Selective non-nuclear estrogen receptor activation with PaPE-1 evokes neuroprotection involving mTOR/MEK in an in vitro model of hypoxic/ischemic injury.

BACKGROUND: Hypoxic/ischemic brain injuries, including ischemic stroke and perinatal asphyxia, remain major causes of mortality and long-term neurological disability, establishing a demand for therapeutic strategies suitable against the multifactorial nature of underlying mechanisms. Estrogen receptors (ERs) signaling is known to exert neuroprotective effects, however, genomic ER activation is associated with serious adverse effects, including carcinogenesis and thromboembolisms. Pathway Preferential Estrogen-1 (PaPE-1), a compound that selectively activates the non-nuclear subset of ERs, may

Safety and benefits of adult-worn slings and baby carriers: a narrative systematic review to inform guidance for parents.

BACKGROUND: Adult-worn slings and carriers (AWSC) are common components of infant care globally. Several widely publicised deaths of babies have occurred during AWSC-use, raising concerns about their safety and the strength of existing parental guidance. As part of a wider project to inform recommendations for UK guidance for the safe use of AWSC, this review was undertaken to assess the risks, benefits, harms and parental motivations for AWSC-use. METHODS: A systematic review of databases PubMed, Web of Science and CINAHL (2000-2024) was undertaken following Preferred Reporting Items for Sys

Therapeutic Hypothermia Ameliorates Apoptosis and Cerebral Injury by Upregulating HECTD1-mediated Ubiquitination and VDAC3 Degradation in a Rat CPR Model.

BACKGROUND AND OBJECTIVE: Post-cardiac arrest (CA) brain injury is a leading cause of death and disability in patients with return of spontaneous circulation (ROSC). Our previous study showed that hypothermia-induced VDAC3 ubiquitination attenuated apoptosis of microglia cells following simulated ischemia/reperfusion (I/R). This study aimed to further investigate the role of E3 ubiquitin ligase, HECTD1, in the ubiquitination and degradation of VDAC3, which is involved in hypothermic neuroprotection. METHODS: Sprague-Dawley rats were randomized into two groups, namely the normothermic (T37) gr

Chest compression synchronized ventilation versus chest compression superimposed by sustained inflation in asphyxiated newborn piglets: a randomized animal trial.

BACKGROUND: Guidelines on neonatal resuscitation recommend 90 chest compressions (CCs) and 30 ventilations (3:1 C:V) per minute in newborns. We have described an alternative resuscitation strategy where CCs are superimposed with sustained inflation (CC + SI), which allows for passive ventilation during compression. A more recent strategy is CCs with synchronized ventilation (CCSV), in which a ventilator flow sensor recognizes airflow during the downward phase of compression and thereby triggers an inflation. No study has compared CCSV with CC + SI in an asphyxiated newborn piglet model. Newbor

Postresuscitation Cerebral Vasospasm and Capillary Failure After Experimental Asphyxial Cardiac Arrest.

BACKGROUND: Disrupted cerebral blood flow is suggested to contribute to secondary brain injury after cardiac arrest (CA). This study aimed to investigate cerebral microcirculation after return of spontaneous circulation (ROSC) following asphyxial CA. We hypothesized that contraction of pericytes after ROSC compromises cerebral capillary blood flow. METHODS: Transgenic C57BL/6NRj mice (n=19) of both sexes were studied. Mice were expressing tdTomato as a fluorescent reporter under the control of the platelet-derived growth factor receptor β promoter to label pericytes. Chronic cranial windows w

Vertical versus transverse abdominal incision in placental abruption: a propensity score-matched study on the trade-off between speed and maternal-neonatal safety.

OBJECTIVE: To compare maternal and neonatal outcomes between midline vertical and transverse abdominal incisions during emergency cesarean delivery for placental abruption. METHODS: In this retrospective cohort study, 152 patients were included. Propensity score matching (PSM) was used to balance baseline characteristics, resulting in 106 well-matched patients (41 vertical, 65 transverse). Surgical metrics, laboratory parameters, and a composite adverse outcome were compared. Independent predictors were identified via multivariate logistic regression. RESULTS: After PSM, the vertical group h

Analysis and comparative study on the influencing factors of bronchopulmonary dysplasia in premature infants with gestational age ≤32 weeks based on logistic regression and decision tree models.

BACKGROUND: Bronchopulmonary dysplasia (BPD), as a common and serious complication in premature infants, has a high mortality rate, and the prognosis of most premature infants with BPD is poor. Early prediction and intervention treatment can improve the prognosis of children to a certain extent. This study aimed to explore the risk factors affecting BPD in premature infants with gestational age ≤32 weeks, provide scientific basis for the prevention and control of BPD. METHODS: We retrospectively collected clinical data from 478 premature infants with gestational age ≤32 weeks admitted to the

Cardiopulmonary resuscitation with synchronized ventilation versus 3:1 compression to ventilation ratio in a neonatal porcine model.

INTRODUCTION: Current neonatal resuscitation guidelines recommend a 3:1 Compression:Ventilation ratio (3:1 C:V) during cardiopulmonary resuscitation. One of the concerns with 3:1 C:V is lung derecruitment, which contributes to a delay in achieving return to spontaneous circulation (ROSC). An alternative approach might be chest compression synchronized ventilation (CCSV), which delivers a ventilation with each chest compression thereby achieving lung recruitment. We hypothesized that in asphyxiated newborn piglets with cardiac arrest CCSV vs 3:1 C:V would decrease the time to ROSC. METHODS: Ne

Impact of interprofessional blended learning with low-fidelity simulation on neonatal resuscitation skills among interns: a quasi-experimental study in Tanzania.

BACKGROUND: Despite the national rollout of the Helping Babies Breathe (HBB) program, neonatal mortality due to birth asphyxia remains a major challenge in Tanzania. Evidence shows that there is inadequate competence (including knowledge, skills, and attitudes) in newborn resuscitation among medical and nursing interns as they transition into practice. The traditional method of a one-day instructor-led HBB model provides little hands-on practice. Integrating low-fidelity simulation with blended learning may contribute to better transfer of learning and offer a more sustainable model for skills

Multi-organ biomarkers for the identification of neonatal encephalopathy in the absence of traditional fetal acidemia: A three-year clinical cohort study.

BackgroundTraditional diagnostic criteria for hypoxic-ischemic encephalopathy (HIE) rely heavily on documented fetal acidemia, which may be missing or unavailable in resource-limited settings. This study evaluated the diagnostic utility of multi-organ biomarkers in identifying neonates with HIE who do not meet classic acidemic criteria ("non-classic HIE").MethodsThis clinical population study analyzed 416 full-term and late-preterm neonates over 3 years at a tertiary neonatal intensive care unit (NICU). Participants were categorized into "classic HIE" (acidemia present) and "non-classic HIE" (

Enhancing perinatal care in Rwanda: building quality improvement into system strengthening.

Rwanda has achieved substantial gains in maternal, newborn and child health over the last two decades. But reduction in neonatal mortality has slowed, with most deaths now occurring in hospitals and health centres. Mortality is mainly caused by prematurity, perinatal asphyxia, infection and gaps in quality of care. From 2017 to 2023, the Rwanda Pediatric Association and the Royal College of Paediatrics and Child Health Global, in partnership with UNICEF, the Ministry of Health and Rwanda Biomedical Center, implemented a multiphase programme across 21 hospitals designed to enhance perinatal car

Aetiology, survival time and mortality predictors among early neonates at two tertiary hospitals of Lusaka, Zambia.

BACKGROUND: Over 2.3 million neonatal deaths occurred globally in 2022, representing 1 in 58 newborns, with neonatal deaths of about 6,300 every day. Of these, 73% died in the early neonatal period and one third within 24 h. Despite the 47% globally neonatal deaths decline from 2000 to 2015, Zambia's perinatal and neonatal mortality rates have remained high at 33 and 27 per 1000 total births, respectively. This study aimed to determine the aetiology of early neonatal deaths and to analyse survival time and mortality predictors among normal and low birthweight early neonates in the neonatal int
