Update in childhood asthma.

The areas covered represent a personal selection in the field. Asthma is defined in this manuscript as a clinical syndrome of wheeze, breathlessness and chest tightness, sometimes with excess cough. No assumptions are made about underlying pathology, and asthma thus becomes a clinical description, not a diagnosis. The areas covered include the need never to forget the importance of getting the basics right, including correct diagnosis and appropriate management; most children with asthma do not need biologics. Recent advances in preschool wheeze are covered next, especially the beginnings of p

Allergic reaction to nalbuphine administration in a pre-adolescent patient: a case report and literature review.

BACKGROUND: Nalbuphine is synthetic agonist-antagonist opioid compound. Allergic reactions to nalbuphine hydrochloride are exceedingly rare. We present a case of hypersensitivity reaction to nalbuphine in a pre-adolescent patient. CASE PRESENTATION: A male patient, aged 11 years, underwent general anesthesia for the correction of congenital strephexopodia. Nalbuphine hydrochloride was administered as part of the postoperative analgesia regimen. However, the patient subsequently developed an allergic reaction characterized by pulmonary edema, wheezing, tachycardia and cutaneous rash. Prompt in

Type I and III interferons in asthma and respiratory infections: New insights for potential treatments.

Asthma is a chronic lower respiratory system disease with high global prevalence and mortality rates. It is characterized by several clinical symptoms, including coughing, wheezing, dyspnea, and breathing difficulties. Despite extensive research on asthma treatment, no definitive cure exists. Current treatments primarily focus on symptom relief through corticosteroid agents and bronchodilators. Various factors contribute to the pathogenesis of asthma, with an imbalance in type 1/2 immune responses and a shift toward type 2 responses being a primary cause. Environmental factors, such as infecti

Assessment of particulate matter exposure levels and associated respiratory symptoms among oil seed processing factories workers.

Studies have shown that levels of fungal contamination and mycotoxin concentrations in oil-producing seeds vary across regions. In addition, extreme exposure to seeds dust particulate matter can lead to respiratory health problems. In Ethiopia oil-seeds processing factories workers exposure to particulate matter and magnitude of its respiratory symptoms have not been widely explored. To determine particulate matter concentration exposure levels and identify factors associated with respiratory symptoms among oil-seed processing factories workers. Using cross-sectional study design, 14 factories

Use of respiratory oscillometry in infants: a scoping review.

We mapped the evidence on the use of respiratory oscillometry (RO) in infants through a scoping review of the literature. We conducted a systematic search of Medline, Embase, Cochrane Library, Web of Science, Google Scholar, Cochrane Controlled Register of Trials, and clinical trial registries from inception to 1st September 2025. We included documents on the use of RO in infants up to 1 year. We assessed risk of bias using the Risk of Bias In Non-randomised studies of Interventions (ROBINS-I) and the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). Reporting adheres to the PRIS

Overlooked and Undernourished: A Case Report of Scurvy Linked to Food Insecurity.

UNLABELLED: Food insecurity is a commonly underrecognized social determinant of health and is a risk factor for severe, though uncommon, nutritional deficiencies. This report describes a 60-year-old male with a history of asthma and housing instability who presented to the emergency department (ED) for shortness of breath and was found to have classic findings of vitamin C deficiency, colloquially known as scurvy. Examination revealed diffuse wheezing, poor dentition with palatal ecchymosis, and a perifollicular rash with scattered bruising on his bilateral lower extremities. Due to financial

Evaluating the Efficacy of Intranasal Montelukast in Pediatric Acute Asthma Attacks: A Single-blinded, Placebo-controlled Clinical Trial.

Asthma is a common chronic respiratory disease in children, often leading to acute exacerbations marked by dyspnea, cough, and wheezing, which frequently necessitate emergency medical care. While standard therapies are effective, the exploration of novel drug delivery routes continues. Oral montelukast is a recognized treatment, but the efficacy of its intranasal formulation for acute attacks remains underexplored. This study aimed to evaluate the clinical effectiveness of intranasal montelukast as an adjunct therapy for pediatric asthma exacerbations. A single-blinded, placebo-controlled, sin

Effect of Azithromycin in Addition to Inhaled Corticosteroids in Severe Preschool Wheeze: A Retrospective Cohort Study.

INTRODUCTION: Preschool wheeze is common, and treatment options are limited, consisting mainly of inhaled corticosteroids (ICS). We report here a retrospective study in severe preschool wheezers not controlled by high doses of ICS, who were given azithromycin as long-term additional therapy. METHODS: We conducted a single-center, retrospective study between 2020 and 2025, including children less than 6 years old with severe recurrent wheeze, partially or uncontrolled despite a high dose of ICS and who were given azithromycin three times a week for 6 months as maintenance treatment. RESULTS:

Reduced baseline lung function in asymptomatic and symptomatic infants with recurrent asthma-like symptoms.

BACKGROUND: Recurrent asthma-like symptoms (RALS) in infants, defined as three or more physician-diagnosed episodes of wheezing and cough in the past 12 months, are the most common clinical presentation of asthma during the first two years of life. During exacerbations, patients may experience severe episodes and often require emergency department visits or hospitalisation. However, there is limited information on lung function (LF) during symptomatic periods compared to asymptomatic periods and in healthy controls. METHODS: The baseline LF of 264 infants aged < 15 months with RALS (94 sympto

Wildfire smoke exposure and persistent respiratory symptoms and illness: a retrospective cohort study.

BACKGROUND: Wildfire smoke is a growing public health hazard yet there is limited evidence on its intermediate or long-term respiratory health effects. Studying the association between wildfire smoke exposure and indicators of impaired respiratory health such as persistent respiratory illness and respiratory symptoms can help elucidate the associated risk of developing chronic lung disease. We sought to determine whether wildfire smoke exposure is associated with medically-attended respiratory illness (MARI) and respiratory symptoms persisting beyond the acute wildfire period. METHODS: This w

Association between volatile organic compound concentrations in daycares and child wheeze: the French CRESPI study.

BACKGROUND: Concern is increasing about of the role of poor indoor air quality in respiratory disorders especially in children. While the deleterious respiratory health impact of some volatile organic compounds (VOCs) in the home environment has been suggested, daycares have never been investigated. This study aims to investigate the association between VOCs concentrations in daycare facilities and children's respiratory health. METHODS: This cross-sectional study included 532 children (mean age: 22.3 months; 47.4% female) of the French CRESPI cohort (100 daycares, 2019-2022). VOC/aldehyde (n

Prenatal environmental exposures and the risk of repeated childhood wheeze and diagnosed atopic disease.

BACKGROUND: Early pregnancy represents a critical developmental period and may influence later development of respiratory and allergic diseases in children. A growing body of evidence links prenatal exposures to atopic diseases, such as asthma, with sex-specific differences. METHODS: Children from the F2-generation (i.e., children born to male or female F1 participants of the original 1989 birth cohort) of the Isle of Wight Birth Cohort (IOWBC) (n = 617) were followed from pregnancy through 10 years. Prenatal exposures (12 weeks), including cooking heat source, dampness or fungi in the home,

Lifetime urban exposure, socioeconomic position and respiratory health among adolescents.

INTRODUCTION: Early-life exposure to urban environmental factors, including air pollution, greenness or climate-related factors, is suspected to be associated with respiratory health but evidence among adolescents is scarce. We estimated the association between early-life exposure to urban factors and respiratory health among adolescents and addressed whether the socioeconomic position (SEP) could modify these associations. METHODS: This study was based on 3731 mother-child pairs from the HELIX and Generation R cohorts with 29 exposures from 6 domains (air pollution, built environment, natura

Gestational and postnatal age define airway epithelial maturation and RSV responses during the first year of life: results of an ex-vivo/in-vitro Airway Epithelial Cell Study.

INTRODUCTION: Respiratory syncytial virus (RSV) is the leading cause of severe lower respiratory tract infection in infants globally. Newborns, especially those born prematurely, are at increased risk of severe RSV-related illness, yet the influence of age on early-life airway epithelium innate immune responses remains relatively understudied. METHODS: We established ex-vivo/in-vitro well-differentiated paediatric nasal epithelial cell (WD-PNEC) cultures derived from preterm and term infants at birth and again at 1-year old. Infection characteristics and innate immune responses were determine
