammation in RDEB patients due to their anti-inflammatory properties and capacity to express C7. We aim to optimize in vitro conditioning of human bone marrow-d

Revisiting Type VII Collagen Mislocalisation in Dystrophic Epidermolysis Bullosa Using High-Resolution Imaging.

Type VII collagen (COL7), a major component of anchoring fibrils, is essential for dermal-epidermal adhesion and pathogenic variants in COL7A1, encoding COL7, cause dystrophic epidermolysis bullosa (DEB). In normal skin, COL7 is localised just beneath the lamina densa. In contrast, previous immunoelectron microscopy (IEM) studies of DEB skin with residual COL7 expression demonstrated aberrant localisation of COL7 above the lamina densa and hemidesmosomes of epidermal basal keratinocytes, possibly reflecting impaired secretion of defective COL7. However, conventional microscopy techniques lack

Eliciting health state utilities for open wounds in dystrophic epidermolysis bullosa.

PURPOSE: Qualitative accounts and studies employing disease-specific outcome measures have demonstrated that people with dystrophic epidermolysis bullosa (DEB) display impaired health-related quality of life (HRQoL) due to wound-related challenges. There is a paucity of utility data necessary to inform the health economic evaluation of emerging treatments for cutaneous open wounds. METHODS: The objective was to implement a vignette study with a representative sample of the United Kingdom (UK) general population (N = 1139) and people with DEB (N = 44) to estimate utility values. Gene Therapy for Dystrophic Epidermolysis Bullosa.

Dystrophic epidermolysis bullosa (DEB) is a rare, debilitating genodermatosis caused by loss-of-function variants in COL7A1, resulting in type VII collagen (C7) deficiency and defective anchoring fibrils, which are essential for dermal-epidermal adhesion. COL7A1 is a large gene, and C7 is synthesized by both keratinocytes and fibroblasts, and extracellularly secreted, where it forms a supramolecular network that stabilizes the dermal-epidermal junction. Human genotype-phenotype correlations suggest that residual C7 expression (~10% of normal levels) may confer a milder disease phenotype, where
