Congenital Dyserythropoietic Anemia Type III Associated With a Novel KIF23 Variant (c.2132A>G; p.Gln711Arg): A Case Report.

Congenital dyserythropoietic anemia type III (CDA III) is an extremely rare inherited disorder characterized by ineffective erythropoiesis, multinucleated erythroblasts in the bone marrow, and variable clinical gravity. We report the case of a 6-year-old boy, presenting with abdominal distension, failure to thrive, dark urine, intermittent itching, and recurrent infections. Physical examination revealed pallor, hepatomegaly, and splenomegaly. Laboratory investigations showed mild normocytic anemia, high liver enzymes, and erythroid hyperplasia. The sequencing of the whole exoma identified a va

Does volume change of the spleen correlate with the therapy response in uveal melanoma patients with liver metastases undergoing hepatic artery infusion chemotherapy?

BACKGROUND: Uveal melanoma (UM) patients with liver metastases often undergo hepatic artery infusion therapy (HAIC). Due to diffuse metastatic spread in the liver, patients often develop hepatomegaly and secondary, portal hypertension which may lead to splenomegaly. This study aimed to compare spleen volumetry and the change of spleen volume (SV) for the evaluation of HAIC treatment response. PATIENTS AND METHODS: In this study, 179 UM patients (mean age 64.8 ± 11.0y, 53% female) with liver metastases undergoing HAIC were included. Treatment response was analyzed by RECIST 1.1 and SV on CT im

Refractory cold autoimmune hemolytic anemia as initial presentation of gastric B cell lymphoma: a case report.

BACKGROUND: Cold agglutinin disease is one of main two types of cold-antibody autoimmune hemolytic anemias, which can be either primary or, commonly, secondary to an infection or underlying lymphoproliferative disease [1]. We report a rare case of refractory cold autoimmune hemolytic anemia as an initial presentation of gastric B cell lymphoma. CASE PRESENTATION: A 53-year-old Sri Lankan woman with diabetes mellitus, dyslipidemia, and hyperthyroidism presented with increased fatigue and shortness of breath on mild exertion for 1 month. She reported yellowing of eyes and passage of dark urine.

Utility of 18F FDG PET/CT in Hemophagocytic Lymphohistiocytosis.

PURPOSE: Hemophagocytic lymphohistiocytosis (HLH) is a fatal systemic inflammatory syndrome caused by a wide array of causes, which may be detected on 18F fluorodeoxyglucose positron emission tomography/computed tomography (18F FDG PET/CT). This study explores the utility of 18F FDG PET/CT in HLH. MATERIALS AND METHODS: Retrospective data of HLH patients referred for whole-body 18F FDG PET/CT were analyzed for abnormal findings, and quantitative analysis using standardized uptake value (SUV), spleen-to-liver ratio (SLR), and bone-to-liver ratio (BLR) was done and correlated with laboratory pa

Challenges in diagnosis and management of pulmonary valve endocarditis associated with autoimmune lymphoproliferative syndrome (ALPS): A case report and literature review.

Isolated infection of the pulmonary valve is rarely encountered. The diagnosis is not considered based on clinical presentation or even on transthoracic echocardiography. A conservative approach is generally recommended for most patients with infective endocarditis. Our patient also had an autoimmune lymphoproliferative syndrome (ALPS), described as a rare hereditary disorder of lymphocyte homeostasis, resulting from mutations in the Fas apoptosis pathway. The elevated level of double-negative T cells (DNT) is considered a major diagnostic marker for this rare pathology. An 18-year-old female

Minimal Criteria to Screen for Wilson Disease: A Delphi Consensus in the United States.

BACKGROUND: The objective was to develop consensus on minimal screening criteria for Wilson disease (WD) diagnosis in US gastroenterology and neurology settings for implementation in clinical practice to support the timely diagnosis of WD. METHODS: A modified Delphi panel with three rounds was conducted. The first round survey was developed with input from a steering committee of four clinical experts in WD who set the analysis rules (consensus: ≥ 80%). Other US gastroenterologists/hepatologists or neurologists with experience treating WD were recruited using purposive sampling, and 32 were i

Primary Hemophagocytic Lymphohistiocytosis: A Severe Immune Dysregulatory Disease with Various Genotypic Features and Outcomes: A Cross-Sectional Study from a Tertiary Pediatric Center.

OBJECTIVE: Inborn errors of immunity (IEIs) are caused by deficiencies or functional abnormalities in the immune system, leading to increased susceptibility to infections, autoimmunity, autoinflammatory diseases, allergies, and/or malignancies. Primary hemophagocytic lymphohistiocytosis (HLH) arises from genetic mutations affecting the function of cytotoxic T lymphocytes and natural killer cells, while secondary HLH is triggered by infections, malignancies, rheumatologic disorders, or immune deficiencies. Treatment consists of remission induction, control of triggers, maintenance of remission,

Wilson's Disease with Autoimmune Hepatitis Manifestation in a Pediatric Patient: A Case Report and Literature Review.

Introduction: Wilson's disease (WD) and autoimmune hepatitis (AIH) are important causes of acute and chronic hepatitis; each can lead to serious complications. The coexistence of these two diseases in the same patient is rare and poses significant diagnostic and therapeutic challenges. The pathophysiological mechanism in WD involves hepatocellular necrosis and the exposure of intracellular antigens to the immune system, resulting in the production of low-titer autoantibodies, which may be misleading and complicate the differentiation between WD and AIH. Case Presentation: We report the case of

[Castleman disease with systemic manifestation: a case report].

Castleman's disease is a rare lymphoproliferative disorder. It typically presents with lymphadenopathy and may be associated with sometimes severe systemic manifestations. It can occur in two clinical forms: the unicentric form, which is limited to a single lymph node or lymph node region, generally has a favorable prognosis and is primarily treated with surgical excision; and the multicentric form, which is more severe, characterized by diffuse lymph node involvement and excessive cytokine production, particularly interleukin-6 (IL-6), which accounts for the systemic symptoms. Treatment in th

Hearing Loss in Infants and Children with Asymptomatic Congenital Cytomegalovirus Infection: An Update in Diagnosis, Screening and Treatment.

Cytomegalovirus (CMV) represents the most prevalent cause of congenital viral infection in newborns and the leading non-genetic etiology of sensorineural hearing loss (SNHL) in children. Notably, only 10-15% of congenitally infected infants possibly present with classic clinical symptoms at birth, including Small for gestational age, Microcephaly, Petechiae or purpura, Blueberry muffin rash, Jaundice, Hepatomegaly, Splenomegaly and abnormal neurologic signs. In contrast, approximately 90% of infected neonates exhibit no apparent symptoms initially. Current research predominantly focuses on sym

[Successful treatment of rapidly progressive Epstein-Barr virus-associated lymphoproliferative disorder with upfront allogeneic hematopoietic cell transplantation].

A 54-year-old woman presented with a chief concern of right back pain that had persisted for 3 weeks. Laboratory tests revealed pancytopenia, liver dysfunction, and coagulation abnormalities. She had hepatomegaly and splenomegaly but no lymphadenopathy. No abnormal cells were detected in the bone marrow or on a liver biopsy. Epstein-Barr virus (EBV) DNA levels in the peripheral blood were high at 6.44 log IU/ml, and clonality of EBV-infected cells was confirmed by Southern blot hybridization with a probe targeting the EBV terminal repeat. EBV-infected NK cells were detected using the magnetic
