BACKGROUND: While typhoid conjugate vaccines (TCV) offer promise for reducing risk in endemic settings, their population-level impact remains unclear. In 2022, Nepal introduced TCV nationally on the heels of the COVID-19 pandemic, which disrupted healthcare services, surveillance, and potentially typhoid transmission dynamics, complicating vaccine impact evaluation. We investigated the impact of TCV introduction amid shifting typhoid burden during the pandemic.
METHODS: We analyzed blood culture data from four Kathmandu Valley health facilities, comparing culture positivity for Salmonella Typ

INTRODUCTION: Enteric fever, primarily caused by Salmonella enterica Typhi and Salmonella enterica Paratyphi A (SPA), is endemic mainly in South Asia, disproportionately affecting school-age children. Although typhoid conjugate vaccines (TCVs) are effective and implemented in many countries, no licensed vaccine exists against paratyphoid A. Bivalent vaccines targeting both S. Typhi and SPA may address this gap. Although field efficacy trials are not considered feasible, controlled human infection models (CHIMs) offer an alternative pathway for evaluating vaccine efficacy. This will be the firs

BACKGROUND: Typhoid poses a significant challenge in India, which has one of the highest burdens of the disease globally, making the implementation of cost-effective vaccination strategies essential. The success of typhoid vaccination, whether through clinical trials or large-scale public health programmes, depends on community acceptance. In the post-COVID-19 period, vaccine hesitancy and scepticism have emerged as key challenges to both vaccine trials and mass vaccination efforts. This study examines the barriers to and motivations for vaccine uptake during a mass vaccination campaign. METHO

Typhoid fever, predominantly caused by Salmonella Typhi, remains a significant global health concern and hence requires early-stage diagnosis to control the outbreaks of this pathogen. The currently used methods for detecting Salmonella Typhi require more turnaround time, up to 2-3 days, to yield results due to the requirement of bacterial culture and identification. This study represents the robust and specific detection of Salmonella Typhi using quantitative polymerase chain reaction (qPCR) in clinical and biological samples. Here, we describe the development and validation of a qPCR-based a

Typhoidal Salmonella continues to pose a severe public health threat, with its management increasingly complicated by the rise of antimicrobial resistance. This study investigated 50 clinical isolates of Salmonella Typhi (S. Typhi) and S. Paratyphi to delineate the association between antibiotic resistance, biofilm formation, and nanoscale mechanical traits. Our results revealed that 22% of isolates were multidrug-resistant (MDR), displaying the classical resistance pattern against ampicillin, chloramphenicol, and trimethoprim-sulfamethoxazole. Among these resistant isolates, 54% formed biofil

We report two such cases. Case 1: A 30-year-old male presented with right forearm swelling and pain; examination revealed a non-mobile, fluctuant intramuscular swelling. Case 2: A 30-year-old female had a 3-month history of midline chest pain with a mobile, fluctuant swelling over the xiphisternum. In both cases, aspirated pus showed numerous pus cells with few Gram-negative bacilli. Culture revealed non-lactose fermenting, smooth convex colonies identified as Salmonella Typhi by biochemical tests, Vitek 2 (bioMérieux), and serology. The isolates were susceptible to ceftriaxone, ciprofloxacin,

Antimicrobial resistance (AMR) poses a major challenge in treating infections such as pneumonia and typhoid fever, necessitating novel therapeutics. Plant-derived natural products provide a promising alternative. This study evaluated dichloromethane (DCM) and methanol (MeOH) extracts of Dipsacus inermis against six bacterial strains: Staphylococcus aureus, Bacillus subtilis, Escherichia coli, Pseudomonas aeruginosa, Salmonella typhi, and Enterobacter aerogenes. Antibacterial activity was assessed following standardized CLSI guidelines for both zone of inhibition (ZOI) and minimum inhibitory co

INTRODUCTION: Typhoid fever is an infection of the gastrointestinal tract caused by Salmonella Typhi. Ciprofloxacin is the most widely used second-line therapy; it provides good results in therapy. Indonesia has a unique resistance pattern of fluoroquinolones. Earlier studies of the gyrA and gyrB from local S. Typhi strains showed no mutation, and amino acid replacement was identified in all reported codons. In this study, we explored the whole sequence of the gyrA and gyrB further to understand the association of the unique characteristics of our local strains with fluoroquinolone resistance.

BACKGROUND: Typhoid fever, a potentially fatal disease caused by Salmonella enterica serovar Typhi, requires effective vaccines. This study aimed to design a recombinant subunit vaccine using the most immunogenic proteins from the Salmonella Typhi proteome.
METHODS: Initially, the most antigenic proteins were selected to predict linear T-cell, B-cell, and IFN-γ epitopes. A recombinant construct incorporating these epitopes, peptide linkers, and a molecular adjuvant was designed. Comprehensive evaluation assessed physicochemical properties, solubility, secondary/tertiary structure, antigenicit

The systemic establishment of the strictly human-adapted pathogen Salmonella Typhi critically depends on functional interplay between its virulence effectors and the host gut microbiome. Organized within a "colonization-toxicity-immunity- microecological intervention" framework, this review synthesizes recent molecular and multi-omics evidence to delineate key host-pathogen-microbiota dynamics. During colonization, a healthy gut microbiota exerts multi-layered colonization resistance through nutrient and niche competition, as well as via microbial metabolites such as short-chain fatty acids an

Enteric fever, a systemic infection caused by Salmonella enterica serovars Typhi and Paratyphi, continue to pose a substantial public health concern, with an estimated 20 million cases annually. It is transmitted via the consumption of contaminated food or water and is a substantial public health issue in low- and middle-income nations, resulting in extended fever, stomach pain, and severe sequelae if not addressed. The clinical diagnosis of enteric fever is complicated by nonspecific symptoms, the low sensitivity and specificity of blood cultures and serological methods, and the restricted ap

BACKGROUND: Typhoid fever remains a significant public health challenge in Malawi, particularly in urban and peri-urban areas with limited access to safe water, sanitation and hygiene. Although the typhoid conjugate vaccine has been introduced into the national immunization schedule, data on community knowledge, attitudes and practices (KAP) toward typhoid and its prevention remain scarce. This study assessed KAP levels among high-risk populations in Lilongwe to inform targeted public health interventions.
METHODS: A cross-sectional community-based study was conducted in May 2025 across selec

A primigravida at 26 weeks' gestation visiting from Pakistan presented with fever, abdominal pain and hypotension suggestive of sepsis and required admission to the intensive care unit. Blood culture grew a multidrug-resistant (MDR) Salmonella enterica serovar Typhi isolate that was resistant to cephalosporins but susceptible to carbapenems. Intravenous meropenem was commenced, resulting in a rapid clinical improvement within 48 hours. She completed a 14-day course of meropenem without maternal or fetal adverse effects, and repeat blood cultures were sterile. The remainder of her pregnancy was
