Situs Inversus Totalis and Severe Early-Onset Developmental Epileptic Encephalopathy in a Child With a Homozygous CFAP52 Mutation: A Case Report.

We report a 2-year-old boy with a homozygous CFAP52 mutation presenting with situs inversus totalis, severe epileptic developmental encephalopathy, and global developmental delay, expanding the phenotypic spectrum of CFAP52-related ciliopathies. Clinical findings included multifocal epileptiform discharges, thin corpus callosum, and a small subdural hematoma. Trio whole-exome sequencing also revealed a homozygous NCAPG2 variant of uncertain significance, however, its clinical significance remains unproven and no causal relationship can be established. This case highlights the diagnostic value

Genotype and Ultrastructure Correlation in Primary Ciliary Dyskinesia Among Saudi Children: A Case Series.

Background: Primary ciliary dyskinesia (PCD) is a rare inherited disorder of motile cilia causing chronic oto-sinopulmonary disease. Transmission electron microscopy (TEM) remains essential for confirming ultrastructural defects, especially in resource-limited settings. This study characterizes the clinicopathologic and molecular spectrum of Saudi children with PCD. Methods: Ten Saudi children with confirmed PCD (2018-2025) were retrospectively reviewed. Clinical, radiological, ultrastructural, and whole exome sequencing (WES) data were analyzed to assess genotype-phenotype correlation. Result

Clinical and Genetic Study of a Pseudo-Dominant Primary Ciliary Dyskinesia Pedigree: The First DNAAF1-Associated Family Reported in Chinese Population.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare, genetically heterogeneous disorder typically inherited in an autosomal recessive pattern. Pseudo-dominant inheritance is exceptionally uncommon and remains poorly characterized in PCD. METHODS: We conducted a clinical and genetic study of a non-consanguineous Chinese PCD pedigree with four affected individuals across two generations. Whole exome sequencing and Sanger sequencing were performed to identify pathogenic variants. RESULTS: Two variants in DNAAF1, c.1930A>T (p.Arg644Ter) and c.1022_1023del (p.Gln341ArgfsTer10), were identified

Impact of Structural Anomalies and Anatomical Variations on Operative Time in Abdominal Surgery-Systematic Review.

Prolonged operative time is associated with increased surgical complications, resource utilization, and cost. While factors such as surgeon experience and patient habitus are well recognized, the impact of anatomical anomalies and variations remains poorly quantified. This systematic review evaluates the influence of abdominal structural anomalies and anatomical variations on operative time. Following PRISMA 2020 guidelines, a search of PubMed (2000-2025) identified 82 eligible studies. Operative-time impact was assessed using added operative time relative to reference benchmarks. The unit of

Molecular characterization of FAM222B as a novel disease gene for dominant cardiovascular laterality defects.

Cardiovascular laterality defects occur with an estimated birth prevalence of 1.1/10,000 live births, associated with congenital heart defects (CHD) and situs abnormalities. Known disease genes explain about 20% of all cases often correlated with primary ciliary dyskinesia (PCD). We aimed to identify disease genes beyond PCD-related aetiologies using exome sequencing in 16 case-parent trios followed by exome survey in 2,109 individuals with situs inversus totalis, heterotaxy, or isolated CHD. We identified six different variants in FAM222B, previously discussed as a candidate gene for cerebral

Pancreatic cancer in situs inversus totalis: case presentation and literature review.

INTRODUCTION AND IMPORTANCE: Situs inversus totalis (SIT) is a rare anatomical variation of the internal organs in which the thoracic and abdominal viscera organs are mirrored from their normal anatomical position. Surgical treatment of cancer in this anatomical variation represents a great challenge due to the altered surgical anatomic landmark. SIT has been related to a higher risk of cancer without evidences behind this hypothesis. CASE PRESENTATION: We report a 61-year-old male with SIT who presented with synchronous primitive pancreatic-head and lung cancers. After multidisciplinary disc

A Rare Presentation of X-Linked Primary Ciliary Dyskinesia Due to Xq22.3 Deletion .

BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare genetic disorder caused by mutations affecting the structure or function of motile cilia, with more than 50 causative genes identified to date. PCD is predominantly inherited in an autosomal recessive manner; however, X-linked recessive inheritance is exceptionally rare. CASE PRESENTATION: We report a male pediatric patient with recurrent pulmonary infections, chronic sinusitis, and situs inversus. Genetic analysis identified an approximately 720.3 kb hemizygous deletion in the Xq22.3 region, consistent with a rare X-linked recessive form

Asymmetrical modulation of fear expression via GABAB receptors in the mouse medial habenula.

The medial habenula (MHb) is implicated in regulating emotional responses to aversive events. Studies in zebrafish have identified a remarkable morphological left-right asymmetry in the dorsal habenula (zebrafish equivalent of mammalian MHb)-interpeduncular nucleus (IPN) pathway and its asymmetrical roles in behavior. However, there is little evidence for structural or functional lateralization in the mammalian MHb-IPN pathway. Here, we investigated the synaptic properties of left- and right-MHb afferents to the IPN and their roles in the expression of conditioned fear in mice. We found that e

Flipping the Script: Pharmacologic Myocardial Perfusion Imaging and Percutaneous Coronary Intervention in a Patient With Dextrocardia and Situs Inversus Totalis.

Situs inversus totalis (SIT) is an extremely rare congenital abnormality in which the positions of the body's organs are mirrored. It can be accompanied by dextrocardia, where the heart is located in the right thorax with its apex pointing to the right side of the body. Its rarity poses diagnostic and therapeutic challenges, particularly in patients with coronary artery disease (CAD). We report the case of a 74-year-old woman who presented with dyspnea. Her medical history included hypertension, diabetes mellitus, hypothyroidism, and dextrocardia with SIT, incidentally diagnosed at age 45 foll

Transcatheter closure of ostium secundum atrial septal defect in congenitally corrected transposition of great arteries with visceral situs inversus and dextrocardia: Rationale and technical challenges.

Congenitally corrected transposition of the great arteries (CC-TGA) may occur in isolation or may be associated with defects in the ventricular septum and stenosis of the pulmonary outflow tract. A large ostium secundum atrial septal defect (ASD) as the only other anomaly is rare in CC-TGA. We report successful use of an Amplatzer septal occluder to close an ASD in a patient with visceral situs inversus, dextrocardia, and CC-TGA.

When Fetal Anomalies Lead to Prognostic Clarification: An Underrecognized Application of Exome Sequencing.

OBJECTIVES: To describe a cohort of fetuses with abnormal prenatal findings in whom exome sequencing (ES) identified genetic diagnoses that refined prognostic assessment and reduced prognostic uncertainty. METHODS: We retrospectively reviewed all fetal ES studies performed at our center between 2017 and 2025 for structural or biochemical abnormalities. Cases with a definitive molecular diagnosis were identified, and those with predicted mild, isolated, transient, or treatable phenotypes were classified as having comparatively favorable prognostic implications. Clinical findings and expected p
