Placebo and nocebo responses in gabapentin enacarbil and pregabalin trials for restless legs syndrome: time-course model-based network meta-analysis and meta-analysis of proportions.

PURPOSE: Placebo responses are often stronger in randomized controlled trials (RCT) on conditions that measure subjective improvement in symptoms, such as restless legs syndrome (RLS). This meta-analysis sought to quantify both placebo and nocebo responses in published RCTs of gabapentin enacarbil (GEn) and pregabalin. METHODS: In the first meta-analytic approach, we evaluated the time-courses taken by the participants randomized to the different doses of GEn and pregabalin and to placebo in their responses in International RLS Study Group Rating Scale (IRLS) scores. This was primarily evaluat

Pregabalin and duloxetine combination for painful diabetic neuropathy: a systematic review and meta-analysis.

BACKGROUND: Diabetic neuropathy is one of the most common complications of diabetes, affecting about half of all people with the disease. Among these, 30%-50% experience nerve-related pain, characterized by abnormal sensations, burning, or stabbing pain, a condition known as painful diabetic neuropathy (PDN). PDN not only severely impairs quality of life but is also closely associated with sleep disturbances, depression or anxiety, and foot complications. Together, these problems substantially increase healthcare costs and place a considerable economic burden on both families and society. MET

Effectiveness and safety of cenobamate after conversion to monotherapy or dual therapy in real-world clinical practice.

OBJECTIVE: Patients with drug-resistant epilepsy (DRE) typically take multiple anti-seizure medications (ASMs) and are at risk of treatment-related adverse events (AEs). This study assessed the impact of cenobamate monotherapy or dual therapy in patients with drug-resistant epilepsy. METHODS: This was a multicenter (six Spanish epilepsy units), retrospective, observational study in patients with drug-resistant epilepsy who had received treatment with cenobamate as monotherapy (after conversion from polytherapy) or in dual therapy for at least three consecutive months, between April 2017 and N

Effectiveness of adjunctive low-dose clobazam in adults with focal drug-resistant epilepsy and incomplete response to cenobamate: A real-world study.

OBJECTIVE: Cenobamate (CNB) is a highly effective antiseizure medication (ASM) for focal drug-resistant epilepsy (DRE); nevertheless, even with CNB treatment and optimized therapy, a substantial proportion of patients continued to experience seizures. Preliminary observations suggest that clobazam (CLB), a 1,5-benzodiazepine, may enhance CNB efficacy. We aimed to evaluate the real-world effectiveness and safety of adjunctive low-dose CLB in adults with focal DRE who continued to have incomplete seizure control despite CNB. METHODS: We conducted a retrospective single-center study including ad

First-year Experience with Brivaracetam in Japan: Real-world Data from Juntendo Epilepsy Center.

Approximately 30% of patients still develop drug-resistant epilepsy despite the introduction of newer antiseizure medications. Brivaracetam, a high-affinity synaptic vesicle protein 2A ligand with a mechanism of action similar to that of levetiracetam, was only recently approved in Japan in June 2024 and became available at our institution in September 2024. Clinical data in real-world settings remain limited. To date, no real-world clinical data have been systematically reported in patients in Japan, and the present study aimed to characterize the initial real-world experience with Brivaracet

Is academic mobility good for you? A case report on the effects of frequent, two-weekly travel across one time zone on sleep-wake indices.

The aim of the present study was to investigate the actigraphic sleep-wake and light exposure patterns of a 48-y-old healthy woman who travelled across one time zone (Italy to UK and vice versa) every two weeks for over 2.5 years (i.e. 852 night of sleep data, with 80% of recorded days retained for analysis after exclusion of travel and non-UK/Italy stays; 58% of time spent in Italy), and reported early evening somnolence and unpleasantly early wake-up time in the UK, especially immediately after travelling. She therefore started to wear an actigraph to monitor rest-activity and light exposure

Successful treatment of carbapenem-resistant Acinetobacter baumannii and Enterobacterales coinfection with sulbactam-durlobactam combined with aztreonam: a case report.

BACKGROUND: Carbapenem-resistant Gram-negative bacilli (CRGNB) often cause nosocomial infections difficult to treat. Their widespread dissemination, associated high mortality, and increasing disease burden constitute a significant global public health challenge. This situation is further exacerbated by mixed CRGNB infections, which exhibit elevated mortality rates and increased therapeutic complexity. Thus, it is imperative to establish effective antibiotic strategies with minimal adverse effects to address these polymicrobial infections. We present this case to evaluate the efficacy of sulbac

Safety evaluation of an effervescent curcuminoids-ascorbic acid-polysaccharide-β-cyclodextrin (CUR-A-Poly-β-CD) inclusion complex powder in healthy volunteers: A non-placebo-controlled pilot study.

Colorectal cancer (CRC) is a prevalent and aggressive malignancy with a high mortality rate. Although chemotherapy can improve survival outcomes, it is frequently associated with undesirable side effects. Curcuminoids, the principal polyphenol derived from Curcuma longa L., have demonstrated promising potential as an alternative anticancer agent. Previous studies have shown that the effervescent curcuminoid-ascorbic acid-polysaccharide-β-cyclodextrin (CUR-A-Poly-β-CD) inclusion complex exerts anticancer effects against colorectal cancer cells, with no acute toxicity observed in animal models.

Ketamine for Depression in Serious Illness: Evidence, Safety, and Practical Approaches.

Patients with serious illnesses and short prognoses often experience depression and suicidal ideation. Traditional antidepressants are limited by delayed onset, creating a need for rapidly acting therapies. In this Palliative Care Rounds, we examine the evidence for ketamine/esketamine's efficacy as antidepressants, including evidence specific to people with serious illnesses. In psychiatric studies, intravenous ketamine produces rapid (1-24 hours), moderate-to-large antidepressant effects lasting one to two weeks, with a number needed to treat of three in the first week. Esketamine nasal spra

Efficacy of Low-Dose Olanzapine for the Prevention of Conditioning Therapy-Induced Nausea and Vomiting in Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation.

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a curative treatment for hematological malignancies. Conditioning therapy often causes severe chemotherapy-induced nausea and vomiting (CINV). To prevent CINV, olanzapine (OLZ) is recommended as part of a four-drug antiemetic regimen for highly emetogenic chemotherapy; however, evidence in the allo-HSCT setting remains limited. This study aimed to evaluate the effectiveness and safety of low-dose OLZ in preventing CINV in patients undergoing allo-HSCT. We conducted a retrospective single-center cohort study of adult patients wit

Post-acute sequelae after Nipah virus infection: a systematic review and meta-analysis.

BACKGROUND: Nipah virus was first detected in the 1998-1999 Malaysia outbreak and remains a significant public health concern due to its high epidemic potential and recurrent outbreaks in South Asia. Incidence patterns of post-acute sequelae, characterised by persistence or delayed onset after the acute phase of an infection, are not well documented after infectious disease outbreaks. We aimed to address this knowledge gap. METHODS: We conducted a systematic review and meta-analysis on the prevalence, incidence, duration, and characteristics of post-acute sequelae in survivors of Nipah virus
