Annually, there are 5.57 to 9.23 million typhoid fever cases and 46,200 to 163,000 associated deaths, mainly in low- and middle-income countries. High incidence is estimated in the Southeast Asian, African, and Eastern Mediterranean (EMR) regions of the WHO. We reviewed EMR typhoid surveillance and vaccination recommendations to inform regional prevention and control efforts. Separate surveys were sent to surveillance and immunization officers in all 21 EMR countries and the occupied Palestinian territory, with 17 surveys (77%) and 13 surveys (59%) completed, respectively. Fifteen countries ro

BACKGROUND: In endemic countries, people remain at risk of contracting both malaria and typhoid concurrently. Though experimental animal models are very reliable for understanding human diseases, the rat model for Plasmodium-Salmonella coinfection remains underresearched.
OBJECTIVES: The present study was performed to characterize the rat model of malaria-typhoid coinfection focusing on physical, hematological, and biochemical parameters.
METHODS: Thirty-four Wistar rats were distributed into five groups including normal control, immunosuppressed, Salmonella-infected, Plasmodium-infected, an

Pathogen genomic surveillance is essential for monitoring high-risk lineages and antimicrobial resistance in Salmonella enterica serovar Typhi (S. Typhi), yet implementing whole-genome sequencing in sentinel hospital laboratories remains challenging. We evaluated whether mechanically sheared DNA, paired with high accuracy or super accuracy (SUP) basecalling, enables reliable Oxford Nanopore Technologies (ONT)-only sequencing for multiplexed S. Typhi genomic surveillance. Genomic DNA from three benchmark strains (Ty2, Ty42 and Ty43) was either mechanically sheared to ~15 kb or left unsheared an

Existing methods to identify patients infected with Salmonella enterica serovar Typhi or Paratyphi are not adequately accurate, affordable, or efficient. We evaluated the performance of antibodies to Salmonella Typhi hemolysin E (HlyE) and lipopolysaccharide (LPS) in Bangladesh, Nepal, and Pakistan for enteric fever case identification. We measured plasma concentrations of HlyE and LPS IgA in blood culture-confirmed enteric fever case-patients and in febrile controls with laboratory-confirmed alternative etiologies. Combining LPS and HlyE IgA discriminated enteric fever cases from other febril

Typhoid fever and malaria co-infection have been documented since the 19th century in addition to its significant impact on global population. The aim of the present study was to investigate the potential neurological damages of typhoid fever-malaria co-infection in rats. To perform this, 34 Wistar strain rats were divided into five groups, including a control group which received distilled water, an immunosuppressed group treated only with cyclophosphamide, a group mono-infected with Salmonella typhimurium; a group mono-infected with Plasmodium berghei, and a co-infected one which received bo

Typhoid fever, caused by Salmonella enterica subsp. enterica serovar Typhi (Salmonella Typhi), remains a significant global health challenge that is increasingly complicated by the emergence and spread of multidrug-resistant (MDR) and extensively drug-resistant strains. Growing limitations of antibiotic-centered treatment strategies have stimulated interest in anti-virulence approaches targeting bacterial regulatory networks rather than viability alone. Among these, quorum-sensing (QS), particularly the LuxS-mediated autoinducer-2 (AI-2) signaling system, has emerged as a potential regulator o

Licensed typhoid vaccines contain Vi polysaccharide (ViPS), a capsular antigen of Salmonella. Typhi (S.typhi), either unconjugated or conjugated to carrier proteins like tetanus toxoid (TT), diphtheria toxoid (DT), and Cross-reactive material 197 (CRM-197). Quantitation of ViPS during vaccine development, is challenging, as ViPS demonstrates limited adsorption to solid phases, resulting in poor binding and reduced sensitivity in conventional ELISA formats. We developed a novel competitive enzyme-linked immunosorbent assay (c-ELISA) using biotinylated ViPS as a detector probe for ViPS quantitat

PURPOSE OF REVIEW: Typhoidal and invasive non-typhoidal Salmonella (iNTS) serovars remain leading causes of bloodstream infection, mortality and morbidity, particularly in South Asia and sub-Saharan Africa. The rising burden of antimicrobial resistance has strengthened the case for vaccination as a control strategy. This review summarises recent advances in Salmonella vaccine development, highlighting progress, remaining challenges and future directions.
RECENT FINDINGS: The introduction of typhoid conjugate vaccine (TCV) programmes across some endemic countries represents a major public heal

BACKGROUND: Typhoid fever remains a health threat in endemic countries. In Indonesia, it causes an estimated 470,000 cases and 6000 deaths annually. We assessed the cost-effectiveness and budget impact of introducing the typhoid conjugate vaccine (TCV) into Indonesia's routine National Immunisation Program (NIP) at 9 months of age, with or without a catch-up campaign up to 15 years of age.
METHODS: A validated dynamic model of typhoid transmission was used to project health outcomes over a 30-year analytical horison for each scenario. Cost-of-illness data were drawn from the Surveillance of T

BACKGROUND: Membranoproliferative glomerulonephritis (MPGN) is an uncommon cause of renal dysfunction and is increasingly being reclassified into immune complex-mediated and complement-mediated forms. Infections, particularly in endemic regions, may act as disease triggers. Pregnancy further complicates renal pathology through physiological and immunological changes.
CASE PRESENTATION: We describe a 22-year-old pregnant woman presenting with generalized edema, proteinuria, and cola-colored urine. Renal biopsy revealed diffuse endocapillary proliferation with mesangial and capillary wall C3 de

Typhoid remains endemic in many low and middle-income countries, with the highest incidence reported in children under 15 years. No published study has estimated the cost of hospitalised paediatric typhoid cases in Kenya. This analysis estimated the direct medical, direct non-medical and indirect costs from the healthcare provider and societal perspectives and assessed the proportion of households that would experience catastrophic health expenditure (CHE) due to the disease.Patient-level data on hospitalised children with typhoid were drawn from 16 public hospitals that are part of the Clinic

BACKGROUND: Diseases caused by Salmonella enterica serovar Typhi and Salmonella enterica serovar Paratyphi remain a major public health concern. Due to overlapping clinical symptoms and the limitations of diagnostic methods, distinguishing S. Typhi from S. Paratyphi (A, B, and C) is challenging. Delayed and inaccurate diagnoses increase the risk of complications and transmission, highlighting the need for rapid, reliable molecular markers.
AIM OF STUDY: This study aimed to develop and validate gene-based molecular markers, using optimized PCR conditions, for the accurate identification of S.

Antimicrobial-resistant enteric infections may be difficult to treat, associated with severe outcomes, and costly. To determine if resistant enteric infections are more costly, we described direct hospitalization costs for hospital admissions for typhoid fever, salmonellosis, and shigellosis infections by resistance status. We included all inpatient admissions with culture-confirmed infections from approximately 300 hospitals in the Premier Healthcare database during 2012-2019. We classified typhoid fever, salmonellosis, and shigellosis infections as clinically resistant if laboratory testing

Salmonella enterica serovar Typhi (S. Typhi) is the aetiologic agent of typhoid fever in humans. The burden of typhoid is highest in low- and middle-income countries such as Pakistan. Moreover, the increasing emergence of antibiotic-resistant and hypervirulent S. Typhi strains highlights the need for deeper genomic understanding as current treatment regimens become progressively less effective. Therefore, this study analyzed publicly available S. Typhi genomes from Pakistan to characterize genomic diversity, sequence types, plasmid content, and antimicrobial resistance and virulence profiles.

BACKGROUND: The spread of extensively drug-resistant (XDR) Salmonella Typhi variants in South Asia has severely limited treatment options for typhoid fever, a life-threatening systemic infection affecting millions. Azithromycin has become the last effective oral drug against XDR typhoid; however, despite increasing azithromycin resistance, limited data exists concerning the correlation between in vitro susceptibility and clinical treatment efficacy.
METHODS: We developed a THP-1 macrophage infection model to evaluate time- and concentration-dependent azithromycin activity against intracellula

Chronic infection is important for the transmission of Salmonella enterica serovar Typhi (S. Typhi), an obligate human pathogen responsible for typhoid fever. The immunomodulatory action of typhoid toxin helps establish an initial stealth invasion of the intestinal tract that can lead to systemic spread, colonization of the gallbladder, and persistent shedding. Typhoid toxin contains a cell-binding pentamer and two catalytic subunits: cytolethal distending toxin B (CdtB, a DNase) and pertussis-like toxin A (PltA, an ADP-ribosyltransferase). It undergoes endocytosis and transport to the endopla

The human-restricted enteric pathogen Salmonella enterica serovar Typhi (S. Typhi) is the causative agent of the life-threatening typhoid fever. Although S. Typhi incidence is relatively low in the USA, routine surveillance of S. Typhi is critical to track the emergence and spread of high-risk lineages in non-endemic areas. In this study, we analysed 151 genomes of S. Typhi isolates from patients who were clinically confirmed with typhoid fever across New York State between 2016 and 2023. We used the GenoTyphi classification scheme and identified established multidrug-resistant and extensively
