Colorectal cancer presenting with dysarthria and ataxia: a case of isolated leptomeningeal metastasis.

We report the case of a 72-year-old man who presented with weight loss, anorexia and a change in bowel habit. Computed tomography (CT) of the chest, abdomen and pelvis demonstrated widespread thoracic and abdominal lymphadenopathy and a lesion within the splenic flexure. This was confirmed as an adenocarcinoma after a colonoscopic biopsy. A CT guided biopsy of the abdominal lymph node confirmed the adenocarcinoma. Within three days of admission, the patient developed worsening, progressive cerebellar symptoms that left the patient aphasic and bed bound. CT of the head was reported as normal. M

The endoscopically normal colon: when is mapping biopsy histopathologically justifiable?

OBJECTIVE: Mapping biopsy of endoscopically normal colon is a contentious area and generates considerable work for histopathology services. Managing demand for pathological testing is a current healthcare priority. In this retrospective audit, the authors aimed to establish diagnostic yield of mapping biopsy in this specific subgroup and identify situations where practice could be safely streamlined. DESIGN: Cases were retrieved over a 10-month period. Histopathology results were correlated with relevant endoscopy reports. The data were anonymised and analysed. SETTING: Department of Cellula

Colon cancer presenting with polymyositis-A case report.

INTRODUCTION: Colorectal cancer most often presents with a change in bowel habit, weight loss or with bleeding per rectum. Much less commonly, colorectal cancer may present as part of a paraneoplastic syndrome. Polymyositis is a rare disease most often considered a complement-mediated idiopathic inflammatory myopathy manifested by proximal muscle weakness. However, polymyositis may also be part of a paraneoplastic syndrome associated with an underlying malignancy. The relationship between polymyositis and malignancy is well known, but it has been suggested that tumours of the large bowel are r

Is earlier referral and investigation of bowel cancer patients presenting with rectal bleeding associated with better survival?

AIM: This study was carried out to determine whether rectal bleeding is related to stage of bowel cancer and whether earlier diagnosis and treatment are associated with improved survival. METHOD: Eight hundred and forty-five patients were identified in the Wessex Bowel Cancer Audit (1991-1994). Presenting symptoms were identified from case notes. Outcome measures included 5-year survival, Dukes' stage, metastatic disease at surgery and time from onset of symptoms to treatment, in patients presenting with rectal bleeding or other symptoms and signs. RESULTS: Six hundred and seventy-six (80%)

Cannabinoid hyperemesis.

A 21-year-old woman presented with a 4-week history of sudden onset vomiting, nausea and anorexia. Questioning revealed that she had a 7-year history of heavy cannabis use (smoking). She did not describe abdominal pain, change in bowel habit, antibiotic use, foreign travel or contact with gastroenteritis. Biochemistry results demonstrated mild metabolic derangement with a low potassium and a low bicarbonate, and urine toxicology was positive for cannabinoids. Other investigations, including a full blood count, renal function tests, liver function tests, a coagulation sample, an ECG, urinary β-

A human volunteer study to assess the impact of confectionery sweeteners on the gut microbiota composition.

Sweeteners are being sourced to lower the energetic value of confectionery including chocolates. Some, especially non-digestible carbohydrates, may possess other benefits for human health upon their fermentation by the colonic microbiota. The present study assessed non-digestible carbohydrate sweeteners, selected for use in low-energy chocolates, for their ability to beneficially modulate faecal bacterial profiles in human volunteers. Forty volunteers consumed a test chocolate (low-energy or experimental chocolate) containing 22.8 g of maltitol (MTL), MTL and polydextrose (PDX), or MTL and res

Laparoscopic enucleation of a jejunal mesenteric cyst: a case report.

Mesenteric cysts (MC) are a rare surgical condition occurring approximately in 1/200.000-350.000. The aetiology is unknown and the rarity of the tumor has led to confusion about their nature and classifi cation. They can be uni- or multi-locular, and are mostly benign. Approximately 830 cases have been reported in the literature and only four of them were found to be malignant. Cysts are usually diagnosed during routine abdominal examinations, they can present with various signs, such as acute abdominal pain, chronic abdominal pain, nausea and vomiting, or change in bowel habit. Although rare,

Who needs colonoscopy to identify colorectal cancer? Bowel symptoms do not add substantially to age and other medical history.

BACKGROUND: Many bodies advise that people with bowel symptoms undergo colonoscopy to detect colorectal cancer. AIM: To determine which bowel symptoms predict cancer on colonoscopy. METHODS: Information was collected on symptoms, demographics and medical history from patients subsequently undergoing colonoscopy. Multiple logistic regression modelling was used to identify predictors of colorectal cancer. An ROC curve was estimated for each model, and the area under the curve (AUC) was computed. RESULTS: Cancer was found in 159 patients and no cancer or adenoma in 7577 patients. Bowel symptom

Positive predictive values of ≥5% in primary care for cancer: systematic review.

BACKGROUND: The positive predictive value (PPV) for cancer of symptoms, signs, and non-diagnostic test results of patients routinely consulting a GP (unselected primary care populations) can help to determine when malignancy should be excluded. Comparisons with other illness indicate that a value of 5% or more may be regarded as highly predictive. AIM: To identify symptoms, signs, and non-diagnostic test results in unselected primary care populations that are highly predictive of cancer. DESIGN OF STUDY: Systematic review. SETTING: Primary care. METHOD: Fourteen bibliographic databases wer

Serum fatty acid profile in subjects with irritable bowel syndrome.

OBJECTIVE: Irritable bowel syndrome (IBS) is a fluctuating disorder of the gastrointestinal tract, characterized by abdominal pain and change in bowel habit. We wanted to investigate subjects with IBS for signs of disturbed intestinal absorption of fatty acids, as reflected in serum composition. MATERIAL AND METHODS: Serum samples were obtained from 32 adults with IBS, and from 59 controls. Serum fatty acids were analyzed by capillary gas-liquid chromatography. RESULTS: Especially the proportions of arachidonic acid (20:4 n-6) and the long-chain polyunsaturated fatty acids of the n-3 family

Improving detection of colorectal cancer.

Colorectal cancer is the third most common cancer in the U.K., with an annual incidence of 36,100 in England and Wales. It is also the second leading cause of death from cancer in the U.K. However, there has been a significant increase in five-year survival over the past decade, from 22% to 50% despite more than 55% of patients presenting with lymph node or distant metastases. Around 80% of colorectal cancer is sporadic, i.e., caused by the interaction of genetic and environmental factors via the adenoma-carcinoma sequence and cancer may take up to ten years to develop in this way. Adenomas ar

Detection and survival of colorectal cancer from a 2 week wait service.

AIM: To determine detection strategies for colorectal cancer (CRC) and to analyse subsequent survival from a 2 week wait (2WW) service. METHOD: Retrospective analysis of 2WW and hospital CRC databases from January 2006 to July 2009. Survival was assessed using Kaplan Meier survival curves and Cox's proportional hazard models. Proximal cancers were those proximal to and including the splenic flexure. RESULTS: From 1725 patients seen in a 2WW clinic, 108 were identified with CRC. Median follow-up in survivors was 514 days (interquartile range 160-788 days). Of 23 patients investigated for iron

Coeliac disease causing symptomatic hypocalcaemia, osteomalacia and coagulapathy.

A 36-year-old gentleman presented with 6 months of poor energy, tingling in fingers and weight loss with a change in bowel habit. He appeared cachectic and had clubbing, demineralisation of teeth, pectus carinatus, kyphosis, spinal tenderness, proximal muscle weakness and generalised muscle atrophy. Chvostek's and Trosseau's signs were positive. His haemoglobin (Hb) was 8.7 g/dl, MCV 64.7 fl with low iron. Calcium corrected was 1.30 nmol/l, parathyroid hormone 440.4 ng/l, vitamin D <12.5 nmol/l; INR was 2.7 with coagulation inhibitor studies negative. Radiographs of spine and pelvis commented

Anorectal pain, bleeding and lumps.

BACKGROUND: The patient presenting with anal pain, anal lump or rectal bleeding is a common occurrence in the general practice setting and the combination of symptoms usually gives an indication of the most likely diagnosis. However, careful examination including digital rectal examination is always required. OBJECTIVE: This article discusses three common anorectal conditions: perianal haematoma, haemorrhoids and anal fissure, and briefly discusses the less common, but not to be missed conditions: anal carcinoma and low rectal carcinoma. DISCUSSION: The majority of first degree haemorrhoids

Constipation does not develop following elective hysterectomy: a prospective, controlled study.

Although there have been reports that women develop constipation following hysterectomy, previous studies were either retrospective or uncontrolled. The aim of this prospective, controlled study was to assess whether constipation develops after elective hysterectomy. Women undergoing elective gynaecological surgery were compared to matched non-surgery controls at enrollment and 3 and 12 months after surgery. The subset of women who underwent elective hysterectomy was the study group for the present report. Fifty-eight of the 132 elective surgery patients underwent hysterectomy and were compare

The use of symptoms to predict colorectal cancer site. Can we reduce the pressure on our endoscopy services?

OBJECTIVE: Our objective was to determine the value of gastrointestinal symptoms and signs in predicting the site of colorectal cancer (CRC). These symptoms can subsequently be used in determining first-line investigation with either sigmoidoscopy or colonoscopy. METHOD: We interrogated the endoscopic and CRC databases ('Infoflex'), for patients diagnosed with CRC between April 2005 and March 2006 inclusive. These patients were cross-referenced with the pathology database and patient records. Information gathered from these databases include: age, gender, symptoms, site of cancer, histology,

Are screen detected colorectal cancers asymptomatic?

OBJECTIVE: Colorectal cancer (CRC) screening aims to detect asymptomatic disease and thus provide the opportunity for early diagnosis and treatment. This study assesses the prevalence of significant symptoms in patients found to have CRC detected through the NHS National Bowel Cancer Screening Programme (NHS NBCSP) pilots. METHOD: All patients in the NHS NBCSP pilots with a positive faecal occult blood completed a standardized symptomatology questionnaire before colonoscopy. This data was entered into the NHS BCS pilot database, data from the English arm has been analysed retrospectively. RE

The risk of colorectal cancer with symptoms at different ages and between the sexes: a case-control study.

BACKGROUND: Colorectal cancer is generally diagnosed following a symptomatic presentation to primary care. Although the presenting features of the cancer are well described, the risks they convey are less well known. This study aimed to quantify the risk of cancer for different symptoms, across age groups and in both sexes. METHODS: This was a case-control study using pre-existing records in a large electronic primary care database. Cases were patients aged 30 years or older with a diagnosis of colorectal cancer between January 2001 and July 2006, matched to seven controls by age, sex and pra

Management of irritable bowel syndrome.

Irritable bowel syndrome (IBS) is a functional disorder of the gastrointestinal tract, characterised by abdominal pain and change in bowel habit, with a fluctuating natural history. The exact etiology remains unknown, but it is unlikely there is a single unifying explanation. The prevalence in the general population is between 5% and 20%, and the condition represents a considerable financial burden to the health service. Guidelines for the management of IBS recommend that symptom-based diagnostic criteria should be used to make a positive diagnosis, without the need for recourse to investigati
