BACKGROUND: Febrile diseases such as Malaria, Typhoid fever, HIV/AIDS, Tuberculosis, respiratory tract infections, and urinary tract infections remain major public health concerns in low-to-middle-income countries (LMICs). Their prevalence is driven by interacting socioeconomic, environmental, behavioral, and biological factors such as vector exposure, poor sanitation, overcrowding, and high-risk behaviors including intravenous drug use and smoking. This study investigates the key risk factors influencing the prevalence and diagnosis of febrile diseases in Southern Nigeria. METHODS: A cross-se

INTRODUCTION: Typhoid intestinal perforation (TIP) remains a significant cause of pediatric morbidity in resource-limited settings, with prolonged hospitalization straining healthcare resources. Accurate prediction of hospital stay duration could optimize resource allocation and clinical decision-making, yet no TIP-specific prediction models exist. The objective of this research was to develop and validate a machine learning models for predicting prolonged hospital stay (>8 d) in pediatric TIP patients using readily available admission parameters, comparing logistic regression and random fores

In this work, a compartmental model of disease transmission due to typhoid fever, taking into account both the human-to-human transmission and the environmental route due to bacterial contamination of water resources, has been developed and analyzed. The human population under consideration are subdivided into four classes based on the epidemiological status of individuals with respect to typhoid fever transmission dynamics, these are, susceptible human, carrier humans, infectious humans, and recovered humans. There exist additional compartment which represent the concentration of bacteria in

The duration of fever defervescence in enteric fever has been observed to be increasing in recent times. An evaluation of the factors influencing fever defervescence in 150 children aged 1 month-18 years with confirmed (blood culture yielding Salmonella typhi or S. paratyphi) enteric fever was undertaken, and the minimum inhibitory concentrations (MICs) of ceftriaxone and azithromycin were determined. The median (q1, q3) time to fever defervescence was found to be 88 (58.75, 117.25) hours. Prior antibiotic therapy, splenomegaly and systemic complications were significantly associated with late

Salmonella enterica serovar Typhi (S. Typhi) is a leading cause of typhoid fever, significantly impacting morbidity and mortality in Ghana. However, genome-resolved data on circulating typhoidal strains remains scarce. We conducted this study to explore the genetic diversity, virulence, and antimicrobial resistance (AMR) profiles of S. enterica strains isolated from clinically diagnosed typhoid patients to inform targeted management and surveillance strategies. Twenty-eight S. enterica isolates recovered from stool and blood cultures were confirmed by PCR targeting 211 bp amplicon of the bcfD

INTRODUCTION: Typhoid fever remains a major Global public health concern, with treatment outcomes strongly influenced by antimicrobial resistance (AMR) and inter-patient variability. Determining the most appropriate antibiotic for an individual patient remains clinically challenging. Machine learning-based clinical decision support systems (CDSS) offer a promising avenue for improving diagnostic precision and guiding antibiotic selection using routinely collected clinical data.
METHODS: We developed a machine learning-based decision-support framework using XGBoost models to predict (i) treatm

BACKGROUND: Early diagnosis of typhoid is important for reducing mortality. In the absence of a blood culture, the quantitative tube Widal test is the standard serological test used for the diagnosis of Salmonella typhi , but rapid test kits are largely used in Northern Ghana. Variations in the performance of rapid test kits have been noted in other studies. We compared the diagnostic performance of two rapid test kits to the quantitative Widal method.
METHODS: This was a cross-sectional study conducted from April to May 2024 in the Northern Region of Ghana. Quantitative tube Widal and rapid

Typhoid toxin, a genotoxin secreted by typhoidal Salmonella serovars, has been implicated in diverse host responses ranging from DNA damage to cellular senescence. In this Research Highlight, we discuss how recent findings reveal an unexpected link between toxin-induced DNA damage and a noncanonical interferon-like response centered on ISG15. Using transcriptomic profiling and functional infection models, a recent report by Stark and colleagues shows that intoxicated cells mount a type-I interferon-like transcriptional program independent of interferons. Instead, ISG15 is upregulated via a STI

BACKGROUND: Typhoid fever incidence estimates are central to policy decisions on vaccine introduction and investments in non-vaccine prevention and control but are often unavailable. We explored whether prevalence metrics from sentinel studies of community-onset bloodstream infections could accurately predict local Salmonella enterica serovar Typhi (Salmonella Typhi) incidence.
METHODS: Using a previous systematic review (January 2018-December 2024), we identified studies reporting both typhoid incidence and prevalence of community-onset bloodstream infections from sentinel sites. From author

Background. Salmonella enterica serovar Typhi (S. Typhi), the causative agent of typhoid fever, continues to pose a major public health challenge in many low- and middle-income countries. The World Health Organization recommended typhoid conjugate vaccine (TCV) use in countries with a high burden of disease and/or high rates of antimicrobial resistance (AMR). Recent introductions of TCVs into national immunization programmes are expected to substantially reduce this burden. However, the impact of vaccine introduction on pathogen populations has not been widely investigated.Methods. To fill thi

Infected pancreatic necrosis is a major determinant of morbidity in acute pancreatitis and is usually caused by enteric flora. Isolation of Salmonella enterica serovar Typhi from pancreatic necrosis is exceptionally rare, and secondary superinfection by Enterococcus avium is even more uncommon. We report a middle-aged male who developed walled-off pancreatic necrosis following ERCP for gallstone disease. Initial drainage yielded S. Typhi. Persistent sepsis prompted repeat culture, which isolated E. avium, indicating sequential superinfection within the same cavity. This case highlights the imp

Despite the availability of conventional drugs, the use of medicinal plants for typhoid and malaria remains widespread in Nigeria. This study investigated the indigenous use of Cajanus cajan (pigeon pea) and Azadirachta indica (neem) in rural and urban Nsukka. We assessed: (i) socio-demographic factors influencing knowledge and use; (ii) the association between treatment practices and perceived efficacy; and (iii) socio-economic drivers of continued reliance on these herbal remedies. A cross-sectional survey of 400 respondents selected via stratified random sampling (200 urban, 200 rural) was

Quorum sensing (QS) in Salmonella enterica serovar Typhi (S. Typhi) is a cell-cell communication process mediated by molecules like Autoinducer-2 (AI-2). The production and detection of these molecules are population density-dependent, regulating behaviours essential to virulence and survival. Since the QS stage lies between the initial planktonic stage and the formation of a stable, persistent biofilm, it allows the pathogen to survive and become more virulent. Eradication of S. Typhi requires investigation using effective therapeutic agents, such as biologically-derived therapeutics. Convent

This prospective study reports an outbreak of enteric fever from Thane and nearby areas in Maharashtra, India, over a 4-month period between April 2025 and July 2025. Out of 167 culture-proven enteric fever cases, 44 (26.35%) exhibited ceftriaxone resistance. The majority of these (36 out of 44; 81.82%) were reported from a small geographical area of GB Road in Thane. Tanker water supply was significantly associated with ceftriaxone resistance.

BACKGROUND: This study investigates the transmission dynamics of typhoid fever and assesses the impact of environmental factors and public health interventions on disease spread. Typhoid fever, caused by Salmonella Typhi, remains a major public health concern in regions with poor sanitation, high population density, and limited access to clean water. Although environmental contamination plays a critical role in sustaining transmission, its contribution is often under explored in mathematical modeling studies.
METHODS: We developed a deterministic compartmental model incorporating environmenta

Enteric fever is a common systemic infection characterised by high grade fever and variable gastrointestinal symptoms. Splenic abscesses, thrombosis, and infarcts are uncommon complications. We report a case of splenic infarction in a young female presenting with persistent fever and abdominal pain. Her blood culture showed drugresistant Salmonella Typhi (S.Typhi). Due to persistent abdominal pain, abdominal imaging was performed, which revealed multiple splenic infarcts. Treatment with appropriate antibiotics led to complete resolution of symptoms. This case emphasises the importance of timel
