BACKGROUND AND OBJECTIVES: Congenital contractural arachnodactyly (CCA) is a rare autosomal dominant connective tissue disorder, and FBN2 is its only known causative gene. CCA is characterized by joint contractures, arachnodactyly, scoliosis, and crumpled ears. Due to its rarity, phenotypic diversity, heterogeneity, and clinical overlap with conditions such as Marfan syndrome (MFS), the diagnosis remains challenging, and genetic screening plays a critical role in facilitating accurate diagnosis. We recruited a CCA family with three patients across three generations and detected their genetic e

BACKGROUND: The vascular system is the largest organ in the body and underlies most chronic diseases, yet the molecular mechanisms that govern its plasticity remain poorly defined.
METHODS: We applied single-cell proteomics in vascular disease, integrating it with single-cell transcriptomics to map protein regulation in healthy and Marfan syndrome aortas.
RESULTS: This approach uncovered cell type-specific and cell state-specific proteins missed at the transcriptional level. Notably, we identified a decoupling of fibrillin-1 RNA and protein abundance, suggesting altered protein regulation as

BACKGROUND: Patients with pectus excavatum occasionally develop hypotension after prone positioning during scoliosis surgery, which may result in cancellation of the surgery. The imaging characteristics associated with severe hypotension in patients with pectus excavatum remain unclear. The authors compared the cases of 3 scoliosis patients with pectus excavatum who developed hypotension after prone positioning, focusing on the spatial relationship between the great vessels and the thoracic cage on CT.
OBSERVATIONS: In only 1 of the 3 patients was the hypotension refractory to fluids and vaso

BACKGROUND: Hackman-Di Donato syndrome (Online Mendelian Inheritance in Man no. 301,039), caused by mutations of the NKAP gene, located in Xq24, is a rare genetic disorder characterized by a Marfanoid phenotype, intellectual disability, and abnormalities of the musculoskeletal system. This condition has only been described in ten patients and two fetuses, who were identified owing to severe heart defects. All previously reported patients had mutations located in exons 8 and 9 of NKAP.
CASE PRESENTATION: The proband, a 12-year-old Slavic male individual, presented with a Marfanoid habitus, tal

Compressed air injuries to the globe are rare and typically associated with orbital fractures or a visible entry wound. The authors report the case of a 16-year-old male with Marfan syndrome who developed orbital emphysema strictly confined to the sub-Tenon's space following a compressed air injury, without any visible entry wound or orbital fracture. Clinical examination demonstrated a well-demarcated subconjunctival air pocket located approximately 3-4 mm posterior to the limbus, consistent with the anterior insertion of Tenon's capsule. Computed tomography confirmed a localized intraorbital

PURPOSE: Bone mineral density (BMD) in pediatric Marfan syndrome (MFS) patients remains debated. While dual X-ray absorptiometry for BMD assessment is gold standard, it is unreliable in scoliosis. Hounsfield Units (HU) measurement from lumbar CT scans in pediatric patients is increasingly used. The aim of this study is to evaluate BMD in patients with MFS by comparing HU values with those of adolescent idiopathic scoliosis (AIS) patients scheduled for spinal correction surgery. METHODS: Propensity score matching (1:2, ages 10–19) was performed between MFS and AIS patients based on age, sex, bo

BACKGROUND: Pathogenic ACAN variants ('aggrecanopathies') are increasingly recognised as a non-syndromic cause of skeletal dysplasias and short stature. Unlike many other aetiologies, ACAN-related disorder is reportedly associated with advanced bone age due to early epiphysial fusion, although presentation can vary. This study aimed to further characterise skeletal findings in paediatric and adult patients.
METHODS: A retrospective study of confirmed ACAN-related disorder cases from a tertiary genetics centre in England was performed. Data collected included patient demographics, antenatal ul

Pediatric endophthalmitis is a rare but vision-threatening condition, most commonly associated with trauma worldwide. A case of delayed-onset postoperative Staphylococcus warneri endophthalmitis was identified in a 7-year-old boy with Marfan syndrome after scleral-sutured Akreos AO60 intraocular lens (IOL) implantation. After lensectomy, pars plana vitrectomy, and Gore-Tex-fixated IOL placement with intraoperative moxifloxacin, early postoperative examinations were unremarkable. At postoperative day 59, the patient developed progressive vision loss with white, spherical deposits coating the IO

Marfan syndrome is a heritable connective tissue disorder associated with cardiovascular complications, including progressive aortic aneurysm and dissection, caused by mutations in the fibrillin-1 (FBN1) gene. We generated human induced pluripotent stem cells (hiPSCs) from skin fibroblasts of 3 patients carrying FBN1 mutations (FBN1 c.6554 T > C, FBN1 c.4425-4426delinsC, FBN1 c.6806 T > C) using an episomal vector reprogramming system. These hiPSC lines exhibit pluripotent stem cell morphology, express established markers of undifferentiated hPSC state, maintain a normal karyotype, and differe

Marfan Syndrome, a heritable connective tissue disorder caused by mutations within the fibrillin-1 (FBN1) gene, can have deleterious effects on heart and aorta, eyes, the skeletal system and bone. FBN1 mutations that result in increased aortic vulnerability to rupture are associated with high mortality rates. Here, we describe an induced pluripotent stem cell line (JHUi006-A) generated from patient-derived human dermal fibroblasts harboring a heterozygous c.5225-2A > C intronic splice acceptor site variant preceding Exon 43 of FBN1 that results in exon skipping. The clonal line has a normal ka

BACKGROUND: Marfan syndrome (MFS) is associated with a high risk of aortic complications and premature mortality. This study aimed to evaluate long-term outcomes and identify clinical predictors of mortality in patients with MFS.
METHODS: We conducted a retrospective cohort study of consecutive patients diagnosed with MFS according to the revised Ghent criteria who presented between February 2000 and June 2022. Kaplan-Meier analysis was used to estimate overall survival, and survival was compared according to preoperative left ventricular end-diastolic dimension (LVEDD). Univariable and multi

BACKGROUND: Marfan syndrome (MFS) is a heritable connective tissue disorder associated with a heightened risk of acute type A aortic dissection (ATAAD). The long-term impact of MFS on outcomes following total arch replacement with frozen elephant trunk (TAR with FET) remains poorly characterized. This study aimed to evaluate this impact in a large cohort from a single high-volume aortic center.
METHODS: Patients with ATAAD who underwent TAR with FET at Fuwai Hospital (2010-2018) were categorized into MFS and non-MFS groups. Long-term follow-up was complete for all surviving patients, with a m

BACKGROUND: Pregnancy in women with Marfan syndrome (MFS) increases the risk of aortic dissection, yet management based solely on aortic root diameter often fails to predict this complication, particularly Stanford type B dissection. Haploinsufficient (HI) FBN1 variants are associated with more severe aortic phenotypes. We investigated the relationship between FBN1 genotype and pregnancy-related aortic dissection in women with MFS.
METHODS: We retrospectively analyzed women with genetically confirmed MFS whose pregnancies progressed beyond the second trimester and who were managed at a single

Thoracic aortic aneurysm (TAA) is a dilation of the aorta that may eventually dissect and/or rupture. It is associated with genetic disorders such as Marfan syndrome (MFS) and is a life-threatening cardiovascular condition if left untreated. Current clinical guidelines for TAA management are primarily based on maximum diameter thresholds that are often inadequate, particularly in MFS patients. Moreover, the diameter thresholds are not sex-specific, despite growing evidence that TAA outcomes in MFS are influenced by sex. The aim of this study was to identify non-invasive biomarkers for better m

This case report describes selective revascularization of the superior mesenteric artery in a patient in the third trimester of pregnancy with probable Marfan syndrome and an acute complicated type B aortic dissection. Open surgical revascularization was preferred over endovascular intervention, given the patient's anatomic and clinical high-risk features for retrograde type A aortic dissection.

Congenital contractural arachnodactyly (CCA) is a rare, autosomal dominant connective tissue disease characterized by arachnodactyly, camptodactyly, multiple joint contractures, tall and slender habitus, crumpled ears, and scoliosis. It shares overlapping features with Marfan syndrome (MFS). This condition is caused by pathogenic variants in the fibrillin 2 (FBN2) gene. Currently, approximately 200 variants in FBN2 have been identified, with most of the variants located in the middle region of the gene (Exons 24-35). Here, we investigated the genetic etiology of CCA in two unrelated Chinese fa
