Association of preoperative frailty with health-related quality of life and urinary function after robot-assisted radical prostatectomy.

BACKGROUND: Frailty may influence outcomes after robot-assisted radical prostatectomy (RARP), but its association with longitudinal patient-reported outcomes remains unclear. This study aimed to examine the association of preoperative frailty, assessed using the Geriatric (G) 8 screening tool, with longitudinal health-related quality of life (HRQOL) and urinary status in patients who underwent RARP. METHODS: We retrospectively analyzed 328 patients who underwent RARP at a single institution. Patients were preoperatively classified into high, intermediate, and low G8 groups. HRQOL was assessed

Surgical architecture and component identifiability in robot-assisted radical prostatectomy: a systematic review and network meta-analysis.

Robot-assisted radical prostatectomy (RARP) combines preservation, reconstruction, and support maneuvers within named surgical packages, making component attribution difficult. To determine whether comparative evidence can identify independent surgical-component effects on strict early perfect continence while preserving oncological context. Comparative RARP studies were synthesized within a prespecified surgical-architecture framework. The primary outcome was perfect continence (0 pads/day) at 10-14 weeks. All prespecified bibliographic databases and registries were rerun through 26 September

Evaluation of nanofibrillated cellulose as a urethral bulking agent in female dogs with acquired urethral sphincter mechanism incompetence: a prospective clinical study.

Urethral bulking agent (UBA) implantation is a minimally invasive, effective and safe alternative treatment for refractory urethral sphincter mechanism incompetence (USMI). However, repeated implantations are frequently needed to maintain continence. Therefore, a prospective interventional clinical study with eight client-owned neutered female dogs was designed to evaluate the clinical efficacy and 12-month outcome of nanofibrillated cellulose (NFC), a novel biomaterial as a UBA. Concurrent causes of urinary incontinence were excluded with clinical and neurological assessment, blood and urine

Combined Sensate Focus and Pelvic Floor Muscle Training versus Pelvic Floor Muscle Training Alone for Sexual Function in Women with Stress Urinary Incontinence: A Randomized Controlled Trial.

BACKGROUND: Stress urinary incontinence (SUI) is the most common subtype of urinary incontinence among women, with reported prevalence ranging from approximately 15% to 35% depending on population and diagnostic criteria, and it impairs sexual function through physiological and psychological mechanisms. Pelvic floor muscle training (PFMT) addresses anatomical deficits but not psychosocial barriers such as intimacy avoidance and performance anxiety. Sensate focus, a psychobehavioral technique, reduces performance demand and reconnects couples with pleasurable bodily sensations. This trial evalu

Comparative outcomes of radical prostatectomy, radiotherapy, and radiotherapy plus androgen deprivation therapy for high-risk and very high-risk prostate cancer: a narrative review.

BACKGROUND AND OBJECTIVE: High-risk and very high-risk prostate cancer (PCa) accounts for approximately 15-20% of newly diagnosed cases and are associated with substantial risk of disease progression, metastasis, and cancerspecific mortality. The comparative effectiveness of radical prostatectomy (RP), external beam radiotherapy (EBRT), and radiotherapy combined with androgen deprivation therapy (RT + ADT) remains uncertain because randomized head-to-head trials are lacking. This narrative review aims to synthesize the comparative effectiveness of RP, EBRT, and RT + ADT in patients with high-r

Clinical Role of anogenital distance in Predicting pelvic floor training outcomes for stress urinary incontinence.

AIM: To evaluate the predictive value of anogenital distance (AGD) measurements, including the anus-to-clitoris distance (AGDAC) and genital hiatus (GH), on the success of pelvic floor muscle training (PFMT) in women with stress urinary incontinence (SUI). METHODS: This prospective study included 150 adult women with clinically confirmed stress urinary incontinence who met the inclusion criteria. All patients' demographic data were recorded, and the anogenital distance (AGDAC, AGDAF) and genital hiatus (GH) were measured using a digital caliper. Patients were then classified as responders or

[TRANSVAGINAL BLADDER NECK CLOSURE FOR PARAPLEGIC ADULTS - REPORT OF TWO CASES].

We report our initial experiences with transvaginal bladder neck closure for two females with a devastated bladder neck. In each, the bladder neck was closed following a circumferential dissection and the suture line covered using an anterior vaginal flap, with urine draining via a suprapubic cystostomy tube. (Case 1) A female in her twenties was presented with urinary incontinence. The patient had been paraplegic since childhood and initially managed with intermittent catheterization. Later, severe incontinence developed, and Foley catheter was placed. At presentation, the urethra and bladder

The link between inflammation and autonomic dysfunction in alpha-synucleinopathies - narrative review.

INTRODUCTION: Alpha-synucleinopathies are a group of disorders characterized by the accumulation of the pathological form of alpha-synuclein. This group comprises Parkinson's disease, Parkinson's disease dementia, multiple system atrophy, dementia with Lewy bodies, pure autonomic failure, and idiopathic REM sleep behavior disorder. Despite sharing a similar molecular basis, these disorders differ in their clinical presentation. Characteristic manifestations include signs of autonomic failure such as orthostatic hypotension, postural orthostatic tachycardia syndrome, constipation, gastroparesis

The pelvic floor: myth or hype?-a narrative review.

BACKGROUND AND OBJECTIVE: The pelvic floor represents a complex, dynamic neuromuscular structure integral to urinary, bowel, and sexual function. Despite its fundamental role, pelvic floor dysfunction remains under-recognised in both diagnosis and management within urology and gynaecology. Traditional perspectives have often confined its clinical relevance to stress urinary incontinence (SUI), overlooking its contribution to voiding dysfunction, chronic pelvic pain (CPP), sexual dysfunction (SD), and functional bladder outlet obstruction (FBOO). This narrative review aims to examine the pelvic

Shift work and hormonal-circadian disruption raise urinary incontinence: a narrative review.

BACKGROUND AND OBJECTIVE: Pelvic floor dysfunction (PFD) is a multifactorial condition that affects a large proportion of women, compromising urogenital support and producing symptoms such as urinary incontinence (UI). The structural and functional integrity of the pelvic floor depends on coordinated interactions among muscular, hormonal, and neuroendocrine systems. Evidence indicates that circadian misalignment, particularly in women exposed to night-shift work, sleep deprivation, and endocrine disruption, can influence pelvic floor physiology through melatonin suppression, cortisol dysregula
