Neuroimaging features of incontinentia pigmenti: prompting genetic confirmation.

A term female neonate presented with vesicular skin lesions along Blaschko's lines and refractory seizures in the first week of life. Brain MRI revealed multifocal T2/fluid-attenuated inversion recovery hyperintensities and restricted diffusion in the periventricular and subcortical white matter, basal ganglia, thalami, corpus callosum and cerebellum, with microhaemorrhages on susceptibility-weighted imaging. These imaging findings, along with clinical history, suggested incontinentia pigmenti (IP), prompting genetic testing, which confirmed a heterozygous IKBKG exon 4-10 deletion. Seizures we

Long-Read Sequencing is Required for Precision Diagnosis of Incontinentia Pigmenti.

Incontinentia pigmenti (IP) is caused by loss-of-function variants in IKBKG, with molecular genetic diagnosis complicated by a pseudogene. We describe seven individuals from three families with IP but negative clinical testing in whom long-read sequencing identified causal variants. Concurrent methylation analysis explained disease severity in one individual who died from neurologic complications, identified a mosaic variant in an individual with an atypical presentation, and confirmed skewed X-chromosome inactivation in an XXY individual.

Single-Cell RNA Sequencing in Incontinentia Pigmenti With Neonatal Encephalopathy Reveals Broad Immune Activation Moderated by Steroids.

BACKGROUND AND OBJECTIVES: DNA variations in the NF-kappa-B essential modulator (NEMO) gene are linked to incontinentia pigmenti (IP) and also immunodeficiency and autoinflammatory conditions. Some patients with IP present with neonatal vasculitis-like brain changes, although pathogenesis is unclear. We investigated cell-specific gene expression in a neonate with IP, who had encephalopathy, seizures, and vasculitis-like brain changes, and responded to steroid treatment. METHODS: Single-cell RNA (ribonucleic acid) sequencing (scRNAseq), using the HIVE single-cell system, was performed on a neo

Incontinentia Pigmenti: A Rare Case of Survival of a Male Infant.

This case describes a seven-month-old male infant with incontinentia pigmenti (IP), a rare X-linked dominant disorder that is typically lethal in male fetuses. The infant presented with blaschkoid hyperpigmentation but showed no neurologic, dental, or ocular abnormalities. Genetic testing revealed a normal karyotype without IKBKG (inhibitor of nuclear factor kappa B kinase regulatory subunit gamma) mutations, suggesting possible somatic mosaicism, a rare survival mechanism in male patients with IP. This case underscores the rarity of IP in male infants and highlights the need for multidiscipli

Incorporating Nanopore Sequencing Into a Diverse Diagnostic Toolkit for Incontinentia Pigmenti.

Incontinentia pigmenti (IP) is a rare hereditary disorder affecting 1.2 in 100,000 live births, predominantly females. Genetic analysis of IP is complicated by a homologous pseudogene, making conventional short-read sequencing challenging. While long-range PCR is typically used to overcome this, skewed X-inactivation detection can also aid in assigning variants to IKBKG. We employed a comprehensive approach, incorporating whole-exome sequencing (WES), long-range PCR, RT-PCR, X-inactivation analysis, and nanopore sequencing, to identify and accurately phase a small heterozygous deletion, NM_001

Congenital and Neonatal Skin Disorders: Histopathological Diagnosis and Syndromic Associations.

Congenital and neonatal skin disorders encompass a broad spectrum of conditions ranging from transient benign rashes to severe genodermatoses and multisystem syndromes. Accurate diagnosis in the newborn period is critical, as cutaneous findings may be the earliest clue to underlying genetic, infectious, or systemic diseases. A comprehensive literature review was performed, drawing from up-to-date peer-reviewed studies, landmark dermatology and pathology reviews, and authoritative texts. Key information on clinical presentation, histology, and syndromic associations was extracted and synthesize

A Fulminant Case of Incontinentia Pigmenti Presenting With Early Neuro-Ophthalmologic Manifestations.

Incontinentia pigmenti (IP) is a rare, X-linked dominant, multisystemic genetic disorder. We report the case of a female neonate presenting with vesiculobullous and erythematous cutaneous lesions involving the hands and feet. Concurrently, she exhibited repetitive flexion movements of all four limbs, which rapidly progressed to status epilepticus. The clinical course was rapidly unfavorable, marked by diffuse retinal ischemia and cerebral palsy. This patient's clinical history illustrates the potential severity of the neurological and ophthalmologic manifestations of IP. It underscores the nee

Diagnostic Utility of Optical Genome Mapping in X-Linked Dominant Genodermatoses: Incontinentia Pigmenti and CHILD Syndrome.

INTRODUCTION: Incontinentia pigmenti and CHILD syndrome are genodermatoses characterized by an X-linked dominant inheritance pattern, resulting from pathogenic variants in the IKBKG and NSDHL genes, respectively. METHODS: This study examines 3 pediatric patients exhibiting a compatible phenotype with inconclusive genetic studies, aiming to evaluate the diagnostic utility of optical genome mapping (OGM) in detecting alterations that could elucidate these conditions. RESULTS: The identified structural variants consisted of deletions of varying sizes in the Xq28 cytoband, encompassing regions t

Clinical relevance of loss-of-function mutations of NEMO/IKBKG.

Dysfunctional inhibitor of nuclear factor-κB (NF-κB) kinase regulatory subunit gamma (IKBKG) is known to trigger incontinentia pigmenti (IP), anhidrotic ectodermal dysplasia with immunodeficiency (EDA-ID), immunodeficiency (ID), and IKBKG deleted exon 5 autoinflammatory syndrome (NDAS). The correlation between genotype and phenotype remains elusive because of the considerable variability in IKBKG genes. This study aimed to systematically describe IKBKG gene mutations and clinical characteristics. Cases with IKBKG mutations and thorough clinical features were gathered using PubMed, Web of Scien

Whole Exome Sequencing in Incontinentia Pigmenti: A ROP Mimicker and the Genetic Detection Dilemma.

Incontinentia pigmenti is an X-linked dominant multisystemic disorder caused by pathogenic variants in the inhibitor of nuclear factor kappa B kinase regulatory subunit gamma (IKBKG) gene. Mutations in the IKBKG gene, encoding the NF-kappa-B essential modulator protein, underlie its pathogenesis, leading to inflammation-driven vascular ischemia and significant ocular manifestations. The authors report a case of an Asian-Indian infant diagnosed as having stage 3 incontinentia pigmenti in both eyes treated with anti-vascular endothelial growth factor and laser photocoagulation where standard who
