Relationship between prodromal symptoms of psychosis and suicidal ideation among Colombian school adolescents.

Prodromal symptoms of psychosis are a public health problem that causes distress and are linked to schizophrenia. However, knowledge among school students from the community is limited. The study aimed to know the relationship of prodromal symptoms of psychosis with suicidal ideation in school adolescents from a city in the Caribbean Region of Colombia. A cross-sectional study was conducted, with participants being tenth- and eleventh-grade students. The participants filled out the prodromal symptoms questionnaire (PQ-16) and an item that assessed suicidal ideation in the last year. Odds ratio

EEG changes associated with hallucinations caused by Charles Bonnet Syndrome.

Charles Bonnet syndrome (CBS) is characterised by the presence of visual hallucinations following visual loss in many patients. The neuropathophysiology of CBS is poorly understood. We used electroencephalography (EEG) in individuals with frequent hallucinations in order to identify changes in neural activity that co-occur with hallucination onset. We found reduced α power in occipital electrodes at the onset of the hallucinations compared to offset (p < 0.05), suggesting that intermittent periods of low endogenous alpha may create the neural conditions for hallucination to emerge in CBS patie

First-in-human low-intensity focused ultrasound targeting striatal circuits in schizophrenia: feasibility, safety, and effects on hallucinations and striatal-temporal functional connectivity.

BACKGROUND: Auditory hallucinations are among the most disabling symptoms in individuals with schizophrenia (SZ) and are linked to aberrant signaling within deep-striatal circuits, such as the nucleus accumbens (NAc) and caudate head (CH). However, causal tests of striatal involvement have been limited by the inaccessibility of these structures using noninvasive neuromodulatory techniques. Low-intensity focused ultrasound (LIFU) provides millimeter-scale precision capable of modulating deep-brain circuits, but its feasibility and impact on hallucinations in SZ remain unknown. METHODS: SZ part

The subjective experiences of patients recovering from delirium in acute geriatric care: An analysis of quantitative and qualitative interview data.

BACKGROUND: Delirium occurs in about one fourth of hospitalized older adults and is associated with multiple adverse outcomes. Despite efforts to improve the prevention and treatment of delirium, it remains a burden for patients, their relatives, and healthcare professionals. Currently, little is known about how people experience delirium episodes. METHODS: A sample of 77 inpatients who had been recently treated for delirium were recruited from acute geriatric care units at four hospitals. A semi-structured interview was conducted to explore memories and feelings associated with the delirium e

Drug-Induced Psychosis: A Drastic Turn After a Kidney Transplant.

Drug-induced psychosis is an uncommon but clinically significant complication in solid-organ transplant recipients, particularly those receiving calcineurin inhibitors such as tacrolimus. Neuropsychiatric toxicity can occur even at therapeutic drug levels, and concurrent corticosteroid use may further increase vulnerability. Early recognition is essential, as symptoms may be mistakenly attributed to metabolic disturbances, primary psychiatric illness, or postoperative delirium, thereby delaying appropriate intervention. We report the case of a previously healthy 34-year-old man with end-stage

Mapping Symptom Complexity in Dementia With Lewy Bodies: A Network Analysis Approach.

OBJECTIVE: Dementia with Lewy bodies (DLB) is the second most common neurodegenerative dementia after Alzheimer's disease but remains underdiagnosed because of its heterogeneous symptomatology. Network analysis may capture interrelationships among symptoms, potentially improving diagnostic precision. The authors applied network analysis to a cohort of patients with DLB to map symptom connections, identify central features, and explore clustering patterns. METHODS: Clinical records of 107 patients with a confirmed DLB diagnosis from Brugmann University Hospital, Brussels, were retrospectively

Mindfulness-based group therapy for auditory hallucination management in schizophrenia: A randomized controlled trial.

Auditory hallucinations, a core symptom of schizophrenia, affect 50-70% of patients and significantly impair their quality of life. Although multimodal treatments, including pharmacotherapy, psychotherapy, and neuromodulation, are commonly employed, their efficacy remains limited. This randomized controlled trial aimed to assess the effects of Mindfulness-Based Auditory Hallucination Management (MBAHM) on auditory hallucinations, anxiety, depression, and quality of life in patients with schizophrenia. Eighty patients with schizophrenia were randomly assigned to the MBAHM group (n = 40) or the

Neurocognitive Effects of Neurofeedback: A Systematic Review of the Applicability and Therapeutic Effect in Patients with Schizophrenia Spectrum Disorders, Psychosis or Clinical High Risks for Psychosis.

Schizophrenia Spectrum Disorders are complex mental health conditions that significantly impact cognitive function and quality of life. While pharmacological and psychotherapeutic interventions are available, their effectiveness remains limited, particularly for negative symptoms and cognitive impairments. These limitations, alongside drug side effects and adherence difficulties, highlight the need for new treatments. Cognitive remediation strategies like Neurofeedback show promise by harnessing neuroplasticity. This systematic review aims to evaluate the neurocognitive and humoral changes ind

Compensatory hallucinogenesis across three neuropsychiatric disorders: a Bayesian account.

Emerging evidence suggests that hallucinations may arise because of an over-reliance on prior knowledge during perception. While best established in psychosis-spectrum illness, data also support the presence of this abnormality in other hallucination-prone neuropsychiatric illnesses that vary in their association with disruption of sensory circuits. In this piece, we ask whether an over-weighting of expectations may be conceived of as a compensatory response to degraded incoming sensory information. We make the case that visual hallucinogenesis across a wide array of neuropsychiatric disorders

Neuropsychiatric Symptoms in Patients With Pathologically Confirmed Comorbid Alzheimer Disease and Frontotemporal Lobar Degeneration.

BACKGROUND AND OBJECTIVES: Little is known about the clinical presentation in patients with comorbid Alzheimer disease neuropathologic change (ADNC) and frontotemporal lobar degeneration (FTLD) neuropathology, despite frequent comorbidity of neurodegenerative diseases on autopsy. In other neurodegenerative conditions, multiple pathologies alter the presentation of neuropsychiatric symptoms, complicating clinical care. We examined whether neuropsychiatric symptoms differ in patients with comorbid ADNC/FTLD compared with patients with each pathology alone. METHODS: This was a retrospective exam

Unprovoked Cerebral Venous Infarction Presenting as Audio-Visual Hallucinations: A Case Presentation.

Cerebral venous sinus thrombosis (CVST) is a rare type of stroke with a variable presentation. Without a prompt diagnosis, subacute presentation can delay treatment with devastating results. Our case presents an unusual clinical pattern of a young patient presenting with audio-visual hallucinations who was diagnosed with CVST. A 26-year-old man with no neurological or psychological history presented with audio-visual hallucinations, transient uncontrolled movements of his left upper extremities, and right-sided headache. Physical examination was nonfocal, and he demonstrated normal awareness,

Specific psychiatric symptoms during cannabis use and their relation to psychotic disorders.

BACKGROUND AND OBJECTIVES: Cannabis use is an increasing phenomenon worldwide and represents a controversial issue, as it has direct implications in the development of psychosis. There are no widely accepted tools of evaluating the risk for users and therefore no prevention strategies. As psychosis and schizophrenia are two major psychiatric disorders, developing a screening tool is of great need. The current study evaluates the feasibility of the cannabis experience questionnaire 6 (CEQ-6) questionnaire as a screening tool for cannabis users that are at high risk of developing psychosis or sc

Evaluation and Treatment Considerations for Children and Adolescents Presenting With Psychosis Symptoms in Primary Care.

Pediatricians and all other pediatric primary care clinicians are likely to encounter youth who report symptom concerns of psychosis, such as auditory or visual hallucinations. It is essential for clinicians to be comfortable evaluating and triaging these symptoms, including assessment of co-occurring safety concerns, to ensure access to appropriate treatment. Due to limited specialty mental health resources nationwide, pediatricians may also be tasked with initiating medications or referring directly to evidence-based psychosocial interventions in the community. This article reviews different

Neuropsychiatric Presentation of Non-paraneoplastic Voltage-Gated Calcium Channel Autoimmune Encephalitis: A Case Report.

Autoimmune encephalitis is an immune-mediated inflammatory disorder of the brain with diverse neurological and psychiatric manifestations. Voltage-gated calcium channel (VGCC) antibody-associated autoimmune encephalitis is rare and has been predominantly described in paraneoplastic settings. Emerging reports suggest a broader range of central nervous system involvement, but presentations dominated by psychiatric symptoms remain uncommon and diagnostically challenging. A 39-year-old man with no prior medical illness presented with subacute progressive cognitive decline, behavioral changes, and

Unveiling the Complex Interplay: Sudden Emergence of First-Rank Schneiderian Symptoms Following TMS in a Patient With Severe Depression and Complex Trauma.

Schizophrenia-spectrum disorders are characterized by psychotic symptoms, including hallucinations, delusions, and disorganized thinking. Schneider's first-rank symptoms (FRS)-including thought broadcasting and experiences of external control-are clinically salient but not ergonomic and require careful differential diagnosis. The emergence of new-onset psychosis in midlife, particularly in individuals without prior psychotic history, warrants a broad biopsychosocial evaluation. We present the case of a 47-year-old male with chronic major depressive disorder and complex developmental trauma who

HDL cholesterol indicates neurodegenerative vulnerability in dementia with lewy bodies.

Altered lipid metabolism has been implicated in neurodegenerative disorders; however, its relevance to dementia with Lewy bodies (DLB) remains incompletely characterized. This study examined associations between plasma lipid profiles, lipid-related inflammatory indices, and clinical and neurodegenerative features in drug-naive patients with DLB, with comparisons to age-matched healthy controls. Plasma levels of high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol did not differ significantly between patients with DLB and controls, whereas triglyceride (TG) level

Case Report: Pediatric Hallucinations and Anti-Neuronal Intermediate Filament Autoimmune Encephalitis.

INTRODUCTION: Patients with psychiatric concerns often present to an emergency department (ED) for medical evaluation prior to inpatient psychiatry placement. One diagnosis to consider prior to disposition is autoimmune encephalitis (AIE). This report describes a pediatric patient who presented with psychiatric symptoms that required inpatient admission and workup to diagnose a rare form of AIE. CASE REPORT: A 16-year-old female with no known past medical history presented as a transfer from an outside hospital for medical evaluation of two days of auditory and visual hallucinations. A Qualitative Investigation of the Experience of Taking Xanomeline and Trospium Chloride for Schizophrenia, Part 1: Perceived Impact on Symptoms.

INTRODUCTION: Xanomeline and trospium chloride (formerly KarXT) is a muscarinic M1 and M4 receptor agonist recently approved for the treatment of schizophrenia in adults. Unlike all previously approved antipsychotics, it does not directly block dopamine D2 receptors. Given its novel mechanism of action, understanding patients' subjective treatment experiences, including perceived symptom changes, is clinically important. METHODS: This is a qualitative study embedded within a larger 52-week open-label long-term safety study that investigated patient perspectives on xanomeline/trospium monother
