Clofazimine-induced interstitial lung disease with haemoptysis during therapy for Mycobacterium abscessus pulmonary disease.

We report a woman in her late 60s with a 6-month history of productive cough, low-grade fever and fatigue. Bronchoscopy yielded Mycobacterium abscessus, and she was diagnosed with M. abscessus pulmonary disease. She initially received intravenous imipenem and amikacin plus oral clarithromycin for 1 month, followed by maintenance therapy with clarithromycin and clofazimine (CFZ). One year after starting CFZ, she developed high fever, worsening cough, exertional dyspnoea and haemoptysis. Chest high-resolution CT revealed bilateral upper-lobe predominant reticular ground-glass opacities, bronchov

A rare cause of massive haemoptysis in an adolescent.

Haemoptysis is defined as the expectoration of blood or blood-tinged sputum originating from the lower respiratory tract. Massive haemoptysis, although rare, can often cause life-threatening asphyxia, respiratory failure, shock and, rarely, mortality. Pulmonary tuberculosis (PTB), although common, rarely presents with massive haemoptysis. This case report highlights an adolescent male who presented with life-threatening haemoptysis owing to PTB which was managed by successful pulmonary vascular embolisation.Abbreviations: AV: arteriovenous; BAE: bronchial artery embolisation; BPF: bronchopulmo

Tuberculosis or vasculitis? Granulomatous inflammation of the pancreatic tail as an atypical presentation of granulomatosis with polyangiitis.

Granulomatosis with polyangiitis (GPA), a subtype of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV), is a systemic inflammatory disease that typically affects the respiratory tract and kidneys. However, atypical pancreatic manifestations have been reported, which may present clinically as acute pancreatitis or exocrine insufficiency, and radiologically as pancreatic enlargement or pseudotumour.A female in her 40s presented with nasal crusting and epistaxis. She subsequently developed fever, weight loss, haemoptysis, bilateral pulmonary nodules and a pancreatic tail mass

Histological insights into sudden, unexpected death due to tuberculosis: two autopsy case reports and review of the literature.

We report two singular cases of sudden death in which the cause was advanced tuberculosis infection. In the first case, a 24-year-old man died suddenly following massive hemoptysis due to erosion of the pulmonary vessels. The second case involved a 29-year-old man who died unexpectedly from asphyxia secondary to hemoptysis caused by fibrocavitatory tuberculosis. Toxicological screening and HIV testing were negative in both cases. Medico-legal autopsy, combined with detailed histological examination, was essential to determine the exact cause of death.

Guidance for chest-CT in children and adults with cystic fibrosis: A European perspective (part 2: special situations/respiratory complications).

This manuscript covers guidance proposed by The European Cystic Fibrosis Society Clinical Trials Network (ECFS-CTN) for the use of chest computed tomography (CT) scans in special situations such as haemoptysis, aspergillus-related lung disease, non-tuberculous mycobacterial (NTM) infection and pneumothorax in people with CF. Guidance development followed a three-step process. The most important questions were first identified via anonymous online survey, then a comprehensive literature search was made to support a first set of statements. They were finally validated by a Delphi process to obta

Histologic Assessment of Medial and Lateral Menisci in Degenerative Knees Using Special Stains: A Histomorphometric Study.

Forequarter amputation (interscapulothoracic amputation) includes surgical removal of an upper limb and the shoulder girdle, including the scapula and a portion of the clavicle. We aim to report about our recent experience of having to resort to this mutilating surgery and the clinicopathological variables in that context. The study was done at a cancer centre in Northeast India. It was an ambispective study design, where the patient cohort who underwent FQA was identified retrospectively from the operative register of major surgeries for the time period 1st June 2020 to 31st May 2022 (24 mont

Deciphering the "Doxycycline Deficiency Syndrome": Defining the Characteristic Clinical, Laboratory and Imaging Findings in Patients Presenting with Leptospirosis, Q Fever and Rickettsial Diseases in Far North Queensland, Tropical Australia.

Background: The "doxycycline deficiency syndrome" is a colloquial term used in tropical Australia to describe the overlapping, non-specific findings seen in leptospirosis, Q fever, and the rickettsial diseases scrub typhus and Queensland tick typhus (QTT). A laboratory confirmed diagnosis is frequently delayed, however, early differentiation of these potentially life-threatening infections has important implications for clinical management. Methods: We examined consecutive laboratory-confirmed cases of leptospirosis, Q fever, and rickettsial diseases managed at a referral centre in tropical Au

Recurrent Cryptogenic Ulcerative Tracheobronchitis 6 Years After Lung Transplantation: Case Report.

BACKGROUND: Tracheobronchitis is an uncommon pattern of airway disease and is typically caused by infection, inhalation injury, infiltrative disorders and more rarely, systemic autoimmune disease. Rarer again is the development of bronchial webbing following tracheobronchitis. The underlying aetiology is usually identified through biopsy, culture or a detailed clinical history. We present a case of cryptogenic ulcerative tracheobronchitis with bronchial webbing being successfully managed by bilateral lung transplantation that has, however, been complicated by recurrence. CASE PRESENTATION: In

Pseudo-heart failure in pregnancy complicated by preeclampsia: giant mediastinal teratoma with extrinsic cardiopulmonary compression.

A woman in her early 20s at 30 weeks of gestation presented with elevated blood pressure, transaminitis, progressive dyspnoea, orthopnoea, exertional chest pain and peripheral oedema, raising concern for preeclampsia-associated cardiopulmonary disease or peripartum cardiomyopathy. Transthoracic echocardiography showed preserved left ventricular systolic function, normal left-sided filling pressures, right ventricular dilatation and an estimated right atrial pressure of 15 mmHg; N-terminal pro-brain natriuretic peptide was 57 pg/mL. Cross-sectional imaging revealed a large multi-cystic anterior

When rare becomes real: diagnostic challenges and use of a multidisciplinary team-based decision in primary pulmonary synovial sarcoma.

Primary pulmonary synovial sarcoma (PPSS) is a rare and aggressive malignancy accounting for 0.5% of lung cancers, with a 5-year survival rate of only 30%-50%. Diagnosis is challenging as it has no specific clinical or radiological features that distinguish it from other lung malignancies. We report a man in his 20s who presented with a 6-week history of cough and haemoptysis. Initial chest CT revealed a 2.2 cm endobronchial lesion in the left mainstem bronchus. Bronchoscopy confirmed the obstructing mass but initial biopsy was non-diagnostic. Positron emission tomography-CT scan demonstrated r

Paediatric tracheal carcinoid with carinal involvement and nodal metastasis: challenging airway reconstruction with oncological clearance.

Tracheal carcinoid tumours are exceptionally rare neuroendocrine neoplasms, particularly in the paediatric population. We present the case of a teenage girl with longstanding dyspnoea and intermittent haemoptysis, ultimately diagnosed with a typical carcinoid involving the distal trachea and both main bronchi. Bronchoscopy biopsy confirmed the diagnosis. She underwent tracheal resection and complex carinal reconstruction with cardiopulmonary bypass. Histopathology revealed a grade 1 typical carcinoid with metastatic involvement in two of five subcarinal lymph nodes. Postoperative recovery was

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A rare case: adult cor triatriatum sinister with tricuspid regurgitation.

BACKGROUND: Cor triatriatum sinister is a rare congenital cardiac anomaly, occurring in approximately 1 in 1,000 congenital cases. Although typically diagnosed in infancy because of significant haemodynamic consequences, cases in adulthood are uncommon and may present with nonspecific symptoms. CASE PRESENTATION: A 31-year-old male presented with progressive dyspnoea and haemoptysis. Transthoracic echocardiography revealed a fibrous membrane dividing the left atrium, with a 5-mm fenestration permitting communication between an accessory chamber and the main left atrial cavity; four pulmonary

Imaging and endovascular management of haemoptysis in congenital heart disease.

Haemoptysis, a rare but serious complication that can arise in patients with congenital heart disease, necessitates prompt diagnosis and specialized care. The radiologist plays a critical role in this scenario, including identifying the source of haemoptysis, devising treatment plans, and delivering endovascular interventions. This article highlights the importance of imaging techniques, especially computed tomography, in identifying the cause of haemoptysis and the therapeutic value of endovascular interventions. Furthermore, a suggested algorithmic approach is presented to assist with the di

Same story, different endings: clinical course and outcomes of two dogs treated differently for delayed fulminant pulmonary haemorrhage 20 h after eastern brown snake (Pseudonaja textilis) envenomation.

This report presents the clinical course and outcomes of two dogs that were treated differently upon recognition of respiratory distress due to delayed fulminant pulmonary haemorrhage (DFPH) 20 h after eastern brown snake envenomation. Two dogs from the same household were likely envenomated at the same time. Pulmonary haemorrhage was diagnosed based on pleural and lung ultrasound, decreasing packed cell volume and haemoptysis. Case 1 received a total of 7000 units of brown snake antivenom within the first 10 h after envenomation. The dog developed severe respiratory distress and anaemia appro

Pulmonary sequestration 1: right lower lobe.

Pulmonary sequestration is a rare congenital anomaly, characterized by aberrant lung tissue supplied by an aberrant systemic artery or arteries coursing within the inferior pulmonary ligament. The intralobar variety is the most frequent form. Clinical presentation may include recurrent haemoptysis and infection. Standard treatment involves surgical resection of the affected tissue with maximal preservation of healthy lung. This video tutorial presents the surgical technique for resecting intralobar pulmonary sequestration involving the right lower lobe. Rather than attempting to skeletonize th

Haemoptysis as a presentation of an infected aortic aneurysm rupture.

INTRODUCTION: Infective thoracic aortic aneurysms are uncommon, especially presenting with haemoptysis. CASE PRESENTATION: We report the case of an 81-year-old male who presented with fever and pleuritic chest pain and was initially misdiagnosed with community-acquired pneumonia. A CT scan later  revealed a saccular, ruptured thoracic aortic aneurysm. Despite antibiotic therapy, the patient developed haemoptysis, necessitating thoracic endovascular aortic repair (TEVAR). Post-procedure, the patient showed significant clinical improvement and was discharged in stable condition 45 days later.
