Cerebellar Ataxia, Neuropathy, and Vestibular Areflexia Syndrome (CANVAS): A case report of RFC1-associated disease.

Cerebellar ataxia, neuropathy, and vestibular areflexia syndrome (CANVAS) is a late-onset neurodegenerative disorder associated with biallelic RFC1 repeat expansions. We report a 55-year-old woman with progressive imbalance, sensory neuronopathy, bilateral vestibular hypofunction, chronic cough, and orthostatic symptoms. MRI demonstrated cerebellar atrophy involving the vermis, particularly the posterior vermis, with additional involvement of the posterosuperior cerebellar hemispheres, including Crus I, and upper cervical spinal cord atrophy. Integration of the characteristic imaging pattern w

Splenic Silicosis Accompanying Chronic Complicated Silicosis: A Rare Systemic Manifestation of Silica Exposure - A Case Report.

Silicosis is an occupational pulmonary disease caused by prolonged inhalation of crystalline silica particles. Although it predominantly affects thoracic structures, extrapulmonary or systemic manifestations are rare. We report a 60-year-old male who worked as a grinder in a quartz mill for 7 years and presented with advanced dyspnea and chronic cough. Imaging revealed bilateral progressive massive fibrosis and radiodense nodules in the spleen. Splenic involvement, also known as splenic silicosis, is an exceptionally rare entity and was detected incidentally during radiological evaluation. Thi

Unravelling A Rare Diagnosis: Pulmonary Alveolar Proteinosis.

Pulmonary alveolar proteinosis (PAP) is a rare disorder characterized by abnormal accumulation of surfactant within the alveoli, leading to impaired gas exchange and progressive respiratory symptoms. We report a 40-year-old stone grinder presenting with chronic cough, dyspnea, weight loss and hypoxemia. High-resolution computed tomography showed bilateral ground-glass opacities, while bronchoalveolar lavage (BAL) yielded characteristic milky fluid. Cytological examination demonstrated PAS-positive eosinophilic material with foamy macrophages, confirming secondary PAP related to silica exposure

Estimating the Range and Duration of Post-influenza Symptoms, Conditions, and Diseases.

BACKGROUND: Acute symptoms of influenza are understood; however, less is known about potential postacute sequelae. We conducted a retrospective matched cohort study to identify conditions with elevated risk following influenza. METHODS: We analyzed data from the Merative MarketScan Research Databases (2001-2019), including 3.3 million patients with a diagnosis of influenza. Cases were matched 1:1 to patients based on similar age, sex, care setting, and enrollment period with no influenza diagnosis. We examined the frequency of diagnosis codes in the year before and after the index influenza e

Efficacy and safety of negative-pressure nasal irrigation in pediatric chronic rhinosinusitis: A retrospective cohort study.

BACKGROUND: Pediatric chronic rhinosinusitis (CRS) affects 6.37% of Chinese children. Negative-pressure nasal irrigation (NNPI) is recommended by Chinese guidelines, but international evidence remains limited. OBJECTIVE: To evaluate efficacy and safety of NNPI combined with standard medical therapy in pediatric CRS. METHODS: Retrospective cohort study of 200 children (3-6 years) with CRS. One hundred children receiving NNPI plus standard therapy were matched with 100 receiving standard therapy alone for 14 days. All children were treatment-naïve to intranasal corticosteroids and nasal irriga

A complex pediatric cervical lymphadenopathy: case report.

BACKGROUND: Cervical lymphadenopathy associated with Mycoplasma pneumoniae (M. pneumoniae) is an uncommon manifestation. Nevertheless, it should be considered in the differential diagnosis of prolonged fever combined with other symptoms and not overlooked as a potential underlying cause. CASE DESCRIPTION: This case report describes a 5-year-old girl presenting with high-grade fever, cough, neck and throat pain, and enlarged cervical lymph nodes. At initial presentation, this case appeared relatively straightforward, with laboratory findings suggestive of a bacterial infection. Accordingly, a

Cultivar differentiation of fritillary using machine learning model-driven untargeted metabolomics.

Fritillaria cirrhosa (F. cirrhosa), a medicinal and edible herbs, is highly regarded for its remarkable efficacy in relieving cough and eliminating phlegm. However, surging demand and wild resource shortages have caused widespread adulteration of F. thunbergii with F. ussuriensis. Therefore, conducting a comparative study of these types of fritillaries may be helpful for the quality control and identification of the F. cirrhosa medicinal material. This study aims to conduct a comparative study of the chemical components of three types of fritillaries by integrating plant metabolomics combined

Decoding functional cough in pediatrics: differentiating tic, psychogenic, and compulsive phenotypes for targeted intervention.

Chronic cough without clear cardiopulmonary pathology poses diagnostic and therapeutic challenges in children and adolescents. While psychosocial factors play a crucial role in its pathogenesis, historical labels such as "habit cough" or "psychogenic cough" have often been vague and stigmatizing. Despite contemporary consensus guidelines advocating for the term "somatic cough syndrome," unified diagnostic criteria and standardized care pathways remain lacking. A rapid review approach was employed to search the PubMed database for empirical studies published between 2021 and May 2026 on chronic

Laryngopharyngeal Symptoms and Laryngopharyngeal Reflux Disease: Diagnosis and Treatment.

Chronic cough, hoarseness, and other laryngopharyngeal symptoms (LPS) are common complaints that often prompt a referral to an allergist/immunologist. Laryngopharyngeal reflux disease (LPRD) can cause LPS, and due to its symptom overlap with many other conditions, it is critical to understand how to clinically evaluate and treat this disease. Assessing for the presence or absence of concomitant gastroesophageal reflux disease symptoms is critical. The importance of direct visualization via laryngoscopy, and in some cases endoscopic evaluation and pH testing, also highlight the importance of ta
