BACKGROUND: Fragile X Syndrome (FXS) is an X-linked neurodevelopmental disorder characterised by intellectual disability, developmental delays, anxiety, and social and behavioural challenges. Currently, no effective treatments exist to address the root cause of FXS. Mouse models are the most widely used for studying molecular pathogenesis and conducting preclinical treatment testing. However, therapeutic interventions that show promise in rodent models have yet to succeed in clinical trials. After evaluating the current models, we have developed an ovine model to address this clinical translat

BACKGROUND/OBJECTIVES: Angelman syndrome is a neurodevelopmental disorder resulting from a deficiency of the maternally inherited UBE3A gene. In mature neurons, UBE3A expression is restricted to the maternal allele due to tissue-specific genomic imprinting, while the paternal allele is silenced in cis by the UBE3A antisense transcript (UBE3A-ATS). To date, numerous strategies have been employed to activate paternal UBE3A expression. In this study, we utilized RNA interference (RNAi) to investigate the downregulation of UBE3A-ATS in mouse primary neurons and human induced pluripotent stem cell

BACKGROUND: Complex neurodevelopmental disorders frequently reflect multiple neurologic symptoms which have shared molecular and network level mechanisms. Advances in genomic medicine have redefined these conditions as overlapping manifestations of brain circuit dysfunction with significant variability. This review examines the intersection of genomics, epilepsy, and neurodevelopment in complex neurodevelopmental disorders, emphasizing Dup15q syndrome as a model for understanding phenotypic variability.
METHODS: Authors conducted a clinical (non-systematic) review of the literature based on t

BACKGROUND: Duplications involving Xp22.33, particularly within the pseudoautosomal region 1 (PAR1), are rare. While copy number variants (CNVs) involving SHOX, a dosage-sensitive gene in PAR1, are known to cause growth disorders, large duplications encompassing the entire PAR1 region and beyond show variable associations with skeletal and neurodevelopmental abnormalities. Duplication of the near-complete, isolated PAR1 with a comprehensive clinical description has not been reported.
CASE PRESENTATION: We report a male patient with a 2.49 Mb duplication encompassing nearly the entire PAR1 reg

Background: Neurodevelopmental disorders (NDDs) are a heterogeneous group of conditions characterized by cognitive, behavioral, and developmental impairments, frequently linked to structural genomic alterations. Copy number variants (CNVs) involving chromosome 16, particularly the short arm 16p, are recognized contributors to neurodevelopmental variability. Despite increasing international evidence, data from Italian clinical cohorts are still limited. Methods: We investigated 1200 patients referred for genetic evaluation due to suspected NDDs, including autism spectrum disorder (ASD), intelle

Aneuploidies-whole-chromosome copy number imbalances arising from nondisjunction-underlie numerous congenital and somatic disorders, but unlike many other disease-causing variants, they can readily revert back to euploidy through subsequent errors of the same type. The extent to which this inherent plasticity impacts the stability and persistence of aneuploid karyotypes in populations remains poorly understood, a gap in knowledge that continues to limit our understanding of aneuploidy-driven disease incidence, penetrance, and progression. To assess how reversion shapes aneuploid population dyn

Challenging behaviors are common in rare neurogenetic conditions and significantly impact family life. Families were especially vulnerable during COVID-19, yet little is known about behavior trajectories considering pre-pandemic risks. The Purdue Early Phenotype Study has followed children with and without neurogenetic conditions since 2017, including during the pandemic. Analyses examined patterns and predictors of challenging behaviors among 228 children (Williams = 52; Angelman = 64; Prader-Willi = 29; fragile X = 23; controls = 60), ages 1 month-10 years (57% male, predominantly White), fo

OBJECTIVE: Investigate transient, non-averaged electroencephalography (EEG) signals in the gamma frequency band during an auditory chirp paradigm in individuals with Fragile X syndrome (FXS), a population characterized by elevated background gamma power, and to assess their relationship with clinical features.
METHODS: Source-localized EEG responses to auditory chirp stimulus were studied in 36 individuals with FXS and 39 typically developing controls (TDC). Transient high-power gamma-band events were extracted using a thresholding approach. Gamma event properties were compared between groups

Intellectual disability (ID) and autism spectrum disorders (ASD) are complex neurodevelopmental disorders (NDDs) with a strong genetic component, and recent studies have highlighted the POGZ gene on chromosome 1q21.3 as a significant contributor, particularly linked to White-Sutton syndrome (WHSUS). POGZ-associated disorders are most commonly caused by heterozygous loss-of-function (LOF) variants, frequently arising de novo. In this study, we aimed to further this understanding by exploring novel pathogenic variants in the POGZ gene and their impact on the molecular landscape of NDDs. We condu

INTRODUCTION: It is unknown if neurodegeneration trajectories differ between Down syndrome (DS) and autosomal dominant Alzheimer's disease (ADAD), both of which are genetic forms of Alzheimer's disease (AD).
METHODS: We compared brain volumes in DS, ADAD, and unaffected family members serving as controls. Participants underwent magnetic resonance imaging (MRI) and amyloid positron emission tomography (PET), deriving volumetric and amyloid burden, respectively. Nonlinear associations between regional volumes and estimated years to clinical symptom onset (EYO) were evaluated using generalized a

Stimulants are the first-line pharmacological treatment for ADHD and generally effective, yet 35-61% of individuals discontinue treatment within a year. We investigated the contribution of common and rare genetic variants to early stimulant discontinuation using data from 18,362 individuals with ADHD (31% female) initiating stimulants in iPSYCH, a Danish population-based case-cohort linked to national registers. Discontinuation was defined as a ≥ 180-day gap between dispensations within one year of initiation. We examined genetic differences by age groups, estimated SNP-heritability (h2SNP), c

For many neurodevelopmental disorders, genetic treatment discovery is complicated by non-homology between human and non-human model system genomes. Antisense oligonucleotides (ASOs) influence protein expression by sequence-specific binding of RNA, resulting in targeted degradation or alternative splicing, and thereby provide a promising avenue for clinical therapeutics. Angelman Syndrome (AS), caused by loss-of-function of maternal UBE3A, has emerged as a monogenic neurodevelopmental disorder for which ASO therapy might be particularly amenable due to genomic imprinting. In an AS mouse model,

BACKGROUNDEstrogen deficiency and progressive hearing loss (HL) are significant concerns in individuals with Turner syndrome (TS). However, whether childhood estrogen deficiency increases HL risk and whether estrogen replacement therapy (ERT) prevents hearing deterioration are still unclear.METHODSThis prospective cohort study recruited children with TS from a tertiary referral center between 2016 and 2024. All participants received standardized recombinant human growth hormone therapy. Longitudinal monitoring data of hormone levels, metabolic parameters, and annual audiological examinations w

OBJECTIVE: Non-invasive prenatal testing (NIPT) is the most accurate method for screening major trisomies. However, its diffusion is limited by high costs, and in many countries, the first-tier combined test remains the standard of care. This study compared the performance and outcomes of the Vanadis NIPT platform at 10 weeks' gestation with the combined first-trimester test.
METHODS: This prospective cohort study included unselected women undergoing both first-tier NIPT and combined tests. Positive results led to genetic counselling and invasive procedures. Diagnostic accuracy was compared.

Chromoanasynthesis is a form of complex chromosomal rearrangement (CCR) commonly detected in cancers and congenital disorders, but the mechanism underlying its generation remain elusive. Here we develop a single-molecule long-read DNA sequencing approach to characterise ultra-complex mutational events, consistent with chromoanasynthesis, occurring at shortened telomeres and sub-telomeric DNA double-strand breaks in human cells. Our data reveal that chromoanasynthesis is generated by microhomology-mediated break-induced replication (MM-BIR), occurring specifically in mitosis. Surprisingly, this

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder caused by CGG repeat expansions in the FMR1 gene. While CGG repeat toxicity is established, the precise molecular mechanisms driving neurodegeneration remain unclear. Here, we show that a multi-omics strategy combined with TWAS reveals brain-region-specific molecular signatures and striking gene dysregulation in inhibitory neurons. Using conditional mouse models, we demonstrate that selective expression of expanded CGG repeats in GABAergic neurons is sufficient to recapitulate key pathologic hallmarks of FXTAS.

BACKGROUND: Decsions about prenatal screening for trisomies are complex for some women. Mobile decision aids (mDAs) can help them make informed decisions based on their values and preferences.
OBJECTIVE: To assess pregnant women's intention to use an mDA for prenatal screening of trisomies 21, 18, and 13.
METHODS: This mixed-methods study in Quebec recruited a purposeful sample of women aged 18 or over, at least 16 weeks pregnant or who had given birth in the past year after a low-risk pregnancy. After they completed a paper-based decision aid, we assessed their intention, and its psychosoci

Common fragile sites (CFSs) are large genomic loci that are frequently deleted under replication stress and are thought to play a role in carcinogenesis as well as developmental disorders during early embryogenesis. They often appear as gaps or breaks in mitotic chromosomes by conventional optical microscopy and are also loci that are replicated in mitosis in a process called mitotic DNA synthesis (MiDAS). However, we still have a very poor understanding of the potential mechanisms underlying their genomic instability. We recently found that in normal mitotic chromosomes, there is a two-level
