Emerging Noninvasive Approaches for the Suppression of Pathological Tremor.

Pathological tremor affects over 40 million people worldwide, significantly impairing daily activities and quality of life. Pharmacological treatments show limited efficacy, with up to 30% discontinuation rates, while surgical interventions like deep brain stimulation achieve significant tremor reduction but are often unsuitable due to age, comorbidities, or personal preference. Recently, the need for safe and effective alternatives has led to the development of innovative, noninvasive, and patient-friendly technologies for tremor management. This clinical application review analyzed 134 studi

Real-Time Model-Free Adaptive Dual Control in Closed-Loop Deep Brain Stimulation: A Path to Individualized Parkinson's Treatment.

Deep brain stimulation (DBS) is an advanced clinical treatment for suppressing tremors in Parkinsonian patients. However, traditional open-loop DBS systems remain unable to adapt to patient-specific neural dynamics, often leading to suboptimal results. To address these limitations, this paper proposes a novel closed-loop DBS scheme based on a data-driven model-free adaptive control (MFAC) strategy, designed to effectively suppress pathological tremors hindering overstimulation and providing less power consumption. Using the basal ganglia (BG) system dynamics which is assumed to be completely u

Structural barriers control the spatial extent of slow earthquake slip.

Recent seismological and geodetic observations have revealed shallow slow earthquakes on subduction megathrusts, offering an opportunity to explore the factors controlling megathrust slip. Identifying these controlling factors helps constrain the potential updip extent of future megathrust ruptures. Here, we investigate the factors controlling the spatial extent of shallow slow earthquakes by integrating tremor locations with uncertainty estimates, detailed bathymetry data, and multichannel seismic data for the 2020-2021 slow earthquake sequence in the Kumano-nada region of the Nankai Trough.

Consensus expert recommendations for referral of Parkinson's disease patients for deep brain stimulation surgery.

Deep brain Stimulation (DBS) surgery effectively alleviates troublesome motor symptoms of Parkinson's disease (PD) such as tremor, rigidity, bradykinesia, motor fluctuations and dyskinesia. It also improves some non-motor symptoms and quality of life. DBS should thus be considered when these symptoms interfere with quality of life despite optimal medical treatment. While DBS benefits are clear, access by eligible patients remains low. Patients and their caregivers should be educated regarding DBS surgery and referrals for surgical evaluation should occur alongside ongoing medication adjustment

Integrative transcriptome-wide association analyses reveal PRKCG-linked GABAergic dysfunction in Fragile X-associated tremor/ataxia syndrome.

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder caused by CGG repeat expansions in the FMR1 gene. While CGG repeat toxicity is established, the precise molecular mechanisms driving neurodegeneration remain unclear. Here, we show that a multi-omics strategy combined with TWAS reveals brain-region-specific molecular signatures and striking gene dysregulation in inhibitory neurons. Using conditional mouse models, we demonstrate that selective expression of expanded CGG repeats in GABAergic neurons is sufficient to recapitulate key pathologic hallmarks of FXTAS.

Extremely rare case of isolated cerebellar amyloidoma: a short report.

Brain amyloidoma is an exceedingly rare localized deposition of amyloid that often radiologically mimics neoplastic or inflammatory disease. We report the case of a previously healthy 53-year-old woman who developed subacute right-hand tremor, pain, and weakness, followed by gait instability. Magnetic resonance imaging (MRI) showed a 35-45 mm hypervascular, infiltrative lesion in the right cerebellar hemisphere, extending into the vermis and middle cerebellar peduncle, with ill-defined margins suggesting of posterior fossa tumor. Subtotal resection was performed. Histopathology confirmed amorp

Low-frequency earthquakes track the motion of a captured slab fragment.

Accurate tectonic models are essential for assessing seismic hazard and fault interactions. However, the plate configuration at the complex Mendocino triple junction, where the San Andreas Fault and the Cascadia subduction zone meet, remains uncertain. We analyzed fault slip associated with a recently identified zone of tectonic tremor and low-frequency earthquakes (LFEs) near the southern edge of the subducting Gorda slab. Based on tidal sensitivity and P-wave first motions, we show that the LFEs are generated by dipping, strike-slip motion. This suggests that a former Farallon slab fragment,

Are symptoms clustered into latent classes in pediatric functional neurological disorder?

OBJECTIVE: DSM-5-TR and ICD-11 classify functional/dissociative seizures (FDS, formerly known as psychogenic nonepileptic seizures [PNES]) and functional movement disorder (FMD) under the broad category of functional neurological disorder (FND). However, substantial heterogeneity within FND has led some to hypothesize that FDS and FMD may represent distinct disease entities. To test this hypothesis, we aimed to determine whether the variation in FND symptoms among pediatric patients is best explained by either a single disease entity or two or more distinct disease entities. METHODS: We analy

Vertebrobasilar dolichoectasia compresses midbrain, worsens Parkinson symptoms, and evokes Holmes tremor: A case report.

RATIONALE: Parkinson disease (PD) is characterized by bradykinesia, resting tremor, rigidity, as well as other motor and non-motor symptoms due to damage to dopaminergic neurons in the substantia nigra. Holmes tremor (HT), otherwise, manifests itself as resting, intentional, and postural tremors in the limbs, resulting from lesions in the red nucleus of the midbrain. These 2 distinctive maladies seldom emerge simultaneously. This study delineates a case with both PD and HT occurring after vertebrobasilar dolichoectasia. PATIENT CONCERNS: A 54-year-old male patient experienced a gradual aggreg

Machine learning models for the prediction of levodopa response to tremor in Parkinson's disease.

OBJECTIVES: To develop and validate machine learning models to predict levodopa responsiveness of tremor in Parkinson's disease (PD) patients. METHODS: A total of 197 PD tremor patients underwent Levodopa Challenge Tests and were classified as having levodopa-responsive or levodopa-resistant tremor. Clinical and electromyogram (EMG) tremor analysis variables were recorded. The dataset was randomly divided into a training set (80%) and a test set (20%). To distinguish between the two groups, Support vector machine (SVM), random forest (RF), and logistic regression (LR) models were developed us

Discovering Causality Representations in Graph Convolutional Network for Automated Assessment of Parkinsonian Hand Postural Tremor.

Postural tremor of hands, a typical symptom of Parkinson's disease (PD), is assessed using the Movement Disorder Society-sponsored revision of the Unified PD Rating Scale (MDS-UPDRS). However, the manual assessment process is both subjective and time-consuming, underscoring the necessity for automated rating models in clinical practice. To accommodate large-scale applications, such models must effectively identify and mitigate the impact of confounding factors in complex clinical settings, ensuring stable and robust assessment performance. Therefore, we propose a causality-representation graph
