Alveolar echinococcosis drives functional reprogramming of hepatic CD8+ T cells.

BACKGROUND: Alveolar echinococcosis (AE), caused by the larval stage of Echinococcus multilocularis, exhibits infiltrative, tumor-like behavior in the liver and persists within its tolerogenic immune environment. Although T cells are central to host defense, the stage-specific remodeling of their lineage states during AE remains unclear. METHODS: A secondary AE infection model was established by portal vein injection of approximately 1,000 viable protoscoleces in C57BL/6 mice. Liver tissues collected at 3 days (3 dpi) and 3 months (3 mpi) post-infection were analyzed using single-cell RNA seq

A neuregulin-like ligand and EGF receptor underpin Echinococcus multilocularis development.

Alveolar echinococcosis (AE), a neglected zoonotic disease, is caused by the infiltrative growth of the metacestode larval stage of the cestode Echinococcus multilocularis within host organs. We previously showed that metacestode development relies on the mitotic activity of a population of parasite stem cells known as germinative cells. However, the molecular mechanisms that control Echinococcus stem cell dynamics such as cell-cycle progression, self-renewal, and differentiation remain poorly understood. Building on earlier reports implicating epidermal growth factor (EGF) signalling in stem

Population-based ultrasound prevalence and risk factors for cystic echinococcosis in endemic Kazakhstan.

BACKGROUND: Cystic echinococcosis (CE), caused by Echinococcus granulosus sensu lato, remains a significant public health concern in endemic areas of Kazakhstan. Despite global control efforts, CE persists due to insufficient deworming of dogs, the absence of control programs, and low public awareness. METHODS: A cross-sectional study was conducted between September 2023 and June 2024 in 51 remote villages across two endemic regions of Kazakhstan. Participants underwent abdominal ultrasound screening and completed a structured questionnaire to assess potential risk factors. Cyst staging was p

Inflammatory exacerbation of hepatic alveolar echinococcosis during dupilumab therapy: A case report.

BACKGROUND: Alveolar echinococcosis is a rare but potentially fatal parasitic disease characterized by slow, infiltrative hepatic growth. Disease progression is strongly influenced by host immune status, and atypical courses have been reported in immunocompromised individuals. Dupilumab, a monoclonal antibody targeting interleukin-4 and interleukin-13 signaling, is increasingly used for atopic diseases; however, its impact on latent helminth infections remains unclear. We report a case of hepatic alveolar echinococcosis with pleural involvement occurring during dupilumab therapy, suggesting a

Small particles of Echinococcus granulosus (spegs) and Echinococcus multilocularis (spems) promote follicular T helper cell expansion and are associated with IgE and IgG4 class switching in human lymph nodes.

BACKGROUND: Cystic echinococcosis (CE) and alveolar echinococcosis (AE) are zoonotic diseases caused by the larval stages of Echinococcus granulosus and E. multilocularis. Prior studies have identified small particles from the outer layer of parasitic lesions termed spegs (from E. granulosus) and spems (from E. multilocularis), which appear to interact with the human immune system, leading to lymph node enlargement. METHODS: We analyzed lymph nodes from CE (n = 3) and AE (n = 3) patients by immunohistochemical staining for spegs/spems and CD57. Automated image analysis revealed a significantl

Canonical WNT signalling governs Echinococcus metacestode development.

Alveolar echinococcosis (AE) is a lethal zoonosis caused by infiltrative growth of the metacestode larva of the tapeworm Echinococcus multilocularis in host organs. We previously showed that the Echinococcus metacestode is an evolutionarily unique, broadly posteriorized tissue, leading us to hypothesize that canonical WNT (cWNT) signalling, which patterns the body axis across metazoans, might be critical for metacestode formation. Here, we report effective RNAi-mediated knockdown of the E. multilocularis β-catenin gene (bcat-1), the central effector of cWNT signalling, in a primary parasite ce

Detection of Echinococcus multilocularis in coyotes in Washington State, USA highlights need for increased wildlife surveillance.

Echinococcus multilocularis is a zoonotic cestode that uses canids as definitive hosts and rodents as intermediate hosts. In humans, this parasite is the causative agent of alveolar echinococcosis. Recently, its range has been expanding across the Northern Hemisphere, and it is increasingly detected in wild canids, domestic dogs, and humans across Canada and the United States. While this expansion has been documented in isolated studies across the continent, a lack of routine sampling in wildlife hinders our ability to anticipate and mitigate further spread of E. multilocularis. We confirmed t

Hepatic alveolar echinococcosis mimicking cholangiocarcinoma: a case study.

BACKGROUND: Hepatic alveolar echinococcosis (HAE) is associated with significant disability and mortality. After infection, there is a prolonged asymptomatic latent period, and most patients present at a late stage when they seek medical treatment. The radiological features of HAE can resemble those of liver malignancies, posing diagnostic challenges. A 52-year-old male was admitted to our hospital with discomfort and pain in the upper right abdomen persisting for more than six months without an apparent trigger. Abdominal ultrasound revealed a heterogeneous mass in the right posterior lobe of

Diagnostic pearls and pitfalls in the radiologic evaluation of hepatic alveolar echinococcosis.

Hepatic alveolar echinococcosis is a tumor-mimicking parasitic disease characterized by infiltrative hepatic involvement and substantial radiologic overlap with malignancy. Predominantly solid morphology, pseudo-capsular margins, and deceptive enhancement patterns frequently contribute to misclassification as primary or secondary liver cancer. Intralesional calcifications on non-contrast computed tomography (CT) and the absence of true arterial phase hyperenhancement on CT and magnetic resonance imaging represent important diagnostic clues. Diffusion-weighted imaging may further aid characteri

Tearing Down the Wall: emu-miR-745-3p in Echinococcus multilocularis Exosomes Attenuates Host Liver Fibrosis to Permit Invasive Lesion Development.

BACKGROUND: Alveolar echinococcosis (AE), caused by Echinococcus multilocularis (E. multilocularis), exhibits tumor-like, invasive growth in the liver. Unlike cystic echinococcosis, AE lesions are bordered by loose fibrosis, not a dense capsule. How the parasite weakens this host fibrotic response remians unknown. METHODS: We identified functional miRNAs in parasite-derived exosomes via small RNA sequencing and multi-step screening, validated them using bioinformatics, luciferase assays, and in vitro studies in hepatic stellate cell studies (HSCs) and inhibited the target in vivo with AAV6-de

Compound Biejia-Ruangan tablets activate the STING-TBK1 pathway to alleviate hepatic fibrosis in alveolar echinococcosis.

UNLABELLED: Alveolar echinococcosis (AE) is a lethal parasitic disease that leads to progressive liver fibrosis, a process characterized by the dominance of M2 macrophages in advanced stages. While the compound Biejia-Ruangan tablet (CBRT) has demonstrated efficacy in reversing hepatic fibrosis, its underlying mechanisms in the context of AE-induced fibrosis are not fully understood. This study aimed to investigate the therapeutic potential of CBRT against AE-induced liver fibrosis and to elucidate its mechanisms of action. Serum levels of liver fibrosis markers and cytokines were measured by

[Pediatric Alveolar Echinococcosis with Pulmonary and Hepatic Involvement and Literature Review: A Rare Case].

Alveolar echinococcosis (AE) caused by Echinococcus multilocularis, is a rare but potentially fatal zoonotic infection. AE most commonly presents with multiple cysts in the liver, may spread to other organs through infiltration or metastasis and can mimic malignancy with fatal consequences if not diagnosed early. Its occurrence in childhood is extremely rare and simultaneous hepatic and pulmonary involvement has been reported only in a few cases. This report aimed to discuss a pediatric AE case with concurrent hepatic and pulmonary involvement, initially misdiagnosed as malignancy, in the ligh

Blockade of the CCL2-CCR2 axis attenuates fibrosis and parasite load in vesicular echinococcosis by inhibiting the PI3K-AKT pathway that regulates angiogenesis and hepatic stellate cell apoptosis.

BACKGROUND: The C-C motif chemokine ligand 2 (CCL2)-C-C chemokine receptor type 2 (CCR2) signaling axis is critically involved in angiogenesis and cellular invasion; however, the therapeutic potential of its targeted blockade on alveolar echinococcosis (AE) remains largely unexplored. This study aimed to determine whether RS504393, a selective CCR2 antagonist, can inhibit the progression of AE by blocking the CCL2-CCR2 axis and modulating its downstream pathogenic mechanisms. METHODS: In this study, bioinformatics analysis of the GSE124362 dataset was combined with molecular docking simulatio
