Family burden and caregiver psychological stress in epidermolysis bullosa in China: a cross-sectional survey.

BACKGROUND: Epidermolysis Bullosa (EB) is a rare genetic skin disorder requiring long-term intensive care. The disease places considerable economic and caregiving burdens on families, yet factors associated with caregivers' psychological stress remain inadequately understood. METHODS: A cross-sectional questionnaire survey was conducted among families of patients with EB. Statistical analysis was performed at the household level. In families with multiple affected individuals, the youngest patient was selected as the representative case, as Epidermolysis Bullosa is a rare inherited disorder t

Mesenchymal stromal cell infusions of umbilical cord-derived mesenchymal stromal cells in children with Recessive Dystrophic Epidermolysis Bullosa (MissionEB): a qualitative sub study of a randomised, double-blind, placebo controlled, crossover, phase 3 trial.

BACKGROUND: Recessive Dystrophic Epidermolysis Bullosa (RDEB) is a rare genetic skin condition causing fragile skin, blistering, and scarring. It leads to chronic pain, slow wound healing, and severe limitations, profoundly impacting patient and family quality of life. Umbilical cord tissue-derived mesenchymal stromal cells (UC-MSC) have shown therapeutic promise. The Mission EB trial (ISRCTN14409785; registration date 25/03/2021) assessed UC-MSC safety and effectiveness in children with RDEB in a placebo-controlled, double-blinded, crossover study. METHODS: This study used a qualitative rese

Population-Based and Routinely Collected Health Data Studies of Immunobullous Diseases: A Scoping Review.

BACKGROUND: Validated case definitions are essential for reliable epidemiological research using routinely collected health data. The extent to which population-based studies on autoimmune blistering diseases (AIBDs) use validated case definitions is uncertain, and variability in definitions may limit the generalizability of findings. OBJECTIVE: To describe definitions of AIBDs used in population-based epidemiologic studies and whether these definitions have been validated. METHODS: MEDLINE and Embase were searched for epidemiologic studies on pemphigoid, pemphigus and their variants, dermat

So Much More Than Just Structural Support: Laminin-332 and Its Multifaceted Functions in Skin Homeostasis and Disease.

The extracellular matrix (ECM) is increasingly recognised as a dynamic regulator of tissue function beyond its traditional structural role. In the skin, the ECM supports tissue integrity while also controlling processes such as stem cell maintenance, wound healing and disease progression. This review focuses on laminin-332, a key component of the epidermal-dermal junction. Its essential role in maintaining epidermal cohesion is illustrated by junctional epidermolysis bullosa, a severe inherited blistering disorder and recent advances in laminin-332-targeted genetic therapies are discussed. Identification of a long-term surviving human mesenchymal stromal cell subpopulation and implications for recessive dystrophic epidermolysis bullosa treatment.

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is a severe skin disease caused by loss-of-function pathogenic variants in COL7A1 encoding type VII collagen (C7). Patients with RDEB suffer since birth from skin and mucosal blistering and develop severe local and systemic complications resulting in poor prognosis. Mesenchymal stromal cells (MSCs) have demonstrated their potential to enhance wound healing and reduce skin inflammation in RDEB patients due to their anti-inflammatory properties and capacity to express C7. We aim to optimize in vitro conditioning of human bone marrow-d

A Cross-sectional Study on Quality of Life in EB: Validation of the Italian QOLEB and Assessment in Italian Patients.

Inherited epidermolysis bullosa comprises a heterogeneous group of skin fragility diseases, presenting with a range of manifestations and complications that profoundly affect quality of life (QoL). A disease-specific instrument, Quality of Life in EB (QOLEB), has been developed to assess the impact of epidermolysis bullosa signs and symptoms on QoL. The aim of this cross sectional study, conducted as part of a European project - BUR-EB, was to test the psychometric properties of the Italian version of the QOLEB and to assess QoL in Italian epidermolysis bullosa patients. Demographic, clinical,

HLA genotype, clinical and immunofluorescence findings of 30 patients with type VII collagen antibody positive subepidermal bullous disease.

Epidermolysis bullosa acquisita (EBA) and mucous membrane pemphigoid are mucocutaneous blistering disorders that are associated with antibodies that target type VII collagen (anti-col7). There are relatively few studies that closely investigate the clinical phenotype and HLA genotype of patients who are anti-col7 positive. We therefore characterized all patients with anti-col7-positive subepidermal bullous disease that were treated at our centre. In total, 30 patients were identified, 29 had mechanobullous skin lesions and all had oral ulceration. The most common sites for oral lesions were th

Whole-Exome Sequencing Identified a Novel Mutation in an Iranian Patient with Epidermolysis Bullosa.

BACKGROUND: Epidermolysis bullosa (EB) is a rare, genetically heterogeneous disorder characterized by skin fragility. EB is categorized into four types: simplex, junctional, dystrophic, and Kindler syndromes. The condition is caused by mutations in several genes that are important for skin integrity and dermal-epidermal adhesion. In the present study, we recruited a patient with EB from an Iranian pedigree for genetic evaluation. METHODS: Whole-exome sequencing (WES) and bioinformatics analysis were performed using genomic DNA from the patient with EB. The potential variant was confirmed by S

Dermal papillary fibroblasts promote persistent granulation tissue formation in junctional epidermolysis bullosa.

The skin is composed of multiple fibroblast subpopulations with different functions in homeostasis and repair, but their role in skin diseases is largely unknown. Junctional epidermolysis bullosa (JEB) is a hereditary skin disorder characterised by severe skin fragility and aberrant granulation tissue formation, caused by loss-of-function variants in basement membrane proteins, including laminin-332. We developed JEB-like organotypic (OT) cultures with distinct fibroblast subpopulations and explored their role in an inducible JEB in vivo disease model, mimicking key features of the human disea

Revisiting Type VII Collagen Mislocalisation in Dystrophic Epidermolysis Bullosa Using High-Resolution Imaging.

Type VII collagen (COL7), a major component of anchoring fibrils, is essential for dermal-epidermal adhesion and pathogenic variants in COL7A1, encoding COL7, cause dystrophic epidermolysis bullosa (DEB). In normal skin, COL7 is localised just beneath the lamina densa. In contrast, previous immunoelectron microscopy (IEM) studies of DEB skin with residual COL7 expression demonstrated aberrant localisation of COL7 above the lamina densa and hemidesmosomes of epidermal basal keratinocytes, possibly reflecting impaired secretion of defective COL7. However, conventional microscopy techniques lack

Expert consensus on oral management in autoimmune bullous diseases, erythema multiforme and SJS/TEN.

BACKGROUND: Autoimmune bullous diseases (AIBD), Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS-TEN) and erythema multiforme (EM) often present with clinically similar features in the oral mucosa: classically widespread mucosal erosions or ulceration. Despite this, there are no formalized guidelines for the specific management of the oral manifestations of these conditions. OBJECTIVES: We sought to establish an consensus on the management of oral involvement of AIBD, SJS-TEN and EM using the Delphi method. METHODS: Participants were sent a survey comprising 62 statements organiz

Eliciting health state utilities for open wounds in dystrophic epidermolysis bullosa.

PURPOSE: Qualitative accounts and studies employing disease-specific outcome measures have demonstrated that people with dystrophic epidermolysis bullosa (DEB) display impaired health-related quality of life (HRQoL) due to wound-related challenges. There is a paucity of utility data necessary to inform the health economic evaluation of emerging treatments for cutaneous open wounds. METHODS: The objective was to implement a vignette study with a representative sample of the United Kingdom (UK) general population (N = 1139) and people with DEB (N = 44) to estimate utility values. Vignettes were

HCA2 Receptors in Lymphocytes and Keratinocytes Affect Murine Contact Allergic Inflammation.

The hydroxycarboxylic acid receptor HCA2 is expressed in keratinocytes and immune cells. In mice, the anti-inflammatory potential of HCA2 receptor signalling in the skin was first described in experimental models of psoriasis-like inflammation and bullous pemphigoid-like epidermolysis bullosa acquisita. We examined contact allergic immune responses to the obligate contact sensitiser DNFB in Hcar2-/- and wild-type C57BL/6 mice. Basal mRNA levels of pro-inflammatory mediators like IFNγ were already increased in the ear tissue of naïve Hcar2-/- animals. After sensitisation and challenge with DNFB

Bullous Diseases in the Perioperative Setting: Anesthetic Considerations.

Bullous skin disorders, including epidermolysis bullosa, pemphigus, and bullous pemphigoid, pose distinctive anesthetic challenges due to extreme cutaneous fragility, mucosal involvement, fluid imbalances, an increased risk of infection, and multisystem comorbidities. Although these patients frequently require surgical or procedural care, evidence guiding perioperative management is limited to isolated case reports and small series. This systematic review aimed to synthesize contemporary data on anesthetic and perioperative strategies for patients with autoimmune and inherited bullous diseases

CD19×CD3 bispecific T cell engager treatment induces remission in experimental pemphigoid disease.

Pemphigoid diseases (PDs) are autoimmune disorders marked by autoantibodies (Aabs) against skin and mucous membrane proteins, causing mucocutaneous blistering in predominantly elderly patients. Since current therapeutics are often insufficient, PDs impose a significant morbidity and mortality burden. CD19×CD3 bispecific T cell engagers (TCEs)-originally developed for B cell malignancies-have shown promise in treatment-refractory autoimmune diseases such as systemic sclerosis and rheumatoid arthritis. To explore their potential in PDs, we tested a murine CD19×CD3 TCE in a mouse model of epiderm

Diagnostic challenge: Epidermolysis bullosa acquisita of the palms and soles.

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disorder that can be difficult to diagnose, especially when its presentation mimics other blistering conditions. We present the case of a 56-year-old male who developed widespread blistering on flexural surfaces, palms, soles, and target lesions, with clinical features resembling bullous pemphigoid (BP), erythema multiforme (EM), and infectious aetiologies. A skin biopsy showed subepidermal bullae with linear IgG and C3 deposition at the basement membrane, with additional focal IgM and IgA positivity, and ELISA testing confi

Gene Therapy for Dystrophic Epidermolysis Bullosa.

Dystrophic epidermolysis bullosa (DEB) is a rare, debilitating genodermatosis caused by loss-of-function variants in COL7A1, resulting in type VII collagen (C7) deficiency and defective anchoring fibrils, which are essential for dermal-epidermal adhesion. COL7A1 is a large gene, and C7 is synthesized by both keratinocytes and fibroblasts, and extracellularly secreted, where it forms a supramolecular network that stabilizes the dermal-epidermal junction. Human genotype-phenotype correlations suggest that residual C7 expression (~10% of normal levels) may confer a milder disease phenotype, where
