Risk-Stratified Cardiac Monitoring During Antipsychotic Treatment in Adolescents and Young Adults.

INTRODUCTION: Antipsychotic-associated heart rate-corrected QT (QTc) prolongation and sudden cardiac death are uncommon in young people, but risk is concentrated in patients with identifiable clinical vulnerability and during selected drug- or exposure-related situations. We mapped the evidence and developed a risk-stratified framework for clinical monitoring. METHODS: This structured narrative review searched PubMed in August 2026 using one comprehensive query and 41 targeted subqueries. After PMID-based deduplication, 934 unique records underwent title screening, 382 abstract review, and 15

Postoperative Delirium and the Association to Continuously Monitored Severe Deviations of Vital Signs at the General Ward After Major Abdominal Surgery.

BACKGROUND: Delirium is the most frequent neurological complication after surgery and is associated with increased postoperative morbidity and mortality. The underlying pathophysiology is not fully understood, but is suggested to be multifactorial, including hypoperfusion and hypoxia; however, the role of deviating vital signs prior to postoperative delirium (POD) remains unclear. This study aimed to examine the association between severe physiological deviations detected by continuous wireless monitoring and the occurrence of POD within the first three postoperative days after major abdominal

Blast injury simulation in pigs using an air-driven shock wave generator: effects of shock waves of lateral shock wave exposure.

PURPOSE: We have hypothesized that respiratory arrest caused by temporary brainstem dysfunction is the primary cause of death during the hyperacute phase of blast injury and have demonstrated that this is highly likely. To date, we have examined the effects of shock waves on pigs by exposing them to shock waves from both the dorsal and ventral sides. Therefore, in this study, we investigated the effects of lateral shock wave exposure in pigs and identified priority regions for protection against blast injuries. METHODS: Eight male hybrid pigs (age, 10-12 weeks; mean weight, 36.6 kg) were anes

The p.Arg52Leu variant in GNB2 causes a gain-of-function due to an increase of IK, ACh in stimulated hiPSC-CMs.

BACKGROUND: Initially discovered and published in 2017, the heterozygous mutation (p.Arg52Leu) in the GNB2 gene is associated with sinus node dysfunction (SND) and atrioventricular block (AVB). Electrophysiological characterization using heterologous expression systems has demonstrated that this R52L GNB2 variant results in a larger G-protein-activated K+ (GIRK/IK, ACh) current. So far, this has not been functionally validated in patient-derived disease modelling using human induced pluripotent stem cell-derived cardiomyocytes (CM). METHODS: Here, we differentiated CMs derived from human indu

Catecholaminergic polymorphic ventricular tachycardia with diagnostic difficulty differentiating seizures from syncope: a case report.

INTRODUCTION: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a potentially fatal inherited arrhythmia syndrome triggered by exercise or emotional stress that may mimic epileptic seizures, resulting in delayed diagnosis. CASE PRESENTATION: We report the case of a 6-year-old girl with recurrent transient loss of consciousness (T-LOC) who was initially misdiagnosed with epilepsy. She experienced three episodes of T-LOC between 4 and 6 years of age. The first two episodes occurred during or immediately post-exercise, whereas the third appeared non-exertional; however, retrospecti

Favorable outcome of fetal bradyarrhythmia: Two case reports and literature review.

Fetal bradycardia is primarily a symptom of fetal distress or intrauterine injury. This can result in fetal cardiac insufficiency, fetal edema, or even fetal death. This condition once perplexed doctors and often resulted in the induction of labor, breaking the hearts of many families. Here, we present two cases of fetal bradycardia with favorable outcomes, offering hope for the survival of affected babies.

Effects of dexmedetomidine on postoperative outcomes in adult non-cardiac surgical patients admitted to the intensive care unit: a systematic review and network meta-analysis of randomized controlled trials.

BACKGROUND: The role of dexmedetomidine in patients admitted to the intensive care unit (ICU) following non-cardiac surgery remains unclear. While dexmedetomidine is widely used for its sedative and potential delirium-sparing properties, its pooled effects on clinically relevant postoperative outcomes in this population are not well established. This systematic review and network meta-analysis evaluated the comparative effectiveness and safety of dexmedetomidine compared with other sedative strategies in postoperative non-cardiac surgical ICU patients. METHODS: We systematically searched PubM

Initial side effects of roxadustat on thyroid function in patients undergoing hemodialysis.

Roxadustat, a hypoxia-inducible factor prolyl hydroxylase (HIF-PH) inhibitor, is effective for treating renal anemia, with efficacy noninferior to erythropoiesis-stimulating agents. However, data on its adverse effects remain scarce. In medical practice, roxadustat administration reportedly leads to central hypothyroidism; however, the frequency of its occurrence remains unclear. We investigated thyroid function in patients undergoing dialysis who received roxadustat at Saiseikai Yokohamashi Tobu Hospital, with particular focus on the timing of the decrease in serum thyroid-stimulating hormone

Left Bundle Branch Area Pacing: Current Clinical Practice and Future Perspectives.

Conventional right ventricular pacing (RVP) is associated with electromechanical dyssynchrony and may contribute to pacing-induced cardiomyopathy (PICM). Left bundle branch area pacing (LBBAP) has emerged as a physiological pacing strategy that engages the native conduction system. This review summarizes the current clinical evidence and future perspectives for LBBAP in both bradycardia and heart failure indications. LBBAP has been associated with improved electrical synchrony, preservation of left ventricular function, and lower rates of heart failure hospitalization and mortality compared wi

The impact of Centruroides tecomanus scorpion venom on cardiac function in rats during early envenomation.

Scorpion envenomation is a medical emergency that can produce severe cardiovascular complications. Centruroides tecomanus, an endemic scorpion species from western Mexico, is medically relevant, although its early cardiac effects remain poorly characterized. This study evaluated the initial cardiotoxic effects of C. tecomanus venom in Wistar rats using serial electrocardiographic recordings, algorithm-based ECG signal processing, histopathology, and serum biochemical markers. Fourteen rats were allocated to control and venom-treated groups (n = 7 per group). The experimental group received an

Subcutaneous and Extravascular Implantable Cardioverter-Defibrillators: A Clinical Framework for Breast Surgery, Pregnancy, Lactation, Radiation Therapy, and Breast Imaging.

The subcutaneous implantable cardioverter-defibrillator and extravascular implantable cardioverter-defibrillator do not require intravascular, intracardiac, or epicardial leads, making them attractive alternatives to transvenous implantable cardioverter defibrillators in patients without bradycardia pacing or cardiac resynchronization therapy indications. These systems are especially useful for patients with venous access challenges and those at risk for endovascular infections, including patients with renal failure or those who are immunocompromised. Women with prior or anticipated breast sur

Cardiac Adaptations in Endurance Athletes: A Bibliometric Analysis of Exercise‑Induced Physiologic Remodeling.

BACKGROUND: Endurance exercise induces cardiovascular adaptations known as exercise-induced physiologic remodeling of the athlete's heart, including sinus bradycardia, chamber enlargement, enhanced vagal tone, and atrial and ventricular remodeling. These changes are generally physiologic, but concern remains about their association with arrhythmias and long-term outcomes. This study aimed to characterize the research landscape, identify influential contributors, and explore emerging themes in this field. METHODS: A bibliometric analysis was conducted using the Web of Science Core Collection d
