Primary Small Cell Carcinoma of the Liver Presenting With Acute Liver Failure.

Primary small cell carcinoma of the liver is an exceptionally rare malignancy and must be distinguished from metastatic pulmonary small cell carcinoma by excluding a pulmonary primary. We report a 55-year-old man with suspected alcohol-related cirrhosis and diabetes who presented for elective lithotripsy and was found to have jaundice, hepatomegaly, and acute liver dysfunction. Laboratory evaluation demonstrated marked hyperbilirubinemia and transaminitis with normal alpha-fetoprotein. Imaging revealed interval hepatomegaly with diffuse hepatic involvement compared to CT performed three months

Prolonged Survival in Metastatic Pancreatic Acinar Cell Carcinoma Treated with Hepatic Arterial Infusion of Oxaliplatin: A Case Report with Longitudinal Molecular Profiling.

INTRODUCTION: Pancreatic acinar cell carcinoma (PACC) is a rare malignancy, representing 1-2% of pancreatic cancers. Although it generally has a better prognosis than ductal adenocarcinoma, optimal systemic management remains undefined. Hepatic arterial infusion (HAI) of chemotherapy has shown efficacy in colorectal liver metastases but is rarely reported in pancreatic cancer. CASE PRESENTATION: We present a 36-year-old man diagnosed with metastatic PACC after developing abdominal pain and hepatomegaly. Imaging revealed a pancreatic tail mass and multiple liver metastases. After an initial pa

Pediatric acute liver failure in a child with arsenic toxicity and underlying rotor syndrome.

Pediatric acute liver failure can result from infections, metabolic disorders, autoimmune diseases, toxins, or malignancies. It is crucial to consider herbal medicines as an etiological factor in such cases. We report a 7-year-old boy presenting with PALF from arsenic toxicity after consuming herbal medicine. He presented with jaundice, fever, vomiting, and lethargy, with hepatomegaly and signs of raised intracranial tension on examination. He had taken herbal medicine for the jaundice. His toxicology screen revealed high levels of arsenic and lead in serum and urine. Whole exome sequencing in

Discovery of DDO-4279 as a PP5 degrader for the treatment of obesity.

PP5, a key member of the serine/threonine phosphatase family, is closely linked to tumors and other diseases such as obesity. However, developing highly specific and potent PP5 modulators remains challenging due to the high homology and strong electronegativity of its catalytic domain. Serendipitously, we identified the PP5 degrader DDO-4279 from our previously established PP5 inhibitor library. Mechanistically, DDO-4279 selectively degrades PP5 via the ubiquitin-proteasome pathway, thereby downregulating PP5 substrate proteins PPARγ and GR, a mechanism entirely distinct from that of conventio

Hepatobiliary Abnormalities in Children With Sickle Cell Anemia Attending a Tertiary Hospital in Northwestern Tanzania.

Hepatobiliary complications are common in children with sickle cell anemia (SCA) due to chronic hemolysis and vaso-occlusion, but are often underdiagnosed in low-resource settings where imaging is limited. This hospital-based cross-sectional study, conducted from March to May 2023, assessed the prevalence and characteristics of hepatobiliary abnormalities using ultrasound among children younger than 18 years with SCA at a tertiary hospital in Tanzania. Demographic and clinical characteristics, laboratory findings, and abdominal ultrasonography were obtained. Logistic regression was used to ide

Orthotopic Liver Transplantation for Giant Hepatic Hemangioma Involving the Entire Liver With Intra-Abdominal Hypertension: A Case Report.

INTRODUCTION: Hepatic hemangioma (HH) is the most common benign liver tumor, usually <4 cm and asymptomatic. Rarely, lesions become massive or diffuse, causing severe abdominal mass effect or complications such as Kasabach-Merritt syndrome with consumptive coagulopathy. When surgical resection is unfeasible, liver transplantation (LT) may be considered. Adult LT for unresectable HH is extremely rare, with only 27 cases reported worldwide. The aim of this study is to describe a case of giant HH that encompassed the whole liver and caused abdominal hypertension, eventually requiring LT. CASE RE

Coronary computed tomography angiography findings of anomalous left hepatic vein drainage into the coronary sinus with complex anomalies: a case report.

BACKGROUND: Drainage of the left hepatic vein into the coronary sinus is an extremely rare anomaly reported in the literature. Typically, haemodynamics remain unimpaired in the absence of concomitant anomalies, and detection is expected to be incidental. CASE SUMMARY: We herein report a case of a 7-year-old male patient presenting with atypical chest pain, exertional dyspnoea, and mild congestive hepatomegaly, in whom a left hepatic vein draining into a dilated coronary sinus was detected. This anomaly was associated with a superior-inferior type sinus venosus atrial septal defect, persistent

Successful propranolol therapy for infantile hepatic hemangioma with cardiac failure.

Infantile hepatic hemangioma (IHH) can cause severe complications such as high-output cardiac failure and growth faltering, requiring prompt diagnosis and intervention. In the absence of standardized treatment guidelines, propranolol has emerged as an effective and well-tolerated therapy. We report a female infant with multifocal IHH who presented with high-output cardiac failure, hepatomegaly, and growth faltering. She experienced rapid and sustained clinical as well as radiologic improvement with propranolol therapy. This case reinforces propranolol's role as a safe, effective first-line the

Association of Helicobacter pylori infection with insulin resistance, tumor necrosis factor-alpha, and dyslipidemia in obese children with metabolic syndrome.

BACKGROUND: Previous studies have suggested a link between Helicobacter pylori (H. pylori) and metabolic complications outside the gastrointestinal tract, but the relationship with pediatric obesity remains unclear. We aim to study the correlation between H. pylori infection and components of insulin resistance (IR), inflammation [tumor necrosis factor-alpha (TNF-α)], and cardiometabolic parameters in obese children/adolescents with a metabolic syndrome (MetS) phenotype. AIM: To elucidate the association between H. pylori infection and metabolic disturbances in children with obesity and MetS,

Urgent Chemotherapy for Aggressive Metastatic Neuroendocrine Cancer With Impending Hepatic Failure: A Case Report.

Neuroendocrine carcinoma is a rare, biologically diverse malignancy that frequently presents with metastatic disease, which can carry a poor prognosis. Its symptomatology is often nonspecific, resulting in delayed diagnosis with substantial tumor burden. Platinum-based chemotherapy remains the cornerstone of treatment, although outcomes remain poor and novel therapeutic strategies are needed. We report a case of newly diagnosed metastatic neuroendocrine cancer in a patient presenting with vague constitutional and gastrointestinal symptoms, ultimately complicated by impending hepatic failure fr

Predictive factors for severity and poor treatment response in children with Evans syndrome: A retrospective cohort study.

Evans syndrome (ES) is a rare disorder characterized by autoimmune cytopenias affecting multiple blood cell lineages. In children, management remains particularly challenging due to the absence of clear guidelines for acute treatment and escalation to second-line therapies. We conducted a retrospective, longitudinal study (2010-2024) including pediatric patients (<18 years) with ES to identify predictors of severe presentation, need for second-line therapy, and fatal outcome. Predictors were identified using Cox Regression. Fifty patients were included (sequential = 27, concomitant = 23), with
