Validity of EQ-5D-5L breathing and cognition bolt-ons in non-hospitalized patients after COVID-19.

PURPOSE: The EQ-5D has been criticized for lacking dimensions for breathing and cognition. In acute COVID-19 and Long COVID, breathing problems and cognitive complaints are common. This study evaluated the inclusion of EQ-5D-5L (bolt-on) items that assess these dimensions. METHODS: In a follow-up of community-based non-hospitalized patients with COVID-19 in 2020 (n = 450), we used respiratory and cognitive bolt-on items alongside other questionnaires about 30 months later (n = 220). We assessed data quality and construct validity for the bolt-ons, including comparison with concurrently used i

Evaluation and validation of a revised VetCompass clinical grading tool for heat-related illness in dogs.

The original VetCompass clinical grading tool for heat-related illness (HRI) in dogs was developed using general primary-care clinical records. Using two novel HRI emergency-care datasets from Vets Now clinics during 2022 and 2023, this current study aimed to firstly evaluate and revise the original tool, then to validate the revised tool. Evaluation of the original grading tool using 364 HRI cases treated during 2022 prompted three revisions; dyspnoea moved from Mild to Moderate grades, episodic collapse from Moderate to Mild, and new critical temperature criteria (≥ 41.0 °C) were added to bo

Frequent users of the ambulance service in a Swedish region: a retrospective cohort study.

BACKGROUND: This study aimed to describe the characteristics of frequent users of ambulance services and their corresponding ambulance missions. METHODS: A retrospective cohort study using register data from a Swedish region (approximately 308,000 inhabitants) was conducted. The dataset covered ambulance missions dispatched between 2019 and 2023; data from 2020-2022 were analyzed, with the extended timeframe (2019-2023) used to identify frequent users. Frequent users were defined as patients with four or more ambulance missions within a 365-day period, divided into moderate (patients with 4-1

Non-antimicrobial interventions in recovery from community-acquired pneumonia in adults.

PURPOSE OF REVIEW: We review recent evidence on the effectiveness of non-antimicrobial adjunctive interventions on the recovery of adults diagnosed with community-acquired pneumonia (CAP). RECENT FINDINGS: Respiratory physiotherapy, early mobilization or tailored exercises may decrease length of stay (LoS), dyspnoea and readmissions, but there is little evidence of an effect on mortality. Nutritional interventions may decrease readmissions and improve 30-day mortality, but there are few studies on the effect of individual micronutrient supplementation. Strategies to improve discharge communic

Dyspnoea-referenced minimal clinically important difference in the resting inspiratory capacity response to short-acting bronchodilators in COPD patients.

BACKGROUND: In COPD, the clinical significance of bronchodilator-induced changes in inspiratory capacity (IC) at rest remains uncertain. Unlike forced expiratory volume in one second (FEV1), IC may better reflect symptom improvement, but no minimal clinically important difference (MCID) has been formally established. RESEARCH QUESTION: To relate short-acting bronchodilator (SABD)-induced dyspnoea relief in stable COPD patients at rest with the concomitant IC changes and determine the MCID of the latter. STUDY DESIGN AND METHODS: In 106 stable COPD outpatients undergoing routine lung function

Concavity of the maximal expiratory flow-volume curve, and incidence of COPD and respiratory symptoms: a population-based cohort study.

BACKGROUND: Concavity of the maximal expiratory flow-volume curve is widely regarded as an early indicator of obstructive airways disease. However, its discriminatory accuracy for respiratory outcomes has remained poorly defined. We aimed to examine the discriminatory accuracy of concavity of the maximal expiratory flow-volume curve versus post-bronchodilator (BD) forced expiratory volume in 1 s/forced vital capacity (FEV1/FVC) as predictors of incidence of COPD and respiratory symptoms. METHODS: Spirometry was performed on a subset of the Tasmanian Longitudinal Health Study cohort at age 45

Sequential radiation-induced and immune checkpoint inhibitor-induced pneumonitis in a patient with breast carcinoma.

We report a woman in her early 60s with right breast carcinoma who received chemotherapy, whole breast radiotherapy and pembrolizumab (immune checkpoint inhibitor). During the sixth pembrolizumab cycle, she developed progressive cough and exertional dyspnoea. CT of the thorax revealed a right upper lobe consolidation with air bronchogram and surrounding ground-glass opacities, while bronchoalveolar lavage excluded infection and malignancy. A diagnosis of overlapping radiation-induced and pembrolizumab-induced pneumonitis was made. Oral corticosteroids led to rapid clinical improvement, with ra

Assessing congestion in heart failure: tools are already available, but integration is key.

Congestion represents the principal clinical manifestation and hemodynamic hallmark of heart failure (HF), reflecting elevated cardiac filling pressures. It remains the leading cause of HF hospitalization and is consistently associated with adverse outcomes across all stages of the disease. Despite its major prognostic implications, the assessment of congestion remains suboptimal in both acute and chronic settings, with up to one-third of patients discharged while still fluid overloaded. This review, developed following the Critical Care Clinical Trialists (3CT) Workshop 2025, summarizes curre

Preoperative Botulinum Toxin for Complex Diaphragmatic Paralysis: A Case Series.

INTRODUCTION: The management of giant diaphragmatic paralysis remains a significant surgical challenge, frequently associated with high rates of recurrence and the risk of developing abdominal compartment syndrome. While the use of Botulinum Toxin Type A (BTX- A) as an adjuvant therapy has been established in complex ventral hernia repair, its application in diaphragmatic paralysis is novel and sparsely documented. This study aims to present our institutional experience with BTX-A as a prehabilitation strategy in patients with complex diaphragmatic paralysis and to evaluate short- and long-ter

The relationship between muscle strength and oxygenation and activities of daily living in adolescents and young adults with pulmonary arterial hypertension.

Pulmonary arterial hypertension leads to dyspnoea, fatigue, and oxygen desaturation, limiting activities of daily living and functional capacity. This study aimed to evaluate the impact of pulmonary arterial hypertension on activities of daily living performance, functional capacity, peripheral muscle strength, oxygen dynamics, and energy expenditure. Eighteen adolescents and young adults with pulmonary arterial hypertension (10 females, 8 males; age 9-30 years) and 15 healthy controls (10 males, 5 females; age 9-30 years) were included. Peripheral muscle strength was assessed using a handheld

Unmasking hepatopulmonary syndrome: the 6-min walk test as a key to a missed diagnosis.

Hepatopulmonary syndrome (HPS) is an under-recognised complication of chronic liver disease, affecting 10%-30% of patients with cirrhosis. It involves intrapulmonary vascular dilatations, arterial hypoxaemia and an elevated alveolar-arterial gradient, causing impaired functional status and increased peri-transplant morbidity. Early recognition is crucial for timely liver transplantation referral. We report a woman in her mid-50s with alcohol-related liver disease admitted for a urinary tract infection, in whom severe exertional hypoxaemia was incidentally revealed. She developed positional dysp

Evaluating Changes in Physical Activity and Clinical Outcomes During Post-Hospitalisation Rehabilitation for Persons with COPD: A Prospective Observational Cohort Study.

Physical activity often remains low after hospitalisation for acute exacerbation of Chronic Obstructive Pulmonary Disease (AECOPD). Although post-hospitalisation rehabilitation has shown to support recovery, its impact on daily activity levels in the early post-exacerbation phase is unclear. This study describes the changes in physical activity (PA) and clinical outcomes during an 8-week rehabilitation following hospitalisation for AECOPD. This prospective observational cohort study included patients recently discharged after AECOPD from Aalborg University Hospital, Denmark. Participants recei

Time to next procedure in patients with malignant pleural effusion undergoing aspiration: derivation and initial validation of the RED score.

INTRODUCTION: In patients with malignant pleural effusions (MPE), pleural fluid reaccumulates at variable rates following therapeutic aspiration. The aim of this study was to identify variables which predict time to next procedure and use them to develop a predictive score. METHODS: This prospective observational cohort study in 10 British hospitals recruited patients with known or suspected malignant effusions undergoing therapeutic aspiration. Follow-up lasted 3 months and assessed time to next clinically indicated pleural procedure. Regression analysis was performed to identify independent

SSVEP-based brain-computer interface enabling graded dyspnoea self-report: proof-of-concept study in healthy volunteers.

BACKGROUND: Mechanically ventilated patients may experience respiratory suffering, which is difficult to assess when verbal communication is impaired. We evaluated the performance of a steady-state visual evoked potential (SSVEP)-based brain-computer interface (BCI) designed to enable self-reporting of dyspnoea in this context. METHODS: Forty-nine healthy volunteers were studied under five respiratory conditions: normal breathing (NB), inspiratory resistive loading (IRL), inspiratory threshold loading (ITL), CO₂ inhalation (CO₂), and a return to NB as wash-out (NBWO). Respiratory discomfort w

Intestinal acinar adenocarcinoma in a blue-fronted Amazon parrot (Amazona aestiva).

A 54-year-old captive male blue-fronted Amazon parrot (Amazona aestiva) was presented with progressive weight loss, diarrhoea and dyspnoea. Contrast radiographs suggested a neoplastic process by revealing an opaque soft tissue mass occupying most of the coelomic cavity. Post-mortem findings included approximately 80 ml of serous ascitic fluid. The intestines had a diffuse firm consistency, and the serosal surface was covered by multifocal to coalescent, irregularly shaped, white to off-white elevated plaques and nodules, which obscured visualization of the pancreas. Histologically, there was a
