Successful Awake Blind Nasal Intubation for Airway Management of a Giant Parapharyngeal Space Tumor in a Resource-Limited Setting.

INTRODUCTION: Parapharyngeal space (PPS) tumors are rare and account for approximately 0.5% of all head and neck tumors. Their close proximity to the upper airway can result in significant anatomical distortions, making airway management challenging. Awake tracheal intubation is recommended when a difficult airway is anticipated; however, advanced airway devices such as flexible fiberoptic bronchoscopes and videolaryngoscopes may not be available in low-resource settings. Awake blind nasal intubation (ABNI), although rarely practiced today, may be a feasible alternative in carefully selected p

Beyond the Acute Phase: Prevalence and Clinical Characteristics of Unreported Swallowing Impairments in Ambulatory Outpatient Stroke Survivors.

PURPOSE: Unreported dysphagia, imaging-identified swallowing impairment without symptom reporting, remains poorly characterized beyond the acute phase of stroke. This study aimed to (a) quantify the point prevalence of unreported dysphagia in ambulatory stroke survivors, (b) examine demographic and clinical factors associated with the presentation of this phenomenon, and (c) evaluate concordance between patient-reported symptoms and imaging-derived impairments. METHOD: This retrospective secondary analysis included 45 ambulatory adults ≥ 3 months post-ischemic stroke who underwent videofluoro

Deep learning-based automated scanline localization in suprahyoid muscle ultrasound for quantitative dysphagia assessment.

Ultrasound-based assessment of suprahyoid muscle (SHM) motion has emerged as a promising approach for evaluating dysphagia. However, quantitative analysis remains limited by operator-dependent scanline placement and variability in manual interpretation. This study proposes an integrated framework combining mathematical modeling, automated SHM segmentation, scanline determination, and deep learning-based analysis for objective dysphagia assessment. A total of 371 ultrasound images acquired from patients with normal, mild, and severe dysphagia were analyzed. Clinically relevant SHM-derived varia

Prevalence and appropriateness of solid oral dosage form manipulation in long-term care facilities.

PURPOSE: Medication administration in long-term care facilities (LTCFs) is complicated by multimorbidity, frailty, cognitive impairment, polypharmacy, and dysphagia. Solid oral dosage form (SODF) manipulation is common but may compromise medication safety. We aimed to determine the prevalence of SODF manipulation and evaluate its appropriateness in Italian LTCFs. METHODS: We conducted a national multicenter point-prevalence study (Prescription-Day LTCFs 2024) in 82 Italian LTCFs. Of the 3400 enrolled residents, 3174 were included after excluding residents with acute conditions and facilities

Avoidant/restrictive food intake across the gastrointestinal tract.

BACKGROUND: Avoidant/restrictive food intake disorder (ARFID) is characterized by reduced dietary intake leading to medical and/or psychosocial impairment, independent of body image concerns. We aim to assess ARFID screening and characteristics in patients with gastrointestinal disorders versus healthy volunteers (HV), and its associations with symptom severity, extraintestinal symptoms and depression scores. METHODS: We analyzed 92 HV and 467 patients from seven tertiary cohorts: irritable bowel syndrome (IBS, n=88), functional dyspepsia (FD, n=83), constipation (n=59), reflux (n=107), globu

The Gut-Brain Axis in Parkinson's Disease: From Clinical Observation toward Mechanism-Based Precision Medicine.

Gastrointestinal dysfunction is increasingly recognized as a core feature of Parkinson's disease (PD), affecting quality of life, nutrition, medication response, and potentially the earliest stages of pathogenesis. This perspective traces the evolution of the gut from a neglected source of non-motor symptoms to a central component of biological models of PD. Gastrointestinal manifestations are physiologically heterogeneous and require objective phenotyping to distinguish dysphagia, gastroparesis, slow transit, defecatory dysfunction, and overlapping mechanisms. The body-first/brain-first frame

Surgical management of an unstable occipital condyle fracture in a dog using a patient-specific CNC-milled titanium implant: a case report.

A 10-year-old castrated male Pomeranian was evaluated after a fall and presented with tetraparesis, dysphagia, and headturn. Magnetic resonance imaging (MRI) revealed a right occipital condyle fracture that caused brainstem compression and cerebellar infarction from a displaced bony fragment. According to the Anderson and Montesano classification in human medicine, this case was presumed to be Type 3, indicating instability, and surgical correction was considered. A bone fragment located in the medulla oblongata was left in situ owing to the potential risk of severe complications from removal.

Gastric volvulus following Nissen fundoplication: a case report and review of reported cases, risk factors, and management strategies.

Gastric volvulus is a rare but potentially life-threatening complication following Nissen fundoplication, with delayed diagnosis increasing the risk of gastric ischemia, necrosis, and perforation. We report the case of a 72-year-old man who presented on postoperative Day 10 with progressive dysphagia, severe epigastric pain, and intolerance to oral intake following robot-assisted laparoscopic Nissen fundoplication. Computed tomography and upper gastrointestinal contrast study demonstrated marked gastric distension with obstruction at the gastroesophageal junction. Urgent diagnostic laparoscopy

The Contemporary Association of Hospital volume with Outcomes of Surgery for Vestibular Schwannoma in the United States.

BACKGROUND: Despite advancements in microsurgical technique, perioperative morbidity following resection for vestibular schwannoma (VS) remains significant. Given contemporary care paradigms, we characterized the presence of a volume-outcome relationship in a contemporary national cohort. METHODS: The 2013-2022 Nationwide Readmissions Database was queried to identify adults undergoing VS resection. Hospitals were classified as low- (LVH) and high-volume hospitals (HVH), based on annual thresholds developed from restricted cubic spline models. Multivariable logistic regressions were subsequent

Current status of minimally invasive surgery for thoracic esophageal diverticula: systematic review and meta-analysis.

INTRODUCTION: Thoracic esophageal diverticula, including epiphrenic and parabronchial types, are rare clinical entities representing about 30% of all esophageal diverticula. Thoracic diverticula of the pulsion type have traditionally been associated with motility disorders and treated by stapled diverticulectomy with or without myotomy/fundoplication in patients experiencing disabling symptoms of dysphagia, regurgitation or pulmonary aspiration. Purpose of this study was to review the existing literature and estimate outcomes of minimally invasive surgery. METHODS: Systematic review and meta-

Technology to remotely capture key elements of swallowing and speech: State of the science in Parkinson's disease.

Swallowing and speech impairments are common in Parkinson's disease (PD) and can significantly affect health and quality of life. Traditional methods for assessing and managing these functions rely on in-person visits and large, expensive equipment, which are inaccessible to many patients. Recent advances in health technologies offer opportunities for remote monitoring and assessment, enabling early detection, personalized intervention, and longitudinal tracking of swallowing and speech deficits in PD. This review synthesizes evidence on current and emerging technologies designed to remotely c

A comparison of the upright seated versus left lateral decubitus position during pediatric sedation-free transnasal endoscopy.

OBJECTIVES: Although sedation-free transnasal endoscopy (TNE) is conventionally performed in an upright seated position in pediatric patients, emerging data suggest that the left lateral decubitus (LLD) position may improve both endoscopic visualization and patient comfort and safety. This study compared TNE outcomes and tolerance in pediatric patients using the upright versus LLD position. METHODS: A retrospective review was conducted on 110 pediatric patients who underwent sedation-free TNE from September 2024 to November 2025. Three cases were excluded for scope failure during the procedur

Patient-reported swallowing burden and FEES findings in disease-free oropharyngeal cancer survivors after chemoradiotherapy.

PURPOSE: Oropharyngeal dysphagia is a frequent long-term sequela in survivors of oropharyngeal cancer treated with chemoradiotherapy (CRT), with significant impact on quality of life (QoL). Flexible endoscopic evaluation of swallowing (FEES) provides objective information on swallowing physiology, whereas patient-reported outcome measures (PROMs) capture perceived dysphagia burden. This study explored how FEES findings and PROMs provide complementary information regarding dysphagia in disease-free survivors treated with CRT. METHODS: A cross-sectional study was conducted in 38 disease-free su

Biomechanical Assessment During High-Resolution Impedance Manometry Enhances Characterization of Inconclusive Esophagogastric Junction Outflow Obstruction.

BACKGROUND: Esophagogastric junction outflow obstruction (EGJOO) identified on high-resolution manometry (HRM) remains a clinically heterogeneous and inconclusive finding. Additional complementary testing with functional lumen imaging probe (FLIP) or timed barium esophagram (TBE) is recommended to confirm clinically relevant obstruction. We aimed to assess the association of novel Four-Dimensional (4D) HRM metrics against FLIP and TBE outcomes among symptomatic patients with inconclusive EGJOO on HRM. METHODS: Adult patients with inconclusive HRM-EGJOO who completed corresponding FLIP and TBE

Association between perioperative factors and the occurrence of postoperative cerebellar mutism syndrome in children.

PURPOSE: Postoperative cerebellar mutism syndrome (pCMS) is a complex condition involving neurological impairment including mutism, hypotonia and dysphagia, that occurs after resection of infratentorial lesions predominantly in children. While the literature has reported several preoperative risk factors, there is little knowledge on the surgeon's role and perioperative factors. Thus, we aimed to examine the association between perioperative features and pCMS. METHODS: This single-center study included 83 patients who underwent microsurgical resection of a cerebellar lesion in 2013-2022. Cont
