BACKGROUND: Adult-onset leukoencephalopathy with axonal spheroids and pigmented glia (ALSP) is a rare, progressive white matter disorder caused by pathogenic variants in the CSF1R gene. It typically presents in mid-adulthood with progressive cognitive, motor, and neuropsychiatric decline, but its phenotypic variability often leads to delayed diagnosis.
CASE PRESENTATION: We describe a 41-year-old woman with no prior neurological history or relevant psychiatric tendencies who developed progressive cognitive impairment, urinary incontinence, gait disturbance, and sleep disruption over 3-4 years

Leptomeningeal metastasis (LM) is a rare complication of esophageal adenocarcinoma, with reported cases typically presenting with meningitic, encephalopathic, or multifocal neurologic symptoms. We report a 70-year-old man with metastatic esophageal adenocarcinoma with signet-ring cell features who developed subacute, painless, bilateral visual loss with bilateral optic disc edema as the initial manifestation of probable LM. Symptoms arose four months into first-line folinic acid, fluorouracil, and oxaliplatin (FOLFOX) plus pembrolizumab while all measurable extracranial disease was responding

BACKGROUND: Stress urinary incontinence (SUI) affects nearly half of U.S. women. Central adiposity, measured by waist circumference, may be more pathophysiologically relevant than BMI due to its direct impact on intra-abdominal pressure. However, whether age modifies this association remains unclear.
METHODS: We conducted a cross-sectional analysis of 894 non-pregnant women (55.8 % with SUI) from the 2021-2023 National Health and Nutrition Examination Survey (NHANES). Weighted multivariable logistic regression examined associations between SUI and waist circumference, age, parity, race/ethnic

BACKGROUND: Adolescent stress urinary incontinence (SUI) is challenging to manage and significantly impacts quality of life. Treatment with urethral bulking is well established in adults but is understudied in the pediatric and adolescent population.
CASE: A young female athlete presented with refractory SUI beginning at age 10. After conservative measures failed, she underwent urethral bulking injection at age 14 with temporary benefit. Repeat injection at age 16 provided seven months of complete continence and uninhibited athletic participation.
DISCUSSION: Urethral bulking is effective fo

BACKGROUND: Perineal urethrostomy (PU) is a surgical procedure performed in male cats with recurrent urethral obstructions or irreversible urethral damage, creating a permanent urinary stoma. Although PU effectively relieves obstruction, it can lead to postoperative complications that affect feline quality of life.
CASE DESCRIPTION: The present study assessed the short and long-term outcomes and complications following PU in 10 male cats treated at the Small Animal Hospital, Faculty of Veterinary Medicine, University of Tehran. A retrospective review of the medical records was conducted for m

Urinary incontinence (UI), an involuntary loss of urine, is a widespread pelvic floor disorder affecting 20-50% of women worldwide, with a 21% prevalence in Sub-Saharan Africa. Contributing factors include genetics, childbirth, and chronic conditions, leading to significant physical, social, and emotional challenges such as fear of leakage and isolation. Despite its impact, many affected women lack awareness of UI and available treatments, which include medication, surgery, and pelvic floor muscle training (PFMT). Knowledge gaps and misconceptions persist, hindering timely diagnosis and manage

IMPORTANCE: Stress urinary incontinence (SUI) imposes a substantial burden on patients and the health care system. Understanding cost differences among common treatment strategies can inform efficient, evidence-based care models.
OBJECTIVES: The objective of this study was to evaluate and compare the 1-year costs associated with 3 initial treatment strategies for SUI: pelvic floor muscle therapy (PFMT), vaginal pessary, and sling surgery.
STUDY DESIGN: This was a retrospective cost-minimization analysis using a decision-tree model informed by a 5% national Medicare Fee-For-Service claims dat

IMPORTANCE: Limited data describe short-term Medicare-allowed amounts associated with treatments for stress urinary incontinence (SUI).
OBJECTIVE: The objective of this study was to compare total Medicare-allowed amounts for pelvic floor muscle therapy (PFMT), pessary, and sling surgery as the first SUI treatments observed.
STUDY DESIGN: We analyzed 5% Medicare fee-for-service claims from 2008 through 2016 for women aged 66 years or older with SUI or mixed urinary incontinence. Episode allowed amounts were summed for 90 days after PFMT or pessary treatment and 180 days after sling surgery, a

INTRODUCTION: Type 2 diabetes mellitus (T2DM) is often comorbid and, along with non-glycemic complications such as bladder dysfunction and sleep disturbances, significantly reduces the health-related quality of life (HRQoL). In Pakistan, these associations have remained unexplored. The present study sought to investigate whether diabetic bladder dysfunction (DBD) was associated with the sleep quality and HRQoL in adults with T2DM.
METHODS: This study employed a cross-sectional design among 300 T2DM patients selected from outpatient departments in Islamabad and Rawalpindi using convenience sam

INTRODUCTION: Hospitalized older adults are often routinely placed in diapers without prior assessment of continence status. This practice leads to several adverse outcomes, including skin complications.
OBJECTIVES: This evidence implementation project aimed to achieve 80% compliance among nursing staff with continence promotion strategies and a 50% reduction in the incidence of diaper-related complications.
METHODS: The project was conducted over a 21-month period in four medical wards at a public university hospital in Singapore. The project used the JBI Evidence Implementation Framework,

PURPOSE: Hip fracture in older men is associated with disproportionately poor outcomes compared to women, yet the prevalence and prognostic significance of incontinence in this population remain understudied. This study aimed to determine the prevalence of urinary (UI) and fecal incontinence (FI) in older men with hip fracture, evaluate their association with 1-year mortality, and identify factors linked to UI at post-fracture out-patient comprehensive geriatric assessment (CGA).
METHODS: This retrospective study analyzed data from 644 men aged ≥65 with first hip fracture (2007-2019) at Seinä

Background There is a pressing need among individual surgeons and busy clinics for a time-efficient method to record, assess, and monitor cases going to surgery. Purpose To develop a compact, symptom-based, simplified diagnostic model and database, designed to be sufficiently comprehensive, accurate and predictive of causation to guide pelvic floor surgeries and monitor their results postoperatively. Methods We condensed the four parts of the validated comprehensive Integral Theory Diagnostic System into a 10-minute assessment, including QOL and "simulated operations" to test the possibility o

BACKGROUND: Axial spondyloarthritis (axSpA) is a chronic inflammatory disease of the sacroiliac joints and spine whose burden extends beyond the musculoskeletal system. Lower urinary tract symptoms are increasingly recognised in these patients, whereas anorectal symptoms have received almost no attention. Because the lower urinary tract and the anorectal continence mechanism share pelvic floor musculature and sacral neural regulation, symptoms of these two systems might be expected to co-occur, yet this has not been examined in axSpA. Anxiety and depression are also common in this population a

BACKGROUND: Neurogenic lower urinary tract dysfunction (NLUTD) severely affects patients' quality of life, and conventional therapies have limited efficacy. Direct comparisons between SNM and peripheral nerve electrical stimulation are lacking. This study employed a network meta-analysis to evaluate the efficacy of these two approaches on voiding-related parameters.
METHODS: A computerized search of PubMed, Embase, Scopus, Web of Science, VIP, Wanfang, and CNKI databases was conducted for publicly available randomized controlled trials (RCTs) on sacral neuromodulation and peripheral nerve ele

The Weiss nocturia indices have structured clinical thinking about nocturia for over 2 decades. By deriving simple ratios from the frequency-volume chart, they classify nocturia into nocturnal polyuria, reduced nocturnal bladder capacity, and mixed etiology. The arithmetic is sound. But the framework rests on an unexamined assumption: that the maximum voided volume, almost always the first morning void, represents "bladder capacity," and that the gap between this value and daytime voided volumes reflects normal physiology. We propose that what the frequency-volume chart measures during wakeful

INTRODUCTION: Urinary incontinence (UI) is a prevalent geriatric syndrome that significantly affects the physical, psychological, and social well-being of older adults. Non-pharmacological interventions are recommended as the first-line approach, especially in frail older adults.
DESIGN: This was a prospective pre-post observational study.
METHODS: The study included sixty-one patients with rehabilitable UI who were admitted to an intermediate care hospital. The nurse-led multidisciplinary program integrated education, hygiene-dietary measures, pelvic floor physiotherapy, behavioral strategi

Intermittent and uncontrolled loss of urine is common among children but requires medical attention when it persists beyond age 5 years. Mostly functional in origin, urinary incontinence is caused by a complex inter-relationship of biopsychosocial factors, and evidence shows that mental health problems and adverse childhood experiences (ACEs) are common in children and adolescents with urinary incontinence. Mental health problems and ACEs (including abuse, neglect, and household dysfunction) could be both causes and consequences of paediatric urinary incontinence, but research is needed to und
