Direct Carotid Artery Exposure for Mechanical Thrombectomy in a Patient with Marfan Syndrome and Prior Total Arch Replacement.

Marfan syndrome presents unique challenges for mechanical thrombectomy in acute ischemic stroke due to vascular fragility and complex aortic anatomy, often from prior surgeries. We describe the case of a man in his 50s with Marfan syndrome, total aortic arch replacement, mechanical valve replacement, and extensive chronic dissections, who presented with acute ischemic stroke due to right internal carotid artery occlusion. Conventional transfemoral access failed due to anomalous prosthetic brachiocephalic artery graft anatomy. Consequently, mechanical thrombectomy was performed via direct surgi

Kikuchi Disease With Marfanoid Features in a Background of Fever of Unknown Origin.

Kikuchi-Fujimoto disease (KFD) is a self‑limiting necrotizing lymphadenitis that usually affects young women. It can mimic infection, malignancy, or autoimmune disease, which often leads to diagnostic delays. Marfanoid habitus encompasses a constellation of physical traits associated with Marfan syndrome: tall stature, disproportionate limbs, arachnodactyly, joint hypermobility, and chest wall deformities. Co-existence of KFD and prominent marfanoid features has not previously been reported. Based on a structured PubMed/MEDLINE search from database inception through January 2026 using MeSH ter

Spontaneous pneumothorax-associated with genetic disorders.

Spontaneous pneumothorax (SP) is defined by the presence of air in the pleural space arising from neither trauma nor iatrogenic causes. It can develop secondary to various etiologies. The familial clustering observed in some patients with SP supports the view that genetic factors play a role in the pathogenesis of SP. Several recognizable pneumothorax-associated genetic syndromes exist, including Birt-Hogg-Dubé syndrome (BHD), tuberous sclerosis complex-associated lymphangioleiomyomatosis (TSC-LAM), Marfan syndrome (MFS), cystic fibrosis (CF), alpha-1 antitrypsin deficiency (AATD), vascular Eh

Sulcus fixation of a standard capsular tension ring in cataract surgery with capsular instability.

AIM: To introduce a novel technique for stabilizing the capsular bag in cataract surgery. METHODS: This retrospective observational case series included eyes with unstable capsular bag that underwent cataract surgery. A standard capsular tension ring (CTR) with 10-0 Prolene sutures was inserted into the capsular bag and fixated to the sulcus to stabilize the bag, enabling in-the-bag intraocular lens (IOL) implantation. Uncorrected distance visual acuity, corrected distance visual acuity (CDVA), and slit-lamp examination were performed preoperatively and at 1 and 6mo postoperatively. RESULTS:

Integrated Forward and Reverse Degradomics of Aortic Aneurysms Uncovers Their Proteolytic Landscapes and the Roles of MMP9 and Mast Cell Chymase.

BACKGROUND: Dysregulated proteolysis is implicated in thoracic (thoracic aortic aneurysm [TAA]) and abdominal aortic aneurysm (AAA) pathogenesis, but proteolytic landscapes (degradomes) of aneurysmal and normal aorta and contributions of individual proteases remain undefined. Here, a proteome-wide approach was used to define and compare TAA and AAA degradomes and uncover the specific role in aortic remodeling of 2 proteases consistently identified in the aneurysms, CMA1 (mast cell chymase) and MMP9 (matrix metalloprotease 9). METHODS: The mass spectrometry-based N-terminomics strategy, termin

Ross Procedure After Valve-Sparing Root Replacement With Marfan Syndrome.

BACKGROUND: The Ross procedure is one of the optimal surgical options for aortic valve pathologies in relatively young patients. However, its application in patients with Marfan syndrome (MFS) is not only debatable but may even be considered a contraindication. CASE SUMMARY: A 31-year-old woman with MFS had undergone valve-sparing root replacement 2.5 years earlier and now presented for treatment of severe aortic insufficiency (AI). A Ross procedure with stabilization of the aortic root was performed. Two years after the procedure, mild AI was observed; however, the patient was asymptomatic,

Arteriopathies: Too big to be true.

Arteriopathies in early childhood present significant diagnostic challenges due to phenotypic overlap between Marfan syndrome, Loeys-Dietz syndrome, Ehlers-Danlos syndrome, and hereditary cutis laxa. We report a 2-year-old male, born of consanguineous parentage, evaluated for persistent cough and hypertension. Clinical features included coarse facies, pectus excavatum, and mild hepatomegaly. Echocardiography revealed a massively dilated ascending aorta (56 mm), aortic annular dilatation, moderate aortic insufficiency, and severe post-subclavian coarctation with marked post-stenotic dilatation.

Beyond the Aorta: Incidental Atrial Septal Defect in a Patient With Marfan Syndrome and Severe Aortic Dilation.

Marfan syndrome (MFS) is an autosomal dominant connective tissue disorder caused by mutations in the FBN1 gene, which carries a high risk of cardiovascular morbidity and mortality. We present the case of a 31-year-old man with a severe Marfanoid phenotype who was admitted with decompensated heart failure, septic shock, and renal failure. After stabilization, multimodal evaluation revealed a severely dilated aortic root and severe aortic regurgitation, along with an incidental 11 mm ostium secundum atrial septal defect (ASD). This unusual combination caused critical biventricular overload. The

New CT-based dural ectasia criteria using machine learning to diagnose Marfan and Loeys-Dietz syndromes.

AIMS: Dural ectasia (DE) is a criterion for Marfan syndrome (MFS) diagnosis. However, there is no agreement on its diagnosis. We identified new DE criteria on CT-scan imaging using machine-learning (ML) and aimed to evaluate their performance for MFS and Loeys-Dietz detection. METHODS AND RESULTS: MFS patients with FBN1 pathogenic variant, who underwent a complete CT-scan were included and matched 1:1 with controls. The conventional criteria for DE were assessed, as well as new criteria identified by ML (generalized linear model and random forest), regarding MFS diagnostic performance. The ne

Double endovascular repair in a patient with Marfan syndrome and postdissection thoracoabdominal aortic aneurysm: Twelve years of follow-up.

A patient with Marfan syndrome and prior open aortic surgery presented with a chronic post-type B dissection thoracoabdominal aneurysm measuring 6.5 cm in diameter. He underwent successful endovascular repair using a surgeon-modified fenestrated endograft. During follow-up, he developed infectious aortitis and permanently lost right kidney function. Multiple endovascular reinterventions were performed, including complete relining with a T-branch endograft to address graft degeneration. After 12 years, the aneurysm sac remained stable at 6.3 cm; renal function stabilized without dialysis, and h

Case Report: A rare coexistence with severe aortic root dilatation and nutcracker phenomenon in pediatric Marfan syndrome.

Marfan syndrome (MFS) is a multisystem connective tissue disorder affecting the cardiovascular, ocular, and skeletal systems. We report a case of a 13.5-year-old boy who presented with excessive linear growth. Diagnostic evaluations revealed severe aortic root dilatation, repeatedly positive occult blood in urine, and ultrasonographic findings suggestive of left renal vein entrapment. Genetic testing identified a pathogenic variant in the FBN1 gene. The patient was ultimately diagnosed with Marfan syndrome complicated by left renal vein entrapment syndrome (nutcracker phenomenon). The co-occur

Long-term echocardiographic and clinical outcomes after aortic root replacement in patients with Marfan syndrome.

OBJECTIVE: We compared longitudinal echocardiographic data between patients with Marfan syndrome who underwent aortic valve-sparing (AVS) and aortic valve-replacing (AVR) aortic root replacement procedures. METHODS: Of 316 patients enrolled at 19 sites in 5 countries, 239 (75%) were treated with AVS and 77 (25%) with AVR. Transthoracic echocardiograms were collected at postoperative years 1, 3, 5, 7, 10, 12, 14, 16, 18, and 20. Mixed-effects models were used to evaluate echocardiographic changes over time. Inverse probability-weighted Cox regression models were used to compare the risk of per

Primary adrenal insufficiency resulting in diagnosis of rare ABCD1 pathogenic variant in X-linked adrenoleukodystrophy.

X-linked adrenoleukodystrophy (X-ALD) is a rare neurodegenerative demyelinating disorder with variable presentations. We report a case of a 32-year-old African American male with adult-onset X-ALD presenting with primary adrenal insufficiency (PAI) and marfanoid musculoskeletal features. He was initially diagnosed with PAI of unclear etiology and taking only hydrocortisone monotherapy. Further workup revealed negative 21-alpha-hydroxylase antibodies and elevated plasma very long chain fatty acids. Genetic testing confirmed the diagnosis of X-ALD resulting from novel ATP-binding cassette transp

Fluoroscopy-guided surgical retrieval of a retained intradural spinal catheter following major aortic surgery.

BACKGROUND: Retained intradural spinal catheter fragments are rare but potentially serious complications that may lead to neurological injury, infection, and catheter migration. CASE DESCRIPTION: Herein, we describe a fluoroscopy-guided surgical technique to retrieve a retained intradural spinal catheter in a patient with Marfan syndrome and dural ectasia following major aortic surgery. Intraoperative fluoroscopy enabled precise localization and targeted exposure and safe catheter removal while minimizing neural manipulation. Complete retrieval was confirmed, and the patient experienced no po

Mosaicism in Marfan Syndrome: Clinical Implications and Variant Allele Frequency Interpretation in a Narrative Review.

Marfan syndrome (MFS) is an autosomal dominant disorder of the connective tissue caused by pathogenic variants in the FBN1 gene, characterized by cardiovascular, ocular, and skeletal involvement. Although some of these cases are considered sporadic, recent advances in next-generation sequencing (NGS) have revealed that certain cases might be attributable to parental mosaicism. Mosaicism, defined as the presence of genetically distinct cell populations arising from postzygotic mutations, retains important implications in diagnosis, phenotypic variability, recurrence risk, and genetic counseling
