BACKGROUND: Charles Bonnet syndrome (CBS) refers to visual hallucinations that can occur following a decline in vision. Although vision loss increases fall risk and susceptibility to falls-related variables such as activity limitation, the potential contribution of CBS is unclear. The present study adopted a qualitative approach to explore experiences of CBS from different perspectives, including its impact on physical activity and falls.
METHODS: Semistructured interviews were conducted with 17 older adults living with CBS (mean age = 76 years) and 12 community falls and sight loss professio

Hyoscyamus niger (henbane) is a toxic plant containing tropane alkaloids that can cause anticholinergic toxicity. Pediatric presentations may differ from adults and may include atypical behavioral manifestations such as hyperactivity and euphoria without agitation. An 11-year-old previously healthy boy presented with sudden behavioral changes, hyperactivity, and unexplained euphoria. Approximately four hours earlier, he had ingested about one tablespoon of fresh Hyoscyamus niger seeds. Physical examination revealed minimal mydriasis and mild flushing. The patient was cooperative but disoriente

IMPORTANCE: Auditory hallucinations (AHs) are a hallmark characteristic of schizophrenia spectrum disorders (SSDs) and are often associated with severe distress. Previous research lacks comprehensive evidence on the overall efficacy of approaches targeting AHs in SSDs.
OBJECTIVE: To evaluate the efficacy and acceptability of all available psychological and psychosocial interventions targeting AHs in SSDs.
DATA SOURCES: Embase, MEDLINE, PsycArticles, PsycInfo, PSYNDEX, and the Cochrane library were searched for eligible studies published until August 18, 2025.

Background and clinical significance: Autoimmune encephalitis (AE) is an inflammatory brain disorder that manifests through a diverse, unspecific range of neuropsychiatric symptoms. When AE occurs alongside a primary neurodegenerative disorder, the shared symptoms can create a mixed clinical profile, making diagnosis more difficult and potentially postponing effective management and treatment. Case presentation: We describe the case of a 58-year-old female with a one-year history of progressive behavioral and personality changes who presented a subacute confusional state, psychomotor retardati

Virtual reality (VR)-assisted avatar therapy has shown promising effects for reducing voice-related distress and overall severity of persistent auditory verbal hallucinations (AVH) in psychosis, yet the mechanisms underlying these improvements remain unclear. Emotional processes may play a role during therapeutic encounters with simulated voices, but it is unknown whether treatment effects involve broader changes in emotion regulation. We conducted secondary analyses of the CHALLENGE randomized clinical trial (N = 270), comparing a seven-session immersive VR-based avatar therapy with enhanced

Introduction: A vast body of cognitive research in psychosis has focused on auditory hallucinations, though more recent studies are turning towards other sensory modalities. This study aimed to compare the cognitive profile of persons experiencing uni- or multisensory versus multimodal hallucinations. Methods: Participants with primary diagnosis of a psychotic disorder were subdivided into those experiencing uni- or multisensory (UMS; n = 31) versus multimodal (MM; n = 28) hallucinations relative to non-clinical controls (NC; n = 32). Cognitive assessment comprised the MATRICS Consensus Cognit

OBJECTIVE: Hallucination studies in psychiatry have typically focused on the auditory domain, despite mounting evidence that non-auditory hallucinations are prevalent, and elicit distress and functional interference. Proponents have called for more in-depth phenomenological enquiries into hallucinations across multiple sensory modalities, but there are no comprehensive, validated assessment tools to date. This study aimed to document the design and pilot validation of such a transdiagnostic instrument - the Multisensory Hallucinations Schedule.
METHODS: Leveraging off a precursor measure, the

BACKGROUND: Hallucinations are common yet under-recognized after cardiac surgery and contribute to postoperative morbidity. Hypertension (HTN), the most prevalent comorbidity in surgical candidates, may alter susceptibility through impaired cerebrovascular autoregulation. Whether HTN modifies predictors of postoperative visual and auditory hallucinations remains unclear.
METHODS: In the prospective, multicenter VAACS study, 1,332 adults undergoing CABG or valve surgery were stratified by hypertension (HTN, n = 517; non-HTN, n = 815). Visual and auditory hallucinations within 7 days were asses

BACKGROUND: Cognitive impairment is a core feature of schizophrenia and bipolar disorder (BD), yet few studies have explored its association with specific psychopathological symptoms.
AIM: We aimed to analyse the relationship between psychotic symptoms and cognitive impairment using bedside cognitive tests in patients with schizophrenia and BD.
METHODS: This cross-sectional study included 100 patients (50 with schizophrenia and 50 with BD) from a tertiary care psychiatry department. Participants were selected based on inclusion and exclusion criteria and assessed using standardized psychopat

INTRODUCTION: Charles Bonnet Syndrome (CBS) is a condition characterized by visual hallucinations in individuals with visual impairments. Among military veterans, CBS can cause considerable distress, leading to health-related anxiety and delays in seeking help. Despite its prevalence, awareness of CBS remains limited across healthcare services, resulting in underdiagnosis and inadequate support. There is a need to better understand experiences of managing CBS and accessing healthcare services, from both healthcare and service user perspectives. Additionally, tailored resources are needed to ad

OBJECTIVE: To develop a multimodal deep learning-based classification model for adolescent bipolar depression (ABD) with verbal auditory hallucinations (AVHs).
METHODS: A retrospective analysis was conducted on 47 untreated ABD patients within 30 days, between January 2024 and August 2025. Comprehensive clinical data were collected, including sex, age, age at onset, years of education, and the presence of suicidal or self-harming behaviors. Based on the PANSS P3 score and the presence of AVHs, patients were divided into a hallucination group (P3 score > 3, n = 24) and a non-hallucination grou

Background: Hypothyroidism is a prevalent endocrine disorder capable of affecting multiple organ systems. While somatic symptoms are common, prominent neuropsychiatric manifestations such as visual hallucinations and cognitive deficits are exceptionally rare as an initial presentation. This atypical clinical picture frequently leads to misdiagnosis as primary psychiatric disorders or neurodegenerative conditions. Case Presentation: We report the case of a 75-year-old male who presented with a three-month history of complex visual hallucinations and progressive cognitive decline. The hallucinat

BACKGROUND: Rapid eye movement sleep behavior disorder (RBD) and visual hallucinations (VH) are prognostically relevant non-motor symptoms in Parkinson's disease (PD), but their combined effects on local brain dysfunction remain unclear.
OBJECTIVE: To characterize regional brain dysfunction patterns associated with RBD and VH in PD and to explore candidate-region symptom-related effects within regions showing overall between-group differences.
METHODS: In this cross-sectional study, 96 patients with PD were divided into four groups according to the presence or absence of RBD and VH (24 per g

The management of patients with schizophrenia is a complex undertaking informed by a diverse evidence base addressing pharmacological, psychosocial and physical therapies. The journal has recently published a clinical guideline, using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach, to assess the evidence for the effectiveness of a range of treatments, providing a valuable guide to practice and service planning. However, the guideline process did not assess the evidence for forms of non-invasive brain stimulation such as repetitive transcranial magnetic

Bupropion toxicity can present as neurologic and autonomic manifestations that closely resemble alcohol withdrawal syndrome, a diagnostic challenge that becomes compounded when patients are unable to provide reliable histories and when confirmatory toxicology results are delayed. We present the case of a 55-year-old male who presented with agitation, encephalopathy, auditory and visual hallucinations, hypertension, tremors, and suspected seizure activity. Based on his substance use history and clinical presentation, he was managed as alcohol withdrawal using symptom-triggered benzodiazepines p

OBJECTIVES: Postoperative hallucinations are underrecognized neuropsychiatric complications of cardiac surgery. Glycemic control may influence their occurrence, particularly in patients with diabetes. This study aimed to estimate the incidence of visual and auditory hallucinations in diabetic and non-diabetic patients after cardiac surgery, and to identify factors independently associated with hallucinations within each group.
METHODS: In this prospective multicenter cohort, hallucinations were assessed daily for 7 days using the Questionnaire for Psychotic Experiences. Cox regression identif

BACKGROUND: Voice hearing can occur in healthy people without a trigger, be caused by a traumatic event, or occur as part of a psychiatric disorder. There are still many gaps in our understanding of voice hearing, and research in this field can be challenging, maybe because it is associated with shame and stigma. Those who experience distress by voice hearing could benefit from a treatment, aimed directly at that. The ECHO manualized treatment is based on the principles of compassion-focused therapy (CFT).
METHODS: Treatment will take place on several different psychiatric hospitals in the Re

Background: Substance Use Disorder (SUD) is frequently associated with psychiatric comorbidity, including psychotic symptoms, impulsivity and neurodevelopmental traits. The influence of age and duration of substance use on these clinical characteristics and on treatment retention remains insufficiently understood. Objectives: To examine the influence between age, duration of substance use, clinical presentation, patterns of violence, and treatment retention in individuals with SUD. Methods: A prospective 6-month cohort study was conducted at the Alcoholism Treatment Unit of the CAUSA Hospital
