AMPK-mediated HCN4 channel phosphorylation contributes to age-related intrinsic bradycardia.

The regulation of the hyperpolarization-activated cyclic nucleotide-gated 4 (HCN4) channels in pacemaker myocytes is essential for maintaining physiological cardiac rhythm. HCN4 dysfunctional behavior is among the major factors contributing to sinus node disease, a primary cause of pacemaker implantation. Previous work has shown that AMP-activated protein kinase (AMPK) activation leads to sinus bradycardia, a process attributable to cardiac remodeling that involves a decrease in HCN4 membrane expression, but the mechanism underlying this event remains unclear. We show here that AMPK can act as

Comparison of opioid-free versus opioid-based total intravenous anaesthesia in elderly patients undergoing short-duration surgery: a randomized controlled trial.

INTRODUCTION: Older adults who undergo short-duration surgery are vulnerable to opioid-related complications. It is uncertain whether an opioid-free total intravenous anaesthesia (OFA) can reduce these events. We aimed to determine whether OFA reduces the incidence of major postoperative adverse events compared with standard opioid-based total intravenous anaesthesia (OBA). PATIENTS AND METHODS: This single-center randomized clinical trial was conducted in China. From May to August 2025, 400 patients aged ≥60 years undergoing elective, short-duration surgery (anticipated duration of less than

Surgical management of malignancy-associated carotid sinus syndrome: a case report and literature review.

A 59-year-old male was admitted to hospital for workup of a high-grade atrioventricular block, where he had recurrent episodes of symptomatic bradycardia and hypotension. Imaging performed in anticipation of pacemaker insertion demonstrated a right-sided neck mass obliterating the jugular vein and compressing the carotid artery. He continued to have episodes of hemodynamic instability despite transvenous pacing. Decision was made to proceed with excisional biopsy and surgical debulking as a therapeutic attempt for suspected mass effect on the carotid sinus. Intraoperatively, there was gross va

Catheter Ablation and Device Therapy in Patients With Transthyretin Amyloid Cardiomyopathy: A Review of Non-Pharmacological Therapy.

In recent years, the number of cases diagnosed with wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM) has been increasing. However, ATTRwt-CM frequently coexists with atrial fibrillation (AF), atrial flutter (AFL), and atrial tachycardia (AT), often necessitating management for arrhythmias. Additionally, ventricular arrhythmias sometimes occur, or conduction disturbances often develop, requiring management for bradycardia, frequently needing device therapy such as pacemakers, implantable cardioverter defibrillators (ICDs), or cardiac resynchronization therapy defibrillators. Therefore

Olanzapine Overdose-Induced Bradycardia in a Young Adult With Major Depressive Disorder: A Case Report and Literature Review.

Olanzapine, an atypical antipsychotic widely used for psychiatric disorders, is typically associated with tachycardia and QT prolongation in overdose, while bradycardia remains a rare and underrecognized effect. We report a 20-year-old woman with major depressive disorder who presented 4 h after ingesting 100 mg of olanzapine in a suicide attempt. She exhibited marked sinus bradycardia (41 bpm) with stable hemodynamics and no QT prolongation. Laboratory and toxicology results were unremarkable. Supportive care, including oxygen, fluids, and cardiac monitoring, led to complete recovery within 4

Palliative pacemaker deactivation for a pacing-dependent patient.

A man in his 90s was frail, symptomatic of heart failure and dependent on a pacemaker inserted decades prior for third-degree atrioventricular block. He requested his cardiologist to deactivate the pacemaker so he could die naturally of his underlying cardiac pathology. After 135 days of consultations and advice, the pacemaker was deactivated in a hospice in England. After deactivation, the patient developed bradycardia and hypoxia but remained comfortable and conversant, then gradually became unconscious. Opioid and benzodiazepine were administered at deactivation and later in response to mil

Survival benefit of guideline-directed medical therapy in older patients with heart failure: a systematic review and meta-analysis.

BACKGROUND: Heart failure-related mortality in older patients has increased. Guideline-directed medical therapy (GDMT) in heart failure with reduced ejection fraction (HFrEF) remains insufficient in this population. This study assessed the survival benefit of GDMT in elderly patients aged ≥65 years old with HFrEF. METHODS: MEDLINE, EMBASE and Cochrane Library were searched from inception to 5 February 2025. Inclusion criteria were age ≥65 years old with HFrEF, GDMT vs no GDMT and reported survival data. Pooled prevalence, HR and risk ratio (RR) with 95% CI were analysed. Heterogeneity was fur

Increased Arrhythmia Risk in Long COVID: A Systematic Review and Meta-Analysis.

BACKGROUND: COVID-19 infection can cause significant long-term health problems for patients. While there is no universally accepted definition for long COVID, it is usually identified by persistent symptoms that extend past 4 weeks after the initial SARS-CoV-2 infection with no other explanation. The cardiovascular system is one of the most important systems involved in long COVID, and even asymptomatic patients have evidence of cardiovascular injury after COVID-19. This study aims to determine the long-term risk of developing cardiac arrhythmias after SARS-CoV-2 infection. METHODS: A compreh

Whole-heart electro-mechanical optimization: an integrative systems approach to modern cardiac pacing.

INTRODUCTION: Modern-day cardiac pacing is no longer limited to the treatment of bradycardia but can be tailored to optimize cardiac hemodynamics through coordinated modulation of heart rate, atrioventricular (AV) coupling, and atrial and ventricular activation patterns as an integrated system. While traditional approaches have addressed these components in isolation, this review summarizes the mechanisms and clinical evidence of a 'whole-heart electro-mechanical optimization' approach for the treatment of bradycardia. AREAS COVERED: Accelerated atrial pacing can lower left-sided filling pres

Proactive haemodynamic management using the hypotension prediction index during caesarean section: a randomised controlled study.

INTRODUCTION: Intra-operative hypotension is common during caesarean section and may result in adverse maternal effects, such as nausea and vomiting. While oscillometric blood pressure and continuous non-invasive arterial pressure monitoring enable reactive treatment, the hypotension prediction index offers a machine learning-based approach that may allow proactive haemodynamic intervention. This study investigated whether hypotension prediction index-guided management could reduce the incidence of intra-operative hypotension compared with oscillometric and continuous non-invasive arterial pre

Targetable Effects of the Anesthetic, Ubiquinone-5, on Murine Cardiac Rhythm.

General anesthetics can adversely affect the heart, negatively impacting chronotropy, electrical conduction, and myocardial contractility. The intravenous sedative-hypnotic, propofol, for example, impairs ventricular contraction at clinically relevant doses and can cause dysrhythmias and atrioventricular block with acute administration. In addition, high cumulative propofol doses can induce bradyarrhythmias, cardiac conduction abnormalities, and myocardial failure. As with propofol, the recently identified intravenous anesthetic agent, ubiquinone-5 (Ub5), causes bradycardia and complete heart

Symptomatic sinus bradycardia during the recovery phase of high-altitude pulmonary edema: a case report.

This case report describes a 28-year-old healthy male who developed high-altitude pulmonary edema (HAPE) after a rapid ascent. While his condition improved with standard therapy (including corticosteroids, diuretics, and supplemental oxygen), he developed symptomatic sinus bradycardia (heart rate nadir: 37 bpm) 2 days after discontinuation of these medications. The bradycardia resolved with intravenous potassium supplementation and fluid resuscitation, and his heart rate recovered to 59 bpm. This case highlights that a rebound in vagal tone and steroid withdrawal may contribute to severe brady
