Early Transient Hypothyroidism Is Associated With Long-Term Outcomes of 131I Treatment in Graves' Disease: A Retrospective Cohort Study.

BACKGROUND: Radioactive iodine (131I) is a cornerstone therapy for Graves' disease (GD), yet predicting long-term thyroid functional outcomes remains challenging. Early transient hypothyroidism (ETH) is a common phenomenon post-therapy, but its prognostic value for differentiating between permanent hypothyroidism and persistent hyperthyroidism is not fully elucidated. METHODS: This retrospective cohort study included 222 GD patients treated with 131I. Patients were categorized based on thyroid function within the first 6 months: the ETH group (occurrence of transient hypothyroidism, n = 59) a

Effect of Multi-Antioxidant Supplement on Lipid Profile, Occupational Fatigue, Work Stress, and Hair Cortisol in Administrative Workers with and Without Obesity: A Quasi-Experimental Pilot Study.

Background/Objectives: Work stress (WS), occupational fatigue (OF), and Burnout syndrome (BS) among administrative workers are associated with negative psychosocial and metabolic effects. Although antioxidant-rich nutritional strategies have been proposed to help manage stress, evidence from real-world occupational settings is still limited. This study evaluated the total antioxidant capacity (TAC) of a multi-antioxidant dietary supplement 2.0 (DS2.0; apple polyphenols, [APP], astaxanthin [AXT], and fucoxanthin [FXT]; 387:12:1 ratio) and explored its association with metabolic parameters, OF,

Iatrogenic phrenic nerve injury in early-onset recurrent cardiac myxoma: a case report.

INTRODUCTION: Cardiac myxomas (CMs) are the most common benign primary heart tumors. CMs can present with diverse symptoms including embolic events and neurological signs. Diagnosis relies heavily on imaging modalities like transthoracic echocardiography. Surgical excision is the definitive treatment due to the high risk of embolism. CASE PRESENTATION: A 19-year-old woman presented with dyspnea, palpitations, and dizziness. Echocardiography revealed a right atrial mass. Surgical excision via sternotomy confirmed a 2 cm CM. Eighteen months later, the patient presented with symptomatic recurren

Dual AV Nodal Non-Reentrant Tachycardia as a Suspected Cause of Tachyarrhythmia-Induced Cardiomyopathy.

We describe the case of a 69-year-old female patient who presented with a sensation of her heart pounding and an electrocardiogram showing tachycardia with unusual grouped beating. She was recently diagnosed with heart failure with reduced ejection fraction presumed to be secondary to atrial fibrillation which did not improve with beta-blocker therapy. She underwent an electrophysiology study which demonstrated Dual AV nodal non-reentrant tachycardia status post slow pathway ablation. She had significant improvement in her ejection fraction and palpitations thereafter. This case highlights the

Left posterior fascicular ventricular tachycardia associated with a septal fissure complicated by iatrogenic ventricular septal defect: a case report.

BACKGROUND: Left posterior fascicular ventricular tachycardia (LPF-VT) is considered the most common type of fascicular ventricular tachycardia. False tendons (FTs) are reported to be common in patients with LPF-VT and may contribute to the pathogenesis of this condition. However, LPF-VT cases with congenital septal structural abnormalities have not been reported. CASE SUMMARY: This case report investigated an 86-year-old woman diagnosed with LPF-VT who underwent radiofrequency ablation. The preoperative transthoracic echocardiography (TTE) results indicated no anatomic abnormalities. During

Disulfiram-like reaction with ornidazole: A case report of a rare interaction.

A disulfiram-like reaction (DLR) associated with ornidazole is a rare but important adverse drug-alcohol interaction. We report a 44-year-old female with no known comorbidities or drug allergies who presented to the emergency department with palpitations, nausea, vomiting, facial flushing, and acral hyperemia. Her symptoms began approximately 30 min after consuming two glasses of raki while receiving ornidazole for a pilonidal cyst. On examination, she was found to be hypotensive and tachycardic. Laboratory results were within normal limits. Her symptoms improved significantly with supportive

Presumed Postural Tachycardia Syndrome: 12-Lead Electrocardiograms During Tilt Table Testing Unmask an Atrial Tachycardia.

BACKGROUND: Tilt table testing is commonly used to evaluate orthostatic intolerance and syncope and is typically performed with three-lead electrocardiography (ECG) monitoring, which limits P-wave interpretation. CASE SUMMARY: A 21-year-old woman with congenitally corrected transposition of the great arteries presented with recurrent tachy-palpitations and near syncope and was presumed to have postural tachycardia syndrome (POTS) after extensive noninvasive testing. During head-up tilt table testing incorporating intermittent 12-lead ECGs, she developed a sustained narrow-complex tachycardia

The Sound of Silence and its Echo.

BACKGROUND: Recognition of sinus echo beats on surface electrocardiography is exceedingly uncommon and may be confounded by sinus arrhythmia or premature atrial complexes with similar P-wave morphology. CASE SUMMARY: A 29-year-old postpartum woman presented with intermittent palpitations. A standard 12-lead electrocardiogram demonstrated sinus bradycardia with sinus arrhythmia and 2 premature atrial complexes exhibiting distinct electrophysiological behavior. Careful cycle-length analysis revealed that the first premature atrial complex reset the sinus node, whereas the second mimicked comple

Malignant Extra-adrenal Paraganglioma of the Bladder: A Rare Clinical Entity.

Bladder paragangliomas are uncommon extra-adrenal neuroendocrine tumors that arise from chromaffin tissue within the bladder wall. Clinical presentation varies from hematuria to catecholamine-related paroxysms, and malignant transformation, although uncommon, carries significant morbidity. We report a 45-year-old woman who presented with painless hematuria. Contrast-enhanced computed tomography (CECT) revealed a 3.6×3 cm lobulated, enhancing posterior bladder wall mass. Transurethral resection of the bladder tumor (TURBT) was performed, and histopathology confirmed paraganglioma. The patient w

Supravalvular catheter ablation of idiopathic ventricular tachycardia arising from the mitral annulus: a case report.

BACKGROUND: Although idiopathic ventricular arrhythmias most commonly originate from the outflow tracts, they may also arise from the mitral annulus and the left ventricular (LV) summit. While acute ablation outcomes have improved, catheter ablation of arrhythmias arising from these regions remains particularly challenging due to their complex anatomy. CASE SUMMARY: A 19-year-old male athlete experienced sustained palpitations during exercise, followed by syncope. An implantable loop recorder documented wide QRS complex tachycardia during exercise. An electrophysiological study induced ventri

Adolescent presentation of L-type ccTGA with pulmonary valve hypoplasia and RV dysfunction: from cardiogenic shock to surgical repair.

BACKGROUND: Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect involving both atrioventricular (AV) and ventriculoarterial (VA) discordance. This condition is further complicated by additional anomalies such as a hypoplastic pulmonary valve, ventricular septal defect (VSD), and dextroversion. These abnormalities may contribute to an increased risk of heart failure and cardiogenic shock, making early detection and appropriate intervention essential for better patient outcomes. CASE PRESENTATION: A 16-year-old female presented with progress

Exploratory research on drugs after lung surgery based on real-world data from the FDA adverse event reporting system database.

BACKGROUNDS: Lung surgery is the most common surgery in thoracic surgery, but postoperative dyspnea, cough, palpitations, and chest pain seriously affect the quality of life of patients and hinder their postoperative recovery. METHODS: To reveal the potential correlation between drugs and adverse events, we used disproportionation analysis to calculate the ROR and PRR values of the top 30 reported cases of dyspnea, cough, palpitations, and chest pain, respectively. In addition, we defined high and medium ROR signaling drugs after lung surgery by generating Venn diagram and calculated the PRR

Complicated third sternotomy for left ventricular outflow tract obstruction secondary to mitral valve prosthesis strut.

Introduction: LVOT obstruction after surgical bioprosthetic MVP placement is uncommon and the influence of the MVP strut width has seldom been examined. Case presentation: A 74-year-old female presented with palpitations, chest pain, and shortness of breath. She had MVR with 31-mm bioprosthetic valve 2 years prior. TEE confirmed LVOT obstruction secondary to a MVP strut, causing a peak gradient of 55 mmHg. The redo operation was complicated by iatrogenic Type A aortic dissection. The patient was immediately cooled for DHCA with RCP and ascending aortic replacement was performed. MVR was then p

Incessant Focal Atrial Tachycardia Originating From the Right Inferior Pulmonary Vein With Extremely Short Cycle Length and Variable Exit Block.

The underlying mechanism of focal atrial tachycardia (AT) is usually considered to be abnormal automaticity, typically with a cycle length (CL) exceeding 120 ms. We present a case of a 29-year-old female with recurrent symptomatic palpitations. She was referred for electrophysiologic study and catheter ablation. Twelve-lead ECG showed a sinus rhythm with incessant atrial ectopic beats and occasional aberrant conduction. P-wave morphology during ectopic revealed positive deflections in Leads I, II, aVL, aVF, and V1-V6, suggesting a left-sided origin. Intracardiac recordings obtained using a cir

Pulmonary vein and left atrial posterior wall isolation using a balloon-in-basket pulsed-field ablation catheter via the superior vena cava approach in a patient with interrupted inferior vena cava: a case report.

BACKGROUND: Pulmonary vein isolation (PVI) is the cornerstone of catheter ablation for atrial fibrillation (AF) and is typically performed via transfemoral access. In patients with interrupted inferior vena cava (IVC), however, alternative access routes such as the superior vena cava (SVC) approach are required. A balloon-in-basket (BiB)-pulsed field ablation (PFA) is a novel non-thermal ablation modality that offers stable positioning and uniform tissue contact. CASE SUMMARY: A 66-year-old man with symptomatic persistent AF, characterized by palpitations and interrupted, IVC was referred to
