Neurodevelopmental Outcomes of Prenatally Diagnosed Isolated Cerebellar Hemorrhage by Fetal Magnetic Resonance Imaging: Case Series and Review of Literature.

BACKGROUND: The objective was to evaluate imaging and neurodevelopmental features in patients prenatally diagnosed with isolated cerebellar hemorrhage using fetal magnetic resonance imaging (MRI). METHODS: This was a retrospective-cohort case series of patients with isolated cerebellar hemorrhage who underwent fetal MRI over a 9-year period. Existing literature in the field is also reviewed. RESULTS: Nine patients were identified; male predominance (n = 6) was noted. Injury patterns on fetal MRI included left cerebellar hemisphere only (n = 4), bilateral cerebellar hemispheres and vermis (n

When the Electromyography (EMG) and the Patient Disagree: Unraveling a Motor Neuron Disease Mimic With Small-Fiber Dysfunction.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder characterized by upper and lower motor neuron dysfunction. Electromyography (EMG) is central to its evaluation, but neurogenic abnormalities and fasciculations are not specific to ALS and must be interpreted within the clinical phenotype and disease trajectory. This study reports a 52-year-old man with an approximately eight-year history of intermittent muscle twitching, bilateral upper-limb tremor, muscular tightness and warmth, and asymmetric cutaneous hypersensitivity. An initial electrodiagnostic evaluation rai

Impact of type 2 diabetes mellitus on acute levodopa responsiveness in Parkinson's disease: A Multicenter retrospective study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a common comorbidity in Parkinson's disease (PD) and may influence the progression and dopaminergic function, suggesting a potential impact on levodopa responsiveness. OBJECTIVE: To examine the impact of T2DM status on levodopa responsiveness in an acute levodopa challenge in PD. METHODS: We retrospectively analyzed the total and subdomain motor responses in the acute levodopa challenge in 136 PD patients with normal glycemic status, prediabetes, or T2DM across three German centers. RESULTS: Levodopa responsiveness did not differ significantly

Clinical Importance and Usefulness of Direct Targeting of the Ventral Intermediate Nucleus of the Thalamus Using Fluid and White Matter Suppression Imaging at 3T MRI.

BACKGROUND AND OBJECTIVES: Stereotactic targeting of the ventral intermediate nucleus of the thalamus (Vim) in tremor surgery traditionally relies on indirect, atlas-based coordinates, whereas direct visualization of the Vim using advanced MRI sequences has the potential to improve targeting precision and clinical outcomes. We evaluated the feasibility and clinical utility of fluid and white matter suppression (FLAWS) imaging for direct Vim targeting in the MRI-guided focused ultrasound (MRgFUS) procedure. METHODS: We retrospectively reviewed 30 MRgFUS procedures in 29 patients. Cases were di

A trembling detour to the diagnosis of encephalitis: a neuroborreliosis case report.

In this Clinical Rounds, we present a case of neuroborreliosis in a man aged 59 years who initially complained of headache, tremor, and hiccups. Diagnosis was delayed, partly due to an atypical presentation but mainly because subtle MRI findings were overlooked. There is emerging recognition that radial and patchy hyperintensities in cerebral peduncles, mesencephalon, and pons is a distinct MRI feature of neuroborreliosis. Dissemination of this knowledge could contribute to more timely and efficient diagnoses in future patients.

The good, the bad, and the ugly: A three-circuit model of Parkinson's tremor.

Tremor is a common and disabling symptom of Parkinson's disease, which classically occurs at rest and often re-emerges during simple actions. The dimmer-switch hypothesis proposes that the basal ganglia trigger tremor, whereas the cerebello-thalamo-cortical circuit amplifies it. Recent findings suggest that this model may be expanded to include a potential anti-tremor network. This bilateral cortical network, comprising somatosensory, posterior parietal, insular, supplementary motor, and dorsal premotor regions, shows activity that is inversely related to tremor amplitude and clinical tremor s

Clinical progress note: Management of the hospitalized patient with medetomidine withdrawal.

Medetomidine is an ⍺2 adrenergic agonist that has rapidly emerged as an adulterant in the US illicit opioid supply. Clinicians should suspect medetomidine exposure when opioid overdoses respond poorly to naloxone. Medetomidine withdrawal is clinically distinct from opioid and xylazine withdrawal and should be suspected when a patient with opioid withdrawal presents with significant rebound sympathetic activation, including hypertension, tachycardia, tremor, agitation, and refractory vomiting despite opioid agonist treatment. Management requires concurrent treatment of opioid withdrawal, early

Large-Scale EHR-Based Phenotyping of an FMR1 Premutation Cohort at a Referral Hospital.

BACKGROUND: Rare genetic diseases collectively affect hundreds of millions of people, yet their clinical features are usually defined in small and selected patient groups which do not represent the entire population. The FMR1 CGG repeat expansion, one of the most common trinucleotide disorders, illustrates what we know about a genetic carrier's susceptibility that predisposes to multiple medical problems, which unfortunately remains poorly characterised. FMR1 premutation-associated conditions (FXPAC) encompass a spectrum of clinical manifestations. FMR1 premutation carriers are at risk of deve

Beyond the Neck: Long-Term Motor and Non-motor Outcomes of Pallidal DBS in Craniocervical Dystonia.

Pallidal deep brain stimulation (DBS) is effective for treating cranial or craniocervical dystonia, a disorder characterized by abnormal facial expressions, impaired eyelid opening, speech and chewing difficulties, and abnormal head posture, leading to physical disability and functional impairment. However, long-term data on motor, non-motor, and quality-of-life outcomes remain limited with less robust available data than cervical dystonia. In 19 patients with craniocervical dystonia, the Unified Dystonia Rating Scale (UDRS) scores improved by 34.5% at 3-6 months, 51.0% at 1-2 years, 33.3% at

The close relationship between a rare disease and a common disease: Case report of a patient with gaucher disease and parkinsonism.

Gaucher disease (GD) is a lysosomal storage disorder caused by pathogenic variants in the GBA1 gene, which encodes glucocerebrosidase. GBA1 variants are also the most common genetic risk factor for Parkinson's disease (PD), although the penetrance of parkinsonism among carriers is incomplete. Patients with GD who develop parkinsonism may present an earlier onset and a more severe clinical course than individuals with sporadic PD. However, clinical data from underrepresented populations, including the Brazilian Amazon, remain scarce. We report the case of a 51-year-old woman from Belém, Pará, B

A Stand-Alone Wearable Measurement System for Continuous Hand Tremor Monitoring.

Progressive neurodegenerative diseases are often associated with motor symptoms, among which tremor is one of the most debilitating manifestations, significantly compromising daily activities and quality of life. Continuous, non-invasive monitoring of tremor characteristics helps to assess disease progression and optimize treatment strategies. For this reason, wearable measurement systems can provide an objective and non-invasive means for the quantitative characterization and monitoring of tremor-related movements. This study addresses the development of a compact, low-power wearable board fo

Interrater reliability of Tremor Characterization.

BACKGROUND: Tremor is a repetitive, oscillatory movement with multiple subtypes requiring accurate characterization for diagnosis and treatment. While frequency, amplitude, and context are routinely used to describe tremor, recent recommendations have emphasized oscillation shape and regularity. However, these descriptors lack strict operational definitions, and differences in how clinicians interpret them introduce inconsistency into tremor classification. OBJECTIVE: We quantified inter-rater reliability for commonly used tremor descriptors and determined which objectively measured signal pa

Adult-Onset Levodopa-Unresponsive Parkinsonism as the Presenting Feature of Aromatic L-Amino Acid Decarboxylase Deficiency.

INTRODUCTION: Aromatic L-amino acid decarboxylase is a critical enzyme required for the final step in the synthesis of dopamine and serotonin, two important neurotransmitters. Aromatic L-amino acid decarboxylase (AADC) deficiency is a rare autosomal recessive disorder of neurotransmitter biosynthesis caused by pathogenic variants in the dopa decarboxylase (DDC) gene. An absence or decrease of dopamine and serotonin levels may lead to severe motor and neurodevelopmental impairments. It classically presents within the first 6 months of life with a triad of hypotonia, oculogyric crises and develo

A Low-Cost, Open-Source Wireless Platform for Real-Time Tremor Monitoring in Parkinson's Disease.

We present a low-cost, open-source, wireless hardware platform dedicated to real-time tremor monitoring in Parkinson's disease (PD). The system is built on the ESP32-C6 microcontroller integrated with a 3-axis accelerometer and tilt sensor and employs ESP-NOW for wireless communication and multi-device synchronization. It supports up to five synchronized accelerometer nodes for assessing tremor and postural changes at the upper and lower limbs, as well as the jaw or chest. In this study, we demonstrate its application for intraoperative tracking of resting tremor and voluntary hand movements d

Parkinson's Disease: From Bench to Bedside-Advancements in Diagnosis and Therapeutics using Pharmacogenomic Approach.

This study provides a detailed overview of Parkinson's disease (PD), a neurodegenerative ailment mostly known for movement difficulties such tremor, stiffness, and bradykinesia, which affects approximately 1% of persons over the age of 60. Although the precise cause of PD is still unknown, various factors such as pesticide exposure, genetics, and lifestyle choices like smoking and caffeine consumption are thought to play a role in its development. The presence of Lewy bodies characterizes the disease, the aggregation of alpha-synuclein, the loss of dopaminergic neurons in the substantia nigra,

Rapamycin and Autophagy: Potential Therapeutic Approach for Parkinson's Disease Treatment.

Parkinson's disease (PD) is a chronic, progressive neurodegenerative disorder marked by the degeneration of dopaminergic neurons in the substantia nigra, leading to characteristic motor symptoms such as bradykinesia, tremor, and rigidity, as well as a range of non-motor manifestations including cognitive impairment, mood disturbances and autonomic dysfunction. Among the multiple cellular mechanisms implicated in PD, the dysregulation of autophagy has gained significant attention in recent years. Autophagy is a crucial intracellular degradation pathway responsible for the removal of misfolded p

Past genomes guide future conservation: insights from extinct populations of the endangered Pacific pocket mouse.

Efforts to recover endangered species often rely on restoring populations to their historical range, yet reestablishing lost genetic variation is challenging when the ancestral genetic landscape is poorly understood. The Pacific pocket mouse (Perognathus longimembris pacificus), a federally endangered heteromyid rodent, has been extirpated from most of its range in coastal southern California. Recovery efforts call for establishing new populations in their historic range through translocation, but the extent to which historical patterns of genetic variation can be recapitulated is unknown. Investigating the Effectiveness of Haptic Resistive Force Feedback to Improve Tremors in Parkinson's Disease: A Training Impact Study.

The goal of this study was to investigate the effectiveness of haptic resistive force feedback in improving the upper limb tremors in Parkinson's Disease (PD) during a button push task. We developed a haptic-mixed reality (HMR) button push simulation system that provides virtual button touch feedback synchronized with mixed reality 3D buttons and various levels of kinesthetic resistance force against hand movements. Two user studies were conducted: one to ensure the system's usability with 25 healthy individuals, and another to measure tremor improvement over a five-day training program with s
