An incidental heterozygous ATP7B nonsense variant leading to a diagnostic pitfall for Wilson disease: a pediatric case report.

BACKGROUND: Wilson disease (WD) is an autosomal recessive disorder caused by pathogenic variants in ATP7B, resulting in impaired copper transport and progressive copper accumulation, most prominently affecting the liver. With the growing use of genetic testing, incidental detection of ATP7B pathogenic/likely pathogenic variants in individuals without typical hepatic phenotypes can prompt overdiagnosis and unnecessary investigations. This case highlights the importance of interpreting a single heterozygous ATP7B variant in the context of phenotype and a standardized biochemical diagnostic pathw

Connecting gut microbiome composition to symptom severity and host genetics in endometriosis.

BACKGROUND: Endometriosis (EM) is a multifactorial, estrogen-dependent inflammatory disease characterized by high clinical variability. Growing evidence is highlighting the role of gut microbiome dysbiosis in EM pathophysiology; however, associations between microbiome, symptom severity, and host genetics remain poorly characterized. Extra-oral taste receptor genes (TGs), expressed in both the gastrointestinal and female reproductive systems, may represent novel mediators of gut-reproductive tract axis crosstalk. METHODS: A total of 50 women with surgically confirmed EM were deeply clinically

Postvoid Residual Urine as a Clue to the Diagnosis of Acute Bacterial Prostatitis: A Case Report.

Acute bacterial prostatitis (ABP) typically presents with fever and urinary symptoms; however, its diagnosis can be challenging in patients who lack characteristic findings such as perineal pain or prostatic tenderness. Although postvoid residual (PVR) urine may be associated with voiding dysfunction in ABP, its potential role as a diagnostic clue has received little attention. A 44-year-old man presented with a 2-day history of fever ranging from 38 to 39°C, irritative urinary symptoms, back pain, and difficulty initiating urination. At presentation, the patient was afebrile and had no perine

Diagnostic challenges of masked arginine vasopressin deficiency in neurosarcoidosis complicated by sodium-glucose cotransporter 2 inhibitor use.

A wide variety of endocrine disorders caused by sarcoidosis have been reported to date. We report the case of an 82-year-old male patient who presented with hypercalcemia and anterior pituitary hormone deficiency at the time of his sarcoidosis diagnosis and was subsequently diagnosed with arginine vasopressin deficiency (AVP-D) 6 months after the initiation of glucocorticoid therapy. Magnetic resonance imaging revealed an enlarged pituitary gland, thickened pituitary stalk, and loss of the posterior pituitary bright spot, raising the suspicion of hypothalamic-pituitary sarcoidosis. Despite imp

Incidence and prevention of postprocedural bacteraemia and complications following water vapour therapy of the prostate: a retrospective review and quality improvement initiative at a tertiary care centre.

BACKGROUND: Water vapour therapy of the prostate is a minimally invasive treatment for benign prostatic hyperplasia. While generally safe, rare but clinically significant complications, such as postprocedural bacteraemia, have been reported. Differentiating true complications from expected side-effects is essential for accurate risk communication and quality improvement. OBJECTIVE: The aim of this study was to evaluate the incidence of bacteraemia and other adverse events following these treatments and assess the impact of targeted interventions on patient safety. METHODS: A retrospective re

Female genital lichen sclerosus.

Female genital lichen sclerosus (FGLSc) is a chronic inflammatory scarring dermatosis that can lead to scarring and irreversible architectural change, sexual and urological dysfunction and malignancy. Although the aetiology is not completely understood, current evidence supports a multifactorial pathogenesis involving immune dysregulation, genetic susceptibility, epithelial stress and aberrant fibrogenesis, with emerging evidence for chronic irritation and occluded exposure to urine in susceptible individuals. Clinically, FGLSc presents with pruritus, soreness, dyspareunia and progressive scar

Polymers for Pelvic Floor Repair: Current State and Future Prospects.

IMPORTANCE: Surgical repair of pelvic organ prolapse and stress urinary incontinence frequently relies on polymeric biomaterials in sutures, meshes, and membranes to provide structural support during tissue healing and beyond. Polypropylene has long served as the default material owing to its strength, durability, and processability, but the chronic foreign body response and mechanical mismatch between stiff implants and viscoelastic pelvic tissue have been linked to complications, including mesh contraction, exposure, and dyspareunia. These limitations, together with regulatory actions on tra
