Alcohol-Induced Hypersensitivity: A Clinical and Mechanistic Overview.

Patients with reactions to alcohol pose a diagnostic challenge for allergists, since such symptoms can represent true hypersensitivity, alcohol as a cofactor for other allergens, or entirely separate mimicking conditions. The literature on this topic is limited, with much of the current understanding drawn from case reports, small case series, and mechanistic studies rather than large scale trials. Although the underlying mechanism is not fully understood, several pathways have been proposed to explain this cofactor effect, which this review will discuss in detail. Alcohol has also been identi

Vulvar Mastocytosis in Children and Adolescents: A Case Series.

BACKGROUND: Pediatric cutaneous mastocytosis is a rare condition characterized by clonal mast cell accumulation in the skin, most commonly presenting on the trunk and extremities. Vulvar involvement is exceedingly rare, particularly as an isolated lesion. METHOD: We retrospectively reviewed five pediatric cases of vulvar mastocytosis: one case from a children's hospital and four cases enrolled in an NIH protocol for mastocytosis (NCT00044122). RESULTS: Five girls (aged 16 months to 15 years), initially presented to the NIH or tertiary children's hospital with lesions on the vulva characteriz

Myeloid neoplasm with PDGFRA rearrangement presenting in blast-phase with basophilia.

An adult man, from Southern Africa, presented to a public health centre with symptoms of progressive fatigue. Peripheral blood morphology showed marked basophilia and 21% myeloblasts, confirmed by flow cytometry, suggesting acute myeloid leukaemia with basophilia. Bone marrow biopsy revealed increased mast cells, raising systemic mastocytosis as a differential. However, next-generation sequencing identified a PDGFRA fusion, establishing the diagnosis of blast phase of a myeloid/lymphoid neoplasm with PDGFRA rearrangement. The presence of cells with abnormal basophilic granulation and absence o

Identification of Homing Receptors and Activation-Linked Surface Antigens on Human Mast Cells and Basophils.

Mast cells (MC) and basophils (BA) are key effector cells in atopic disorders. Development, homing, and activation of these cells are regulated by various surface antigens. We examined the expression and cytokine-mediated regulation of functionally relevant surface antigens on human BA and MC. Primary BA were obtained from healthy or allergic donors, or patients with chronic myeloid leukemia (CML). Primary MC were isolated from lung, skin, normal/reactive bone marrow (BM) or BM of patients with mastocytosis. The BA cell line KU812 and the MC lines HMC-1, ROSA, and MCPV-1 were also examined. As

Generation of a novel knock-in mouse line expressing oncogenic Kit D814V in the hematopoietic stem cell compartment as a preclinical model for systemic mastocytosis.

Research in mastocytosis, characterized by clonal expansion of mast cells (MC), and driven by the KIT D816V mutation in >90% of patients, has been hampered by the lack of in vivo models. Here, we describe a novel knock-in mouse line expressing Kit D814V (murine homolog to human KIT D816V) in the hematopoietic compartment by Scl-CreER-mediated recombination. Scl-Cre;KitD814V fl/+ mice presented increased MC progenitors in the bone marrow (BM) leading to MC expansion in the skin, stomach, spleen and liver, and increased serum levels of murine MC protease-1 (mMCP-1) and mMCP-6. Additionally, mice

Predicting the clinical course of advanced systemic mastocytosis on treatment with avapritinib or midostaurin.

Advanced systemic mastocytosis (AdvSM) is driven by KIT D816V in approximately 95% of cases. Accurate assessment and prediction of clinical course and disease outcome before and during treatment with the KIT inhibitors midostaurin and avapritinib may facilitate optimization of treatment strategies. We retrospectively evaluated clinical, morphologic, laboratory, and genetic predictors of outcome in 79 midostaurin- and 176 avapritinib-treated patients. The number of mutations at baseline in SRSF2/ASXL1/EZH2 (midostaurin) and in SRSF2/RUNX1/SETBP1 (avapritinib) delineated highly predictive 3-tier

Adult-Onset Polymorphic Maculopapular Cutaneous Mastocytosis.

Cutaneous mastocytosis commonly manifests with maculopapular lesions. In children, the lesions tend to be large and polymorphic, while in adults, they are typically small and monomorphic. Adults who develop cutaneous manifestations are usually found to have systemic mastocytosis (SM).  In this report, we describe the case of a 42-year-old female patient who developed polymorphic hyperpigmented cutaneous lesions scattered over her chest and abdomen. Punch biopsies of two lesions were obtained and demonstrated increased perivascular and interstitial mast cells. Over the following month, she cont

In-hospital sting challenge during venom immunotherapy has a high negative predictive value for subsequent sting reactions.

AIMS: Hymenoptera venom allergy is the most frequent elicitor of anaphylaxis in adults. Biomarkers predicting success of venom immunotherapy (VIT) are lacking. Only a re-sting may help to detect insufficiently protected individuals. The aim of the study was to investigate the prognostic value of a tolerated sting challenge with regard to future field stings. MATERIALS AND METHODS: A retrospective study included 585 consecutive patients of the AllergieZentrum Munich, Germany, who had tolerated a sting challenge test during VIT. Patients who were still on VIT were examined with a questionnaire

Species-specific outcomes in wasp sting challenges during venom immunotherapy and their respective follow-up.

BACKGROUND: Insect venom allergy is a major cause of life-threatening anaphylaxis, with venom immunotherapy (VIT) being the gold standard for prevention. Sting challenges (SC) are used to assess immune tolerance and VIT efficacy, but factors influencing outcomes remain unclear. This study examined whether the stinging insect species affects SC outcomes during VIT and evaluated outcomes during long-term follow-up. MATERIALS AND METHODS: A retrospective analysis of 1,163 patients undergoing SC during wasp venom VIT was performed. The stinging species was entomologically confirmed. Associations

More Than Just a Gut Feeling: Unmasking Systemic Mastocytosis Behind Chronic Gastrointestinal Symptoms in a 2-Patient Case Series.

Systemic mastocytosis is a rare clonal mast-cell disorder whose gastrointestinal manifestations may mimic common diseases and delay diagnosis. We present 2 women with longstanding gastrointestinal symptoms, including 1 treated for Crohn's disease for more than 15 years. Gastrointestinal biopsies demonstrated mast-cell infiltrates consistent with systemic mastocytosis, supported by elevated serum tryptase and KIT proto-oncogene, receptor tyrosine kinase (KIT) D816V mutations. Both received mast cell-directed therapy, with differing response and tolerability. These cases highlight the importance

The SCF/KIT signaling pathway in multisystem diseases: Mechanisms and therapeutic targeting.

The stem cell factor (SCF)/KIT signaling axis regulates the survival, proliferation, differentiation, and migration of hematopoietic stem and progenitor cells, mast cells, interstitial cells of Cajal, germ cells, and melanocytes through multiple downstream pathways, including PI3K/AKT and RAS/MAPK. Its biological and pathological effects are determined by cell lineage, ligand presentation, and mutational context. At the pathway level, SCF expression and processing, KIT abundance, and activating mutations determine signal initiation and intensity, whereas CBL-mediated receptor degradation and S

Chronic Spontaneous Urticaria and Psychiatric Symptoms: The Blood-Brain Barrier Hypothesis.

Chronic spontaneous urticaria (CSU) is a mast cell-driven inflammatory disease frequently accompanied by anxiety, depression, sleep disturbance, fatigue, and impaired quality of life. Pruritus, disturbed sleep, psychosocial burden, and treatment-related effects are established contributors to this neuropsychiatric and functional burden. Whether immune-to-brain signalling provides an additional mechanism in a susceptible subgroup remains unknown. This hypothesis-driven narrative review examines the blood-brain barrier (BBB) as one potential interface linking CSU-associated inflammation to centr

Mastocytosis: Imaging Spectrum and Diagnostic Insights.

Mastocytosis is a rare clonal disorder marked by the excessive proliferation and accumulation of mast cells in different tissues, with the skin and bone marrow being the most commonly affected sites. Mastocytosis is primarily driven by somatic mutations that most commonly affect the c-KIT proto-oncogene. The World Health Organization classifies mastocytosis into three main categories. Cutaneous mastocytosis involves infiltration limited to the skin and is more prevalent in children than in adults. Systemic mastocytosis involves infiltration of extracutaneous organs, including the liver, spleen

Serum Mas-related G protein-coupled receptor X2 concentration in drug-induced immediate hypersensitivity reactions.

INTRODUCTION: Mas‑related G protein-coupled receptor X2 (MRGPRX2) has emerged as a mediator of mast cell activation in acute and chronic conditions. Exogenous ligands, such as neuromuscular blocking agents (NMBAs) and fluoroquinolones (FQs), can trigger MRGPRX2‑dependent activation and may augment immunoglobulin E (IgE)-mediated pathways. Although investigators have measured serum MRGPRX2 level in asthma, mastocytosis, and chronic urticaria, its role in immediate hypersensitivity reactions (IHRs) to FQs or NMBAs remains unclear. OBJECTIVES: We conducted this study to determine whether increas
