Optimal Treatment for Dysfunctional Breathing in Adults: An International Delphi Study.

BACKGROUND AND OBJECTIVES: Dysfunctional breathing (DB) is common and impairs quality of life. Various non-pharmacological interventions have been identified, but there is a lack of agreement regarding essential treatment components and delivery methods. This study aimed to gain consensus from experts and people with DB regarding the critical components, optimal format and delivery methods of non-pharmacological DB interventions. METHODS: A two-round Delphi process was conducted involving international experts, with each round followed by a consumer focus group. Online surveys were distribute

Yield of Hyperventilation and Other Activation Procedures and Study Parameters in Facilitating Epileptiform Discharges.

PURPOSE: Hyperventilation (HV) during EEG was restricted during the COVID-19 pandemic. The authors compared EEGs with and without HV to place HV in context of other factors that precipitate epileptiform abnormalities (EA). The authors hypothesized HV would not significantly affect the sensitivity of routine EEG in children and adults. METHODS: The authors reviewed routine EEGs completed between March 2018 and March 2022 in subjects older than 4 years. The primary outcome variable was EA (seizures and epileptiform discharges). In a subset of patients with serial EEGs with and without HV, the a

Systemic effects of metabolic acidosis.

Metabolic acidosis is a primary reduction in serum bicarbonate concentration and a consequent decrease in blood pH, which has profound implications for systemic physiology. This literature review synthesizes the current evidence of metabolic acidosis's effect on cardiovascular, vascular, pulmonary, gastrointestinal, endocrine, musculoskeletal, renal, and metabolic systems. Cardiovascular effects include impaired cardiac contractility, altered ion exchange currents, and decreased β-adrenergic response. Vascular response is dependent on vessel size, promoting vasodilation in large arteries and v

The therapeutic effect of breathing interventions for functional seizures: A scoping review.

Functional/Dissociative Seizures (FDS) are presentations that appear like epileptic seizures but occur without abnormal cortical electrical activity in the brain. Abnormal respiratory patterns, particularly hyperventilation, have been frequently observed in people with FDS, suggesting this may be a potential therapeutic target. This scoping review aimed to explore the various applications of breathing techniques in the treatment of FDS, synthesizing evidence from clinical and case-based articles to provide a comprehensive overview of this therapeutic modality. Systematic searches were conducte

Brain strain: Blood flow and metabolism in environmental extremes.

This narrative review compares and contrasts the most commonly encountered environmental stressors on human cerebrovascular functioning. From high altitude and space, extreme apnoea, heat and cold stress, the impact of these stressors on the regulation of cerebral blood flow (CBF) and oxygen metabolism ( CM R O 2 ${\mathrm{CM}}{{\mathrm{R}}_{{{\mathrm{O}}_{\mathrm{2}}}}}$ ) is discussed. As long as consciousness remains, CM R O 2 ${\mathrm{CM}}{{\mathrm{R}}_{{{\mathrm{O}}_{\mathrm{2}}}}}$ and oxygen delivery ( C D O 2 ${\mathrm{C}}{{\mathrm{D}}_{{{\mathrm{O}}_{\mathrm{2}}}}}$ ) rem

Post-Exercise Controlled Breathing Enhances Cardiovascular Recovery and Autonomic Balance: A Randomised Crossover Study.

Background and Objectives: Controlled breathing can influence autonomic regulation and haemodynamics; however, the role of its timing relative to exercise remains unclear. Materials and Methods: Fourteen healthy, physically active men (mean age 21.8 ± 0.7 years; body mass index within the normal range) participated in this randomised crossover study. Each session consisted of five 5 min cycling bouts at 50% of heart-rate reserve, interspersed with 3 min passive recovery periods. The three conditions were: control (no structured breathing), 30 s hyperventilation (approximately 30 breaths·min-1)

Intraoperative seizure in the setting of tumor resection: illustrative cases.

BACKGROUND: Intraoperative seizures pose a serious threat to patients undergoing neurosurgical procedures, potentially disrupting cases and increasing postoperative complications. While established risk factors, such as history of seizures and tumor location, are well documented, emerging evidence indicates hypocapnia, induced by controlled hyperventilation, may further reduce seizure threshold. Nevertheless, clinical data directly linking hyperventilation to intraoperative seizure incidence remain sparse in the current literature. OBSERVATIONS: The authors present the cases of 3 patients wit

Delayed Onset Hypercapnia in Patients With Anemia Undergoing Total Laparoscopic Hysterectomy: A Report of 2 Cases.

BACKGROUND Hypercapnia is a known complication of laparoscopic surgery involving carbon dioxide (CO₂) pneumoperitoneum and is usually managed intraoperatively with ventilatory adjustments. However, delayed-onset postoperative hypercapnia is uncommon. Anemia impairs hemoglobin-mediated buffering and clearance of CO₂, potentially increasing vulnerability to postoperative respiratory acidosis. This report describes 2 cases of delayed-onset postoperative hypercapnia in anemic patients following total laparoscopic hysterectomy with CO₂ pneumoperitoneum, in the absence of subcutaneous emphysema. Mild hyperventilation with preserved exercise capacity in patients with self-reported long-term dyspnea after COVID-19-a prospective cohort study in a primary healthcare setting.

BACKGROUND: Post COVID-19 condition remains a complex and challenging issue, with diverse manifestations, despite accumulating research and clinical experience. Dyspnea is one of the most common symptoms reported in post COVID-19 condition. Cardiopulmonary exercise testing (CPET) offers a cohesive assessment of dyspnea and exercise limitations. The Nijmegen questionnaire is a form for assessment of dysfunctional breathing. AIM: The aim was to explore relationships between self-reported post COVID-19 respiratory symptoms, assessed by Nijmegen questionnaire, and the results of a 6-minute walk t

Quantitative EEG Assessment of Dependence-Related Neurophysiological Patterns Using Rule- and Score-Based Modeling in Substance Use Disorders.

Background and Objectives: Substance use disorders (SUDs) are associated with maladaptive neuroplasticity and chronic dysregulation of cortical arousal. EEG provides a non-invasive tool for quantifying these neurophysiological alterations through spectral power and reactivity indices. Prior research consistently reports elevated beta and diminished alpha activity in SUD, reflecting cortical hyperarousal and reduced inhibitory control. This study sought to identify EEG-based markers of dependence-related neurophysiological alterations by integrating rule-based and score-based models incorporati

Transient neuropsychiatric events after HPV vaccination among adolescent schoolgirls in India.

Clusters of transient neuropsychiatric symptoms following HPV vaccination among adolescents, particularly in school-based immunisation settings, can lead to diagnostic uncertainty, unnecessary medical interventions and public concern regarding vaccine safety. We describe a cluster of five 13-year-old schoolgirls who developed acute dizziness, brief loss of consciousness, hyperventilation and anxiety shortly after vaccination, despite normal examinations and unremarkable investigations. The clinical pattern was most consistent with vasovagal and anxiety-related reactions, although absence seizu

Effect of a Comprehensive Mobile-Based Respiratory Training Program on Respiratory Function in Survivors of Acute Stroke: Randomized Controlled Trial.

BACKGROUND: Respiratory dysfunction frequently occurs during the acute phase of stroke and is associated with reduced ventilatory capacity, respiratory muscle weakness, and increased pulmonary complications. However, delivering standardized respiratory training during hospitalization is often constrained by staffing and service continuity. OBJECTIVE: This study aimed to evaluate the efficacy, safety, and feasibility of a hospital-based comprehensive mobile-based respiratory training program (CMRTP) added to conventional rehabilitation in people with acute stroke who are inpatients. METHODS:

Comparative validation of speckle contrast optical spectroscopy against diffuse correlation spectroscopy for monitoring human cerebral blood flow.

SIGNIFICANCE: Within diffuse optics, speckle contrast optical spectroscopy (SCOS) has emerged as a promising alternative to the state-of-the-art technique of diffuse correlation spectroscopy (DCS) for continuous, noninvasive bedside monitoring of cerebral blood flow (CBF). Although the two methods theoretically yield equivalent relative indices of CBF (rCBFi), in practice, SCOS-based measurements require experimental calibration to obtain unbiased rCBFi values. To date, there are limited validation studies comparing SCOS and DCS in human subjects, particularly at long source-detector separatio

Characterization of asthma comorbidity in patients with functional/dissociative seizures.

INTRODUCTION: Epidemiological studies have suggested an association between asthma and functional/dissociative seizures (FDS). We compared demographic characteristics and pulmonary function test (PFT) findings in patients with FDS and comorbid asthma versus patients with asthma alone. METHOD: Patients with FDS and comorbid asthma and patients with asthma alone who had undergone PFTs were identified using ICD and CPT codes. Demographics, spirometry measures, and objective measures of asthma (bronchodilator reversibility, exhaled nitic oxide and methacholine challenge) were collected. Groups we

Dysfunctional breathing mimicking asthma exacerbation with acute hypoxemia: a case report.

BACKGROUND: Dysfunctional breathing (DB) is characterized by abnormal breathing patterns and is often under-recognized in clinical practice. Although DB is generally regarded to be a functional breathing disorder, its potential to cause clinically significant hypoxemia has not been well documented. CASE REPORT: A case involving a 24-year-old woman diagnosed with asthma and other psychiatric comorbidities, who developed acute wheezing and oxygen desaturation during hospitalization, is reported. Despite treatment with systemic corticosteroids, the patient's symptoms did not improve. Careful phy
