Accuracy of next-generation sequencing for rapid diagnosis of tuberculous pleurisy: A protocol of systematic review and meta-analysis.

BACKGROUND: Early, rapid and precise diagnosis of tuberculous pleurisy is difficult. Next-generation sequencing (NGS) has been used for the diagnosis of TB in some high-income countries. We completed the protocol of the study in order to better guide the systematic review and meta-analysis for evaluating the role of NGS in the diagnosis of tuberculous pleurisy, which was the purpose of the current study. METHODS: We designed and drafted this protocol as per the Preferred reporting items for systematic review and meta-analysis protocols (PRISMA-P) guidelines. Three English databases (Cochrane

Quantification of MPT-64 within pleural fluid extracellular vesicles of tuberculous pleurisy patients by real-time immuno-PCR.

Diagnosis of tuberculous (TB) pleurisy is an exigent task owing to atypical clinical presentations and low bacillary content in clinical samples. Hence, there is a crucial need to deliberate a quick and consistent diagnostic test. We recently quantified Mycobacterium tuberculosis (Mtb)-specific MPT-64 (Rv1980c) within pleural fluid extracellular vesicles (pEVs) of TB pleurisy patients by SYBR Green real-time immuno-PCR (RT-I-PCR) assay and compared its diagnostic efficacy with respective ELISA and GeneXpert assay. The size of pEVs of TB pleurisy patients ranged between 47.7 and 170.2 nm as eva

Clinical predictive model for identifying high-risk factors in pleurisy tuberculoma patients.

INTRODUCTION: Pleurisy tuberculoma (PTM) is a neoplastic lesion that primarily affects the pleural wall or internal organs. The majority of PTM cases are observed during the treatment of tuberculous pleural effusion(TPE), and although the precise pathogenesis remains unclear, there is a significant association between these two conditions. To identify high-risk factors for the development of PTM, we developed a clinical predictive model aimed at providing more insightful information for the development of PTM. METHODS: A retrospective study was conducted on patients diagnosed with PTM or TPE

Delayed diagnosis of pulmonary tuberculosis with pleuritis due to ampicillin/sulbactam: A case report.

BACKGROUND: Tuberculosis (TB) remains a global health concern despite decreasing incidence. Delayed TB diagnosis can exacerbate patient outcomes and lead to broader public health issues such as mass infections. Differentiation between TB and bacterial pneumonia is often complicated by variable clinical and radiological manifestations of TB, leading to diagnostic delays. CASE SUMMARY: An 89-year-old, Japanese male patient with a history of diabetes mellitus, hypertension, and hypothyroidism presented with right-sided chest pain. Based on the elevated inflammatory response, right pleural effusi

Use of Robotic-Like Articulated Instruments in Medical Thoracoscopy: A Case Series.

INTRODUCTION: Medical thoracoscopy (MT) is a minimally invasive procedure primarily used for diagnosing pleural diseases. However, its therapeutic role, especially in complex pleural infections, is underexplored. Rising pleural disease incidence and the need for less invasive treatment options have spurred interest in enhancing MT's capabilities. Disposable articulated wristed instruments, initially designed for laparoscopic surgery, could offer improved dexterity and precision, potentially expanding MT's therapeutic use. CASE SERIES: We present 3 patients with complex pleural effusions manag

Recurrent pneumoperitoneum with pneumatosis cystoides intestinalis in tuberculous pleurisy: Case report.

RATIONALE: Pneumatosis cystoides intestinalis (PCI) is a rare condition for which autopsy studies have reported a prevalence of 0.03% in the general population. Although PCI can be associated with pneumoperitoneum, many cases are benign and do not reflect viscus perforation. Coexistence with tuberculous pleurisy and recurrent pneumoperitoneum is rare and may be underrecognized. PATIENT CONCERNS: In this report, we present the case of a 38-year-old Tibetan male who experienced tuberculous pleurisy accompanied by recurrent pneumoperitoneum with PCI. Despite the presence of free air, he remained

Impact of anti-TB drugs on modulations of T-cell-receptor-mediated signaling events in TB pleurisy patients.

Immunity in tuberculosis (TB) infection is complex as Mycobacterium tuberculosis (MTB) is a highly adaptive pathogen and may escape the immune defense through various ways. During MTB infection, immune modulation involves the activation and regulation of various immune cells and signaling pathways to mount an effective defense against the pathogen while minimizing immune pathology. Host pathogen interactions in TB are complex as MTB is a pathogen that is able to adapt and survive and may escape the immune defense through various ways. The limitations of BCG vaccine have energized researchers t

Primitive Sjögren's Syndrome Revealed by Bilateral Pleurisy in a Teenager: An Unexpected Case.

Primary Sjögren's syndrome (PSS) is a rare autoimmune disease in children and adolescents. The initial pleural manifestations are exceptional. We report the case of a 16-year-old adolescent hospitalized for bilateral chest pain with pleurisy. The initial diagnosis of tuberculosis was suspected due to the clinical context and local endemicity, but microbiological and histological examinations were negative. The presence of anti-Sjögren's syndrome type A (anti-SSA) antibodies and a salivary gland biopsy confirming chronic sialadenitis allowed for the diagnosis of PSS. Management by corticosteroi
