Splenomegaly With Thrombocytopenia: A Case of the Rare and Obvious.

BACKGROUND: A 12-year-old boy presented with incidental splenomegaly, mild anemia, and thrombocytopenia, prompting evaluation, with Gaucher disease considered in the differential diagnosis. OBSERVATION: Enzyme testing revealed low β-glucosidase activity twice; however, Sanger sequencing was negative for mutations in the glucocerebrosidase 1 (GBA1) gene. A peripheral smear review was the turning point; it showed stomatocytes and giant platelets, suggestive of sitosterolemia. Next-generation sequencing confirmed a homozygous, likely pathogenic variant in ATP-binding cassette subfamily G member

Case Report: Breaking the diagnostic dilemma of "lymphoma-like" presentation: a case of adult-onset Still's disease characterized by generalized lymphadenopathy and extremely elevated ferritin.

Adult-onset Still's disease (AOSD) is a rare autoinflammatory disease, the clinical manifestations of which often overlap with other diseases such as lymphoma. This overlap makes differential diagnosis difficult, especially in patients with generalized lymphadenopathy, splenomegaly, and systemic metabolic hyperactivity. In this article, we discuss a 53-year-old East Asian (Chinese) female patient who had a fever, transient rash, generalized lymphadenopathy, and splenomegaly. An 18F-FDG PET/CT scan showed diffuse hypermetabolism in multiple lymph nodes, her spleen, and her bone marrow, and her

Multisystem imaging findings as diagnostic red herrings in Wilson's disease.

A previously well man in his 20s presented with fever, cough, new-onset jaundice and a right-sided pleural effusion. Initial investigations demonstrated marked hepatic synthetic dysfunction. Subsequent imaging demonstrated cirrhotic liver morphology with splenomegaly and ascites. Additional findings of gallbladder wall oedema and focal colonic wall thickening broadened the differentials but following specialist radiology review, these abnormalities were recognised as manifestations of portal hypertension rather than separate disease processes.Further investigations confirmed Wilson's disease i

Ectopic Duodenal Varices Secondary to Portal Hypertension Caused by Primary Hepatic Neuroendocrine Tumor.

Primary hepatic neuroendocrine tumors (PHNETs) are rare, often presenting with abdominal pain, carcinoid syndrome, or incidental imaging findings. PHNET-associated portal hypertension is uncommon. A 45-year-old man with known PHNET presented with massive upper gastrointestinal bleeding from a duodenal ectopic varix and hemorrhagic shock. Emergent esophagogastroduodenoscopy achieved hemostasis with hemoclips. He subsequently developed high-serum-ascites albumin gradient (1.4 g/dL); imaging showed a 23-cm hepatic mass, portal vein dilatation, and splenomegaly. Liver biopsy confirmed a well-diffe

Sitosterolemia presenting as unexplained cytopenias: hematologic manifestations and genetic spectrum.

BACKGROUND: Sitosterolemia is a rare autosomal recessive disorder caused by pathogenic variants in the ATP-binding cassette subfamily G member 5 (ABCG5) or ATP-binding cassette subfamily G member 8 (ABCG8) gene, which increase the intestinal absorption of plant sterols and reduce their biliary excretion. It is classically linked to xanthomas and premature cardiovascular disease; however, many patients first present with haematologic abnormalities, often leading to diagnostic delay. METHODS: We retrospectively reviewed six consecutive patients with clinical features suggestive of sitosterolemi

Angioimmunoblastic T-cell lymphoma with secondary hemophagocytic lymphohistiocytosis initially misdiagnosed as classical Hodgkin lymphoma: A case report.

RATIONALE: Angioimmunoblastic T-cell lymphoma (AITL), classified as nodal T-follicular helper cell lymphoma, angioimmunoblastic type, is an aggressive lymphoma frequently complicated by immune dysregulation. Owing to overlapping histopathological features, particularly Hodgkin/Reed-Sternberg-like cells, AITL may be initially misdiagnosed as Hodgkin lymphoma, leading to delayed recognition and inappropriate management. PATIENT CONCERNS: A 72-year-old man presented with progressive fatigue, recurrent fever, severe pancytopenia, and splenomegaly after an outside diagnosis of Hodgkin lymphoma bas

An Unexpected Cause of Severe Steatohepatitis in a Young Male.

A 24-year-old male with a body mass index (BMI) of 37.4 kg/m2 was referred to our hospital following the incidental discovery of abnormal liver echogenicity on a routine physical examination. The patient denied any history of alcohol consumption or illicit drug use and reported no family history of liver or metabolic diseases. Abdominal ultrasonography performed in December 2025 revealed heterogeneous liver echotexture with diffuse nodular hyperechoic areas, initially suggestive of nodular fatty infiltration or cirrhosis, and was accompanied by mild splenomegaly. Megakaryocytes in myelofibrosis: mechanisms of fibrotic niche remodeling and therapeutic implications.

Myelofibrosis is a clonal myeloproliferative neoplasm characterized by progressive remodeling of the bone marrow microenvironment. Megakaryocyte (MK) abnormalities are defining histological features of prefibrotic and overt primary myelofibrosis and contribute to disease pathogenesis. Dysplastic MKs can directly modify the pericellular extracellular matrix (ECM), but their main fibrogenic influence comes from coordinating stromal, immune, and endothelial responses. This Review synthesizes evidence linking abnormal MK biology to fibrotic progression and examines its implications for biomarkers

Myeloid-Specific Heparanase Aggravates Insulitis in Type 1 Diabetes via Heparan Sulfate Fragment-Dependent Amplification of Macrophage Polarization.

UNLABELLED: The intraislet heparan sulfate (HS) barrier is essential for β-cell survival, yet its role in type 1 diabetes (T1D) remains incompletely understood. We investigated the expression pattern, pathogenic function, and therapeutic potential of the heparanase (HPSE)-HS axis in T1D by integrating single-cell RNA sequencing data sets from human and mouse islets and peripheral immune cells, together with genetic mouse models and pharmacological intervention. HPSE was selectively enriched in myeloid cells, particularly CD14++ classical monocytes from patients with T1D and NOD mice and was as

Novel humanized loss-of-function NF1 mouse model of juvenile myelomonocytic leukemia.

Juvenile myelomonocytic leukemia (JMML) is a fatal pediatric cancer characterized by classical features such as splenomegaly, monocytosis, and granulocyte-macrophage colony-stimulating factor (GM-CSF) hypersensitivity, with RAS pathway mutations being the major drivers. Mutations causing loss-of-function of the Neurofibromin 1 gene (NF1LOF) occur in ∼20% of patients with JMML. NF1LOF drives upregulation of RAS/MAPK/PI3K pathways that leads to aggressive proliferation/differentiation of immature myeloid cells. Hematopoietic stem cell transplantation is the only curative option, but relapse occu

Microenvironmental cell interactions are essential for sustaining functionality of myelofibrosis malignant stem cells.

Cancer develops through the interactions between cancer stem cells and components of the tumor microenvironment (TME). To model in vivo cancer stem cell-TME interactions and elucidate their functional consequences, we focused on myelofibrosis (MF), a stem cell-driven myeloproliferative neoplasm. We cocultured MF hematopoietic stem and progenitor cells (HSPCs) with normal donor endothelial cells (ECs) and mesenchymal stromal cells (MSCs) to investigate the consequences of interactions between malignant MF HSPCs and nonmalignant microenvironmental cells. This tricultivation system proved to be a

Metformin Downregulates the STAT Pathway and Reduces Bone Marrow Fibrosis in Primary Myelofibrosis Patients: Final Results of the Phase II FIBROMET Trial.

Primary myelofibrosis (PMF) is a chronic myeloproliferative neoplasm characterized by the activation of the JAK-STAT pathway. Previous evidence showed that metformin might be a possible therapeutic option for treating JAK2-mediated myeloproliferative neoplasms. In vitro and in vivo studies demonstrated that metformin inhibits the JAK-STAT pathway, induces apoptosis in JAK2V617F-positive cell lines and reduces tumor burden and splenomegaly in Jak2V617F knock-in-induced mice. The FIBROMET trial, an open label phase II study, evaluated metformin effects on 10 primary myelofibrosis patients over 2

Effect of Partial Splenic Embolization on Immune Environment and Hepatic Function in Cirrhosis Patients With Portal Hypertension.

BACKGROUND/AIM: In hepatocellular carcinoma (HCC), portal hypertension (PHT) during the advanced stages of cirrhosis leads to splenomegaly. Additionally, the immune environment is an important factor in HCC treatment. Splenectomy and partial splenic embolization (PSE) are expected to improve this condition; however, data on the effects of PSE on immune function are limited. Therefore, we investigated the effects of PSE on the immune environment and hepatic function in patients with PHT-related cirrhosis. PATIENTS AND METHODS: The study included 117 of 238 patients who underwent PSE for PHT at
