Incorporating Patient Perspectives into a Composite Score for Measuring Disease Progression in Spinocerebellar Ataxia (SCA).

INTRODUCTION: The spinocerebellar ataxia composite score (SCACOMS) comprises items from the functional Scale for the Assessment and Rating of Ataxia (f-SARA) and the Clinician Global Impression of Change (CGI-C). In the derivation of SCACOMS, weights reflecting 1-year responsiveness were assigned to each item using partial least squares (PLS) regression modeling. The current objective was to incorporate patient-feedback into the SCACOMS item weights, examine corresponding responsiveness of the composite scale, and discuss potential implications for future use. METHODS: Item weights derived by

A Feasibility Study of Real-Time FMRI with Neurofeedback of Motor Performance in Cerebellar Ataxia.

BACKGROUND/OBJECTIVES: Neurodegenerative cerebellar ataxia (CA) is a movement disorder caused by progressive cell death in the cerebellum. Motor imagery represents a potential therapeutic tool to improve motor function by "exercising" brain regions associated with movement, without the need for overt activity. This study assessed the feasibility of combining motor imagery with real-time functional magnetic resonance imaging neurofeedback (rt-fMRI-NF) to improve motor function in CA. METHODS: During finger tapping conditions, 16 participants with CA pushed a button at the same frequency in tim

Cerebellar and Outflow Tremor.

Cerebellar and cerebellar outflow tremors are significant movement disorders that pose therapeutic challenges. These tremors result from disruptions in the cerebellothalamocortical network, particularly affecting key nuclei and pathways. The primary clinical manifestation is an intention tremor characterized by low-frequency, high-amplitude oscillations and is often associated with ataxia and other motor deficits. Cerebellar outflow tremors exhibit diverse features across different states and typically occur after structural lesions in the midbrain or along the dentato-rubro-thalamocortical tr

Episodic Ataxia Associated With Synaptosomal-Associated Protein 25 (SNAP25) Variant: Beyond Epilepsy and Developmental Delay.

Developmental and epileptic encephalopathies (DEEs) of infancy and childhood are characterized by early-onset seizures and developmental impairment. Heterozygous missense or loss-of-function variants in synaptosomal-associated protein 25 (SNAP25), a core component of the presynaptic soluble N-ethylmaleimide-sensitive factor attachment protein receptor (SNARE) complex, have been implicated in a spectrum of DEEs with variable neurological features. We report a male child carrying a novel de novo splice-site variant in SNAP25, expanding the known mutational and phenotypic spectrum of this conditi

Novel genetic and nerve imaging characterization of Charcot-Marie-Tooth disease type 4F.

Charcot-Marie-Tooth disease type 4F (CMT4F) is a rare hereditary sensorimotor neuropathy, linked to the periaxin (PRX) gene. Early onset, pronounced sensory ataxia and comparatively moderate muscular weakness are characteristic hallmarks. We here report a child with corresponding features carrying a pathogenic PRX variant in trans with a very rare variant of uncertain significance in the 5’ untranslated region predicted to interfere with splicing. High-resolution ultrasound depicted an age-related pattern of nerve enlargement and muscle hyperechogenicity resembling other CMT4 subtypes with sim

Molecular Effect of Tobacco on Genetic, Epigenetic, and Metabolic Pathways During Cancer Progression.

Tobacco consumption remains a leading global health challenge, driving chronic diseases such as cancer, cardiovascular disorders, and metabolic dysfunction through intricate molecular mechanisms. This study investigates the multifaceted effects of tobacco exposure on genetic, epigenetic, and metabolic pathways, focusing on its role in carcinogenesis. Tobacco smoke, laden with carcinogens like benzopyrene, nitrosamines, and reactive oxygen species (ROS), induces genetic mutations and impairs DNA repair by downregulating tumor suppressor genes like tumor protein 53 (P53), ataxia-telangiectasia m

Targeting ATR in Cancer therapy: A drug discovery perspective.

The ataxia telangiectasia and Rad3-related (ATR) kinase plays a pivotal role in the DNA damage response (DDR), serving as a critical regulator of replication stress and genomic stability. As a central mediator of cell cycle checkpoint signaling, ATR activation enables cancer cells to survive under conditions of DNA damage, making it an attractive therapeutic target, particularly in tumors with high replication stress or DDR deficiencies. This review outlines a comprehensive overview of the biological functions of ATR, highlighting its mechanistic roles in DNA repair, cell cycle control, and ca

How to improve statistical power in a trial with SCA2 patients using natural history data.

Randomized, double-blind, placebo-controlled trials are the gold standard for evaluating treatment effects, as they minimize biases. However, they raise ethical concerns regarding participant exposure to potentially ineffective interventions. For rare neurodegenerative diseases, these trials are further challenged by the limited number of patients. Our goal is to leverage recent advancements in statistical methodologies to enhance the statistical power of these trials. We aim to test these methods to the ATRIL study, which evaluated the efficacy of riluzole in patients with dominant cerebellar

Design and Synthesis of Pyrrolo[3,4-d]pyrimidine-Based ATR Degraders for Effective Treatment of Colorectal Cancer in Mouse Model.

Ataxia telangiectasia and Rad3-related kinase (ATR) is a pivotal DNA damage response regulator. While several ATR inhibitors have entered clinical trials, none have yet been approved for therapeutic use. A potent ATR degrader A12 based on the pyrrolo[3,4-d]pyrimidine scaffold was designed and synthesized, with the ability not only to induce proteasomal degradation of ATR (DC50: 127 nM, Dmax: 72%) but also of CHK1 (DC50: 135 nM, Dmax: 70%) in several colorectal cancer cells. It exhibits strong antiproliferative activity (IC50: 55 nM) and rapidly triggers apoptosis. In LoVo xenograft mouse model

Precision Diagnosis of Wilson Disease Using a MultiGene Panel: Insights From a Prospective Cohort Study.

BACKGROUND AND OBJECTIVES: Wilson disease (WD) is an autosomal recessive disorder of copper metabolism. Conventional genetic diagnostics are low-throughput and may miss intronic, structural, or phenocopy variants, leading to delayed or missed diagnoses. In this study, we evaluate the utility of a custom next-generation sequencing (NGS) panel targeting the full-length ATP7B gene and 10 additional copper metabolism-related genes in patients with clinically suspected WD. METHODS: We conducted a prospective cohort study of 144 individuals at our neurogenetic center. Variants identified by NGS wer

Differential modulation of haematopoietic and oxidative injury by PARP-1 and ATR kinase inhibition in a murine model of acute irradiation.

UNLABELLED: Temporal Modulation of Acute Radiation Injury by Post-Exposure PARP-1 versus ATR Kinase Inhibition. BACKGROUND: PARP-1 and ATR inhibitors have been employed as radiosensitizers in cancer therapy. In addition to this, there is little evidence about the role of these chemicals after a dose of radiation, specifically in relation to reducing damage to healthy cells rather than increasing it. OBJECTIVE: The goal of this study was to determine if PARP-1 or ATR kinase inhibitors, when delayed administered, could decrease the severity of the condition caused by an acute dose of radiation

FET Fusion Oncoproteins Disrupt Physiologic DNA Repair and Create a Targetable Opportunity for ATR Inhibitor Therapy.

UNLABELLED: In cancers with genetic loss of specific DNA damage response (DDR) genes (e.g., BRCA1/2 tumor-suppressor mutations), synthetic lethal targeting of compensatory DDR pathways has translated into clinical benefit for patients. Native FET family members are among the earliest factors recruited to DNA double-strand breaks (DSB), and FET fusion oncoproteins drive a diversity of sarcomas and leukemias. A better understanding of both native FET protein and FET fusion oncoprotein function in DNA repair could reveal tumor-specific vulnerabilities and provide therapeutic opportunities. In thi

A phase 1 study of berzosertib (M6620, VX-970) in combination with cisplatin and radiation in patients with locally advanced head and neck squamous cell carcinoma (ETCTN 9950).

BACKGROUND: Ataxia telangiectasia Rad3-related (ATR) protein kinase regulates DNA damage response and is essential for tumor cell survival. Preclinically, ATR inhibition can sensitize tumor cells to radiation and chemotherapy. The authors conducted a phase 1 trial of berzosertib, a selective ATR inhibitor, in combination with definitive radiation and cisplatin in locally advanced head and neck squamous cell cancers (LA-HNSCC). METHODS: LA-HNSCC patients received daily radiation (2 Gy per fraction to 70 Gy) and weekly intravenous (iv) cisplatin 40 mg/m2. Berzosertib was administered (iv) once

Extracellular vesicles-mediated delivery of SpCas9 RNPs for therapeutic gene editing in Spinocerebellar Ataxia Type 3.

Spinocerebellar Ataxia Type 3 (SCA3) is a neurodegenerative dominantly-inherited disorder caused by an overexpansion of a CAG tract within the ATXN3 gene, conferring toxic properties to the ataxin-3 protein. Genome editing with CRISPR-Cas9 enzymes is a promising strategy to inactivate mutant ATXN3 alleles, however, in vivo delivery remains challenging. Extracellular vesicles (EVs) are promising delivery vehicles for Cas9 and single guide RNA (sgRNA) ribonucleoproteins that minimize genomic exposure to highly active endonucleases. In this study, we designed SpCas9 with a palmitoylation motif th
