Cough and Airway Responses to Direct and Indirect Challenges: An Integrative Review.

Individuals with classic asthma can present with wheezing, dyspnea, chest tightness, and/or cough. Notably, cough can be the main or only symptom of asthma, termed cough variant asthma. Although there are numerous guidelines for asthma, the diagnosis and management of cough in asthma remain challenging because of its complex physiology. By definition, cough variant asthma excludes individuals with normal airway sensitivity to methacholine (MCh). However, many individuals with chronic cough who are being evaluated for possible cough variant asthma have normal airway sensitivity, yet cough durin

Crossing the blood-brain barrier: neurotrauma in disasters and resource-limited settings.

Traumatic brain injury (TBI) is a major cause of morbidity and mortality in natural disasters, armed conflict, and other resource-limited settings, where advanced imaging, invasive neuromonitoring, and specialized neurosurgical care, common in high-resource environments, are often unavailable. This narrative review summarizes the global health impact of TBI in austere conditions and outlines key management strategies and ethical considerations relevant to these contexts. Evidence from civilian disaster responses, military conflicts, and global health guidelines highlight the need for adaptable

Hyperventilation-Induced High-Amplitude Rhythmic Slowing and Its Impact on Migraine Frequency in Children: A Retrospective Observational Cohort Study.

OBJECTIVES: The most common primary headache disorder in children is migraine. Some children with hyperventilation (HV) may exhibit hyperventilation-induced high-amplitude rhythmic slowing (HIHARS) in their electroencephalogram (EEG). The study aims to evaluate the correlation between HIHARS and a child's future migraine onset risk. MATERIALS & METHODS: The present study evaluated HIHARS during the HV maneuver in children aged 7 to 12 admitted to the Electroencephalography Monitoring Unit for diverse referral reasons. After six years, the researchers assessed Long-term video-EEG monitoring da

Electroconvulsive therapy in an older patient with recurrent major depressive disorder and a permanent pacemaker: A case report.

BACKGROUND: Electroconvulsive therapy (ECT) is highly effective for severe or treatment-resistant major depressive disorder, including in older adults. In patients with permanent pacemakers, however, ECT may be avoided because of concern about device malfunction or arrhythmia. CASE PRESENTATION: We report a man in his 80s with recurrent major depressive disorder and a dual-chamber pacemaker for complete atrioventricular block. During the approximately 2 years before admission, the depressive episode achieved only partial remission despite mirtazapine 30 mg/day and trials of duloxetine 60 mg/d

Arterial CO2 partial pressure is a key modulator of active muscle hypoperfusion mediated by diving reflex in exercising young adults.

Voluntary apnea markedly attenuates exercise-induced increases in active muscle blood flow, a response known as the diving reflex, which is thought to preserve oxygen within the body. However, the underlying mechanisms of this response remain unknown. Since voluntary apnea elevates arterial CO2 partial pressure, which can induce vasoconstriction in active muscles during exercise, we hypothesized that arterial CO2 partial pressure modulates diving reflex-related regulation of active muscle blood flow. Fourteen young adults (1 female) performed maximal-duration breath-holding during a dynamic tw

Asthma in athletes: diagnostic challenges and management strategies.

PURPOSE OF REVIEW: Exercise-induced bronchoconstriction (EIB) and exercise-induced asthma (EIA) are common conditions among athletes and represent an important clinical issue with potential implications for both respiratory health and athletic performance. The prevalence of asthma in athletes is higher than in the general population, likely due to the combined effects of increased ventilatory demand and environmental exposures. RECENT FINDINGS: The pathophysiology of EIB is multifactorial and involves osmotic and thermal mechanisms related to airway dehydration during exercise-induced hyperve

Monitoring myocardial oxygenation response to respiratory maneuvers using real-time magnetic resonance imaging.

BACKGROUND: Oxygenation-sensitive (OS) cardiovascular magnetic resonance (CMR), using breathing maneuver-induced stress and blood oxygen level-dependent contrast, is a well-tolerated, non-invasive approach for assessing myocardial oxygenation. This study evaluated two real-time CMR sequences and a semi-automated analysis workflow to enable continuous, motion-robust assessment of myocardial oxygenation dynamics, aiming to overcome limitations of electrocardiogram (ECG)-triggered acquisitions and reduce manual evaluation requirements. METHODS: Signal intensity dynamics of ten young healthy fema

Arterial blood gas changes in progressively deeper breath-hold dives.

Studies involving breath-hold dives as deep as 60 m have identified a spike in arterial Po2 ([Formula: see text]) and Pco2 ([Formula: see text]) apparently associated with lung compression by water pressure as depth increases. Competitive breath-hold dives may exceed 100 m, raising the possibility that a spike in [Formula: see text] might contribute to narcosis-like symptoms sometimes experienced. To predict the magnitude of this spike, three elite breath-hold divers were instrumented with radial artery catheters and had arterial blood specimens taken at the bottom on separate dives to 20, 40,

2025 Korean Guidelines for Cardiopulmonary Resuscitation: Part 8. Pediatric advanced life support.

The 2025 Korean pediatric advanced life support guideline update introduces clinically important revisions emphasizing airway strategy, physiologic resuscitation targets, post-cardiac arrest hemodynamics, neuroprotection, and extracorporeal support. In out-of-hospital pediatric cardiac arrest, bag-mask ventilation is now suggested over endotracheal intubation or supraglottic airway placement. In in-hospital arrest, evidence is insufficient to favor bag-mask ventilation versus advanced airways; however, endotracheal intubation or supraglottic airway insertion is reasonable when performed with m

Hypercapnic Hyperventilation to Accelerate Emergence from Inhaled Volatile Anesthetics: A Practical Method Using Standard Hospital Equipment.

Hypercapnic hyperventilation is a technique used to support emergence from volatile anesthesia by allowing a controlled elevation of arterial carbon dioxide (CO₂) levels. The resulting hypercapnia increases minute ventilation, enhances the washout of anesthetic gases from the lungs, and increases cerebral blood flow, which may facilitate faster elimination of volatile agents from brain tissue. Commercial devices have been developed to achieve this effect; however, their availability is limited in many regions. This protocol describes a practical alternative approach using standard anesthesia e

Pitt-Hopkins syndrome (PTHS)- a case report from Pakistan.

Pitt-Hopkins syndrome (PTHS) is a rare genetic neurodevelopment disorder where affected individuals exhibit symptoms such as severe developmental delays and intellectual disability. To the best of our knowledge, this report presents the first known case from Pakistan where Chromosomal Microarray Analysis (CMA) was employed to diagnose PTHS. The CMA revealed a deletion in the Transcription Factor 4 (TCF4) gene, confirming the diagnosis. This case underscores the clinical features, diagnostic process, and the significance of CMA in diagnosing rare genetic disorders such as PTHS, particularly in

Broadening the inherited ASXL3 spectrum and unveiling molecular mechanisms through detailed genotypic-phenotypic analyses.

PURPOSE: ASXL3-related disorder is a highly heterogeneous neurodevelopmental disorder, with increasing reports of inherited and recurrent variants indicating substantial intrafamilial phenotypic variability, as well as the possibility of interfamilial phenotypic variability. METHODS: In this study, we conducted a detailed genotype-phenotype review of 204 individuals from the literature, including the International ASXL3 Natural History Study (IRAS: 316055). Statistical comparisons were made between individuals with variants leading to no protein product (nonsense-mediated messenger RNA decay

Review: Pig welfare around slaughter and impacts on carcass and meat quality.

Pig welfare around slaughter remains a sensitive issue, as animals are exposed to multiple hazards within a short period. The objective of this review was to provide a comprehensive assessment of the key factors affecting pig welfare in commercial slaughterhouses, to evaluate their impact on meat quality, and to examine contemporary alternatives to high-concentration CO2 stunning with respect to their implications for both animal welfare and meat quality. During lairage, exposure to a novel environment and unfamiliar animals can hinder recovery from transport stress. Limited space allowance ne

A Rare EEG Finding of Eye Closure Sensitivity in a Child with Genetic Generalized Epilepsy: A Case Report.

Background: Eye-closure sensitivity (ECS) is a rare reflex epilepsy phenomenon characterized by epileptiform discharges on electroencephalogram (EEG), triggered by eye closure. It has been reported in all genetic generalized epilepsies (GGEs), particularly in adolescents and adults. However, pediatric cases remain uncommon in the literature. Case Presentation: We report a 10-year-old previously healthy girl who presented with recurrent generalized tonic-clonic seizures beginning at age nine. Seizures occurred every few months without identifiable triggers, lasting 1-2 min with complete loss of

The Relative Contributions of Arousal Presence and Arousal Intensity to Post-Respiratory-Event Ventilation in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is characterised by cyclical respiratory events followed by hyperpneic breaths that frequently coincide with cortical arousals. Whilst this post-event hyperventilation partly reflects accumulated respiratory stimuli, arousal itself appears to contribute independently through both its presence and its intensity. This study aimed to quantify the relative contributions of arousal presence and arousal intensity to post-event ventilation, independent of chemoreflex-driven responses. Two retrospective polysomnography data sets were analysed: a community-based cohort (Mu

Dynamics of Dead Space and its Components During Exercise Testing With Continuous Transcutaneous Carbon Dioxide Monitoring.

OBJECTIVES: To describe the continuous pattern of dead space during exercise and recovery using noninvasive monitoring of transcutaneous carbon dioxide pressure (PtcCO2). METHODS: During routine exercise testing across various conditions in 132 subjects, including healthy individuals and patients, PtcCO2 was validated against arterial sampling of carbon dioxide pressure (PaCO2), and continuous dead-space recording was performed using a transcutaneous probe. RESULTS: During hyperventilation, a lag of 76±12s was observed between arterial and transcutaneous measurements. Comparison of PtcCO2 an

How high ventilation breathwork alters consciousness: Hypoperfusion, network dynamics, and subjective experience.

High ventilation breathwork (HVB) is increasingly used in psychotherapeutic and contemplative contexts to induce altered states of consciousness (ASC), yet its neurobiological mechanisms remain poorly understood. Thirty experienced HVB practitioners (mean age 44.97 ± 11.80 years) underwent perfusion and resting-state fMRI to assess acute cerebrovascular and post-acute network changes. Cerebral blood flow (CBF) was measured using arterial spin labelling during baseline and the interval of maximal subjective effects. Acute psychotropic effects were rated using an online Likert scale, ASC intensi

Impact of Age on the Diagnostic Yield of Routine EEG in People With Childhood or Juvenile Absence Epilepsy: A Cross-Sectional Study.

BACKGROUND: We aimed to identify the proportion of individuals with a confirmed diagnosis of childhood absence epilepsy (CAE) or juvenile absence epilepsy (JAE) who show a negative routine EEG (rEEG), and to determine the main factors associated with this finding. METHODS: Individuals with CAE or JAE underwent both an rEEG and a 24-h ambulatory EEG (24-h-aEEG). Participants were classified as 'rEEG positive' if generalised spike-wave discharges (GSWs) and/or typical absences (TAs) were observed, and as 'rEEG negative' if no such features were detected. 24-h-aEEG recordings served as the groun
