Measurement and management of adult blood pressure in the peri-operative period: updated guidelines from the Association of Anaesthetists and the British and Irish Hypertension Society.

INTRODUCTION: Maintaining stable blood pressure during surgery is a key responsibility of anaesthetists. Peri-operative omission and reintroduction of antihypertensive drugs, general anaesthesia, neuraxial and regional techniques can all cause significant fluctuations in blood pressure, particularly in patients with hypertension. Since the first edition of this guideline there has been more literature regarding peri-operative management, but some areas still lack standardisation. METHODS: This was a planned update of the 2016 guidelines from the Association of Anaesthetists and British Hypert

Undiagnosed and uncontrolled hypertension at a federally qualified health center.

Hypertension is a preventable risk factor for cardiovascular disease and mortality yet is often undiagnosed and uncontrolled. Federally Qualified Health Centers (FQHC) serve vulnerable, understudied populations having characteristics that are associated with greater risk for undiagnosed and uncontrolled hypertension. This study aimed to determine the burden and risk factors associated with undiagnosed and uncontrolled hypertension at a large FQHC, with the co-primary outcomes of yearly rates of undiagnosed and uncontrolled hypertension. The design was a retrospective cross-sectional study of a

Design of Heart Well: A randomized pilot study of a community health worker-delivered nutrition intervention for low-income adults with hypertension.

BACKGROUND: Suboptimal diet and food insecurity contribute to hypertension disparities experienced by individuals with low-income. Clinic-based nutrition counseling is limited by time constraints and low access to dietitians. Integrating community health workers (CHWs) with basic nutrition training into care teams may be an effective strategy to promote sustainable dietary modifications. OBJECTIVE: Describe the design of Heart Well, a study to determine whether adding nutrition counseling to a primary care-based CHW hypertension program is feasible and acceptable and improves diet and blood p

Effect of Antihypertensive Treatment on Hypotension, Mortality and Length of Stay in Orthopedic Trauma and First Detected High Blood Pressure Adults in a Large Urban Hospital: A Retrospective Cohort Study.

Trauma is a frequent cause of hospital admission in adults, and while arterial hypertension is common in the general population, reactive hypertension associated with trauma remains poorly studied from a therapeutic perspective. This retrospective cohort study aimed to estimate the impact of antihypertensive treatment on hypotension occurrence, in-hospital mortality, and length of stay in patients with musculoskeletal trauma. We analyzed data from a high-complexity center in Colombia between 2020 and 2024, including adults over 18 years with musculoskeletal trauma who had no previously known h

Randomised controlled effectiveness study (RCT) of isometric exercise (IE) in adults with stage 1 and 2 hypertension - ISOFITTER study.

Background High blood pressure (BP) affects more than one in four adults in England and only one in three patients are being treated effectively. Treatment of high BP includes changes to lifestyle such as more physical activity and/or taking medication. However, low adoption and high attrition are common with the large amounts of exercise currently recommended (>150 minutes moderate exercise per week plus 2 strength sessions). Evidence suggests that isometric exercise (IE), holding a fixed body position for a period of time, for example a wall squat, results in greater long term BP reductions,

Barriers to effective hypertension control in a low-income healthcare setting: The role of therapeutic inertia and its predictors among hypertensive outpatients.

Despite the rising burden of hypertension (HTN) across Africa, the rate of blood pressure (BP) control among hypertensive patients remains unacceptably low. One of the key contributors to this treatment gap is therapeutic inertia (TI) the failure to initiate or intensify therapy when clinically indicated. However, evidence on the magnitude and determinants of TI in Ethiopia remains scarce. Objective: To assess the prevalence of therapeutic inertia and identify associated factors among hypertensive patients attending the outpatient department of Wolaita Sodo University Comprehensive Specialized

Resistant Hypertension: Integration of Novel Agents and Interventional Approaches in Clinical Practice.

Resistant hypertension (RH) is a high-risk phenotype characterized by blood pressure readings ≥130/80 mmHg despite maximally tolerated therapy with three antihypertensive agents, including a diuretic, or controlled blood pressure requiring four or more medications. The diagnosis of RH requires a structured evaluation that confirms accurate blood pressure measurement, excludes pseudoresistance-particularly nonadherence and white coat hypertension-and identifies secondary causes such as obstructive sleep apnea, primary aldosteronism, renovascular disease, pheochromocytoma, and Cushing syndrome.

Prevalence and factors associated with isolated systolic hypertension among middle-aged adults and older Filipinos: a population-based study.

BACKGROUND: Isolated systolic hypertension (ISH), defined as blood pressure of ≥140/<90 mmHg, is associated with an increased risk of cardiovascular complications yet few estimates of prevalence are recorded globally. Data on blood pressure measurements of adult Filipinos were taken as one of the variables in the National Nutrition Survey; however, the prevalence of ISH and its associated risk factors has not been investigated. This study aimed to determine the prevalence and factors associated with ISH among middle-aged (40-59 years) and older (60 and above) Filipinos. METHODS: A secondary d

Polycythemia vera as a cause of systemic hypertension.

An underlying medical condition as a cause for elevated blood pressure is suspected when hypertension occurs in younger children or those with markedly high readings (hypertensive urgency or emergency). We highlight a unique presentation and underlying medical condition, polycythemia vera (PV), as the cause of secondary hypertension in an asymptomatic 5-year-old child presenting for a routine physical examination and found to have hypertensive urgency. Laboratory evaluations revealed leukocytosis, polycythemia, thrombocytosis, and low erythropoietin level. JAK2-V617F mutational analysis confir

Optimising Prescription of Treatment In older patients with Mild hypertension at Increased risk of Serious adverse Events (OPTIMISE2): protocol for a primary care based, open-label, randomised controlled non-inferiority trial.

BACKGROUND: Antihypertensive treatment is effective at reducing the risk of cardiovascular disease, but is associated with adverse events, particularly in older patients with frailty. As a result, deprescribing antihypertensive medications is recommended in some clinical guidelines despite limited evidence from a few small randomised controlled trials. The aim of the OPTIMISE2 trial is to examine the safety, efficacy and cost-effectiveness of deprescribing antihypertensive treatment in older adults with controlled systolic blood pressure, who are at higher risk of adverse events.
