Background Idiopathic subglottic stenosis is a rare condition characterized by a constriction of the airway passage without a known cause. While there may be many contributing factors, no definitive etiology has been established. Due to its rarity, this condition is frequently overlooked by healthcare providers, resulting in an inaccurate and lengthy diagnosis process. This results in detrimental effects for the patient given the symptoms of progressive dyspnea. Our objective was to retrospectively review patients diagnosed with idiopathic subglottic stenosis and analyze key characteristics a

Background/Objectives: Post-extubation stridor (PES) is common in pediatric critical care and may contribute to extubation failure, particularly in children with heart disease. Prophylactic corticosteroids are frequently used before extubation, but their benefit in pediatric cardiac patients remains uncertain. We evaluated the association of prophylactic corticosteroids with PES and extubation failure and explored whether PES mediated any association with failure. Methods: We performed a retrospective, single-center, observational cohort study of extubation events in a pediatric cardiac critic

BACKGROUND: Diphtheria remains a life-threatening disease in low-resource settings where immunization coverage is poor and access to a laboratory is limited. Somalia is currently experiencing a resurgence of clinically suspected diphtheria among children, yet detailed case-level clinical data remain scarce. The purpose of this study was to illustrate clinical features, vaccination status, complications, and early outcomes of pediatric diphtheria in a resource-limited Somali hospital. Furthermore, it identified the factors associated with severity and mortality for enhanced early recognition an

BACKGROUND: Bronchoscopy is the gold standard for diagnosing inhalation injury in burn patients. However, its availability remains limited even in major cities such as Jakarta. This lack of access presents significant diagnostic challenges, particularly in rural and resource-limited areas across Indonesia. This paper aims to review physical findings for the early diagnosis of inhalation injury in burn patients and to identify indicators for early intubation in settings where bronchoscopy is limited or unavailable.
METHODS: A literature review was conducted using the keywords "inhalation injur

Laryngeal paralysis is an uncommon but potentially life-threatening condition in cats due to upper airway obstruction. Surgical arytenoid lateralization is generally considered the treatment of choice; however, complication rates in cats appear higher than in dogs, and alternative techniques may be desirable, particularly in emergency situations. The aim of this study was to evaluate the clinical outcome of transoral partial arytenoidectomy for the treatment of laryngeal paralysis in cats. Eight client-owned cats diagnosed with laryngeal paralysis underwent transoral partial arytenoidectomy. D

PURPOSE: Dysfunctional breathing contributes to exercise intolerance (EI) but is difficult to diagnose. We developed a novel approach to evaluate the ventilatory response during cardiopulmonary exercise tests (CPET), measuring the average change in respiratory rate (RR) relative to the change in tidal volume (Vt). We hypothesized that this RR:Vt slope ratio will uncover unique ventilatory responses among different groups of patients experiencing EI.
METHODS: An exploratory retrospective review was conducted on CPET for patients with EI from January 2017 to July 2024. Demographics, spirometry,

Cervical auscultation (CA), used an adjuvant to the clinical feeding examination by Speech-Language Pathologists (SLPs), has high sensitivity and reliability for detecting aspiration in neonates and children. CA can only be considered reliable if there are clear distinctions in the acoustic and perceptual swallow sound profiles between swallow sounds associated with and without aspiration. We aimed to determine whether the acoustic and perceptual swallow sound profile differs between swallows associated with and without aspiration in preterm neonates. We simultaneously recorded video and swall

Croup in adults is a rare but potentially life-threatening condition. We describe a case of croup in a 40-y-old female climber, likely precipitated by exposure to cold, dry air at high altitude. To our knowledge, this is the first documented case of croup in an adult associated with exposure to high altitude. This case highlights the importance of early recognition and timely intervention in managing this uncommon presentation in an adult of a common childhood condition.

Airway ultrasound forms an integral part of the Point-of-care ultrasound (POCUS), providing valuable real-time information that supports airway evaluation and management in critically ill patients. POCUS of the airway helps in the airway assessment for difficult laryngoscopy and intubation, confirmation of endotracheal tube (ETT) position, determination of ETT size especially in paediatric patients, identification of the cricothyroid membrane for cricothyrotomy, prediction of postextubation stridor and guidance during percutaneous dilatational tracheostomy. Despite its undeniable utility, airw

OBJECTIVES: In cluster- randomized trials (CRTs), different estimands can be targeted, such as the individual- or cluster-average effect. These two estimands can differ in magnitude when outcomes or treatment effects vary with cluster size (termed informative cluster size). When informative cluster size is present, commonly used estimators for CRTs, such as mixed-effects models and generalized estimating equations with an exchangeable correlation structure (termed GEEs(exch)), can be biased for both these estimands. With little documented evaluation of informative cluster size, it is currently

Recurrent spasmodic croup is uncommon in school-aged children and may reflect an atopy-associated upper-airway dysfunction rather than a viral illness. We reported a 14-year-old boy with perennial allergic rhinitis and cough-predominant asthma who experienced 10 early-morning episodes of barking cough and inspiratory stridor between 2020 and 2024. Despite the long-term prescription of inhaled corticosteroid therapy, seasonal discontinuation and inconsistent adherence were evident, and recurrent episodes persisted. After referral, integrated management including stepwise allergic-rhinitis treat

Background: Eosinophilic esophagitis (EoE) is a chronic type 2 immune-mediated esophageal disease defined by esophageal dysfunction and eosinophil-predominant inflammation. Beyond classic gastrointestinal symptoms, patients may present with aerodigestive complaints such as chronic cough, hoarseness, recurrent croup, stridor, vocal fold abnormalities, and subglottic stenosis, creating diagnostic overlap with laryngopharyngeal reflux, vocal fold dysfunction, asthma, allergic rhinitis, and other airway disorders. Methods: This scoping review mapped evidence on the relationship between EoE and lar

A postintubation tracheal stenotic patient who requires surgical resection and anastomosis is a challenging case for an anesthesiologist, requiring meticulous planning during surgical correction due to a shared airway. This case report is about the anesthetic management of a 23-year-old male who developed severe tracheal stenosis following prolonged mechanical ventilation after decompressive craniectomy and presented with severe dyspnea, wheeze, and stridor five weeks later. Tracheal resection and anastomosis were the ultimate solution for this patient. For that, a sequential airway strategy w

INTRODUCTION AND IMPORTANCE: BCOR family sarcomas are rare mesenchymal tumors, usually arising in bone or soft tissue, and involvement of the upper airway is exceedingly uncommon. Their nonspecific respiratory presentation can mimic asthma, leading to delayed diagnosis. This case highlights a primary subglottic BCOR sarcoma presenting as refractory asthma, emphasizing a rare site and a diagnostic pitfall.
CASE PRESENTATION: A 26-year-old woman presented with persistent wheezing and progressive dyspnea, which were initially treated as asthma with partial improvement. Imaging revealed a subglot

This case report describes a 55-year-old woman presenting with acute dysphagia and neck swelling after a "popping" sensation in her neck. CT and MRI revealed a large retropharyngeal cystic mass and a left thyroid lobe mass, causing airway displacement. Despite significant effacement of the oropharyngeal and supraglottic airway on flexible nasopharyngolaryngoscopy, the patient remained asymptomatic regarding respiratory distress. Subsequent ultrasound-guided fine needle aspiration biopsy diagnosed malignant papillary thyroid carcinoma. Concurrently, previously undiagnosed Graves' disease was id
