Surgical intervention in bronchiectasis and pulmonary tuberculosis.

Bronchiectasis and pulmonary tuberculosis (TB) remain common causes of chronic respiratory morbidity worldwide, despite major advances in antimicrobial therapy and structured airway clearance strategies. In a subset of patients, medical treatment alone is insufficient, and surgical intervention may be required when disease is anatomically localised or when serious complications develop. From a surgical perspective, resection of non-functional, chronically infected lung segments in bronchiectasis can lead to sustained symptom improvement, fewer exacerbations, and effective control of haemoptysi

Massive haemoptysis in a patient with right pulmonary artery atresia with contralateral pulmonary artery pseudoaneurysm: a therapeutic dilemma in collateral-dependent pulmonary circulation.

A woman in her 30s presented with recurrent massive haemoptysis and chronic respiratory symptoms, including fever, cough and breathlessness. She had previously received empirical anti-tubercular therapy without microbiological confirmation or clinical improvement. CT angiography revealed right pulmonary artery atresia with extensive bronchial and non-bronchial systemic collaterals, a partially thrombosed pseudoaneurysm arising from a left lower lobe segmental branch with associated pulmonary infarction and bilateral bronchiectatic changes. She underwent embolisation of the pseudoaneurysm and s

A 25-year-old pregnant woman with recurrent haemoptysis.

Early recognition of hereditary haemorrhagic telangiectasia in pregnancy is crucial. Pulmonary arteriovenous malformations pose high maternal-fetal risk, but timely embolisation in the second trimester with multidisciplinary care ensures safe outcomes. https://bit.ly/44SpaI4.

Risk factors and causes of early death in germ cell tumors: a Global Society for Rare GU tumors study.

BACKGROUND: Germ cell tumors (GCTs) are highly curable, yet a subset of patients with metastatic disease experience early death (ED) soon after starting first-line chemotherapy. These patients are underrepresented in trials, and risk factors remain unclear. METHODS: We performed a retrospective multicenter cohort study including adults ( ≥ 18 years) with metastatic GCT who died within 3 months of completing their last cycle of first-line chemotherapy. Primary outcomes were cause and timing of death; secondary endpoints included clinical predictors of acute respiratory failure (ARF) and very e

Therapeutic evolution and outcomes in EGPA complicated by diffuse alveolar haemorrhage: Case-based review.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare, systemic vasculitis characterised by asthma, eosinophilia, and multi-organ involvement. Diffuse alveolar haemorrhage (DAH) is an uncommon, but life-threatening pulmonary complication in EGPA. We report on a 49-year-old, previously healthy woman who presented initially with asthma-like symptoms and later developed a fever, haemoptysis, cutaneous purpura, and periorbital oedema. Laboratory evaluation revealed marked eosinophilia, anaemia, elevated inflammatory markers, and a strongly positive MPO-ANCA. Bronchoalveolar lavage fluid w

A pulsatile endobronchial lesion: A vascular mimic of airway pathology.

We describe the case of a 68-year-old woman who presented with intermittent haemoptysis and a significant family history of lung cancer. A non-contrast CT thorax was unremarkable aside from minor right lower lobe scarring. Flexible bronchoscopy demonstrated a pulsatile endobronchial lesion within the posterior basal segment of the right lower lobe bronchus. Given its pulsatile appearance, biopsy was deferred because of concern for an underlying vascular lesion. Bronchoalveolar lavage was negative for malignancy and infection. Subsequent rigid bronchoscopy confirmed the lesion, with no evidence

Shear-wave elastography-guided transthoracic biopsy for lung lesions: a randomised controlled trial.

BACKGROUND: Transthoracic shear-wave elastography (SWE) facilitates differentiation between benign and malignant lesions. This pragmatic randomised controlled trial aimed to validate the effectiveness and safety of SWE-guided transthoracic biopsy for diagnosing subpleural lung lesions. METHODS: Eligible patients with peripheral lung lesions suspicious for lung malignancy and suitable for biopsy were randomised in a 1:1 ratio to the SWE-guided or conventional ultrasound-guided transthoracic biopsy groups. Rapid on-site cytological evaluations were performed to guide adjustments to biopsy targe

Intracardiac and pulmonary arterial thrombosis as a first presentation of Behçet's disease.

Behçet's disease (BD) is a rare vasculitis characterised by mucocutaneous lesions and ocular, vascular and neurological manifestations. We report a man in his early 20s who presented with dyspnoea, haemoptysis, pulmonary arterial thromboses and a large right ventricular thrombus. Initial investigations for infectious and autoimmune causes were negative, complicating diagnosis. A revisited history and new examination revealed oral and genital aphthosis, fulfilling the international criteria for BD. The patient was treated with anticoagulation and high-dose immunosuppression, resulting in rapid

Nebulized amphotericin B deoxycholate in management of chronic pulmonary aspergillosis: experience from a tertiary care centre in North India.

BACKGROUND: Chronic pulmonary aspergillosis (CPA) requires prolonged oral azole therapy, yet 30%-40% experience relapse after treatment discontinuation. The role of nebulized amphotericin B (NAB) in CPA as a maintenance therapy remains unexplored. METHODS: We retrospectively reviewed CPA patients treated with NAB deoxycholate (10 mg twice daily) preparation administered either on alternate days (three times weekly) or six times weekly (6/7-day). The primary objective was to compare the efficacy and safety of these two dosage schedules in maintaining CPA remission. RESULTS: We included 36 sub

Massive pulmonary embolism complicated by pulmonary infarction, haemoptysis and hemothorax.

Pulmonary infarction is an uncommon but clinically significant complication of acute pulmonary embolism (PE), resulting from obstruction of distal pulmonary arteries leading to ischaemia, haemorrhage and necrosis of lung parenchyma. We report a case of a man in his 60s who presented with cardiac arrest secondary to massive PE and was treated with systemic thrombolysis. His course was complicated by pulmonary infarction with massive haemoptysis and haemothorax. Due to life-threatening bleeding, anticoagulation was temporarily withheld and an inferior vena cava filter was placed. The patient was

Hydatid Cyst of the Right Atrium With Hepatic and Pulmonary Involvement and Concomitant Pulmonary Embolism, Presenting With Massive Haemoptysis: A Case Report.

Hydatid cyst disease, caused by Echinococcus granulosus, primarily affects the liver and lungs, while cardiac involvement, especially the right atrium (RA), is extremely rare. We report the case of a 21-year-old woman who presented with massive haemoptysis and hypoxemia. Transthoracic echocardiography demonstrated a large multicystic mass occupying the RA cavity, adherent to the atrial wall and interatrial septum, strongly suggestive of a hydatid cyst. Chest computed tomography angiography confirmed multiple pulmonary cystic lesions, intravascular filling defects consistent with pulmonary embo

Changes in lung ultrasound presentation induced by breath-hold diving in a simulated depth competition at Taiwan.

INTRODUCTION: Acute respiratory symptoms after diving are common among competitive breath-hold divers. These symptoms, including shortness of breath, cough, haemoptysis, and chest discomfort, are often linked to immersion pulmonary oedema (IPO) or pulmonary barotrauma. This study aimed to evaluate the incidence, clinical presentation, and risk factors of IPO using portable ultrasound devices in a depth competition for breath-hold divers in Taiwan. METHODS: This observational study was conducted during a competition around Liuqiu Island, Taiwan. Twenty-five breath-hold divers participated. Lun

Predictors of Recurrence Following Bronchial Artery Embolisation (BAE) for Haemoptysis: A Systematic Review and Evidence Synthesis Analyses Using SWiM Guidelines.

PURPOSE: This systematic review aims to identify independent etiological, anatomical, and procedural predictors of recurrent haemoptysis following Bronchial artery embolisation (BAE) using a Synthesis Without Meta-analysis (SWiM) framework. MATERIALS AND METHODS: PubMed, Cochrane, and Scopus were systematically searched in February 2026 for cohort studies analysing post-BAE haemoptysis recurrence in line with PRISMA guidelines. Data were synthesised using a SWiM approach as clinical heterogeneity and intra-cohort mixing of embolic agents precluded a meta-analysis. Multivariable evidence was p

Chronic pulmonary aspergillosis and sarcoidosis.

PURPOSE OF REVIEW: This review addresses the intersection between chronic pulmonary aspergillosis (CPA) and sarcoidosis, a clinically important but relatively understudied association. While few new sarcoidosis-specific data have emerged, recent advances in CPA research across broader underlying conditions (particularly structural lung diseases) provide updated insights into diagnosis, antifungal therapy, and management strategies that can be extrapolated to sarcoidosis. RECENT FINDINGS: CPA affects ~2% of sarcoidosis patients in tertiary cohorts and arises from combined structural lung damag

Negative Pressure Pulmonary Oedema With Pulmonary Haemorrhage Following Brief Neck Compression in an Adolescent.

Negative pressure pulmonary oedema (NPPE) is well recognised in peri-operative settings but is infrequently reported following brief non-operative airway obstruction, particularly in adolescents. We report the case of an adolescent male who developed NPPE with features of concurrent negative pressure pulmonary haemorrhage (NPPH) following approximately 90 s of neck compression. He experienced transient loss of consciousness with hypoxic myoclonic activity, followed by haemoptysis and hypoxia requiring supplemental oxygen. Chest X-ray (CXR) revealed bilateral opacities, which was followed up by

Cryobiopsy in the Diagnosis of Pulmonary Vein Stenosis After Catheter Ablation of Atrial Fibrillation: A Case Report.

Pulmonary vein stenosis-a complication of atrial fibrillation ablation unfamiliar to most pulmonologists-was identified in a 42-year-old man who presented with haemoptysis. Initial bronchoscopy at a previously visited hospital revealed a central airway lesion, but biopsy was declined because of its haemorrhagic nature. Repeat bronchoscopy at our hospital showed oedematous changes and vascular proliferation in the left main bronchus, suggesting adenoid cystic carcinoma. However, cryobiopsy excluded malignancy and suggested a vascular malformation with thickened, irregularly dilated muscular ves

Mucoepidermoid carcinoma originates from the submucosal glands of the tracheobronchial tree. It is structurally homologous with exocrine salivary glands. It is a rare tumour, consisting of 0.1% to 0.2% of primary lung malignancies.Complete surgical resection is the treatment of choice and is associated with an excellent prognosis. Here we present a case where a boy in his teenage years came with a dry cough and haemoptysis.Contrast-enhanced CT confirmed the presence of a polypoidal enhancing space-occupying lesion arising from the right anterolateral aspect of the carina, protruding into the ri
