The Changing Trends in the Incidence and Clinical Presentation of Immunobullous Disorders Before and During the COVID-19 Era in a Tertiary Healthcare Centre in Andhra Pradesh.

Background Autoimmune immunobullous disorders (AIBDs) represent a heterogeneous group of chronic blistering dermatoses mediated by autoantibodies against desmosomal and hemidesmosomal adhesion molecules. The COVID-19 pandemic has generated significant interest in the interaction between SARS-CoV-2 infection, vaccination-related immune modulation, and the clinical behaviour of autoimmune diseases. Objective This study's primary objective is to compare hospital-based temporal trends in the frequency, subtype distribution, and clinical presentation of AIBDs before and during the COVID-19 era in a

SINE Insertion in LAMA3 in Dogs With Junctional Epidermolysis Bullosa.

Junctional epidermolysis bullosa (JEB) is a hereditary skin disorder caused by defects in proteins responsible for dermal-epidermal adhesion. We investigated the genetic cause of JEB in three related mixed-breed puppies presenting with congenital skin blistering and ulceration. Whole-genome sequencing of one affected dog followed by comparison with 1538 control genomes identified a private candidate variant, XM_038543644.1:c.5690_5691ins240, in LAMA3, a known JEB-associated gene. Visual inspection of the short-read alignments and Sanger sequencing revealed a homozygous 240-bp SINE insertion in

Updates in the Treatment of Pain and Pruritus in Epidermolysis Bullosa.

Pruritus and pain are pervasive, often debilitating symptoms in epidermolysis bullosa (EB), arising from complex immune, neural, and barrier dysfunction. Pruritus, especially in dystrophic and junctional EB subtypes, results from Th2/Th17-mediated inflammation, neuropeptide signaling, and neuropathic changes. Management includes antihistamines, topical agents, gabapentinoids, and emerging systemic therapies, such as dupilumab, JAK inhibitors, and neurokinin-1 inhibitors. Pain in EB is multifactorial, combining nociceptive and neuropathic mechanisms due to chronic wound, fibrosis, and nerve sen

Severe Junctional Epidermolysis Bullosa: Honoring Parental Values in Pediatric Palliative Decision-Making.

Severe junctional epidermolysis bullosa (JEB) is a rare, incurable, autosomal recessive blistering disorder with a uniformly poor prognosis. We present the case of a female infant diagnosed shortly after birth with genetically confirmed severe JEB, whose family prioritized comfort-focused care and memory-making over life-prolonging interventions. Her interdisciplinary care centered on symptom relief, bonding, and alignment with family values. This case underscores the importance of early diagnosis and values-based decision-making in managing rare, life-limiting conditions like severe JEB.

Leveraging the Electronic Health Record to Assist in the Management of Patients With Complex Dermatological Conditions: An OurPractice Advisory Pilot for Patients With Epidermolysis Bullosa.

Epidermolysis bullosa (EB) is a group of rare, fragile skin disorders that poses unique challenges. To support clinicians who lack familiarity with caring for patients with EB, we utilized a tool called an "OurPractice Advisory" (OPA) within Epic, our institution's electronic health record (EHR) platform. The OPA displays crucial resources for providers to assist with the management of patients with EB during an emergency department visit and/or hospitalization. The OPA includes practical guidance regarding wound care, supply procurement, consult recommendations, and complication prevention st

Epidermolysis Bullosa Simplex-Severe Caused by KRT5 p.Glu477Lys: Challenges Encountered in This High-Risk Subtype.

Epidermolysis bullosa simplex-severe (EBS-severe) caused by KRT5 p.Glu477Lys is a rare and particularly severe subtype associated with high neonatal morbidity and mortality. We report an infant who during the neonatal period required prolonged multidisciplinary intensive care for the management of several complications, including extensive wounds with blood loss leading to secondary anemia, failure to thrive with gastroesophageal reflux, respiratory distress with stridor and a necrotizing soft tissue infection, ultimately requiring surgical debridement and grafting. Pain control proved inadequ

Epidermolysis Bullosa Classification and Current Approach to Diagnosis.

Epidermolysis bullosa (EB) is a heterogeneous group of rare genodermatoses marked by skin fragility and bullae formation induced by minor trauma. Pathologic variants in at least 21 genes are associated with EB, grouped into four major subtypes based predominantly on the plane of cleavage within the skin. EB simplex is characterized by epidermal bullae formation and is due to gene mutations that affect epidermal proteins, most commonly keratin filaments. Junctional EB is due to gene mutations affecting proteins in the basement membrane zone, causing a split within the lamina lucida of the derma

Update on Approved Therapies for Dystrophic and Junctional Epidermolysis Bullosa.

Dystrophic epidermolysis bullosa (DEB) and junctional EB (JEB) are severe, bullous genodermatoses induced by mutations of genes encoding structural skin proteins that disrupt epidermal adhesion. Until recently, treatment was limited to symptomatic care. Since 2022, three therapies - birch triterpenes gel (Filsuvez), beremagene geperpavec-svdt (B-VEC, Vyjuvek), and prademagene zamikeracel (pz-cel, Zevaskyn) - have received regulatory approval, representing the first specific interventions for epidermolysis bullosa. Herein, we provide an overview on their mechanisms of action, efficacy, safety,

AI-Based Segmentation of Wound Types Caused by Epidermolysis Bullosa.

Epidermolysis bullosa (EB) defines a group of rare, inherited and currently incurable genetic disorders characterized by excessive skin fragility, with blistering and wounding of skin and mucous membranes upon minor mechanical trauma. Accurate wound assessment is imperative for measuring and longitudinal monitoring of disease activity and for determining the most accurate treatment. In this work, we describe the annotation of clinical images from EB patients and the subsequent training of both classical convolutional neural networks and state-of-the-art transformer-based architectures for the

Supraglottic and Glottic Involvement in Epidermolysis Bullosa Simplex: A Pediatric Case Report and Review of Airway Management.

Airway involvement in epidermolysis bullosa (EB) is rare and most commonly associated with junctional EB. Laryngotracheal disease in epidermolysis bullosa simplex (EBS), particularly PLEC (Plectin gene)-related variants, is uncommon but may cause significant morbidity. We report a 7-year-old girl with autosomal recessive PLEC-related EBS who developed progressive supraglottic and glottic stenosis. She presented at 8 months old with severe respiratory distress requiring emergent tracheostomy. Serial endoscopies revealed persistent supraglottic granulation, an anterior glottic web, posterior glo

From the Microscope to the Genome: A New Era in the Molecular Genetics of Epidermolysis Bullosa.

Epidermolysis bullosa (EB) is a heterogeneous group of inherited disorders characterised by skin fragility, caused by pathogenic variants in genes encoding structural components of the dermo-epidermal junction. With the advent of next-generation sequencing (NGS), the diagnostic paradigm has shifted from a morphological to a genotype-oriented approach. This review summarises the genetic architecture of EB, the types of mutations and genotype-phenotype relationships, the challenges in interpreting variants of unknown significance (VUS), and therapeutic strategies targeting specific mutational me

Trophoblast Extracellular Vesicles Rewire Immunity: A Symptom-Relief Avenue for Recessive Dystrophic Epidermolysis Bullosa.

Redirecting immune responses toward tissue repair represents a promising strategy for treating inflammatory and degenerative diseases. Extracellular vesicles (EVs), derived from allogeneic stem cells, have shown potential for modulating proinflammatory immunity. Among these, placental-derived EVs (PEVs) are emerging as candidates for immune-mediated inflammatory disorders due to their role in establishing maternal immune tolerance. However, their applicability beyond pregnancy remains limited by key translational barriers, including the lack of a standardized and scalable source system and nee

Japanese intractable and rare dermatologic diseases: From the official skin-centered nanbyo core to an illustrative mechanism-based framework.

Japan's designated intractable disease (nanbyo) framework combines a population threshold with unresolved pathogenesis or treatment, long-term care needs, objective diagnostic criteria, and severity-linked assistance with medical expenses. We conducted a targeted narrative review of official Japanese sources and biomedical literature through August 3, 2026. Evidence ranged from regulatory approvals and randomized trials to observational, case-report, and preclinical data, so therapeutic maturity was assessed separately from mechanistic plausibility. We distinguished the 12 official skin/connec

Anti-p200 pemphigoid.

Anti-p200 pemphigoid is an autoimmune bullous disorder, first described in 1996. Clinically, it presents with tense blisters, erosions, crusts and erythematous macules/plaques, in addition to mucosal involvement in about 40% of cases, resembling bullous pemphigoid or the inflammatory variant of epidermolysis bullosa acquisita. As in other pemphigoid diseases, autoreactive IgG and/or C3 bind along the cutaneous basement membrane zone. Serum autoantibodies recognize a 200 kDa protein of the dermoepidermal junction that label the dermal side of human salt-split skin by indirect immunofluorescence

Part I. The role of Staphylococcus aureus in the pathophysiology of dermatologic disease.

In this first part of a two-part continuing medical education series, we examine the impact of Staphylococcus aureus (S. aureus) resistance and its pathogenic mechanisms, such as toxins, virulence factors, superantigens, quorum sensing, and biofilm formation. Next, we explore how S. aureus infection contributes to delayed wound healing, impetigo, and staphylococcal scalded skin syndrome. Additionally, S. aureus plays a crucial role in the pathophysiology of dermatologic diseases that goes beyond infection, including atopic dermatitis, cutaneous T-cell lymphoma, epidermolysis bullosa, and other

When the Skin Tells Another Story: Incidental Epidermolysis Bullosa in an Adult Hospitalized for Chronic Respiratory Symptoms.

Epidermolysis bullosa (EB) comprises a heterogeneous group of inherited blistering disorders characterized by skin fragility and trauma-induced blister formation. Although EB is typically recognized in childhood, milder phenotypes may remain underdiagnosed in adults and may be identified incidentally during evaluation for unrelated medical conditions. A 47-year-old man was admitted for chronic respiratory symptoms and a history of prior granulomatous infections. During a routine physical examination, extensive chronic blistering skin lesions were identified incidentally. Inpatient evaluation r
