Efficacy of L-methylfolate and methylcobalamin in treating resistant hypertension associated with elevated serum homocysteine in hemodialysis patients.

BACKGROUND: End-stage renal disease (ESRD) patients receiving hemodialysis are experiencing a considerable increase in the burden of cardiovascular diseases (CVDs). In this patient population, hypertension is a prevalent modifiable cardiovascular risk factor that is associated with poor prognosis. Resistant hypertension in dialysis patients is challenging to manage since some individuals do not respond to antihypertensive medications or volume control. Hyperhomocysteinemia is common among ESRD patients. "H-type hypertension" or hyperhomocysteinemia-associated hypertension refers to resistant h

Real-world outcomes of Finerenone in patients with diabetic kidney disease in Saudi Arabia.

BACKGROUND: Diabetic kidney disease (DKD) represents a significant microvascular complication associated with type 2 diabetes mellitus (T2DM), markedly elevating the risk of kidney failure, cardiovascular events, and premature mortality. Despite advancements in therapeutic management, such as renin-angiotensin-aldosterone system (RAAS) blockade and sodium-glucose cotransporter 2 (SGLT2) inhibitors, residual risk remains significant. Finerenone, a novel nonsteroidal and selective mineralocorticoid receptor antagonist (MRA), has demonstrated substantial cardiorenal benefits in clinical trials; h

Portal vein thrombosis increases short-term mortality among hospitalized patients with cirrhosis in a global cohort.

BACKGROUND: Portal vein thrombosis (PVT) in cirrhosis is associated with adverse outcomes, but determinants of PVT and its impact on short-term outcomes in hospitalized patients across global regions remain unclear. METHODS: The multinational CLEARED consortium prospectively enrolled adults with cirrhosis admitted non-electively after consent. Patients were followed during hospitalization and 30 days post-discharge. Radiologically diagnosed PVT was analyzed in relation to demography, etiology, clinical features, hospital course, and 30-day outcomes, including mortality. RESULTS: Among 7,721

Finerenone in Type 1 Diabetes and Chronic Kidney Disease.

BACKGROUND: The nonsteroidal mineralocorticoid receptor antagonist finerenone has been reported to improve kidney and cardiovascular outcomes in persons with type 2 diabetes and chronic kidney disease (CKD). The efficacy and safety of finerenone in persons with type 1 diabetes and CKD are unknown. METHODS: We conducted a phase 3 trial involving adults who had type 1 diabetes, CKD (estimated glomerular filtration rate [eGFR], 25 to <90 ml per minute per 1.73 m2 of body-surface area), and albuminuria (urinary albumin-to-creatinine ratio [with albumin measured in milligrams and creatinine measur

Home blood pressure telemonitoring reveals race-specific patterns of target organ damage.

BACKGROUND: Racial differences in cardiac and renal target organ damage (TOD) may persist at comparable blood pressure levels. This study compared TOD in high-risk, non-African-American Black and White patients in relation to the home blood pressure (HBP). METHODS: UPRIGHT-HTM (NCT04299529) is an ongoing international trial comparing risk stratification strategies in asymptomatic patients, aged 55-75  years, with ≥5 risk factors. Patients engage in HBP telemonitoring (OMRON HEM 9210-T). After 34.7 months (median), 287 Black and 154 White patients underwent echocardiography. At baseline, their

Evaluation of Blood Pressure Variability With Clevidipine Infusion in Patients With Acute Cerebrovascular Disease.

Blood pressure variability (BPV) has been associated with worse outcomes in patients with acute cerebrovascular diseases, and acute hypertension is routinely treated with intravenous antihypertensives.  Objective: The goal of this study was to determine if clevidipine infusions can safely and effectively reach blood pressure targets and maintain blood pressure control without undesirable variability.  Methods: This was a prospective, single-arm, observational study. We enrolled a convenience sample of adult patients hospitalized for acute ischemic stroke (AIS), intracerebral hemorrhage (ICH),

Unmasking a Hidden Burden: Chemotherapy-Induced and Exacerbated Diabetes in Paediatric Cancer: A Report of Two Cases.

UNLABELLED: L-asparaginase and corticosteroids, essential components of paediatric leukaemia and lymphoma treatment, are associated with hyperglycaemia and, rarely, diabetic ketoacidosis (DKA). Chemotherapy-induced diabetes is an under-recognised complication in pediatric oncology in resource-constrained settings. CASE 1: A 4-year-old female with newly diagnosed Acute Lymphoblastic Leukaemia (ALL) was commenced on chemotherapy with prednisolone, 6-mercaptopurine, vincristine, doxorubicin, cyclophosphamide, L-asparaginase and cytarabine. However, on the 7th day of initiating chemotherapy, urin

Beyond a Single Etiology: Tachy-Brady Syndrome and Possible Coexisting Focal Epilepsy Presenting as Transient Loss of Consciousness.

Transient loss of consciousness has overlapping cardiac and neurologic causes, and distinguishing syncope from seizure can be difficult. The co-occurrence of a primary cardiac rhythm disorder with a possible coexisting epileptic process is infrequently reported, as most cases instead describe one condition mimicking or triggering the other. We present a case of a 77-year-old woman with hypertension, hyperlipidemia, and paroxysmal atrial fibrillation who awoke with an abnormal chest sensation, a dream-like experience, and urinary incontinence and was admitted for evaluation of a possible seizur

Monospecific antibodies reveal reduction of granulins in frontotemporal dementia caused by GRN mutations.

PURPOSE: Primary open-angle glaucoma (POAG) is a common ocular disease, associated with abnormalities in aqueous humor circulation and an increase in intraocular pressure (IOP), leading to progressive optical neuropathy and loss of vision. POAG pathogenesis includes alterations of the structural properties of the sclera, especially in the optic nerve head area, contributing to the degeneration of the retinal ganglion cells. Abnormal sclera biomechanics hinder adequate compensation of IOP fluctuations, thus aggravating POAG progression. The proteomic basis of biomechanical disorders in glaucoma

Incidence of hypertension and factors associated with blood pressure control among older adults living with HIV in Western Kenya: a retrospective cohort study.

BACKGROUND: People living with HIV are living longer due to expanded access to antiretroviral treatment (ART). As they age, their risk of hypertension is greater due to HIV-immune activation and long-term use of some antiretrovirals. Screening and treatment of hypertension and monitoring hypertension control are key strategies for averting morbidity and mortality from cardiovascular disease and improving the health outcomes of older adults living with HIV (OALWH). We sought to estimate the incidence of hypertension and determine the proportion of blood pressure control among OALWH in western K

Timing of Orthostatic Blood Pressure Changes and Its Association With Orthostatic Symptoms and Falls at Very Old Ages: Findings From the ARIC Study.

BACKGROUND: Assessment and treatment of orthostatic hypotension (OH) are recommended to prevent falls in older adults. However, the optimal timing of standing blood pressure (BP) assessments to identify clinically relevant OH remains unclear. METHODS: We measured 3 supine BP readings after 5 minutes of rest at 30-second intervals and 6 standing BP readings (timed at 0, 1, 2, 3, 4, and 5 minutes after standing) during the ARIC study (Atherosclerosis Risk in Communities) visit 10. OH was defined as a drop in BP (systolic ≥20 mm Hg or diastolic ≥10 mm Hg) upon standing from the supine position.

Optimising the patient pathway for macular conditions requiring intravitreal injections.

Macular disease is the leading cause of vision loss in the United Kingdom, affecting nearly 1.5 million people. Vascular endothelial growth factor (VEGF) has a major role in the onset and progression of vision-threatening macular conditions such as neovascular age-related macular degeneration, diabetic macular oedema and retinal vein occlusion. Anti-VEGF treatments, given by intravitreal injection, can prevent vision loss and may improve vision but must be started quickly before sight loss is irreversible. Informed by an observational study using Hospital Episode Statistics and the Emergency C

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

The presence of excess leucocytes in the semen has been associated with male infertility. According to the WHO, concentrations of more than 106 leucocytes ml-1 are considered as leucocytospermia, indicating genital tract infections. Up to now, no consensus has been achieved on how leucocytes should be quantified in semen. Using the peroxidase staining and monoclonal antibodies to CD15, CD45 and CD68, we found significant differences between the detection methods. Only 47.4% of the semen samples that were assessed as leucocytospermic by CD45 were identified as such by peroxidase staining. The c
