A male in his late fifties, a chronic smoker, presented to his primary care physician with multiple painless subcutaneous swellings over the torso, which were misdiagnosed as a cutaneous infection for more than a month. Later, he developed haemoptysis for which he presented to our tertiary care subspecialty unit. Further evaluation with imaging revealed a left lung mass with adrenal and gastric lesions, raising suspicion of a lung malignancy with metastasis. A biopsy from a chest wall swelling confirmed primary lung small cell carcinoma with positive synaptophysin expression. Despite timely wh

Anti-neutrophil cytoplasmic antibodies (ANCA) in the serum of patients with inflammatory bowel disease (IBD) are usually considered an ancillary finding of uncertain clinical significance. We report a patient with longstanding ulcerative colitis who presented with haemoptysis which became severe, concurrent with the worsening of the colitis. Imaging, absence of an alternative aetiology, and rapid response to high-dose methylprednisolone in the presence of p-ANCA suggested an IBD-related small vessel pulmonary vasculitis, a rare observation.

Spontaneous intercostal herniation of the lung (SLIH) is a rare effect of a stubborn cough characterized by protrusion of the lung parenchyma through the thoracic wall. With varying presentations from chest pain and haemoptysis to severe respiratory distress, SLIH often goes overlooked as a potential differential and has no set gold standard of diagnosis or treatment. Here, a middle-aged male with minimal risk factors developed an anterolateral left lung herniation between the 7th and 8th ribs requiring surgical repair. Early recognition via computed topography (CT) and operative management ma

A woman in her 20s was referred to renal clinic for investigation regarding positive anti-glomerular basement membrane (anti-GBM) antibody results. Autoimmune screening was performed to investigate an episode of fever, cough and haemoptysis which presented alongside the formation of a maculopapular rash and mild proteinuria (1+) on urine reagent dipstick.Despite repeated positive anti-GBM results obtained through BioPlex 2200 multiple antibody (multiplex) immunoassay, examination four months later was unremarkable, all signs and symptoms being resolved. Urinalysis, imaging and renal function t

BACKGROUND AND OBJECTIVE: The expected right atrial pressure (RAP) to pulmonary arterial wedge pressure (PAWP) ratio is < 1. Inversion of this ratio (RAP ≥ PAWP) has been linked to poor outcomes in pulmonary arterial hypertension, but its relevance in chronic thromboembolic pulmonary hypertension (CTEPH) is unknown. For patients undergoing pulmonary endarterectomy (PEA), this ratio may be associated with postoperative outcomes and guide postoperative management.
METHODS: We retrospectively reviewed consecutive CTEPH patients who underwent PEA at the University of California San Diego from Jan

Systemic artery-to-pulmonary artery fistulas (SA-PAFs) are rare vascular anomalies, typically associated with chronic pulmonary or pleural inflammation. SA-PAFs are categorised by their systemic arterial origin, arising either from the bronchial arteries or nonbronchial systemic arteries such as the intercostal arteries. Nonbronchial SA-PAFs are uncommon in patients without a history of thoracic trauma, surgery or infection. We report a 55-year-old man with recurrent haemoptysis caused by right intercostal artery-to-pulmonary artery fistulas, despite having no history of pulmonary or pleural d

Metastasising ameloblastoma (MA) is a rare odontogenic tumour with an unpredictable natural history, capable of presenting decades after primary resection. Pulmonary involvement is the most common site of distant spread; however, endobronchial disease-causing airway obstruction and haemoptysis is exceptionally rare, with very few cases reported in the literature. Existing reports describe modest and short-lived responses to endobronchial therapy, with rapid recurrence. We report a case of bilateral endobronchial MA presenting 31 years after mandibular resection, where surgical intervention was

A 39-year-old man with recurrent spontaneous (including bilateral) pneumothoraces and haemoptysis had unusual thoracic computed tomography (CT) abnormalities. In the absence of typical phenotypic features, these radiological findings prompted genetic testing, which confirmed a pathogenic COL3A1 mutation and established the diagnosis of vascular Ehlers-Danlos syndrome. Recognizing these radiological patterns prevents misdiagnosis and enables crucial, potentially life-saving vascular management.

Coronary-bronchial artery fistulas (CBAFs) represent a rare subset of coronary artery fistulas characterised by an abnormal communication between an epicardial coronary artery and the bronchial arterial circulation. Although historically considered incidental findings, the widespread use of multimodality cardiovascular imaging-particularly coronary computed tomography angiography-has led to increasing recognition of these anomalies in contemporary clinical practice. The clinical significance of CBAFs varies widely and depends primarily on fistula size, shunt magnitude, and associated cardiopul

Objective: To evaluate the safety and efficacy of an electrical impedance tomography (EIT)-guided airway clearance protocol in mechanically ventilated patients. Methods: A total of 62 mechanically ventilated patients admitted to the intensive care units of two tertiary Grade A hospitals in Beijing between January 2024 and April 2025 were enrolled. Patients were allocated into two groups: the intervention group (n=31) received EIT-guided airway clearance, while the control group (n=31) received conventional airway clearance based on admission chest CT findings. Airway clearance efficacy and saf

We report a rare and potentially life-threatening case of a young male with Behçet's disease complicated by Hughes-Stovin syndrome who presented with massive haemoptysis secondary to multiple pulmonary artery aneurysms with associated thrombosis. The patient presented to the emergency department with a 20-day history of cough and fever, followed by acute worsening haemoptysis over 2 days, accompanied by haemodynamic instability. Initial CT pulmonary angiography demonstrated multiple bilateral saccular pulmonary artery aneurysms involving the descending and segmental branches, with partial and

BACKGROUND: N-butyl cyanoacrylate (NBCA) embolisation is an effective technique for rapid haemostasis and tumour devascularisation. However, selection of a single glue dilution requires balancing distal penetration against proximal control, often relying heavily on operator experience. Achieving both deep distribution and stable proximal occlusion can be challenging, particularly in complex lesions.
TECHNIQUE AND CASE PRESENTATION: We describe a sequential dual-dilution NBCA embolisation technique ("Nitoryu" technique), in which a dilute NBCA-lipiodol mixture is first administered to achieve

Pulmonary sequestration is a rare congenital malformation of the lower respiratory tract, while situs inversus totalis-characterized by complete transposition of the thoracic and abdominal viscera-represents an equally uncommon developmental anomaly. We report a case of multiple intralobar pulmonary sequestration coexisting with situs inversus totalis in a patient who presented to the emergency department with acute onset haemoptysis. The perioperative management was notably challenging due to the presence of an aberrant feeding artery and the atypical anatomical orientation of the lesions. Th

Pulmonary artery pseudoaneurysms (PAPAs) are very rare but potentially life-threatening complications of blunt thoracic traumas. They can present either as incidental findings or with severe haemorrhagic symptoms, including massive haemoptysis. We present a case of a man in his 30s who developed a PAPA following severe blunt thoracic trauma after a tractor accident. The patient's symptoms developed only weeks after the initial injury and included severe haemoptysis, desaturation and cardiac arrest. The pseudoaneurysm was successfully treated via endovascular approach with an 8 mm Amplatzer Vas

BACKGROUND: PD-1 and PD-L1 inhibitors have been shown to synergise with anti-angiogenic agents in non-small-cell lung cancer (NSCLC). We aimed to compare benmelstobart plus anlotinib with pembrolizumab in patients with previously untreated, driver gene-negative, PD-L1-positive, advanced NSCLC.
METHODS: The blinded, randomised, controlled, phase 3 CAMPASS trial was conducted in 79 centres across China. Patients aged 18-75 years with stage IIIB-IV squamous or non-squamous NSCLC, no previous systemic treatment for advanced, recurrent or metastatic diseases, a PD-L1 tumour proportion score of 1%

Anti-glomerular basement membrane (anti-GBM) disease is a rare, aggressive autoimmune condition that typically presents as rapidly progressive glomerulonephritis and can be complicated by pulmonary haemorrhage. The co-occurrence of anti-GBM disease and IgA nephropathy (IgAN) is extremely uncommon, though increasingly recognised as a distinct clinical and pathological entity. We describe a 51-year-old Sudanese man who presented with acute hypoxaemic respiratory failure, haemoptysis, severe anaemia, and acute kidney injury. Testing showed high levels of anti-GBM antibodies, with negative anti-ne

INTRODUCTION: Intravenous (IV) tranexamic acid (TXA), an antifibrinolytic agent, is routinely used in the treatment of non-massive haemoptysis. Topical TXA has shown haemostatic efficacy in surgical settings and epistaxis. Limited case reports have documented successful management of pulmonary bleeding using nebulised TXA. This study was conducted to evaluate the efficacy of nebulised TXA compared to IV TXA in non-massive haemoptysis.
MATERIALS AND METHODS: This single-centre, randomised controlled trial included 46 adult patients with non-massive haemoptysis presenting to the Emergency Medic
