Helicase-deficient TFIIH causes severe disease features via persistent DNA excision without damage removal.

Nucleotide excision repair (NER) removes helix-distorting DNA lesions through the ten subunit TFIIH complex, whose XPB and XPD translocase/helicase activities unwind DNA to enable damage verification and subsequent endonucleolytic DNA incisions. While most XPD mutations cause xeroderma pigmentosum, specific helicase-deficient mutations cause severe Cockayne syndrome (CS) features, including progressive neurodegeneration, for which the basis remains unclear. Here we show that loss of XPD helicase activity traps TFIIH in a futile repair cycle in which DNA is incised at the wrong position, leadin

Two Atypical Cases of Adams-Oliver Syndrome With DOCK6 Genetic Variants.

Adams-Oliver syndrome (AOS) is characterized by scalp aplasia cutis congenita (ACC) and terminal transverse limb defects (TTLD). Autosomal recessive AOS type 2 caused by DOCK6 variants is usually accompanied by central-nervous-system and ocular anomalies. We report two unrelated children who each carried compound heterozygous DOCK6 variants. Both presented with variable degrees of global developmental delay and visual impairment, but neither showed ACC or TTLD. Our observations highlight the phenotypic variability in DOCK6-related AOS and a possible ascertainment bias in screening patients onl

FBXL21 regulates diurnal proteostasis in skeletal muscle by targeting DNAJB6 and client proteins.

Circadian regulation of proteostasis, a key determinant of muscle health, remains poorly understood. Here, we identify DNAJB6, an Hsp40 (DnaJ) co-chaperone, as a substrate of the circadian E3 ligase FBXL21. FBXL21 mediates the ubiquitination-dependent proteasomal degradation of both DNAJB6 and its client proteins, including Desmin. In contrast, myopathy-causing mutations of DNAJB6 confer resistance to FBXL21-directed degradation. Fbxl21 KO C2C12 cells display aberrant Desmin accumulation, and show aggravated cytoplasmic accumulation of TDP-43, another DNAJB6 client protein, in response to heat

Cutaneous ultrastructure reveals potential biomarkers of ventricular arrhythmias in inflammatory cardiomyopathy.

Inflammatory cardiomyopathy (ICM), resulting from persistent myocardial inflammation, is associated with life-threatening ventricular arrhythmias (VA). Recent studies suggested overlap between ICM-associated VA and desmosomal diseases affecting the intercalated discs (ID), which are critical for electromechanical coupling. Genetic variants in desmosomal genes cause arrhythmogenic cardiomyopathies and genodermatoses, supporting a potential link between desmosomal dysfunction and arrhythmic risk. This single-center pilot study explored cutaneous abnormalities in patients with ICM complicated by

Stealth Pigmented Squamous Cell Carcinoma in Situ: An Undescribed Variant With Deceptive Clinical and Histological Features.

Cutaneous squamous cell carcinoma in situ is characterized by well-established architectural and cytological abnormalities that are readily appreciated at both low and high magnification. We have encountered a subset of lesions with a deceptively bland appearance at scanning magnification that demonstrated unequivocal malignant features on closer examination, which we term "stealth pigmented squamous cell carcinoma in situ." In this study, we describe 51 cases of this entity and compare its clinical, histological, and immunohistochemical features with those of its closest morphologic mimickers

Disseminated Nocardia otitidiscaviarum Presenting as Lung-Brain-Skin Syndrome in a Renal Transplant Recipient: A Case Report.

We present a case of disseminated Nocardia otitidiscaviarum infection in a 72-year-old male with a history of renal transplantation on chronic immunosuppressive therapy with prednisone, cyclosporine, and mycophenolate mofetil. He presented with pneumonia and was initially treated with ceftriaxone and azithromycin. Bronchoalveolar lavage (BAL) and cultures from a biopsied thigh lesion both grew N. otitidiscaviarum. His hospital course was complicated by visual hallucinations, and brain magnetic resonance imaging (MRI) revealed multiple intracranial abscesses. He initially received meropenem, tr

Rezpegaldesleukin and Regulatory T Cell Restoration in Immune-Mediated Skin Diseases.

Inflammatory skin diseases such as atopic dermatitis (AD), psoriasis, and alopecia areata (AA) are characterized by immune dysregulation involving impaired regulatory T cell (Treg) function and persistent activation of effector inflammatory pathways. Current targeted therapies largely suppress downstream inflammatory signaling rather than correcting upstream regulatory abnormalities. Rezpegaldesleukin (REZPEG; NKTR-358) is a first-in-class, Treg-selective interleukin-2 (IL-2) receptor agonist engineered to preferentially expand cluster of differentiation 4 positive (CD4+) FOXP3+ Tregs while mi

A novel stop-loss variant in ISG15 causes C-terminal dysfunction and impairs USP18-mediated negative feedback.

INTRODUCTION: ISG15 is a type I interferon (IFN-I)-inducible ubiquitin-like protein that restrains IFN-I signaling. Classical ISG15 deficiency is thought to cause disease mainly by disrupting USP18 stabilization, which leads to sustained IFN-I signaling and interferon-driven autoinflammation. Whether patient-derived C-terminal ISG15 variants can impair IFN-I negative feedback through altered C-terminal functional presentation has remained unclear. METHODS: We studied a Chinese boy with early-onset recurrent fever, severe ulcerative skin lesions, and elevated liver enzymes. Whole-exome sequenc

More Than Just a Gut Feeling: Unmasking Systemic Mastocytosis Behind Chronic Gastrointestinal Symptoms in a 2-Patient Case Series.

Systemic mastocytosis is a rare clonal mast-cell disorder whose gastrointestinal manifestations may mimic common diseases and delay diagnosis. We present 2 women with longstanding gastrointestinal symptoms, including 1 treated for Crohn's disease for more than 15 years. Gastrointestinal biopsies demonstrated mast-cell infiltrates consistent with systemic mastocytosis, supported by elevated serum tryptase and KIT proto-oncogene, receptor tyrosine kinase (KIT) D816V mutations. Both received mast cell-directed therapy, with differing response and tolerability. These cases highlight the importance

Targeting Ero1L by Parthenolide Alleviates Cellular Senescence and Fibrosis of Localized Scleroderma by Regulating Mitochondria-Associated Endoplasmic Reticulum Membranes Stabilization.

Mitochondrial dysfunction drives scleroderma pathogenesis. The mitochondria-associated endoplasmic reticulum membrane (MAM) regulates mitochondrial function. Parthenolide (PTL) has protective effects on mitochondria. However, its specific role and mechanisms in localized scleroderma (LoS) remain unclear. This study investigates whether PTL reduces senescence and skin fibrosis by stabilizing MAM in LoS and examines the protective effects of a synthesized thermosensitive controlled-release hydrogel (F127@PLGA@PTL) against LoS. We employed transmission electron microscopy (TEM) to observe MAM abn

Identification of an ERCC2 Mutation-Associated Mutational Signature of Nucleotide Excision Repair Deficiency in Targeted Panel Sequencing Data.

PURPOSE: Next-generation sequencing-based mutational signatures are frequently used to identify tumors with specific DNA repair deficiencies for targeted therapeutic strategies. Although mutational signatures are commonly derived from whole-exome or whole-genome sequencing data, patients often undergo tumor sequencing using more limited targeted panels that typically encompass several hundred cancer-associated genes. Identifying clinically relevant mutational signatures from targeted panel data requires new approaches capable of deriving signatures from the more limited sequencing data. METHO

Strain-Dependent Effects of Long-Term Exposure to 27.5GHz 5G Millimeter Waves on Male Reproductive Outcomes in Mice.

BACKGROUND: The biological impact of prolonged exposure to 5G millimeter-wave frequencies (FR2) on male reproductive health remains insufficiently characterized despite the global deployment of this technology in the near future. Here, we investigated the effects of chronic 27.5GHz FR2 exposure on testicular function and sperm quality in three mouse strains with distinct genetic backgrounds (Car-S, CD1, and C57BL/6J). METHODS: Adult males (8-weeks old at the onset of the study) were exposed for 100 days to low (6.67W/m²) or high (20W/m²) power density, or were SHAM-exposed. Reproductive param

Membrane-Coated Iron Oxide (Fe3O4) Nanoparticles Encapsulating Dacarbazine Suppress B16F10 Melanoma Cell Activity in vitro and in a Subcutaneous Mouse Model.

BACKGROUND: Malignant melanoma is a highly aggressive skin cancer. Dacarbazine (DTIC) chemotherapy, while historically used, is limited by poor solubility, short half-life, and systemic toxicity. Fe3O4 nanoparticles have been explored as drug delivery vectors with potential for photothermal conversion and Fenton-like catalytic activity. Cell membrane-camouflaged platforms may offer improved tumor targeting. METHODS: A biomimetic nanoplatform (Fe3O4PD@CCM) was engineered by encapsulating an Fe3O4 core within a DTIC-loaded PLGA-PEG2000 matrix, followed by functionalization with B16F10 cancer ce

The SCF/KIT signaling pathway in multisystem diseases: Mechanisms and therapeutic targeting.

The stem cell factor (SCF)/KIT signaling axis regulates the survival, proliferation, differentiation, and migration of hematopoietic stem and progenitor cells, mast cells, interstitial cells of Cajal, germ cells, and melanocytes through multiple downstream pathways, including PI3K/AKT and RAS/MAPK. Its biological and pathological effects are determined by cell lineage, ligand presentation, and mutational context. At the pathway level, SCF expression and processing, KIT abundance, and activating mutations determine signal initiation and intensity, whereas CBL-mediated receptor degradation and S

Modified Hotz Operation Combined with Modified Medial Canthoplasty to Correct Congenital Lower Eyelid Epiblepharon in a Chinese Population.

BACKGROUND: Epiblepharon correction combined with epicanthoplasty is useful for the treatment of congenital lower eyelid epiblepharon (CE). To further refine this approach, we developed a modified medial canthoplasty (Mod-MC) involving removal of the medial canthus fibrous band (MCFB), combined with a modified Hotz procedure (Mod-Hotz). This study aimed to evaluate the efficacy of Mod-Hotz + Mod-MC in Chinese patients and to investigate the role of the MCFB in surgical outcomes. METHODS: Patients diagnosed with CE were divided into two groups according to the surgical procedure performed: Mod

[Infantile hemangiomas: diagnosis and modern therapeutic approaches].

Infantile hemangiomas are benign vascular tumors that occur exclusively in infants and display a characteristic growth pattern. They usually become noticeable shortly after birth, proliferate over a period of 4-6 months, remain stable in size for a variable duration, and then gradually involute, although residual changes may persist. Diagnosis is typically made clinically, with careful attention to potential complications, functional impairments, and rare associated malformations such as those seen in PHACES and LUMBAR syndromes. A thorough understanding of the different types of hemangiomas,
