Severe Acrodermatitis Continua of Hallopeau With Atypical Dermoscopic Features: A Diagnostic Pitfall.

Acrodermatitis continua of Hallopeau is a rare, chronic, relapsing form of localized pustular psoriasis characterized by painful sterile pustules and progressive destruction of the nail apparatus. Its clinical and dermoscopic features may be atypical, leading to diagnostic delays and inappropriate treatment. We present the case of a 78-year-old man with a one-year history of painful hyperkeratotic lesions affecting several fingers, with recent worsening, extension, and purulent discharge. Examination revealed bilateral chronic paronychia with peripheral erosions involving both thumbs and the l

Isolated Congenital Hyponychia and Anonychia in a Neonate: A Rare Case.

Congenital hyponychia and anonychia are extremely rare nail disorders characterized by the partial or complete absence of nails. These conditions may be inherited or associated with in utero exposure to teratogenic substances. A full-term male neonate underwent routine head-to-toe examination in the labor room. The baby was noted to have both incomplete (hyponychia) and complete (anonychia) absence of fingernails and toenails. No other systemic abnormalities were identified. Isolated congenital hyponychia and anonychia are uncommon findings in neonates. In the absence of syndromic features, th

Multiple congenital malformations in a burrowing owl (Athene cunicularia) from Southern Brazil.

This study reports a case of multiple congenital malformations in a burrowing owl (Athene cunicularia) rescued in southern Brazil. Clinical assessment revealed partial agenesis of the forelimbs, bilateral asymmetric syndactyly, and selective anonychia of the second digit in both pelvic limbs. Despite severe locomotor limitations and the inability to fly, the individual maintained a satisfactory body condition score, demonstrating the species' adaptive and behavioural capacity. The integrity of the epithelial tissues and the symmetry of the digital alterations support a teratogenic etiology, di

Nail Changes in Congenital Insensitivity to Pain with Anhidrosis: A Case Report.

INTRODUCTION: Nail changes often reflect underlying systemic disease and may be the presenting manifestation of rare genetic disorders. Here, we report 2 siblings with congenital insensitivity to pain with anhidrosis (CIPA) with prominent nail findings. CASE REPORT: We report two brothers who presented with prominent nail abnormalities including dystrophy, brachyonychia, anonychia, paronychia, and pseudo-clubbing, along with acro-osteolysis of distal phalanges. Subsequent evaluation revealed insensitivity to pain and temperature, intellectual disability, recurrent fractures, self-mutilation,

Onychoscopic Alterations in Newborns with Early Congenital Syphilis: A Single-Center Cross-Sectional Study of 10 Cases.

INTRODUCTION: Early congenital syphilis (ECS) is a vertically transmitted infection that remains difficult to diagnose in the neonatal period, as many newborns are asymptomatic at birth. Although mucocutaneous manifestations are well recognized, nail involvement has been rarely described. METHODS: We conducted a prospective, single-center study including neonates with ECS fulfilling scenario 1 or 2 criteria established by the Centers for Disease Control and Prevention and the American Academy of Pediatrics. Ten neonates admitted to the neonatal intensive care unit between April and July 2025

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Junctional Epidermolysis Bullosa Caused by a Hemiallelic Nonsense Mutation in LAMA3 Revealed by 18q11.2 Microdeletion.

Inherited epidermolysis bullosa (EB) is a heterogeneous clinical entity that includes over 30 phenotypically and/or genotypically distinct inherited disorders, characterized by mechanical skin fragility and bullae formation. Junctional EB (JEB) is an autosomal recessive disease characterized by an intermediated cleavage level within the skin layers, commonly at the "lamina lucida". Laryngo-onycho-cutaneous syndrome (LOC) is an extremely rare variant of JEB, characterized by granulation tissue formation in specific body sites (skin, larynx, and nails). Although most cases of JEB are caused by p

The concept of onychodermis containing onychofibroblasts has histological (microanatomical), immunohistochemical as well as molecular basis.

The terms "onychofibroblast" (nail-specific fibroblast) and onychodermis (nail-specific dermis) were first introduced in 2006 and 2012, respectively, based on distinctive histologic and immunohistochemical features from the dermis of the surrounding skin and have been demonstrated in multiple studies. Recently, based on molecular research, the definition of onychodermis containing onychofibroblasts has been expanded to encompass the area located between the nail matrix and bed epithelium and periosteum. Single-cell RNA sequencing and in situ hybridization demonstrated that onychofibroblasts wi

Widespread purplish-red plaques with plasma cell infiltrate: A rare type of lichen planus.

Diseases in which cutaneous plasma cell infiltrates predominate are rare and usually of unknown etiology, including those that transition from benign to malignant, such as cutaneous plasmacytosis, multicentric Castleman disease, and extramedullary plasmacytoma. These diseases may present as purplish, reddish-brown cutaneous plaques or nodules. Here, we report an exceptional case of lichen planus (LP) in which the patient had classic histopathological features, but the infiltrating inflammatory cells were plasma cells with restricted light chain expression. The patient presented with severe ras

Critical role of thrombospondin-1 in promoting intestinal mucosal wound repair.

Acrodermatitis continua of Hallopeau (ACH) is a rare, sterile pustular eruption of one or more digits. The condition presents with tender pustules and underlying erythema on the tip of a digit, more frequently arising on a finger than a toe. As far as classification, ACH is considered a localized form of pustular psoriasis. The eruption typically occurs after local trauma or infection, but such a history is not always present and various other etiologies have been described including infectious, neural, inflammatory, and genetic causes. The natural progression of ACH is chronic and progressive

Dermoscopic features of nails in Leprosy patients in a tertiary referral hospital in West Java, Indonesia.

INTRODUCTION: Leprosy is a chronic granulomatous infectious disease, mainly affecting the skin and peripheral nerves, caused by the obligate intracellular bacteria Mycobacterium leprae. The disease has been discussed in several review articles in recent research, but as far as we know, only a few have addressed the effects of leprosy on nails, especially those who examine the dermoscopic features of nails in leprosy patients. PURPOSES: We aimed to document nail changes in leprosy patients and identify any particular findings through dermoscopic examination. METHOD: This was an observational

Description of the most severe signs in nail lichen planus: a strategy to contribute to the diagnosis of the severe stage.

BACKGROUND: Severe nail lichen planus (NLP) does not respond well to treatment and is often poorly considered and described in detail. OBJECTIVE: We sought to describe the characteristics of severe NLP. METHODS: A retrospective data analysis was performed, including the photographic records of the most compromised nails of patients with NLP over 18 years old, who consulted between 2009 and 2019 at the Instituto de Dermatologia Professor Rubem David Azulay in Rio de Janeiro, Brazil. Descriptive and statistical analysis using Fisher's exact test was performed to verify the hypothesis of indepe

Nail involvement in patients with epidermolysis bullosa: A systematic review.

BACKGROUND: Nail changes in patients with congenital epidermolysis bullosa (EB) are caused by abnormalities of the nail matrix and bed secondary to pathogenic alterations of the dermoepidermal junction. Even though ungual alterations are extremely frequent in these patients, there are scarce studies about their frequency and/or association with subtypes or clinical course of EB. OBJECTIVES: To systematically review nail abnormalities in patients with EB reported in the literature. METHODS: We searched all published articles in electronic databases until June 2020 reporting patients with EB w

Anonychia Congenita - Rare Inheritance of a Rare Disorder.

Non syndromic Anonychia congenita or congenital absence of finger and toe nails is a rare disorder known to occur due to autosomal recessive inheritance of mutation in the R-spondin-4 gene. We present a case of a 32 year old female born of a non-consanguineous marriage presenting with complete absence of finger and toe nails since birth and similar presentation in family members over four generations, suggesting an autosomal dominant inheritance.
