Hereditary myopathy with early respiratory failure.

Neuromuscular diseases can present with acute respiratory failure with no other symptoms. A 30-year-old woman presented with progressive dyspnoea, culminating in respiratory failure requiring critical care admission for non-invasive ventilation. On examination, she had proximal and distal muscle weakness with bilateral scapular winging. An MR scan of the muscle showed selective fatty replacement in the semitendinosus, tibialis anterior and peroneus longus muscles. Muscle biopsy identified rimmed vacuoles, core-like structures and desmin-positive aggregates. Genetic testing identified a novel h

Heart failure therapy in patients with advanced cancer receiving specialized palliative care (EMPATICC trial).

BACKGROUND AND AIMS: Advanced cancer may resemble a heart failure (HF)-like phenotype marked by cardiac wasting, dyspnoea, congestion, and/or physical dysfunction. The trial evaluated safety and efficacy of HF therapy among patients with advanced cancer receiving specialized palliative care to improve patients' self-care ability. METHODS: Patients with stage 4 solid tumours with a life expectancy of 1-6 months receiving specialized palliative care were enrolled. Patients were required to meet at least two cardiovascular risk criteria and at least one criterion for functional limitation. Parti

Red blood cell transfusions in advanced cancer near the end-of-life: narrative review.

BACKGROUND: Red blood cell (RBC) transfusions are commonly used to manage anemia-related symptoms, such as fatigue and dyspnea in patients with advanced cancer. Their use in palliative care (PC) remains controversial due to the lack of clear guidelines, potential risks, and ethical considerations. OBJECTIVE: This narrative review aims to evaluate the role of RBC transfusions in patients with advanced cancer and those at the end of life. METHODS AND DATA SOURCES: A comprehensive review of relevant existing literature was performed in various databases, using combinations of relevant keywords.

Efficacy of fentanyl buccal tablet and morphine for exertional dyspnoea in patients with cancer: a double-blind, placebo-controlled, randomised clinical trial.

INTRODUCTION: Exertional dyspnoea is highly debilitating. Previous single-dose studies found that prophylactic administration of fentanyl buccal tablet (FBT) improved exertional dyspnoea. In this randomised trial, we compared daily use of prophylactic FBT, oral morphine and placebo on exertional dyspnoea over 14 days. METHODS: This parallel, double-blind, randomised clinical trial enrolled opioid-tolerant patients with cancer and exertional dyspnoea. After a 5-day observation period, patients were randomised (1:1:1) to receive FBT, morphine or placebo 30 min before daily structured activity f

Full spectrum of symptoms associated with heart failure: a mixed method systematic review and narrative synthesis.

AIMS: To investigate European Society of Cardiology (ESC) guideline-defined 'typical' and 'less typical' heart failure (HF) symptoms, as well as non-ESC symptoms, and to explore how ESC-defined symptoms vary by care setting, age, and sex. METHODS AND RESULTS: Mixed-method systematic review and narrative synthesis through six electronic databases. Quality was assessed using Joanna Briggs Institute critical appraisal checklists. Symptoms were grouped into ESC (typical/less typical) and non-ESC symptoms. Differences in ESC typical and less typical symptoms were investigated in hospital vs. commu

Multidisciplinary, non-pharmacological breathlessness intervention service for patients with moderately severe to severe COPD: a randomised controlled trial.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is an often-progressive respiratory disease associated with disabling breathlessness. Breathlessness intervention services (BIS), which coach patients to self-manage breathlessness using non-pharmacological strategies, are effective in a variety of populations, including those with cancer and serious respiratory disease. This study aimed to compare the impact of the Westmead Breathlessness Service in people with moderate to severe COPD. METHODS: We analysed 113 participants randomised (1:1) with moderate/severe COPD (forced expiratory v

Computed tomography total vessel count and vessel count percentages: associations with exercise capacity, symptoms and rapid FEV1 decline in COPD.

BACKGROUND: Existing computed tomography (CT) vascular pruning measures rely on volumes, such as the proportion of blood volume in vessels with cross-sectional area (CSA) ≤5 mm2 or CSA ≤10 mm2/total blood vessel volume (BV5/TBV or BV10/TBV, respectively), but may underestimate vascular pruning due to blood redistribution to larger vessels in individuals with milder COPD. We aim to develop a novel CT vessel measure, the total vessel count (TVC), quantify small and combined small/intermediate vessel count percentages, and compare these measures with BV5/TBV and BV10/TBV in terms of associations

A point-of-care ultrasound-driven diagnostic pathway for emergency department patients with dyspnoea: a randomised controlled trial.

BACKGROUND: Previous trials have suggested that point-of-care ultrasound (POCUS) for emergency department (ED) patients with dyspnoea increases the proportion of patients discharged within 24 h. We aimed to assess whether this effect could be confirmed. METHODS: This trial was a randomised controlled trial in 10 Danish EDs. Adult patients presenting to the ED with dyspnoea as chief complaint were randomised to the addition or omission of focused lung and cardiac ultrasound. The primary outcome was the proportion of patients discharged alive within 24 h. Secondary outcomes included overall hos

Late Embolization of a Cor-Knot Fastener Device Resulting in Left Anterior Descending Artery Occlusion.

A 57-year-old female patient underwent elective minimally invasive mitral valve repair surgery. Automatic suture fasteners were used for annuloplasty ring implantation. Six months after surgery, the patient presented with new onset exertional dyspnoea, fatigue, and thoracic pain. In transthoracic echocardiography, new regional wall motion abnormalities of the left ventricular apex were seen. A heart catheterization was performed, identifying 2 metallic foreign bodies in the left anterior descending artery, causing an occlusion of the vessel. Cardiac imaging revealed a transmural scar in the ap

Development and validation of PRECISE-X model: predicting first severe exacerbation in COPD.

OBJECTIVES: In patients with chronic obstructive pulmonary disease (COPD), severe exacerbations (ECOPDs) impose significant morbidity and mortality. Current guidelines emphasise using ECOPD history to inform preventive treatments but offer limited guidance for risk stratification for the first severe ECOPD. METHODS: We developed and validated PRECISE-X using a cohort of newly diagnosed COPD patients from the UK's Clinical Practice Research Datalink (2004-2022), to predict first severe ECOPD over 5 years (primary outcome) and 12 months (secondary outcome). Predictors were selected via clinical

Characterisation of the gut-lung axis microbiome in clinically stable patients with chronic obstructive pulmonary disease.

BACKGROUND: Airway and gut dysbiosis have been reported in Chronic Obstructive Pulmonary Disease (COPD); however, their relationship and association with clinical features remain poorly understood. We aimed to characterise the lung and gut microbiome in patients with stable COPD and controls. METHODS: Prospective, multicentre, longitudinal and controlled study of n = 60 stable patients with COPD and n = 30 controls. In them, we analysed 16S rRNA-seq in oropharyngeal (OP) swabs, sputum, bronchoalveolar lavage fluid (BALF) and stool. Weighted gene co-expression network analysis (WGCNA) was empl

Neutrophil-to-lymphocyte ratio and C-reactive protein in patients admitted with Acute Dyspnoea suspected of acute heart failure.

BACKGROUND: Inflammation contributes to adverse outcomes among patients with acute dyspnoea. This study evaluates the prognostic relevance of Neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) in patients presenting to the emergency department (ED) with acute dyspnoea, including those with acute heart failure (AHF). METHODS: This observational study included patients with acute dyspnoea. Diagnoses of AHF and non-AHF were adjudicated by cardiologist. NLR and CRP were measured at admission. Cutoff were determined using Receiver Operating Characteristic curves and Youden's index.

Health status and prognosis of COPD patients in relation to the FEV1/FVC ratio.

Whether COPD should be diagnosed using the lower limit of normal (LLN) or a fixed FEV1/FVC ratio <0.70 (FR) is debated. We compared symptom and disease burden in COPD patients with FEV₁/FVC below both thresholds (FR + /LLN + ) versus those between them (FR + /LLN-). This cohort study included 572 COPD patients from primary and secondary care in the central Swedish regions of Dalarna, Gävleborg, and Uppsala. FR + /LLN + COPD patients with FEV1 ≥ 60% predicted (n = 194) was compared to FR + /LLN- COPD patients (n = 85) in order to have similar FEV1 levels in both groups. The symptom burden was a

Evaluation of a combined symptom-based questionnaire and capillary natriuretic peptides testing for early detection of heart failure: a Heart Failure Awareness Days nationwide screening study (DEPIC FR).

AIMS: Early diagnosis of heart failure (HF) remains challenging, as symptoms are often nonspecific or absent. We aimed to evaluate the performance of a two-step screening strategy combining a symptom-based questionnaire-based on the acronym EPOF (dyspnoea, weight gain, oedema, fatigue in French)-and capillary NT-proBNP fingerstick testing to identify individuals with elevated cardiovascular risk or preclinical HF. METHODS AND RESULTS: We screened 2481 adults without known HF during a nationwide, community-based campaign held in 21 hospital cardiology centres across France. All participants un

Validity of EQ-5D-5L breathing and cognition bolt-ons in non-hospitalized patients after COVID-19.

PURPOSE: The EQ-5D has been criticized for lacking dimensions for breathing and cognition. In acute COVID-19 and Long COVID, breathing problems and cognitive complaints are common. This study evaluated the inclusion of EQ-5D-5L (bolt-on) items that assess these dimensions. METHODS: In a follow-up of community-based non-hospitalized patients with COVID-19 in 2020 (n = 450), we used respiratory and cognitive bolt-on items alongside other questionnaires about 30 months later (n = 220). We assessed data quality and construct validity for the bolt-ons, including comparison with concurrently used i

Evaluation and validation of a revised VetCompass clinical grading tool for heat-related illness in dogs.

The original VetCompass clinical grading tool for heat-related illness (HRI) in dogs was developed using general primary-care clinical records. Using two novel HRI emergency-care datasets from Vets Now clinics during 2022 and 2023, this current study aimed to firstly evaluate and revise the original tool, then to validate the revised tool. Evaluation of the original grading tool using 364 HRI cases treated during 2022 prompted three revisions; dyspnoea moved from Mild to Moderate grades, episodic collapse from Moderate to Mild, and new critical temperature criteria (≥ 41.0 °C) were added to bo
